    Overt and covert effects of cognitive fatigue on attention networks

    Background and aims Cognitive fatigue refers to a variation of the psychophysiological state during or after prolonged periods of mental activity that requires work efficiency, and could lead to temporary deterioration of attentional functioning, especially top-down attention and cognitive control. The present study aims to verify the effects of cognitive fatigue on attention in the context of the three attentional networks described by Posner, by using behavioral and psychophysiological measures, to detect variations in overt and covert responses respectively. Methods Thirty young healthy subjects were enrolled in the study, 15 in the “fatigue” and 15 in the control group. Cognitive fatigue was provoked by a continuous arithmetic task lasting 1 h, and the EEG recordings were conducted before and after the task, while subjects were performing the attention network test. The N1, N2 and P3 components were analyzed for the alerting, orienting and conflict networks, in conformity with behavioral analysis. Results No difference emerged between groups in networks' efficiency scores and in N1 and P3 amplitudes related to the alerting network. As regards the orienting network, P3 amplitude was significantly reduced in the fatigue group alone (p = 0.02), while no differences emerged in N1 amplitude. As regards the conflict network, both N2 and P3 amplitudes were significantly reduced in the fatigue group alone and selectively for the incongruent target (p < 0.001; p = 0.001 respectively). Conclusions Our results suggest that, in young healthy subjects, cognitive fatigue interferes with goal-driven attention especially when the task demand is higher, sparing the bottom-up attention control mechanisms and in absence of any overt observable effect

    Diffusion MRI Findings in Encephalopathy Induced by Immunosuppressive Therapy after Liver Transplantation

    Neurological complications are common after liver transplantation, as they affect up to one-third of the transplanted patients and are associated with significant morbidity. The introduction of calcineurin inhibitors, cyclosporine A and tacrolimus, in immunosuppressive regimens significantly improved the outcome of solid-organ transplantation even though immunosuppression-associated neurotoxicity remains a significant complication, particularly occurring in about 25% of cases after liver transplantation. The immunosuppressant cyclosporine A and tacrolimus have been associated with the occurrence of major neurological complications, diffuse encephalopathy being the most common. The biochemical and pathogenetic basis of calcineurin inhibitors-induced neurotoxicity are still unclear although several mechanisms have been suggested. Early recognition of symptoms could help reduce neurotoxic event. The aim of the study was to evaluate cerebral changes through MRI, in particular with diffusion-weighted images (DWI) and apparent diffusion coefficient (ADC) maps, in two patients undergoing liver transplantation after immunosuppressive therapy. We describe two patients in which clinical pictures, presenting as a severe neurological condition, early after orthotopic liver transplantation during immunosuppression therapy, showed a different evolution in keeping with evidence of focal-multifocal lesions at DWI and ADC maps. At clinical onset, DWI showed hyperintensity of the temporo-parieto-occipital cortex with normal ADC values in the patient with following good clinical recovery and decreased values in the other one; in the latter case, MRI abnormalities were still present after ten days, until the patient's exitus. The changes in DWI with normal ADC may be linked to brain edema with a predominant vasogenic component and therefore reversible, while the reduction in ADC is due to cytotoxic edema and linked to more severe, nonreversible, clinical picture. Brain MRI and particularly DWI and ADC maps provide not only a good and early representation of neurological complications during immunosuppressant therapy but can also provide a useful prognostic tool on clinical outcome of the patient

    P300 component in euthymic patients with bipolar disorder type I, bipolar disorder type II and healthy controls: a preliminary event-related potential study

    The aim of the present study was to investigate P300 event-related potential components in euthymic bipolar disorder type I (BDI) and bipolar disorder type II (BDII) patients and matched controls. A total of 10 BDI patients, 10 BDII patients and 10 healthy individuals were enrolled in the study. Event-related potential data were collected according to a standard auditory 'oddball' paradigm. A significant groups effect in both the peak amplitude (P<0.001) and the mean amplitude (P<0.001) was observed; post-hoc comparisons showed that the peak and mean amplitudes of BDI and BDII patients were significantly lower than the peak and mean amplitudes of the healthy controls. The neurophysiological patterns found in the present study might at least partially reflect the presence of a mild selective cognitive impairment in euthymic BDI and BDII patients. From a clinical point of view, these evidences support the potential role of cognitive interventions in the treatment of BD

    Mental flexibility in Parkinson's disease with central fatigue: Data from the frontal assessment battery

    Background and aims Central fatigue is defined as a reduced energy level or an increased perception of effort, often associated to a failure in initiating and maintaining tasks that require self-motivation. It is common in Parkinson's disease population and it has been hypothesized to be related to a dysfunction in the striato-talamo- prefrontal loop. The aim of the present study was to explore the association between fatigue and executive functions as index of integrity of the striato-thalamo-prefrontal loop. Methods Twenty-nine non-demented PD patients without fatigue - PDnF, 28 non-demented PD patients with fatigue - PDF and 26 age and sex- matched controls underwent an evaluation with the Frontal Assessment Battery (FAB), MMSE, PSQI, BDI, STAI Y1-2, PDQ-39. Differences between groups in FAB scores (total and subitems) were analyzed by means of Kruskal-Wallis test. Moreover, a correlation between fatigue and FAB was also analyzed. Results Overall parkinsonian population displayed worse performance than controls in frontal scores especially inhibitory control (p = 0.008) and sensitivity to interference (p = 0.014). PDF displayed significantly worse than PDnF in verbal fluency (p = 0.05). Fatigue severity inversely correlated with executive performance (p b 0.001). Conclusions Phonemic fluency tasks are thought to reflect the simultaneous engagement of several executive functions such as attention, working memory, retrieval, information processing. The association of central fatigue with a deficit in mental flexibility, could support the hypothesis that central fatigue is a reliable index of the impairment of higher executive functions needed in order to effectively assess costs and benefits related to adaptive decision- making behavior

    Cerebellum in timing control: Evidence from contingent negative variation after cerebellar tDCS

    Background and aims Timing control is defined as the ability to quantify time. The temporal estimation of supra-seconds range is generally seen as a conscious cognitive process, while the sub-seconds range is a more automatic cognitive process. It is accepted that cerebellum contributes to temporal processing, but its function is still debated. The aim of this research was to better explore the role of cerebellum in timing control. We transitorily inhibited cerebellar activity and studied the effects on CNV components in healthy subjects. Methods Sixteen healthy subjects underwent a S1-S2 duration discrimination motor task, prior and after cathodal and sham cerebellar tDCS, in two separate sessions. In S1-S2 task they had to judge whether the duration of a probe interval trial was shorter (Short-ISI-trial:800 ms), longer (long-ISI-trail:1600 ms), or equal to the Target interval of 1200 ms. For each interval trial for both tDCS sessions, we measured: total and W2-CNV areas, the RTs of correct responses and the absolute number of errors prior and after tDCS. Results After cathodal tDCS a significant reduction in total-CNV and W2-CNV amplitudes selectively emerged for Short (p &lt; 0.001; p = 0.003 respectively) and Target-ISI-trial (total-CNV: p &lt; 0.001; W2-CNV:p = 0.003); similarly, a significant higher number of errors emerged for Short (p = 0.004) and Target-ISI-trial (p = 0.07) alone. No differences were detected for Longer-ISI-trials and after sham stimulation. Conclusions These data indicate that cerebellar inhibition selectively altered the ability to make time estimations for second and sub-second intervals. We speculate that cerebellum regulates the attentional mechanisms of automatic timing control by making predictions of interval timing

    Near-infrared spectroscopy as a tool for in vivo analysis of human muscles

    Recent advances in materials and fabrication techniques provided portable, performant, sensing optical spectrometers readily operated by user-friendly cabled or wireless systems. Such systems allow rapid, non-invasive, and not destructive quantitative analysis of human tissues. This proof-of-principle investigation tested whether infrared spectroscopy techniques, currently utilized in a variety of areas, could be applied in living humans to categorize muscles. Using an ASD FieldSpec\uae 4 Standard-Res Spectroradiometer with a spectral sampling capability of 1.4 nm at 350\u20131000 nm and 1.1 nm at 1001\u20132500 nm, we acquired reflectance spectra in visible short-wave infra-red regions (350\u20132500 nm) from the upper limb muscles (flexors and extensors) of 20 healthy subjects (age 25\u201389 years, 9 women). Spectra off-line analysis included preliminary preprocessing, Principal Component Analysis, and Partial Least-Squares Discriminant Analysis. Near-infrared (NIR) spectroscopy proved valuable for noninvasive assessment of tissue optical properties in vivo. In addition to the non-invasive detection of tissue oxygenation, NIR spectroscopy provided the spectral signatures (ie, \u201cfingerprints\u201d) of upper limb flexors and extensors, which represent specific, accurate, and reproducible measures of the overall biological status of these muscles. Thus, non-invasive NIR spectroscopy enables more thorough evaluation of the muscular system and optimal monitoring of the effectiveness of therapeutic or rehabilitative interventions

    Prefronto-cerebellar transcranial direct current stimulation increases amplituded and decreases latency of P3b component in patients with euthymic bipolar disorder

    INTRODUCTION: Neurocognitive impairments have been observed in patients with bipolar disorder (BD) even during the euthymic phase of the disease, potentially representing trait-associated rather than state-associated characteristics of the disorder. In the present study, we used transcranial direct current stimulation (tDCS) applied to cerebellar and prefrontal cortices to improve the neurophysiological performances of patients with euthymic BD. METHODS: Twenty-five outpatients with BD underwent open-label prefrontocerebellar tDCS for 3 consecutive weeks. Neurophysiological performances were assessed through the examination of the P3b and P3a subcomponents of P300 event-related potential at baseline and after stimulation. RESULTS: Compared to baseline, P3b component after tDCS showed significantly higher amplitude and shorter latency (latency: Fz P=0.02, Cz P=0.03, and Pz P=0.04; amplitude: Fz P=0.24, Cz P=0.02, and Pz P=0.35). CONCLUSION: In our sample of patients with euthymic BD, concomitant prefrontoexcitatory and cerebellar-inhibitory modulations led to improved brain information processing stream. This improvement may at least partially result from neuroplastic modulation of prefrontocerebellar circuitry activity

    The Ventricular System Enlarges Abnormally in the Seventies, Earlier in Men, and First in the Frontal Horn: A Study Based on More Than 3,000 Scans

    Objectives: To detect on computed tomography (CT) brain scans the trajectories of normal and abnormal ventricular enlargement during aging. Methods: For each 1-year age cohort, we assessed in 3,193 axial CT scans the Evans’ index (EI) in the anterior frontal horns and the parieto-occipital (POR) and temporal ratio (TR) in the posterior and inferior horns. Cut-off values for abnormal enlargement were based on previous clinical studies. Results: The mean age associated with normal linear measures was 71 years. Values for all three measures increased with age, showing a linear relationship below—but not above—each cut-off value. The mean age of participants with abnormal enlargement on CT progressed from 79 years for EI to 83 years for POR to 87 years for TR. These results suggested that ventricular dilatation progresses in an age–location relationship. First comes enlargement of the frontal horns (13.8% of scans), followed by the parieto-occipital horns (15.1% of scans) and then temporal horn enlargement (6.8% of scans). Scans from men displayed abnormal values earlier than scans from women (on average 6 years). Risk increased 5.1% annually for abnormal EI, 9.0% for abnormal POR, and 11% for abnormal TR (all p &lt; 0.001). The most frequent agreement between categories (normal–abnormal) for values of neuroimaging measures was identified for POR–TR. Conclusion: The results of this large radiological study suggest that the ventricular system enlarges progressively during aging, and in a subset of patients follows an abnormal consecutive geometric dilatation, influenced by age and sex

    Nabiximols effect on blood pressure and heart rate in post-stroke patients of a randomized controlled study

    Background: Cannabinoids may be useful to treat pain, epilepsy and spasticity, although they may bear an increased risk of cardiovascular events. This study aims to evaluate the cardiovascular safety of nabiximols, a cannabis-based drug, in patients with spasticity following stroke, thus presenting an increased cardiovascular risk. Methods: This is an ancillary study stemming from the SativexStroke trial: a randomized double-blind, placebo-controlled, crossover study aimed at assessing the effect of nabiximols on post-stroke spasticity. Patients were treated with nabiximols oromucosal spray or placebo and assessed before and after two phases of 1-month duration each. Only the phase with the active treatment was considered for each patient who completed the study. The average values of blood pressure (diastolic, systolic, differential) and heart rate from the first 5 days of the phase (lowest nabiximols dosage) were compared to the average values recorded during the last 5 days at the end of the phase (highest nabiximols dosage). Baseline comparisons between gender, stroke type and affected side and correlation between age and blood pressure and heart rate were performed. The study was registered with the EudraCT number 2016-001034-10. Results: Thirty-four patients completed the study and were included in the analysis. Thirty-one were taking antihypertensive drugs and, among these, 12 were taking beta-blockers. During the study, no arrhythmic events were recorded, blood pressure and heart rate did not show pathological fluctuations, and no cardiovascular or cerebrovascular events occurred. At baseline blood pressure and heart rate were comparable concerning gender, stroke type and affected side. A significant direct correlation emerged between differential blood pressure and age and an inverse correlation between diastolic blood pressure and age. No correlation emerged between systolic blood pressure or heart rate and age. Blood pressure and heart rate did not change during nabiximols treatment compared to the baseline condition. Conclusion: This ancillary study adds evidence that, in patients who already underwent a cerebrovascular accident, nabiximols does not determine significant blood pressure and heart rate variation or cardiovascular complications. These data support the cardiovascular safety of nabiximols, encouraging more extensive studies involving cannabinoids characterized by slow absorption rates
