Effectiveness of kinesio taping on postoperative morbidity after impacted mandibular third molar surgery: a prospective, randomized, placebo-controlled clinical study
Objective: Our study seeks to investigate the effectiveness of kinesio taping (KT) on postoperative morbidity compared to placebo and control groups after impacted third molar surgery. Methodology: Sixty patients with impacted mandibular third molar were included in this prospective, randomized, placebo-controlled clinical study. After surgical extraction of the impacted tooth, patients were allocated into three groups (20 patients each): group 1 received KT (kinesio), group 2 received placebo taping (placebo), and group 3 received no taping (control). The groups were compared regarding facial swelling, pain and trismus. Swelling was evaluated using a tape measuring method. Pain was assessed by a visual analog scale and the number of analgesic tablets taken. Trismus was determined by measuring maximum mouth opening. Results: In the KT group, all parameters reduced significantly on 2nd and 4th postoperative days compared to other groups; however, placebo and control groups revealed comparable outcomes. On 7th day, all groups showed comparable results. Conclusions: The KT application is an effective method for reducing morbidity after impacted mandibular third molar surgery. However, placebo taping is not as effective as proper taping. Placebo taping shows similar results compared to no taping regarding facial swelling percentage, pain and trismus

(bzs. rnk.), tablo ; 29 cm.İmplant tedavisinin başarısı açısından implantın yerleştirileceği bölgedeki kemik boyutlarının yeterli olmasının yanı sıra kemik kalitesinin uygun olması da büyük önem taşımaktadır. Bu sebeple, tedavinin başarısı için, kemik kalitesinin işlem öncesinde değerlendirilmesi gerekmektedir. Dental implantlar, osseointegrasyon tanıtıldıktan sonra dental rehabilitasyonda popüler bir alternatif tedavi yöntemi olmuştur. Dental implantların başarısı ve sağkalım oranı, yerleştirilmiş dental implantların sayısındakı hızlı bir artış ile birlikte giderek daha önemli hale gelmiştir. Dental implant tedavisinin başarısı, implant yerleştirilecek bölgedeki mevcut kemiğin miktarı ve kalitesinden etkilenmektedir. Çalışmalar kalitesiz ve yetersiz kemik miktarı olan vakalarda yerleştirilen implantlar için yüksek başarısızlık oranları göstermiştir1. Bu nedenle, ameliyat öncesi kemik yapısının kesin bir şekilde değerlendirilmesi tedavi planlaması sırasında çok önemlidir. Kemik kalitesi implant stabilitesinin sağlanmasında dikkate alınması gereken önemli bir faktördür. İyi bir stabilite implant kemik entegrasyonuna olumlu etki etmektedir. Bu nedenle, literatürde kemik kalitesi değerlendirilmesi için çeşitli sınıflandırma sistemleri ve prosedürleri tanımlanmıştır. İmplant yerleştirilme işlemi sırasında kemik kalitesi hakkında cerrahın sübjektif değerlendirmesiyle implant stabilitesi tahmin edilebilir. İmplant yerleştime işlemi sırasında ve sonrasında yerleştirme tork testi ve rezonans frekans analizi (RFA) ile implant stabilitesi objektif olarak değerlendirilebilir. Bütün bu yöntemler implant cerrahisi sırasında kemik kalitesi hakkında bilgi vermektedir. Bununla birlikte; bu öngörülebilir tekniklerin, ameliyat sırasında kullanılan yöntemlerle korelasyon göstererek ameliyat öncesinde yeterli implant stabilitesi sağlanabilmesi konusunda bilgi vermesi gerekmektedir. Bu nedenle, son yıllarda preoperatif kemik yoğunluğunun değerlendirilmesinde CT kullanma konsepti daha objektif bir yöntem olarak tanıtılmıştır. Birçok araştırmada CT kemik yoğunluğu ve primer implant stabilitesi arasındaki ilişki bildirilmiştir. Fakat hastaların CT taraması sırasında yüksek radyasyona maruz kalması olumsuz bir durumdur. Son zamanlarda, özel olarak kafa ve boyun bölgesinin görüntülenmesi için tasarlanmış konik ışınlı CT (CBCT) tekniği tanıtıldı. CBCT CT ile karşılaştırıldığında yüksek çözünürlüklü görüntüleme, potansiyel olarak daha düşük radyasyon dozu ve daha düşük maliyetler gibi avantajlar sayılmaktadır. Öte yandan, CBCT’nin dağınık radyasyon da dahil olmak üzere, X-ray detektörlerinin sınırlı dinamik aralığı ve kemik yoğunluğu ile doğrusal bir korelasyonu olmayan yoğunluk değerlerinin olması gibi birçok dezavantajlarıda vardır. Sınırlı kontrastlı çözünürlülük, düşük kontrastlı CBCT görüntülerini tanımlanabilirliği açısından olumsuzluk oluşturmaya devam etmektedir. Fakat, son yıllarda yapılan çalışmalarda CBCT kemik yoğunluk değerleri ve kesitli CT’den alınmış Hounsfield birimi(HU) arasında anlamlı bir korelasyon bildirilmiştir. Yeni yapılmış bir çalışmada, Sonuç olarak, CBCT görüntülerinden alınmış kemik yoğunluğu değerleri tartışmalıdır. Literatürde, CBCT kullanarak tahmin edilen kemik yoğunluğu ve primer implant stabilitesi arasındaki korelasyonla ilgili çalışmalar sınırlıdır. Thus, pre-operative evaluation of bone quality is necessary for treatment success. Dental implants have been a popular alternative in the dental rehabilitation after the introduction of osseointegration. The success and survival of dental implants have become increasingly important along with the rapid increase in the number of dental implants installed. The success of dental implant treatment is influenced by both the quality and quantity of available bone for implant placement. Studies have shown higher failure rates for implants placed in bone of poor quality and quantity. Hence, a precise evaluation of bone structure before surgery is crucial during treatment planning. Bone quality is a key factor to take into account when predicting implant stability. A good stability favors implant osseointegration. Thus, several classification systems and procedures have been defined for assessing bone quality. Implant stability can be predicted subjectively by surgeon’s assumption about bone quality during bone drilling procedure before implant placement. Implant stability can also be evaluated objectively by the insertion torque test and resonance frequency analysis (RFA) after implant placement. All of these methods provide information about bone quality during implant surgery. However; predictable techniques, which correlate with the methods used during surgery, are necessary to predict implant stability before surgery. Thus, the concept of using CT, as a more objective method for preoperative quantitative assessment of bone density, has had increasing popularity during the past few years. A number of studies have shown the relationship between bone density on CT and primary implant stability. However, patients were exposed to high radiation during CT scanning. Recently, cone beam CT (CBCT) technique, specifically designed for head and neck imaging, was introduced. The advantages of CBCT are imaging with high resolution, potentially lower radiation dose, and reduced costs compared with CT. On the other hand, there are several disadvantages of CBCT including scattered radiation, the limited dynamic range of the X-ray detectors, and density values without a linear correlation with bone density. Limited contrast resolution continues to impair low-contrast detectability in CBCT images. However, significant correlations between the density values of CBCT and Hounsfield unit (HU) of multislice CT were also reported in recent studies. Consequently, deriving bone density values from CBCT images seems controversial. In the literature, there are only limited number of studies about the correlation between bone density estimated by CBCT and primary implant stability. İmplantların maksiller sinüse yer değiştirmesi genellikle hatalı cerrahi planlama veya yetersiz cerrahi teknik ile ilgilidir. Dental implantlar sıklıkla sinüslerde yabancı cisim reaksiyonlarına neden olur ve sinüs komplikasyonlarının gelişmesinden kaçınmak için çıkarılmalıdır. Bu komplikasyonları önleyebilmek için, preoperatif dikkatli klinik ve radyolojik değerlendirmeler ile anatomik oluşumlarla ilgili cerrahi öngörü doğru bir şekilde yapılmalı ve atravmatik çalışılmalıdır. Comparison of piezoelectric bone surgery and traditional rotary instruments in impacted third molar surgery
Amaç: Bu çalışmanın amacı, gömülü alt üçüncü molar operasyonunda kullanılan piezoelektrik cerrahisinin ve döner enstrümanların operasyon sonrası enflamatuar yanıt üzerindeki etkilerini değerlendirmek ve karşılaştırmaktır. Bireyler ve Yöntem: Çalışmaya, simetrik olarak gömülü mandibular üçüncü molar dişlere sahip 26 sağlıklı hasta dahil edildi. Birinci operasyonda, bukkal ostetomi rond frez ile gerçekleştirildi (Frez grubu). İkinci operasyonda, kemik kaldırmak için piezoelektrik cerrahi tekniği kullanıldı (Piezo grubu). Cerrahi işlem süresi, ağrı, trismus ve yüzdeki şişlik değerlendirildi. Bulgular: Ortalama cerrahi işlem süresi piezo grubunda anlamlı olarak daha uzun bulundu (p=0,003). Operasyon sonrası ikinci günde, dinlenme (p=0,005) ve çiğneme (p<0,001) anındaki ortalama VAS ağrı skorları ve operasyon sonrası yedinci günde çiğneme anındaki (p<0,001) ortalama VAS ağrı skoru frez grubunda anlamlı olarak daha yüksek tespit edildi. Operasyon sonrası yedinci günde, piezo grubunda alınan analjezik tabletlerin sayısı frez grubundan anlamlı olarak daha az sayıdaydı (p=0,013). Frez grubunda, ortalama yüz şişliği operasyon sonrası ikinci (p<0,001) ve yedinci (p=0,003) günlerde piezo grubundan anlamlı olarak daha fazla tespit edildi. Operasyon sonrası ikinci (p<0,001) ve yedinci (p<0,001) günlerde frez grubunda trismus anlamlı olarak daha yüksek bulundu. Sonuç: Bu çalışmada elde edilen bulgulara göre, piezoelektrik kemik cerrahisi gömülü üçüncü molar diş operasyonu sonrası morbiditeyi azaltmak ve iyileşmeyi hızlandırmak için kullanılabilecek alternatif bir yöntemdir.Purpose: The aim of this study was to evaluate and compare the effects of piezoelectric surgery and rotary instruments on the inflammatory response in the impacted lower third molar surgery. Patients and Methods: Twenty-six healthy patients with symmetrically impacted mandibular third molars were included. In the first operation, buccal osteotomy was performed with a round burr (Rotary group). In the second operation, piezoelectric surgery was used for bone removal (Piezo group). Duration of surgery, pain, trismus and cheek swelling were evaluated. Results: The mean duration of surgery was significantly longer in the piezo group (p=0.003). On the second postoperative day, the mean VAS pain scores during rest (p=0.005) and chewing (<0.001) and on the seventh postoperative day during chewing (<0.001) were significantly higher in the rotary group. On the seventh postoperative day, the number of analgesic tablets taken in the piezo group was significantly lower than the rotary group (p=0.013). The mean facial swelling in the rotary group was significantly higher than the piezo group on the second postoperative day (<0.001) and on the seventh postoperative day (p=0.003). On the second (<0.001) and seventh postoperative days (<0.001) trismus was significantly higher in the rotary group. Conclusion: According to the findings of the present study, piezoelectric bone surgery is an alternative method to reduce morbidity and to improve recovery after impacted third molar surgery
Contemporary Approaches in the Repair of Alveolar Clefts
Cleft lip and palate is one of the most common craniofacial anomalies. The repair of the alveolar clefts is an important part of the treatment for patients with cleft lip and palate. The treatment concepts of alveolar bone grafting are still controversial. The corresponding controversial issues are; timing of alveolar bone grafting, graft materials, and timing of the orthodontic expansion. In the present article, aforementioned controversial issues and contemporary treatment modalities of the maxillary alveolar clefts were reviewed in the light of current literature. In conclusion, the most suitable time for alveolar bone grafting is mixed dentition period. Grafting procedure may be performed in the early or late phases of this period depending on some clinical features. Adjunct orthodontic expansion procedures should be performed before and/or after grafting depending on the patient's current features. [Archives Medical Review Journal 2014; 23(4.000): 563-574
