Artificial Neural Network Symbol Estimator With Enhanced Robustness to Nonlinear Phase Noise
This letter reports a novel approach for nonlinear phase noise mitigation, based on artificial neural networks (ANNs) tailored to classification applications and a pre-processing stage of feature engineering. Starting with a set of proof-of-concept simulations, we verify that the proposed system can achieve optimal performance for the additive white Gaussian noise (AWGN) channel. Then, considering a dispersion-less channel with strong nonlinear phase noise (NLPN) distortion, we demonstrate a Q-factor increase of 0.4dB, comparing with standard carrier-phase estimation (CPE) followed by minimum distance detection. Finally, simulating the propagation of 64Gbaud PM-16QAM over standard single mode fiber (SSMF), we verify that the ANN-based solution is effective on wavelength-division multiplexing (WDM) transmission conditions, enabling to increase the maximum signal reach by approximately 1 fiber span over the legacy CPE-enabled NLPN compensation
“Bone-shot fracture” – An unusual iliac wing fracture caused by a projectile of autologous bone fragment. A case report
Case: A young adult male sustained a high-energy crash suffering multiple injuries including a comminuted right femoral shaft fracture and an ipsilateral iliac wing fracture. The iliac fracture was caused by a femoral fragment which was projected and pierced the iliac wing. The patient underwent surgery with retrieval of the femoral fragment and fixation of the iliac and femoral fractures. The lesions healed uneventfully. Conclusion: This is the first reported case of an iliac fracture caused by a projectile of autologous bone. High-energy trauma may present unusual or never seen injury patterns to the trauma surgeon
Individuals with type 2 diabetes have higher density of small intestinal neurotensin-expressing cells
Neurotensin (NT) is a gastro-intestinal hormone involved in several pathways that regulate energy and glucose homeostasis. NT was hypothesized to act in synergy with incretin hormones to potentiate its anti-diabetic effects. Additionally, circulating NT levels were shown to rise after bariatric surgery-induced weight loss. Knowledge of NT-secreting cells distribution along the small intestine and its variation according to diabetes status could provide insights on NT role in mediating type 2 diabetes (T2D) improvement after bariatric surgery. So, our aims were to characterize NT-expressing cell distribution along the human small intestine and to compare the relative density of NT-expressing cells in the small intestine of individuals with and without T2D undergoing bariatric surgery for obesity treatment. Autopsy-derived small intestine fragments (n = 30) were obtained at every 20 cm along the entire intestinal length. Additionally, jejunum biopsies (n = 29) were obtained during elective gastric bypass interventions from patients with (n = 10) or without T2D (n = 18). NT-expressing cells were identified by immunohistochemistry and quantified via computerized morphometric analysis. NT-expressing cell density increased along the human small intestine. NT-expressing cell density was significantly higher from 200 cm distal to the duodenojejunal flexure onward, as well as in subjects with T2D when compared to those without T2D. NT-expressing cell density increases along the human small gut, and a higher density is found in individuals with T2D. This finding suggests a potential role for NT in the mechanisms of disease and T2D improvement observed after bariatric surgery. (c) 2023, The Author(s)
The transmission of Leishmania infantum chagasi by the bite of the Lutzomyia longipalpis to two different vertebrates
<p>Abstract</p> <p>Background</p> <p>Sandflies are vectors of <it>Leishmania</it>, the causative agent of leishmaniasis in mammalian hosts, including humans. The protozoan parasite is transmitted by the sandfly bite during salivation that occurs at the moment of blood feeding. The components of vector saliva include anticlotting and vasodilatory factors that facilitate blood flow and immunomodulatory factors that inhibit wound healing and quell the immune response. Not surprisingly, these factors also play important roles in the establishment of <it>Leishmania </it>infection. To date, the majority of knowledge that has been generated regarding the process of <it>Leishmania </it>infection, including <it>L. infantum chagasi </it>transmission has been gathered by using intradermal or subcutaneous inoculation of purified parasites.</p> <p>Findings</p> <p>This study presents the establishment of a transmission model of <it>Leishmania infantum chagasi </it>by the bite of <it>Lutzomyia longipalpis</it>, the vector of American visceral leishmaniasis. The parasites were successfully transmitted by infected sandfly bites to mice and hamsters, indicating that both animals are good experimental models. The <it>L. infantum chagasi </it>dose that was transmitted in each single bite ranged from 10 to 10, 000 parasites, but 75% of the sandflies transmitted less than 300 parasites.</p> <p>Conclusions</p> <p>The strategy for initiating infection by sandfly bite of experimental animals facilitates future investigations into the complex and dynamic mechanisms of visceral leishmaniasis. It is important to elucidate the transmission mechanism of vector bites. This model represents a useful tool to study <it>L. infantum chagasi </it>infection transmitted by the vector.</p
A cross-sectional study of vascular risk factors in a rural South African population : data from the Southern African Stroke Prevention Initiative (SASPI)
Background: Rural sub-Saharan Africa is at an early stage of economic and health transition. It is
predicted that the 21st century will see a serious added economic burden from non-communicable disease
including vascular disease in low-income countries as they progress through the transition. The stage of
vascular disease in a population is thought to result from the prevalence of vascular risk factors. Already
hypertension and stroke are common in adults in sub-Saharan Africa. Using a multidisciplinary approach
we aimed to assess the prevalence of several vascular risk factors in Agincourt, a rural demographic
surveillance site in South Africa.
Methods: We performed a cross sectional random sample survey of adults aged over 35 in Agincourt
(population ≈ 70 000). Participants were visited at home by a trained nurse who administered a
questionnaire, carried out clinical measurements and took a blood sample. From this we assessed
participants' history of vascular risk, blood pressure using an OMRON 705 CP monitor, waist
circumference, body mass index (BMI), ankle brachial index (ABI), and total and HDL cholesterol.
Results: 402 people (24% men) participated. There was a high prevalence of smoking in men, but the
number of cigarettes smoked was small. There was a striking difference in mean BMI between men and
women (22.8 kg/m2 versus 27.2 kg/m2), but levels of blood pressure were very similar. 43% of participants
had a blood pressure greater than 140/90 or were on anti-hypertensive treatment and 37% of participants
identified with measured high blood pressure were on pharmacological treatment. 12% of participants had
an ABI of < 0.9, sugesting the presence of sub-clinical atheroma. 25.6% of participants had a total
cholesterol level > 5 mmol/l.
Conclusion: We found a high prevalence of hypertension, obesity in women, and a suggestion of
subclinical atheroma despite relatively favourable cholesterol levels in a rural South African population.
South Africa is facing the challenge of an emerging epidemic of vascular disease. Research to establish the
social determinates of these risk factors and interventions to reduce both individual and population risk
are required

Demonstration of the event identification capabilities of the NEXT-White detector
In experiments searching for neutrinoless double-beta decay, the possibility of identifying the two emitted electrons is a powerful tool in rejecting background events and therefore improving the overall sensitivity of the experiment. In this paper we present the first measurement of the efficiency of a cut based on the different event signatures of double and single electron tracks, using the data of the NEXT-White detector, the first detector of the NEXT experiment operating underground. Using a 228Th calibration source to produce signal-like and background-like events with energies near 1.6 MeV, a signal efficiency of 71.6 ± 1.5 stat± 0.3 sys% for a background acceptance of 20.6 ± 0.4 stat± 0.3 sys% is found, in good agreement with Monte Carlo simulations. An extrapolation to the energy region of the neutrinoless double beta decay by means of Monte Carlo simulations is also carried out, and the results obtained show an improvement in background rejection over those obtained at lower energies. Radiogenic backgrounds in the NEXT double beta decay experiment
Natural radioactivity represents one of the main backgrounds in the search for neutrinoless double beta decay. Within the NEXT physics program, the radioactivity- induced backgrounds are measured with the NEXT-White detector. Data from 37.9 days of low-background operations at the Laboratorio Subterráneo de Canfranc with xenon depleted in 136Xe are analyzed to derive a total background rate of (0.84±0.02) mHz above 1000 keV. The comparison of data samples with and without the use of the radon abatement system demonstrates that the contribution of airborne-Rn is negligible. A radiogenic background model is built upon the extensive radiopurity screening campaign conducted by the NEXT collaboration. A spectral fit to this model yields the specific contributions of 60Co, 40K, 214Bi and 208Tl to the total background rate, as well as their location in the detector volumes. The results are used to evaluate the impact of the radiogenic backgrounds in the double beta decay analyses, after the application of topological cuts that reduce the total rate to (0.25±0.01) mHz. Based on the best-fit background model, the NEXT-White median sensitivity to the two-neutrino double beta decay is found to be 3.5σ after 1 year of data taking. The background measurement in a Qββ±100 keV energy window validates the best-fit background model also for the neutrinoless double beta decay search with NEXT-100. Only one event is found, while the model expectation is (0.75±0.12) events. [Figure not available: see fulltext.]
