    The role of concomitant vitamin D3 and K2 treatment after laminectomy in spinal stenosis surgery of patients with osteoporosis

    Background: Readmission because of ongoing pain and complaints who undergone spinal surgery with osteoporosis is increasing. This study investigated the efficacy of concomitant vitamin D3 and K2 usage in the surgical treatment of patients with lumbar spinal stenosis who have osteoporosis. The aim of the study was to treatment strategy is to reduce the patient’s low back pain and decrease readmission rates due to ongoing pain. Methods: A total of 22 patients with osteoporosis included in the study who had lumbar decompression via laminectomy surgery with posterior instrumentation for spinal stenosis. Patients were divided into groups according to use additional supplement of vitamin D3+K2. Group 1 was control group who received only vitamin D3 and group 2 was patients who received vitamin D3+K2 treatment. Patients were called to their sixth month check-up and their pre-and post-treatment VAS scores and Roland-Morris scores were filled in and readmission rates due to ongoing pain were compared. Results: 9 male and 13 female participants with a mean age of 64.4 were in the study. The mean T-score in bone mineral density (BMD) was -2.7 in group 1 and -2.6 in group 2. A significant difference was found when the pre-operative and post-operative VAS and RM scores of all patients were compared (p<0.05). The mean readmission rates due to ongoing pain is 4 in group 1 and 7 in group 2. A significant difference was found when readmission rates due to ongoing pain were compared (p<0.05). Conclusions: Concomitant vitamin D3 and K2 usage in the surgical treatment of patients with lumbar spinal stenosis who have osteoporosis is a valuable treatment option to reduce the rate of admission to the hospital because of ongoing pain

    Penelitian bertujuan  untuk mengetahui  pengaruh Nilai Tukar terhadap Pergerakan Indeks Saham Syariah Indonesia (ISSI); pengaruh Sertifikat Bank Indonesia Syariah (SBIS) terhadap Pergerakan Indeks Saham Syariah Indonesia (ISSI); pengaruh Sertifikat Bank Indonesia (SBI) terhadap Pergerakan Indeks Saham Syariah Indonesia (ISSI), dan pengaruh Penawaran Uang (M2) terhadap Pergerakan Indeks Saham Syariah Indonesia (ISSI). Metode analisa data yang digunakan adalah Analisa VAR dengan menggunakan software Eviews 5.0. Data diperoleh selama periode Januari 2016 sampai Januari 2019. Hasil penelitian menunjukkan bahwa Nilai Tukar berpengaruh terhadap pergerakan Indeks Saham Syariah Indonesia (ISSI) dan mendorong menurunnya Indeks Saham Syariah Indonesia (ISSI). Sertifikat Bank Indonesia Syariah (SBIS) berpengaruh terhadap pergerakan Indeks Saham Syariah Indonesia (ISSI) dan mendorong menurunnya Indeks Saham Syariah Indonesia (ISSI).     Analysis of Longitudinal Data in Craniofacial Research: Some Strategies

    Although it is generally acknowledged that longitudinal data provide the most information on growth and development and other time-dependent phenomena, such data are often analyzed by conventional (cross-sectional) statistical methods. This widespread practice ignores the distinctive characteristics (e.g., covariance structure) of longitudinal data and may yield misleading results. The purpose of this article is to present some strategies and make available computer programs for the appropriate analysis of longitudinal data. User-friendly PC programs for the estimation of average growth curves, computation of tracking indices, prediction of future values, diagnosis, classification, clustering, estimation of missing values, and testing hypotheses concerning individual and group differences are presented.     The influence of N-acetyl-L-cystein infusion on cytokine levels and gastric intramucosal pH during severe sepsis

    INTRODUCTION: The purpose of the present study was to evaluate the effects of continuously infused N-acetyl-L-cystein (NAC) on serum cytokine levels and gastric intramucosal pH in humans suffering from severe sepsis. METHODS: Fifty-three patients were included in the study. In the NAC group (n = 27), after an initial intravenous bolus of NAC (150 mg/kg over 5 min), a continuous intravenous infusion of 12.5 mg/kg per hour was given for 6 hours. Patients in the control group (n = 26) were administered dextrose (5% solution) at the same dosage. We recorded the following: haemodynamic parameters, nasopharyngeal temperature, arterial blood gas changes, plasma cytokine levels, biochemical parameters, intramucosal pH, length of stay in the intensive care unit, duration of of mechanical ventilation and mortality. All measurements were taken at baseline (15 min before the start of the study) and were repeated immediately after the bolus infusion, and at 24 and 48 hours after initiation of the continuous NAC infusion. RESULTS: No differences were found between groups in levels of the major cytokines, duration of ventilation and intensive care unit stay, gastric intramucosal pH and arterial oxygen tension/inspired fractional oxygen ratio (P > 0.05). CONCLUSION: We found that NAC infusion at the doses given did not affect cytokine levels, outcomes, or gastric intramucosal pH in patients with severe sepsis. Because of the limited number of patients included in the study and the short period of observation, our findings need confirmation in larger clinical trials of NAC infused in a dose-titrated manner. However, our results do not support the use of NAC in patients with severe sepsis

    An analysis of the activation of the adipogenic differentiation pathway in the bone marrow stroma by members of the nuclear hormone receptor family.

    The bone marrow stroma is composed of a diverse array of cell types that have unique functions. Of the different types, the adipocyte is the most abundant. It is not clear what purpose adipocytes serve in the bone marrow. They may simply occupy space in the marrow cavity or they may play active roles in systemic lipid metabolism and provide an energy reservoir in the bone marrow. Marrow adipocytes also appear to contribute growth factors and cytokines that directly promote hematopoiesis and influence osteogenesis. The enzyme lipoprotein lipase (LPL) is an early marker of adipogenesis and its regulation is complex. It is the enzyme responsible for the hydrolysis of triglycerides into free fatty acids and the clearance of chylomicrons from the blood. It is regulated by a variety of transcription factors. The nuclear hormone receptor superfamily consists of an immense number of genes and is accepted as the largest transcription factor family in eukaryotes. We determine that peroxisome proliferator activated receptors (PPARs) with their ligands induce adipogenesis in bone marrow stromal cells and that they bind and activate the LPL gene promoter in vitro. The chicken ovalbumin upstream promoter transcription factors (Coup-TFs) are orphan receptors that bind certain DNA direct repeats as homodimers and heterodimers with other steroid receptor molecules such as the retinoid X receptor (RXR) and are accepted as repressors of other nuclear hormone receptors. We determine that Coup-TFs bind the LPL promoter in vitro and demonstrate in cotransfection analysis that they act in concert with the PPARγ\gamma2 and RXRα\alpha proteins to multiplicatively activate its transcription. Recently, new regulatory proteins have been described that affect the interaction of nuclear hormone receptors with the transcriptional apparatus. We determine that the co-regulators of nuclear hormone receptors SMRT (silencing mediator for retinoid and thyroid-hormone receptors) and steroid receptor coactivator (SRC-1) help regulate the LPL promoter in conjunction with PPARγ\gamma2, RXRα\alpha and Coup-TFII (ARP-1) in negative and positive fashion respectively. Improved understanding of the mechanisms regulating stromal adipocyte differentiation may lead to therapeutic interventions that can enhance osteogenesis and hematopoiesis in afflicted individuals

    Diagnostic traits of neurosyphilis among people living with HIV

Neurosyphilis is the infection of the central nervous system by Treponema pallidum which can occur at any stage of the syphlis (1). Taylor et al. reported a 2.1% incidence of neurosyphilis among population with HIV compared to 0.6% in people without HIV (1,2). HIV coinfection more often predispose T.pallidum towards neuroinvasion. The presentations of neurosyphilis includes meningitis, meningovascular disease, cranial nerve involvement, general paresis and tabes dorsales (1,3). In contemporary literature its emphasized that HIV coinfection increases the rate of early neurosyphilis with more severe course and higher risk of serological failure. Aim of the study. In present study, we aimed to identify clinical and laboratory features of neurosyphilis presentation among people living with HIV/AIDS. Materials and methods. Retrospective study was carried out in the cohort of patients with HIV/AIDS referred to the inpatient department of Hospital of Dermatology and Communicable Diseases in Chisinau, Republic of Moldova. Results. Since 2017 and up to 2020, a total number of 8 patients with HIV/AIDS manifesting neurosyphilis were determined. 5 out of 8 patients developed early forms of nervous system alteration provoked by Treponema pallidum (T.pallidum) invasion, including meningovascular ischemic presentation and optic nerve involvement. 3 patients were diagnosed with late form of neurosyphilis – general paresis. Rapid plasma reagins (RPR) non-treponemal test was extremely positive in serum of all patients with neurosyphilis enrolled in study, especially among those who manifested the early forms of the disease with titers raged from 1:32 to 1:64. Treponemal tests (TPHA and ELISA) were both positive in patient’s blood. In cerebrospinal fluid (CSF) serological reactions for T.pallidum presented differently: RPR test was positive in early neurosyphilis and respectively negative results were seen among patients with late forms. Treponemal tests in CSF showed peculiar results too, TPHA was positive in all cases together with high levels of anti-treponemal immunoglobulins G (IgG) assessed by immunoenzymatic assay (EIA). Presence of anti-treponemal immunoglobulins M, in patients CSF, were determined in 2 out of 8 patients who manifested early forms of disease and in 1 with general paresis. CD4+ cells count ranged from 36 up to 200 cells/ml. All the patients received antibiotic therapy via intravenous administration. Just 2 out of 8 patients enrolled in this study were diagnosed with clinical and serological cure of neurosyphilis, 2 patients have died, another 4 manifested only partial recover. Conclusions. Evolution of neurosyphilis among patients with HIV depends on the degree of immune system dysfunction together with anatomical sites of brain alteration (meningea, blood vessels and cranial nerves or brain parenchima). Duration of T.pallidum neuroinvasion, initiation of antibiotic therapy along with neuroprotective medication are either important
