    Evolução temporal na prevalência de asma e rinoconjuntivite em adolescentes

    RESUMO OBJETIVO Analisar a tendência temporal da prevalência de asma e de rinoconjuntivite e seus sintomas em adolescentes. MÉTODOS Foram realizados dois estudos transversais utilizando a mesma metodologia e o mesmo questionário em adolescentes de 12 a 14 anos em Florianópolis, SC. Fundamentado no protocolo internacional do estudo International Study of Asthma and Allergies in Childhood (ISAAC), foram avaliados 4.114 adolescentes em 2001 e 3.150 em 2012. As escolas estudadas foram as mesmas de estudo anterior de 2001, selecionadas aleatoriamente após estratificação por rede (pública e privada) e por localização geográfica. A variação percentual anual média total foi estimada para a prevalência de asma e seus sintomas e para rinoconjuntivite. RESULTADOS A prevalência de relato de asma foi de 10,9% em 2001 e de 14,8% em 2012, com variação média no período de 2,8%. A maior variação média no período ocorreu entre os adolescentes do sexo feminino (4,1%). Houve aumento significativo no relato de diagnóstico médico de asma, de 7,3% em 2001 para 11,1% em 2012, com variação anual de 4,5%. Os maiores aumentos no relato de diagnóstico médico de asma ocorreram em alunos do sexo feminino (5,9%) e masculino (4,5%) da rede pública. Houve incremento expressivo no relato de rinoconjuntivite, com variação média no período de 5,2%. O relato de sintomas de asma grave permaneceu inalterado e houve decréscimo na variação anual no período no relato de sibilos atuais (-1,3%) e de sibilos aos exercícios (-1,2%). CONCLUSÕES Os resultados mostraram incremento significativo na variação anual média da prevalência de asma e de rinoconjuntivite no período de 2001 a 2012.ABSTRACT OBJECTIVE To analyze the temporal trend of asthma and rhinoconjunctivitis prevalences as well as their symptoms in adolescents. The highest average variation in the period was observed among female adolescents (4.1%). In parallel a significant increase occurred in reported physician-diagnosed asthma, 7.3% in 2001 and 11,1% in 2012, with an annual variation of 4.5%. The largest increases in reported physician-diagnosed asthma were seen in female (5.9%) and male (4.5%) public school pupils. In addition, a significant increase in reported rhinoconjunctivitis occurred, with the average variation in the period being 5.2%. Reports of severe asthma symptoms remained unchanged during the period, while the annual variation for reported current wheezing (-1.3%) and wheezing during exercise (-1.2%) decreased. CONCLUSIONS The results showed a significant increase in the annual average variation for asthma and rhinoconjunctivitis prevalence during the 2001 to 2012 period

    O objetivo deste estudo foi medir a prevalência, a gravidade dos sintomas de asma e investigar fatores de risco associados entre a presença de chiado nos últimos 12 meses (asma atual) e fatores pessoais, geográficos e sócio-econômicos em adolescentes escolares de 13 e 14 anos matriculados em sétimas e oitavas séries dos municípios de Tubarão e Capivari de Baixo, Santa Catarina, Brasil. Este é um estudo transversal que utilizou o questionário ISAAC (International Study of Asthma and Allergies in Childhood), aplicado em 1.870 adolescentes de 42 escolas dos dois municípios, entre os meses de março a maio de 2005. A prevalência de asma atual foi de 11,8% e de asma "alguma vez na vida" foi 7,8%. A prevalência de despertar uma ou mais noites na semana foi de 2,1%; 8,1% tiveram 1-3 crises de chiado nos últimos 12 meses e 3,7% apresentaram limitação da fala. Na análise multivariada sexo feminino, história familiar, pai e mãe com asma, moradia alugada e fumante dentro de casa foram fatores de risco para asma atual. A prevalência de asma atual e a gravidade dos sintomas foram comparativamente menores em relação ao encontrado em outros estudos brasileiros

    Effects of prednisone on eosinophilic bronchitis in asthma: a systematic review and meta-analysis,

    OBJECTIVE: To evaluate the effect size of oral corticosteroid treatment on eosinophilic bronchitis in asthma, through systematic review and meta-analysis. METHODS: We systematically reviewed articles in the Medline, Cochrane Controlled Trials Register, EMBASE, and LILACS databases. We selected studies meeting the following criteria: comparing at least two groups or time points (prednisone vs. control, prednisone vs. another drug, or pre- vs. post-treatment with prednisone); and evaluating parameters before and after prednisone use, including values for sputum eosinophils, sputum eosinophil cationic protein (ECP), and sputum IL-5-with or without values for post-bronchodilator FEV1-with corresponding 95% CIs or with sufficient data for calculation. The independent variables were the use, dose, and duration of prednisone treatment. The outcomes evaluated were sputum eosinophils, IL-5, and ECP, as well as post-bronchodilator FEV1. RESULTS: The pooled analysis of the pre- vs. post-treatment data revealed a significant mean reduction in sputum eosinophils (&#8595;8.18%; 95% CI: 7.69-8.67; p < 0.001), sputum IL-5 (&#8595;83.64 pg/mL; 95% CI: 52.45-114.83; p < 0.001), and sputum ECP (&#8595;267.60 &#181;g/L; 95% CI: 244.57-290.63; p < 0.0001), as well as a significant mean increase in post-bronchodilator FEV1 (&#8593;8.09%; 95% CI: 5.35-10.83; p < 0.001). CONCLUSIONS: In patients with moderate-to-severe eosinophilic bronchitis, treatment with prednisone caused a significant reduction in sputum eosinophil counts, as well as in the sputum levels of IL-5 and ECP. This reduction in the inflammatory response was accompanied by a significant increase in post-bronchodilator FEV1

    Translation and cultural adaptation of a specific instrument for measuring asthma control and asthma status: the Asthma Control and Communication Instrument

    ABSTRACT Objective: To translate the Asthma Control and Communication Instrument (ACCI) to Portuguese and adapt it for use in Brazil. Methods: The ACCI was translated to Portuguese and adapted for use in Brazil in accordance with internationally accepted guidelines. The protocol included the following steps: permission and rights of use granted by the original author; translation of the ACCI from English to Portuguese; reconciliation; back-translation; review and harmonization of the back-translation; approval from the original author; review of the Portuguese version of the ACCI by an expert panel; cognitive debriefing (the clarity, understandability, and acceptability of the translated version being tested in a sample of the target population); and reconciliation and preparation of the final version. Results: During the cognitive debriefing process, 41 asthma patients meeting the inclusion criteria completed the ACCI and evaluated the clarity of the questions/statements. The clarity index for all ACCI items was > 0.9, meaning that all items were considered to be clear. Conclusions: The ACCI was successfully translated to Portuguese and culturally adapted for use in Brazil, the translated version maintaining the psychometric properties of the original version. The ACCI can be used in clinical practice because it is easy to understand and easily applied
