Effect of flavonoids of extract of cichorium intybus L. leaf on induction of P19 stem-cells differentiation to insulin-producing cells
Background and purpose: Regarding the effective role of plant flavonoids in the prevention and treatment of diabetes mellitus, in this investigation, induction of P19 stem cells differentiation into insulin producing cells by flavonoids of methanol extract of Cichorium intybus L. leaf was examined. Materials and methods: Initially, flavonoids of methanol extract of Cichorium intybus L. leaf were concentrated. The P19-derived embryonic bodies were cultured in α-MEM (Minimum Essential Medium) containing 15 fetal bovine serum for three days. To induce the differentiation, the cells treated with the concentrations of 50, 100 and 200 μg/ml of flavonoids of methanol extract of Cichorium intybus L. leaf for 12 days. Untreated P19 cells and embryonic bodies were used as controls. Dithizone staining, immunoflorecence and real-time polymerase chain reaction (RT-PCR) techniques were used to demonstrate the differentiated cells. Results: Differentiated P19 cells by different concentrations of extract shown positive reaction to dithizone staining. Most percentage of positive dithizone cells was reached in concentration of 100 μg/ml. Immunoflorecence method showed that differentiated P19 cells were able to express the pancreas beta cell-specific markers. Pdx-1 gene expression in the cells was demonstrated by RT-PCR technique. Conclusion: The flavonoids of methanol extract of Cichorium intybus L. leaf are able to induce differentiation of P19 stem cells to insulin producing cells. © 2014 Mazandaran University of Medical Sciences. The eClinical Care Pathway Framework: A novel structure for creation of online complex clinical care pathways and its application in the management of sexually transmitted infections.
Despite considerable international eHealth impetus, there is no guidance on the development of online clinical care pathways. Advances in diagnostics now enable self-testing with home diagnosis, to which comprehensive online clinical care could be linked, facilitating completely self-directed, remote care. We describe a new framework for developing complex online clinical care pathways and its application to clinical management of people with genital chlamydia infection, the commonest sexually transmitted infection (STI) in England.Using the existing evidence-base, guidelines and examples from contemporary clinical practice, we developed the eClinical Care Pathway Framework, a nine-step iterative process. Step 1: define the aims of the online pathway; Step 2: define the functional units; Step 3: draft the clinical consultation; Step 4: expert review; Step 5: cognitive testing; Step 6: user-centred interface testing; Step 7: specification development; Step 8: software testing, usability testing and further comprehension testing; Step 9: piloting. We then applied the Framework to create a chlamydia online clinical care pathway (Online Chlamydia Pathway).Use of the Framework elucidated content and structure of the care pathway and identified the need for significant changes in sequences of care (Traditional: history, diagnosis, information versus Online: diagnosis, information, history) and prescribing safety assessment. The Framework met the needs of complex STI management and enabled development of a multi-faceted, fully-automated consultation.The Framework provides a comprehensive structure on which complex online care pathways such as those needed for STI management, which involve clinical services, public health surveillance functions and third party (sexual partner) management, can be developed to meet national clinical and public health standards. Evaluating the Effects of Dual Frequency Sonication Parameters on Acoustic Cavitation by Chemical Dosimeter Using Iodide
Abstract:
Background and Aims: Production of acoustic cavitation by sonication has been recently recommended as a targeted treatment. The experimental results from studies indicate that the activity of cavitation generated by bi- or multi-frequency ultrasound irradiation is higher than that caused by single frequency irradiation. In this study, effects dual (1 MHz and 40 kHz) and single frequency sonication on acoustic cavitation were investigated by chemical dosimeter using iodide.
Methods: In this study, we investigated the exposure parameters in low level dual-frequency ultrasound in near fields of 40 kHz and 1 MHz in the progressive wave mode by chemical dosimeter using iodide. Finally, the effective protocol for enhancement of the inertial cavitation activity was determined.
Results: It has been shown that the dosimeter absorbance in the continuous mode sonication is more than the pulse mode. Moreover, significant correlations were found between the time and intensity of single and dual frequency sonication and the iodide dosimeter absorbance. In constant energy, inertial cavitation activity in the pulse mode remained approximately constant. Among different combinations, the combined ultrasound sonication in continuous mode could produce a more remarkable enhancement of the inertial cavitation activity (1.78 times) than the algebraic sum.
Conclusion: Simultaneous combined dual frequency sonication is more effective than single frequency sonication in producing the inertial cavitation activity.

A generic system for critiquing physicians' prescriptions: usability, satisfaction and lessons learnt.
Clinical decision support systems have been developed to help physicians to take clinical guidelines into account during consultations. The ASTI critiquing module is one such systems; it provides the physician with automatic criticisms when a drug prescription does not follow the guidelines. It was initially developed for hypertension and type 2 diabetes, but is designed to be generic enough for application to all chronic diseases. We present here the results of usability and satisfaction evaluations for the ASTI critiquing module, obtained with GPs for a newly implemented guideline concerning dyslipaemia, and we discuss the lessons learnt and the difficulties encountered when building a generic DSS for critiquing physicians' prescriptions
Understanding transport mechanisms in ionic liquid/carbonate solvent electrolyte blends
To unravel mechanistic details of the ion transport in liquid electrolytes, blends of the ionic liquid (IL) 1-butyl-1-methylpyrrolidinium bis(trifluoromethylsulfonyl)imide (Pyr14TFSI), ethylene carbonate (EC) and dimethyl carbonate (DMC) with the conducting salts lithium hexafluorophosphate (LiPF6) and lithium bis(trifluoromethylsulfonyl)imide (LiTFSI) were investigated as a function of the IL concentration. Electrochemical impedance, Pulsed Field Gradient Nuclear Magnetic Resonance (PFG NMR) and Raman spectroscopy supported by Molecular Dynamics (MD) simulations allowed the structural and dynamic correlations of the ion motions to be probed. Remarkably, we identified that though the individual correlations among different ion types exhibit a clear concentration dependence, their net effect is nearly constant throughout the entire concentration range, resulting in approximately equal transport and transference numbers, despite a monitored cross-over from carbonate-based lithium coordination to a TFSI-based ion coordination. In addition, though dynamical ion correlation could be found, the absolute values of the ionic conductivity are essentially determined by the overall viscosity of the electrolyte. The IL/carbonate blends with a Pyr14TFSI fraction of ∼10 wt% are found to be promising electrolyte solvents, with ionic conductivities and lithium ion transference numbers comparable to those of standard carbonate-based electrolytes while the thermal and electrochemical stabilities are considerably improved. Global attitudes in the management of acute appendicitis during COVID-19 pandemic: ACIE Appy Study
676sinoneBackground: Surgical strategies are being adapted to face the COVID-19 pandemic. Recommendations on the management of acute appendicitis have been based on expert opinion, but very little evidence is available. This study addressed that dearth with a snapshot of worldwide approaches to appendicitis. Methods: The Association of Italian Surgeons in Europe designed an online survey to assess the current attitude of surgeons globally regarding the management of patients with acute appendicitis during the pandemic. Questions were divided into baseline information, hospital organization and screening, personal protective equipment, management and surgical approach, and patient presentation before versus during the pandemic. Results: Of 744 answers, 709 (from 66 countries) were complete and were included in the analysis. Most hospitals were treating both patients with and those without COVID. There was variation in screening indications and modality used, with chest X-ray plus molecular testing (PCR) being the commonest (19·8 per cent). Conservative management of complicated and uncomplicated appendicitis was used by 6·6 and 2·4 per cent respectively before, but 23·7 and 5·3 per cent, during the pandemic (both P < 0·001). One-third changed their approach from laparoscopic to open surgery owing to the popular (but evidence-lacking) advice from expert groups during the initial phase of the pandemic. No agreement on how to filter surgical smoke plume during laparoscopy was identified. There was an overall reduction in the number of patients admitted with appendicitis and one-third felt that patients who did present had more severe appendicitis than they usually observe. Conclusion: Conservative management of mild appendicitis has been possible during the pandemic.
