    A 3D computed tomography based tool for orthopedic surgery planning

    Series : Lecture notes in computational vision and biomechanics, vol. 19The preparation of a plan is essential for a surgery to take place in the best way possible and also for shortening patient’s recovery times. In the orthopedic case, planning has an accentuated significance due to the close relation between the degree of success of the surgery and the patient recovering time. It is important that surgeons are provided with tools that help them in the planning task, in order to make it more reliable and less time consuming. In this paper, we present a 3D Computed Tomography based solution and its implementation as an OsiriX plugin for orthopedic surgery planning. With the developed plugin, the surgeon is able to manipulate a three-dimensional isosurface rendered from the selected imaging study (a CT scan). It is possible to add digital representations of physical implants (surgical templates), in order to evaluate the feasibility of a plan. These templates are STL files generated from CAD models. There is also the feature to extract new isosurfaces of different voxel values and slice the final 3D model according to a predefined plane, enabling a 2D analysis of the planned solution. Finally, we discuss how the proposed application assists the surgeon in the planning process in an alternative way, where it is possible to three-dimensionally analyze the impact of a surgical intervention on the patient.(undefined

    Changes in bone marrow lesions in response to weight-loss in obese knee osteoarthritis patients: a prospective cohort study

    BACKGROUND: Patients are susceptible for knee osteoarthritis (KOA) with increasing age and obesity and KOA is expected to become a major disabling disease in the future. An important feature of KOA on magnetic resonance imaging (MRI) is changes in the subchondral bone, bone marrow lesions (BMLs), which are related to the future degeneration of the knee joint as well as prevalent clinical symptoms. The aim of this study was to investigate the changes in BMLs after a 16-week weight-loss period in obese subjects with KOA and relate changes in BMLs to the effects of weight-loss on clinical symptoms. METHODS: This prospective cohort study included patients with a body mass index ≥ 30 kg/m(2), an age ≥ 50 years and primary KOA. Patients underwent a 16 weeks supervised diet program which included formula products and dietetic counselling (ClinicalTrials.gov: NCT00655941). BMLs in tibia and femur were assessed on MRI before and after the weight-loss using the Boston-Leeds Osteoarthritis Knee Score. Response to weight-loss in BML scores was dichotomised to patients experiencing a decrease in BML scores (responders) and patients who did not (non-responders). The association of BMLs to weight-loss was assessed by logistic regressions and correlation analyses. RESULTS: 39 patients (23%) were classified as responders in the sum of all BML size scores whereas 130 patients (77%) deteriorated or remained stable and were categorized as non-responders. Logistic regression analyses revealed no association between weight-loss < or ≥ 10% and response in BMLs in the most affected compartment (OR 1.86 [CI 0.66 to 5.26, p=0.24]). There was no association between weight-loss and response in maximum BML score (OR 1.13 [CI 0.39 to 3.28, p=0.81]). The relationship between changes in BMLs and clinical symptoms revealed that an equal proportion of patients classified as BML responders and non-responders experienced an OMERACT-OARSI response (69 vs. 71%, p=0.86). CONCLUSIONS: Weight-loss did not improve the sum of tibiofemoral BML size scores or the maximum tibiofemoral BML score, suggesting that BMLs do not respond to a rapidly decreased body weight. The missing relationship between clinical symptoms and BMLs calls for further investigation

    Circulating microRNAs as potential diagnostic biomarkers for osteoporosis

    Osteoporosis is the most common age-related bone disease worldwide and is usually clinically asymptomatic until the first fracture happens. MicroRNAs are critical molecular regulators in bone remodelling processes and are stabilised in the blood. The aim of this project was to identify circulatory microRNAs associated with osteoporosis using advanced PCR arrays initially and the identified differentially-expressed microRNAs were validated in clinical samples using RT-qPCR. A total of 161participants were recruited and 139 participants were included in this study with local ethical approvals prior to recruitment. RNAs were extracted, purified, quantified and analysed from all serum and plasma samples. Differentially-expressed miRNAs were identified using miRNA PCR arrays initially and validated in 139 serum and 134 plasma clinical samples using RT-qPCR. Following validation of identified miRNAs in individual clinical samples using RT-qPCR, circulating miRNAs, hsa-miR-122-5p and hsa-miR-4516 were statistically significantly differentially-expressed between non-osteoporotic controls, osteopaenia and osteoporosis patients. Further analysis showed that the levels of these microRNAs were associated with fragility fracture and correlated with the low bone mineral density in osteoporosis patients. The results show that circulating hsa-miR-122-5p and hsa-miR-4516 could be potential diagnostic biomarkers for osteoporosis in the future

    Intention Seekers: Conspiracist Ideation and Biased Attributions of Intentionality

    Conspiracist beliefs are widespread and potentially hazardous. A growing body of research suggests that cognitive biases may play a role in endorsement of conspiracy theories. The current research examines the novel hypothesis that individuals who are biased towards inferring intentional explanations for ambiguous actions are more likely to endorse conspiracy theories, which portray events as the exclusive product of intentional agency. Study 1 replicated a previously observed relationship between conspiracist ideation and individual differences in anthropomorphisation. Studies 2 and 3 report a relationship between conspiracism and inferences of intentionality for imagined ambiguous events. Additionally, Study 3 again found conspiracist ideation to be predicted by individual differences in anthropomorphism. Contrary to expectations, however, the relationship was not mediated by the intentionality bias. The findings are discussed in terms of a domain-general intentionality bias making conspiracy theories appear particularly plausible. Alternative explanations are suggested for the association between conspiracism and anthropomorphism

    O objetivo do presente ensaio é o levantamento de polêmicas sobre a educação como um todo e o conhecimento científico como alicerces importantes na formação acadêmica. Acreditando que a busca incessante dos cursos de pós-graduação não se deveria restringir às teses como mera formalização, questiona-se a dinâmica metodológica do conhecimento, bem como verdade e saber. A condição humana é introduzida como um tema que faz um viés com a era dita pós-moderna, discutindo a liquidez e efemeridades humanas. A complexidade e interdisciplinaridade foram questionadas como um novo formato de trabalho. A abordagem da pesquisa qualitativa correlaciona-se com a questão humana e existencial. Além de outros tópicos a respeito de educação e pesquisa tais como bioética, viabilidade da pesquisa desinteressada, produção massificante e teses bem direcionadas, quisemos mostrar o quanto o processo de educação e conhecimento podem formar eventos para que o aluno se torne sujeito, um indivíduo com autocrítica e com postura dialética.<br>The objective of the present article is primordially to rise controversies about the education as a whole and the scientific knowledge as important foundations in the academic formation. Believing that the incessant search of the masters degree courses should not limit to the theories as a mere academic education. The methodological dynamics of the knowledge is questioned, as well as truth and knowledge. The human condition is introduced as a theme that has a tendency to the post-modern era, discussing the liquidity and human fragileness. The complexity and interdisciplinarity were questioned. The approach of the qualitative research was correlated to the human and existential issue. Besides other topics regarding education and research such as bioethics, viability of the uninterested research, mass production and theories well addressed, we aimed at showing that the education process and knowledge can create events so that the student becomes the subject, an individual with self-criticism and with dialectics posture
