    Normal pregnancy is associated with an increase in thrombin generation from the very early stages of the first trimester

    Background: Pregnancy is a hypercoagulable state associated with an increased risk of venous thrombosis, which begins during the first trimester, but the exact time of onset is unknown. Thrombin generation, a laboratory marker of thrombosis risk, increases during normal pregnancy but it is unclear exactly how early this increase occurs. Methods: We assessed thrombin generation by Calibrated Automated Thrombography in women undergoing natural cycle in vitro fertilization, who subsequently gave birth at term following a normal pregnancy (n = 22). Blood samples were taken just prior to conception and repeated five times during very early pregnancy, up to Day 59 estimated gestation. Results: Mean Endogenous Thrombin Potential (ETP), peak thrombin generation and Velocity Index (VI) increased significantly from pre-pregnancy to Day 43 gestation (p = 0.024–0.0004). This change persisted to Day 59 gestation. The mean of the percentage change from baseline, accounting for inter-individual variation, in ETP, peak thrombin and VI increased significantly from pre-pregnancy to Day 32 gestation (p = 0.0351–<0.0001) with the mean increase from baseline persisting to Day 59 gestation. Conclusion: Thrombin generation increases significantly during the very early stages of normal pregnancy when compared to the pre-pregnancy state. The increased risk of venous thrombosis therefore likely begins very early in a woman's pregnancy, suggesting that women considered clinically to be at high thrombotic risk should start thromboprophylaxis as early as possible after a positive pregnancy test

    Simulation and Modeling of Return Waveforms from a Ladar Beam Footprint in USU LadarSIM

    Ladar systems are an emerging technology with applications in many fields. Consequently, simulations for these systems have become a valuable tool in the improvement of existing systems and the development of new ones. This paper discusses the theory and issues involved in reliably modeling the return waveform of a ladar beam footprint in the Utah State University LadarSIM simulation software. Emphasis is placed on modeling system-level effects that allow an investigation of engineering tradeoffs in preliminary designs, and validation of behaviors in fabricated designs. Efforts have been made to decrease the necessary computation time while still maintaining a usable model. A full waveform simulation is implemented that models optical signals received on detector followed by electronic signals and discriminators commonly encountered in contemporary direct-detection ladar systems. Waveforms are modeled using a novel hexagonal sampling process applied across the ladar beam footprint. Each sample is weighted using a Gaussian spatial profile for a well formed laser footprint. Model fidelity is also improved by using a bidirectional reflectance distribution function (BRDF) for target reflectance. Once photons are converted to electrons, waveform processing is used to detect first, last or multiple return pulses. The detection methods discussed in this paper are a threshold detection method, a constant fraction method, and a derivative zero-crossing method. Various detection phenomena, such as range error, walk error, drop outs and false alarms, can be studied using these detection methods

    Large scale prop-fan structural design study. Volume 1: Initial concepts

    In recent years, considerable attention has been directed toward improving aircraft fuel consumption. Studies have shown that the inherent efficiency advantage that turboprop propulsion systems have demonstrated at lower cruise speeds may now be extended to the higher speeds of today's turbofan and turbojet-powered aircraft. To achieve this goal, new propeller designs will require features such as thin, high speed airfoils and aerodynamic sweep, features currently found only in wing designs for high speed aircraft. This is Volume 1 of a 2 volume study to establish structural concepts for such advanced propeller blades, to define their structural properties, to identify any new design, analysis, or fabrication techniques which were required, and to determine the structural tradeoffs involved with several blade shapes selected primarily on the basis of aero/acoustic design considerations. The feasibility of fabricating and testing dynamically scaled models of these blades for aeroelastic testing was also established. The preliminary design of a blade suitable for flight use in a testbed advanced turboprop was conducted and is described in Volume 2

    A gender-sensitised weight loss and healthy living programme for overweight and obese men delivered by Scottish Premier League football clubs (FFIT): a pragmatic randomised controlled trial.

    BACKGROUND: The prevalence of male obesity is increasing but few men take part in weight loss programmes. We assessed the effect of a weight loss and healthy living programme on weight loss in football (soccer) fans. METHODS: We did a two-group, pragmatic, randomised controlled trial of 747 male football fans aged 35-65 years with a body-mass index (BMI) of 28 kg/m(2) or higher from 13 Scottish professional football clubs. Participants were randomly assigned with SAS (version 9·2, block size 2-9) in a 1:1 ratio, stratified by club, to a weight loss programme delivered by community coaching staff in 12 sessions held every week. The intervention group started a weight loss programme within 3 weeks, and the comparison group were put on a 12 month waiting list. All participants received a weight management booklet. Primary outcome was mean difference in weight loss between groups at 12 months, expressed as absolute weight and a percentage of their baseline weight. Primary outcome assessment was masked. Analyses were based on intention to treat. The trial is registered with Current Controlled Trials, number ISRCTN32677491. FINDINGS: 374 men were allocated to the intervention group and 374 to the comparison group. 333 (89%) of the intervention group and 355 (95%) of the comparison group completed 12 month assessments. At 12 months the mean difference in weight loss between groups, adjusted for baseline weight and club, was 4·94 kg (95% CI 3·95-5·94) and percentage weight loss, similarly adjusted, was 4·36% (3·64-5·08), both in favour of the intervention (p<0·0001). Eight serious adverse events were reported, five in the intervention group (lost consciousness due to drugs for pre-existing angina, gallbladder removal, hospital admission with suspected heart attack, ruptured gut, and ruptured Achilles tendon) and three in the comparison group (transient ischaemic attack, and two deaths). Of these, two adverse events were reported as related to participation in the programme (gallbladder removal and ruptured Achilles tendon). INTERPRETATION: The FFIT programme can help a large proportion of men to lose a clinically important amount of weight; it offers one effective strategy to challenge male obesity.     Structural Transition Kinetics and Activated Behavior in the Superconducting Vortex Lattice

    Using small-angle neutron scattering, we investigated the behavior of a metastable vortex lattice state in MgB2 as it is driven towards equilibrium by an AC magnetic field. This shows an activated behavior, where the AC field amplitude and cycle count are equivalent to, respectively, an effective "temperature" and "time". The activation barrier increases as the metastable state is suppressed, corresponding to an aging of the vortex lattice. Furthermore, we find a cross-over from a partial to a complete suppression of metastable domains depending on the AC field amplitude, which may empirically be described by a single free parameter. This represents a novel kind of collective vortex behavior, most likely governed by the nucleation and growth of equilibrium vortex lattice domains.Comment: 5 pages plus 3 pages of supplemental materia

    Helicopter tail rotor thrust and main rotor wake coupling in crosswind flight

    The tail rotor of a helicopter with a single main rotor configuration can experience a significant reduction in thrust when the aircraft operates in crosswind flight. Brown’s vorticity transport model has been used to simulate a main rotor and tail rotor system translating at a sideslip angle that causes the tail rotor to interact with the main rotor tip vortices as they propagate downstream at the lateral extremities of the wake. The tail rotor is shown to exhibit a distinct directionally dependent mode during which tail rotors that are configured so that the blades travel forward at the top of the disk develop less thrust than tail rotors with the reverse sense of rotation. The range of flight speeds over which this mode exists is shown to vary considerably with the vertical location of the tail rotor. At low flight speeds, the directionally dependent mode occurs because the tail rotor is immersed within not only the downwash from the main rotor but also the rotational flow associated with clusters of largely disorganized vorticity within the main rotor wake. At higher flight speeds, however, the tail rotor is immersed within a coherent supervortex that strongly influences the velocity field surrounding the tail rotor

    Changes in laboratory markers of thrombotic risk early in the first trimester of pregnancy may be linked to an increase in estradiol and progesterone

    Background: Pregnant women are at increased risk of venous thrombosis compared to non-pregnant women. Epidemiological and laboratory data suggest that hypercoagulability begins in the first trimester but it is unknown exactly how early in pregnancy this develops. The mechanisms that result in a prothrombotic state may involve oestrogens and progestogens. Methods: Plasma samples were taken prior to conception and five times in early pregnancy, up to Day 59 gestation, from 22 women undergoing natural cycle in vitro fertilization, who subsequently gave birth at term following a normal pregnancy. Thrombin generation, free Protein S, Ddimer, Fibrinogen, factor VIII, estradiol and progesterone were measured. To counter inter-individual variability, the change in laboratory measurements between the pre-pregnant and pregnant state were measured over time. Results: Peak thrombin, Endogenous Thrombin Potential, Velocity Index and fibrinogen significantly increased, and free Protein S significantly decreased, from pre-pregnancy levels, by 32 days gestation. Ddimer and VIII significantly increased from pre-pregnancy levels by 59 days gestation. Estradiol significantly increased by Day 32 gestation with a non-significant increase of 67% by Day 24 gestation. Progesterone significantly increased by Day 32 gestation. Almost all laboratory markers of thrombosis correlated significantly with estradiol and progesterone. Conclusion: Our work is the first to demonstrate that the prothrombotic state develops very early in the first trimester. Laboratory markers of hypercoagulability correlate significantly with estradiol and progesterone suggesting these are linked to the prothrombotic state of pregnancy. Clinicians should consider commencing thromboprophylaxis early in the first trimester in women at high thrombotic risk

    Topological energy barrier for skyrmion lattice formation in MnSi

    We report the direct measurement of the topological skyrmion energy barrier through a hysteresis of the skyrmion lattice in the chiral magnet MnSi. Measurements were made using small-angle neutron scattering with a custom-built resistive coil to allow for high-precision minor hysteresis loops. The experimental data was analyzed using an adapted Preisach model to quantify the energy barrier for skyrmion formation and corroborated by the minimum-energy path analysis based on atomistic spin simulations. We reveal that the skyrmion lattice in MnSi forms from the conical phase progressively in small domains, each of which consisting of hundreds of skyrmions, and with an activation barrier of several eV.Comment: Final accepted versio

    Review: Opportunities and challenges for the genetic selection of dairy calf disease traits.

    Interest in dairy cow health continues to grow as we better understand health's relationship with production potential and animal welfare. Over the past decade, efforts have been made to incorporate health traits into national genetic evaluations. However, they have focused on the mature cow, with calf health largely being neglected. Diarrhoea and respiratory disease comprise the main illnesses with regard to calf health. Conventional methods to control calf disease involve early separation of calves from the dam and housing calves individually. However, public concern regarding these methods, and growing evidence that these methods may negatively impact calf development, mean the dairy industry may move away from these practices. Genetic selection may be a promising tool to address these major disease issues. In this review, we examined current literature for enhancing calf health through genetics and discussed alternative approaches to improve calf health via the use of epidemiological modelling approaches, and the potential of indirectly selecting for improved calf health through improving colostrum quality. Heritability estimates on the observed scale for diarrhoea ranged from 0.03 to 0.20, while for respiratory disease, estimates ranged from 0.02 to 0.24. The breadth in these ranges is due, at least in part, to differences in disease prevalence, population structure, data editing and models, as well as data collection practices, which should be all considered when comparing literature values. Incorporation of epidemiological theory into quantitative genetics provides an opportunity to better determine the level of genetic variation in disease traits, as it accounts for disease transmission among contemporaries. Colostrum intake is a major determinant of whether a calf develops either respiratory disease or diarrhoea. Colostrum traits have the advantage of being measured and reported on a continuous scale, which removes the issues classically associated with binary disease traits. Overall, genetic selection for improved calf health is feasible. However, to ensure the maximum response, first steps by any industry members should focus efforts on standardising recording practices and encouragement of uploading information to genetic evaluation centres through herd management software, as high-quality phenotypes are the backbone of any successful breeding programme

    Influence of blade aerodynamic model on the prediction of helicopter high-frequency airloads

    Brown’s vorticity transport model has been used to investigate the influence of the blade aerodynamic model on the accuracy with which the high-frequency airloads associated with helicopter blade–vortex interactions can be predicted. The model yields an accurate representation of the wake structure yet allows significant flexibility in the way that the blade loading can be represented. A simple lifting-line model and a somewhat more sophisticated liftingchord model, based on unsteady thin aerofoil theory, are compared. A marked improvement in the accuracy of the predicted high-frequency airloads of the higher harmonic control aeroacoustic rotor is obtained when the liftingchord model is used instead of the lifting-line approach, and the quality of the prediction is affected less by the computational resolution of the wake. The lifting-line model overpredicts the amplitude of the lift response to blade–vortex interactions as the computational grid is refined, exposing the fundamental deficiencies in this approach when modeling the aerodynamic response of the blade to interactions with vortices that are much smaller than its chord. The airloads that are predicted using the lifting-chord model are relatively insensitive to the resolution of the computation, and there are fundamental reasons to believe that properly converged numerical solutions may be attainable using this approach
