    The impact of criminal justice involvement: examining experiences in a housing first program.

    Individuals who come into contact with the criminal justice system are more likely to experience housing instability and homelessness, which, in turn, increases the likelihood of subsequent criminal justice involvement. Due to a lack of federal funding, as well as disjointed and inconsistent policies regarding eligibility criteria, people with criminal records are unlikely to receive federal rental assistance. While the exclusion of people with criminal histories is presented as necessary to protect communities and residents from crime, improving access to stable housing may reduce recidivism, incarceration rates, and correctional costs and increase public safety. The present study examined the differences between individuals with and without criminal justice involvement who participated in a Housing First program in Louisville, Kentucky. Between 2008 and 2017, Family Health Centers-Phoenix Health Care for the Homeless enlisted individuals with a history of chronic homelessness and co-occurring substance abuse and/or mental health disorder in the Louisville Housing First Program (LHFP). The 368 who completed an intake interview, were placed in housing, and did not enter the program more than once were included in the analyses. Disparities in mental health and risky behavior were identified at earlier intervals of program exposure, but at 24 months, participants with criminal justice involvement at intake did not report more problems with mental health or risky behavior than their counterparts. Despite these improvements, participants with past criminal justice involvement were less likely to remain housed through LHFP and less likely to have a successful program outcome at 24 months compared to those without criminal histories. Criminal justice involvement at intake, social support, age, and education were significant predictors of housing outcomes at 24 months. Rather than indicators of individual attributes, critical race theory suggests that these variables are structural predictors that may reflect the inequality embedded in the institutions and structures of our society, namely the education and criminal justice systems and the economy. Disparities in the criminal justice system are not due to disproportionate engagement in criminal activity, which calls into question normative expectations of justice, neutrality, fairness, and culpability and suggests that criminal justice involvement likely reflects the prevailing values and shortcomings of our society, as opposed to the character of those targeted by these biased practices. Therefore, rather than excluding individuals from housing assistance and other opportunities on the basis of their criminal justice involvement, such histories should be considered indicators of need

    All Thresholds Barred: Direct Estimation of Call Density in Bioacoustic Data

    Passive acoustic monitoring (PAM) studies generate thousands of hours of audio, which may be used to monitor specific animal populations, conduct broad biodiversity surveys, detect threats such as poachers, and more. Machine learning classifiers for species identification are increasingly being used to process the vast amount of audio generated by bioacoustic surveys, expediting analysis and increasing the utility of PAM as a management tool. In common practice, a threshold is applied to classifier output scores, and scores above the threshold are aggregated into a detection count. The choice of threshold produces biased counts of vocalizations, which are subject to false positive/negative rates that may vary across subsets of the dataset. In this work, we advocate for directly estimating call density: The proportion of detection windows containing the target vocalization, regardless of classifier score. Our approach targets a desirable ecological estimator and provides a more rigorous grounding for identifying the core problems caused by distribution shifts -- when the defining characteristics of the data distribution change -- and designing strategies to mitigate them. We propose a validation scheme for estimating call density in a body of data and obtain, through Bayesian reasoning, probability distributions of confidence scores for both the positive and negative classes. We use these distributions to predict site-level densities, which may be subject to distribution shifts. We test our proposed methods on a real-world study of Hawaiian birds and provide simulation results leveraging existing fully annotated datasets, demonstrating robustness to variations in call density and classifier model quality.Comment: 14 pages, 6 figures, 3 tables; submitted to Frontiers in Bird Science; Our Hawaiian PAM dataset and classifier scores, as well as annotation information for the three study species, can be found on Zenodo at https://doi.org/10.5281/zenodo.10581530.     Unravelling morphoea aetiopathogenesis by next-generation sequencing of paired skin biopsies

    BACKGROUND: Morphoea can have a significant disease burden. Aetiopathogenesis remains poorly understood, with very limited existing genetic studies. Linear morphoea (LM) may follow Blascho's lines of epidermal development, providing potential pathogenic clues. OBJECTIVE: The first objective of this study was to identify the presence of primary somatic epidermal mosaicism in LM. The second objective was tTo explore differential gene expression in morphoea epidermis and dermis to identify potential pathogenic molecular pathways and tissue layer cross-talk. METHODOLOGY: Skin biopsies from paired affected and contralateral unaffected skin were taken from 16 patients with LM. Epidermis and dermis were isolated using a 2-step chemical-physical separation protocol. Whole Genome Sequencing (WGS; n = 4 epidermal) and RNA-seq (n = 5-epidermal, n = 5-dermal) with gene expression analysis via GSEA-MSigDBv6.3 and PANTHER-v14.1 pathway analyses, were performed. RTqPCR and immunohistochemistry were used to replicate key results. RESULTS: Sixteen participants (93.8% female, mean age 27.7 yrs disease-onset) were included. Epidermal WGS identified no single affected gene or SNV. However, many potential disease-relevant pathogenic variants were present, including ADAMTSL1 and ADAMTS16. A highly proliferative, inflammatory and profibrotic epidermis was seen, with significantly-overexpressed TNFα-via-NFkB, TGFβ, IL6/JAKSTAT and IFN-signaling, apoptosis, p53 and KRAS-responses. Upregulated IFI27 and downregulated LAMA4 potentially represent initiating epidermal 'damage' signals and enhanced epidermal-dermal communication. Morphoea dermis exhibited significant profibrotic, B-cell and IFN-signatures, and upregulated morphogenic patterning pathways such as Wnt. CONCLUSION: This study supports the absence of somatic epidermal mosaicism in LM, and identifies potential disease-driving epidermal mechanisms, epidermal-dermal interactions and disease-specific dermal differential-gene-expression in morphoea. We propose a potential molecular narrative for morphoea aetiopathogenesis which could help guide future targeted studies and therapies

    The human dilemma of displacement

    In this book social responsive theological research converges to provide practical theological and ecclesiological perspectives on the growing human dilemma of displacement. The book presents the research of practical theologians, a missiologist and a religious practitioner whose work pertains first and foremost to the (South) African context. The different fields of expertise of the contributors within the broader field of practical theology worked towards a unique compilation of themes, each relevant to the issue at stake. The majority of chapters are theoretically orientated, except where authors refer to empirical work conducted during previous research. The main contribution of this collaborative work is to be sought in the practical theological and ecclesiological perspectives it provides. It engages the critical questions of what kind of church we need, and what kind of care we should provide in the face of the growing predicament of human displacement. The theological and theoretical principles uncovered in the different chapters will be of use to theologians from all theological subdisciplines, as well as to religious practitioners and leaders of faith communities that are challenged with the growing realities of strangers on their doorsteps and in their pews

    Effects of superficial heating and insulation on walking speed in people with hereditary and spontaneous spastic paraparesis: A randomised crossover study.

    OBJECTIVES: Cooling of the lower limb in people with Hereditary and Spontaneous Spastic Paraparesis (pwHSSP) has been shown to affect walking speed and neuromuscular impairments. The investigation of practical strategies, which may help to alleviate these problems is important. The potential of superficial heat to improve walking speed has not been explored in pwHSSP. Primary objective was to explore whether the application of superficial heat (hot packs) to lower limbs in pwHSSP improves walking speed. Secondary objective was to explore whether wearing insulation after heating would prolong any benefits. METHODS: A randomised crossover study design with 21 pwHSSP. On two separate occasions two hot packs and an insulating wrap (Neo-G™) were applied for 30minutes to the lower limbs of pwHSSP. On one occasion the insulating wrap was maintained for a further 30minutes and on the other occasion it was removed. Measures of temperature (skin, room and core), walking speed (10 metre timed walk) and co-ordination (foot tap time) were taken at baseline (T1), after 30 mins (T2) and at one hour (T3). RESULTS: All 21 pwHSSP reported increased lower limb stiffness and decreased walking ability when their legs were cold. After thirty minutes of heating, improvements were seen in walking speed (12.2%, P0.001) in both conditions. CONCLUSIONS: Application of 30minutes superficial heating moderately improved walking speed in pwHSSP with effects maintained at 1hour. The use of hot packs applied to lower limbs should be the focus of further research for the clinical management of pwHSSP who report increased stiffness of limbs in cold weather and do not have sensory deficits

    BHPR research: qualitative1. Complex reasoning determines patients' perception of outcome following foot surgery in rheumatoid arhtritis

    Background: Foot surgery is common in patients with RA but research into surgical outcomes is limited and conceptually flawed as current outcome measures lack face validity: to date no one has asked patients what is important to them. This study aimed to determine which factors are important to patients when evaluating the success of foot surgery in RA Methods: Semi structured interviews of RA patients who had undergone foot surgery were conducted and transcribed verbatim. Thematic analysis of interviews was conducted to explore issues that were important to patients. Results: 11 RA patients (9 ♂, mean age 59, dis dur = 22yrs, mean of 3 yrs post op) with mixed experiences of foot surgery were interviewed. Patients interpreted outcome in respect to a multitude of factors, frequently positive change in one aspect contrasted with negative opinions about another. Overall, four major themes emerged. Function: Functional ability & participation in valued activities were very important to patients. Walking ability was a key concern but patients interpreted levels of activity in light of other aspects of their disease, reflecting on change in functional ability more than overall level. Positive feelings of improved mobility were often moderated by negative self perception ("I mean, I still walk like a waddling duck”). Appearance: Appearance was important to almost all patients but perhaps the most complex theme of all. Physical appearance, foot shape, and footwear were closely interlinked, yet patients saw these as distinct separate concepts. Patients need to legitimize these feelings was clear and they frequently entered into a defensive repertoire ("it's not cosmetic surgery; it's something that's more important than that, you know?”). Clinician opinion: Surgeons' post operative evaluation of the procedure was very influential. The impact of this appraisal continued to affect patients' lasting impression irrespective of how the outcome compared to their initial goals ("when he'd done it ... he said that hasn't worked as good as he'd wanted to ... but the pain has gone”). Pain: Whilst pain was important to almost all patients, it appeared to be less important than the other themes. Pain was predominately raised when it influenced other themes, such as function; many still felt the need to legitimize their foot pain in order for health professionals to take it seriously ("in the end I went to my GP because it had happened a few times and I went to an orthopaedic surgeon who was quite dismissive of it, it was like what are you complaining about”). Conclusions: Patients interpret the outcome of foot surgery using a multitude of interrelated factors, particularly functional ability, appearance and surgeons' appraisal of the procedure. While pain was often noted, this appeared less important than other factors in the overall outcome of the surgery.
