    Language-Based Data Sharing in Web Applications

    Cloud development and virtualization of applications is crucially becoming the common practice in the software engineering industry. Many systems and database tools are available to support applications with many instances and views, but all the orchestration of data and functionality in the so-called multi-tenant applications comes with a high development and maintenance cost. Due to the high costs of developing and maintaining such applications, there is an increasing need for languages and tools that support the gradual development of software for a highly shared environment, at the developer and user level. We extend a typed, reactive and incremental programming environment and language with parameterized modules that increase application modularity, with lenses that provide a (filtered) data sharing mechanism between modules, and the (dynamic) verification of module access conditions to implement data privacy. The combination of these mechanisms is a safe and powerful mechanism to design and evolve cloud and web applications. We present a pragmatic programming language supported by a deployed prototype where several examples of applications illustrate this new programming paradigm. We also provide a largerweb application example as a means of showing how the combination of the introduced mechanisms allows for the development of multi-tenant applications, and to compare it against implementations in modern frameworks

    A poda do olival é uma prática ancestral, ainda que a forma de intervir nas árvores tenha evoluído ao longo dos tempos e varie substancialmente entre diferentes regiões produtoras. Apesar da poda ser uma prática generalizada entre olivicultores nem sempre se sabe muito bem porque se poda e, frequentemente, discute- -se e duvida-se da melhor forma de podar. Tende ainda a avaliar-se a qualidade do podador pela estética com que deixa a árvore, tal como acontece com os podadores de árvores e arbustos ornamentais dos jardins públicos e privados. Aliás, a poda foi no passado um trabalho especializado, só executada por trabalhadores experimentados.     Quality of life in chronic rhinosinusitis with nasal polyposis

    Introduction and Objectives: The Sino-Nasal Outcome Test 22 (SNOT-22) questionnaire has been suggested as the best specific quality of life indicator for sinonasal illnesses. It was recently translated and validated in Portuguese. The objective of our study was to assess improvements after sinonasal endoscopic surgery in patients with chronic rhinosinusitis with nasal polyps via SNOT-22. Material and Methods: Every adult patients suffering from chronic rhinosinusitis with nasal polyps and monitored in consultation with the Otolaryngology department at our hospital and subjected to surgery between September 2012 and February 2014 answered to a questionnaire that included SNOT-22 on the day before surgery and 3 months after. SNOT-22 questions were divided into 4 sub-categories: rhinologic symptoms, facial/otological symptoms, sleep disorders and psychological disorders. For the statistic analysis of the results, the SPSS v22 program was used and a level of statistical significance of p<0.05 was established. Results: 100 patients were included. Total SNOT-22 scores were significantly higher during the pre-operating period (45) when compared to the post-operating period (17.5) (p<0.01). In the individual assessment of sub-categories, there was also a significant decrease after surgery (p<0.01). Discussion: Sinonasal endoscopic surgery improves the scores in the SNOT-22 questionnaire for those suffering from chronic rhinosinusitis with nasal polyps. Nevertheless, these patients report higher scores than individuals with no sinonasal illness. Conclusions: Sinonasal endoscopic surgery improves the quality of life of those patients suffering from chronic rhinosinusitis with nasal polyps.Introducción y Objetivo: El cuestionario Sino-Nasal Outcome Test 22 (SNOT-22), ha sido calificado como el mejor indicador específico de calidad de vida para la patología nasosinusal. Recientemente se ha traducido y se ha validado la versión en portugués. El objetivo de nuestro estudio fue evaluar, mediante el SNOT-22, la mejoría después de cirugía endoscópica nasosinusal, en pacientes con rinosinusitis crónica con poliposis nasal. Material y Método: Todos los pacientes adultos con rinosinusitis crónica con poliposis nasal seguidos en la consulta de otorrinolaringología de nuestro hospital y sometidos a cirugía entre septiembre de 2012 y febrero de 2014 respondieron a un cuestionario en el que se incluyó el SNOT-22, tanto el día anterior a la intervención como tres meses después de esta. Las preguntas del SNOT-22 se dividieron en cuatro subescalas: síntomas rinológicos, síntomas faciales/otológicos, alteraciones de sueño y alteraciones psicológicas. Para el análisis estadístico de los resultados se utilizó el programa SPSS v22 y se atribuyó un nivel de significado estadístico para p<0,05. Resultados: Se incluyeron 100 pacientes. Los resultados totales del SNOT-22 fueron significativamente superiores en el preoperatorio (45) si se comparan con los del postoperatorio (17,5) (p<0,01). En la evaluación individual de las subescalas también se observó una disminución significativa después de la cirugía (p<0,01). Discusión: La cirugía endoscópica nasosinusal permite mejorar los resultados en el cuestionario SNOT-22 en pacientes con rinosinusitis crónica con poliposis nasal. Aún así, estos pacientes presentan resultados más altos que los individuos sin patología nasosinusal.     Thermal contact resistance between mold steel and additively manufactured insert for designing conformal channels: an experimental study

    The focus of this research is on thermal contact resistance between a mold and its insert, specifically inserts made by additive manufacturing (AM). Using a thermal steady-state system and varying contact pressures (0, 50, 75, and 100 bars), we present experimental results of the thermal contact resistance at the contact interface between steel A (1.2344), obtained from an extruded rod, and steel B (1.2709), produced by laser powder bed fusion. Thermal tests were performed for unbonded and bonded configurations. Results showed that increasing the contact pressure allows the system equilibrium to be reached at lower temperatures. Furthermore, thermal tests showed that in the transition zone of the bonded configuration, a well-defined resistance is not formed between the two steel samples as it occurs in the unbonded configuration.     Foreign Body in the Eustachian Tube: A Challenging Diagnosis and Management

    Foreign bodies in the external ear are very common. The same cannot be said about foreign bodies in the Eustachian tube (ET). We report the case of a 63-year-old woman with a history of painless left side otorrhea and hearing loss. She reported a left ear surgery when she was 30-year-old but she did not know the diagnosis that was made at that time neither the kind of surgery performed. Otoscopic examination revealed an inferior perforation of the eardrum. Audiologic evaluation demonstrated a unilateral, moderate-severe mixed hearing loss. Computed tomography scan showed, in left ear, a soft tissue density filling the middle ear cavity and a foreign body in ET. The patient underwent middle ear exploration which required endoscopic assistance to visualize and remove the foreign body. It appeared to be a stapes prothesis of Robinson type. The displacement of a stapes prosthesis to the ET has not been reported in the literature. Surgeries in this region are challenging. This case highlights the importance of the integration of endoscopy into otologic surgery

    Vestibular Disorders after Stapedial Surgery in Patients with Otosclerosis

    Introduction and Objectives. Vertigo is a described complication of stapedial surgery. Many studies have been conducted to assess the improvement of hearing loss, but there are few studies that assess vestibular function after stapedial surgery. The aim of this study was to evaluate the presence and characterize the vertigo after stapedial surgery. Methods. We conducted a prospective observational study. Patients undergoing stapedial surgery in our hospital between October 2013 and December 2014 were invited to participate. The vertigo was assessed before and 4 months after surgery, using the Dizziness Handicap Inventory. Results. We included 140 patients in the study. 12 patients (8.6%) reported vertigo before surgery, and all of them denied vertigo after surgery. 36 patients (25.7%) reported vertigo four months after surgery, and none of them had vertigo before surgery. Postoperative total scores in patients with vertigo ranged between 2 and 18 points. Conclusion. The study shows that vestibular disorders may remain after the immediate postoperative period and reinforces the need for clarification of the patient in the informed consent act

    Does a three-degree hypoechogenicity grading improve ultrasound thyroid nodule risk stratification and affect the TI-RADS 4 category? A retrospective observational study

    ABSTRACT Objective: The aim of this study was to determine whether classifying hypoechogenicity in three degrees (mild, moderate, and marked) could improve the distinction between benign and malignant nodules and whether such an approach could influence Category 4 of the Thyroid Imaging Reporting and Data System (TI-RADS). Materials and methods: In total, 2,574 nodules submitted to fine needle aspiration, classified by the Bethesda System, were retrospectively assessed. Further, a subanalysis considering solid nodules without any additional suspicious findings (n = 565) was performed with the purpose of evaluating mainly TI-RADS 4 nodules. Results: Mild hypoechogenicity was significantly less related to malignancy (odds ratio [OR]: 1.409; CI: 1.086-1.829; p = 0.01), compared to moderate (OR: 4.775; CI: 3.700-6.163; p < 0.001) and marked hypoechogenicity (OR: 8.540; CI: 6.355-11.445; p < 0.001). In addition, mild hypoechogenicity (20.7%) and iso-hyperechogenicity (20.5%) presented a similar rate in the malignant sample. Regarding the subanalysis, no significant association was found between mildly hypoechoic solid nodules and cancer. Conclusion: Stratifying hypoechogenicity into three degrees influences the confidence in the assessment of the rate of malignancy, indicating that mild hypoechogenicity has a unique low-risk biological behavior that resembles iso-hyperechogenicity, but with minor malignant potential when compared to moderate and marked hypoechogenicity, with special influence on the TI-RADS 4 category
