    A prospective assessment of the inter-laboratory variability of biochemical markers of fibrosis (FibroTest) and activity (ActiTest) in patients with chronic liver disease

    BACKGROUND: Biochemical markers for liver fibrosis (FibroTest) and necroinflammatory features (ActiTest) are an alternative to liver biopsy in patients with chronic hepatitis C. Our aim was to assess the inter-laboratory variability of these tests, and their 6 components (γ-glutamyl transpeptidase, alanine aminotransferase, α(2)-macroglobulin, haptoglobin, apolipoprotein A1, and total bilirubin) and to identify factors associated with this variability. RESULTS: Serum of 24 patients with chronic hepatitis C or severe alcoholic liver disease were prospectively recorded and analyzed in one reference center and in 8 additional laboratories. When γ-glutamyl transpeptidase and alanine aminotransferase were expressed in international units, there was no significant difference between laboratories in the results of FibroTest or ActiTest; kappa statistics were greater than 0.50 with only 0.8% of cases (3/384) with a discordance of more than one stage. The main factor significantly associated with variability was the expression of γ-glutamyl transpeptidase and alanine aminotransferase, as multiples of upper limit of reference values. The use of standardized method with pyridoxal phosphate reduced the variability of alanine aminotransferase expression, and standardized original Szasz method reduced the variability of γ-glutamyl transpeptidase expression. CONCLUSIONS: The variability of FibroTest and ActiTest was acceptable without clinical consequences for the prediction of the stage of liver fibrosis and grade of activity. Standardized methods and assay calibration should be used and expression of alanine aminotransferase and γ-glutamyl transpeptidase in multiples of the upper limit of reference values should not be employed

    Adherence to antiretroviral therapy assessed by drug level monitoring and self-report in cameroon

    OBJECTIVES: To compare adherence to antiretroviral therapy using drug level monitoring and self-report and to explore the relation between these 2 methods and viral load measurements. METHODS: Sixty patients received a fixed-dose combination of nevirapine, stavudine, and lamivudine in a clinical study in Cameroon. Adherence was assessed every 6 months until month 36 by nevirapine minimal plasma concentration and self-report. Plasma HIV-1 viral load was determined at the same time. Analyses included 159 complete observations. RESULTS: The proportion of patients labeled as "adherent" was significantly lower using nevirapine monitoring (88.7%, 95% confidence interval [CI]: 82.7 to 93.2) than self-report (97.5%, CI: 93.7 to 99.3; P = 0.002). Virologic failure was associated with the nevirapine concentration (adjusted odds ratio [aOR] = 4.43; P = 0.018) but not with the self-reported adherence (aOR = 0.84; P = 0.9). As compared with the virologic outcome, the sensitivity of nevirapine level monitoring for predicting inadequate adherence was 20.5%, the specificity was 91.7%, the positive predictive value was 44.4%, and the negative predictive value was 78.0%. For self-report, the respective values were 2.6%, 97.5%, 25.0%, and 75.5%. CONCLUSIONS: Drug level monitoring provided a more reliable estimate of adherence than self-report. This method could be used in research settings. Operational research is required to define how to improve the accuracy of the self-report method because it is the most feasible method in clinical practice

    Digital Technology To Support Organic Growers ? Mesclun: A Web App To Help Designing Complex Organic Vegetable Production

    For organic vegetable growers, combining long rotations involving a high level of plant diversity with intercropping can bring economic and ecological benefits but often increase management complexity and workload. To support the decision making of farmers facing such challenges, the research-action objective of the MESCLUN programme is to develop a web application based on the innovative computer technologies of knowledge graphs and semantic web. In this French transdisciplinary project, we articulate methods and frameworks from different fields (agronomy, economy, design, knowledge and computer engineering) with expertise of agricultural practitioners (organic growers, advisors, teachers, organic farming students). Through an iterative and participatory approach based on co-innovation workshops in 4 contrasted regions of France, we design, develop and test web app prototypes to help farmers to appropriate systemic thinking, explore and assess their “own” solutions in the organisation of complex organic vegetables systems. We will present functionalities/interface of the first web app prototype. We will for example show how the web app can help growers to plan their crops in space and time considering contrasted fertility and plants health strategies as well as marketing requirements. We will also illustrate how different simulations can be assessed from a socio-economic perspective (workload and income). Based on those first results, we will examine the specificities, added value and blind spots of our web app compared to other decision making tools in the organic agricultural sector. To feed a more general debate, we will provide critical discussion points on the potentialities and limitations of innovative digital solutions to support decision making in complex organic farming systems

Эта состав- ляющая его стилистического потенциала обусловлена лингвистической природой неопределенной формы глагола, что и описывается в данной статье.Стаття присвячена опису певних особливостей функціонування в поетичному тексті інфінітивів і їх семантиці. Інфінітив розглядається як засіб створення неозначеності в поетичному тексті. Ця складова його стилістичного потенціалу обумовлена лінгвістичною природою неозначеної форми дієслова, що й описується в цій статті.The article is devoted to the description of some features of functioning in the poetic text of infinitives and their semantics. An infinitive is examined as a means of creation of uncertainty in a poetic text.     EUSEDcollab: a network of data from European catchments to monitor net soil erosion by water

    As a network of researchers we release an open-access database (EUSEDcollab) of water discharge and suspended sediment yield time series records collected in small to medium sized catchments in Europe. EUSEDcollab is compiled to overcome the scarcity of open-access data at relevant spatial scales for studies on runoff, soil loss by water erosion and sediment delivery. Multi-source measurement data from numerous researchers and institutions were harmonised into a common time series and metadata structure. Data reuse is facilitated through accompanying metadata descriptors providing background technical information for each monitoring station setup. Across ten European countries, EUSEDcollab covers over 1600 catchment years of data from 245 catchments at event (11 catchments), daily (22 catchments) and monthly (212 catchments) temporal resolution, and is unique in its focus on small to medium catchment drainage areas (median=43km2, min=0.04km2, max=817km2) with applicability for soil erosion research. We release this database with the aim of uniting people, knowledge and data through the European Union Soil Observatory (EUSO)

    Extensive Necrotic Purpura in Levamisole-Adulterated Cocaine Abuse - A Case Report

    International audienceLevamisole, which is used as an adulterated compound of cocaine, is currently being seen year after year in cocaine intoxication. For a few cases in the last decade, necrotic purpura and neutropenia after levamisole/cocaine intoxication have been described in the medical community. Herein, we present an original case of levamisole intoxication of a 40-year-old woman who smoked heroin and cocaine few during a month. She rapidly presented an extensive necrotic purpura of the nose, cheeks and extremities (lower and upper), and immunologic reactions (positive anti-MPO and anti-HNE). Levamisole was detected on hairs with ultra-high performance liquid chromatography and tandem mass spectrometry. The case reports also a probable cocaine supplier deceit, which bring pure drug for hospital investigation after the intoxication of his client. The intoxicated woman had survived with several skin and chronic pain complications. That case recalls the knowledge about levamisole with a short review of the forensic literature
