THU0334 LASER SPECKLE CONTRAST ANALYSIS FOR MEASUREMENT OF PERIPHERAL BLOOD PERFUSION IN SYSTEMIC SCLEROSIS PATIENTS: CAN IT PREDICT FUTURE ISCHEMIC DIGITAL TROPHIC LESIONS?
Background:Vasculopathy is a hallmark of systemic sclerosis (SSc). Laser speckle contrast analysis (LASCA) is a research tool to assess peripheral blood perfusion (PBP) (1). At this moment, its reliability has been attested in SSc patients, but its predictive value for future ischemic digital trophic lesions (DTL) is unknown (1).Objectives:To investigate in an unselected, prospective SSc cohort if baseline LASCA PBP measurements can discriminate between patients who will develop ischemic DTL (iDTL) and those who will not.Methods:Patients (fulfilling 2013 ACR/EULAR criteria and/or 2001 LeRoy and Medsger criteria) were recruited during the period of December 2017 to September 2018. LASCA was performed at baseline, in standardized conditions (1). Regions of interest (ROIs) (diameter 1 cm) were outlined at the 2nd-5thfingertip both volar and dorsal. The 'average PBP' of these ROIs was calculated (expressed in arbitrary perfusion units [PU]). A monthly telephone survey was conducted for 1 year to investigate DTL occurrence. DTL were considered 'ischemic' if not related to calcinosis. Logistic regression and ROC analysis were used to assess if average PBP is predictive of future iDTL.Results:Of the 106 patients with complete follow-up (92 women [86,8%]; 18 limited SSc [17,0%], 82 limited cutaneous SSc [77,4%], 6 diffuse cutaneous SSc [5,7%]), 29 patients (27,4%) had a DTL history. Forty-nine patients (46,2%) were on vasodilator therapy. Only 7 patients developed at least 1 iDTL during follow-up (6,6%) (Figure 1a). Performing univariate logistic regression (ULR), average PBP was not predictive for future iDTL (Table 1). Of note, analyzing only the patients not taking vasodilators, average PBP in the 'iDTL group' (n = 3) was median 46,8 PU (min. 45,6 - max. 68,8) vs. median 141,4 PU (min. 24,4 - max. 269,5) in the 'no iDTL group' (n = 54) (Figure 1b). Even though, the observations in the subgroup of patients not taking vasodilators deserve future investigation to assess whether low PBP values, as measured by LASCA, are associated with a higher iDTL incidence.References:[1]Cutolo M, Vanhaecke A, et al. The early life history of the clam <i>Macoma balthica</i> in a high CO<sub>2</sub> world
This study investigated the effects of experimentally manipulated seawater carbonate chemistry on several early life history processes of the Baltic tellin (Macoma balthica), a widely distributed bivalve that plays a critical role in the functioning of many coastal habitats. We demonstrate that ocean acidification significantly depresses fertilization, embryogenesis, larval development and survival during the pelagic phase. Fertilization and the formation of a D-shaped shell during embryogenesis were severely diminished: successful fertilization was reduced by 11% at a 0.6 pH unit decrease from present (pH 8.1) conditions, while hatching success was depressed by 34 and 87%, respectively at a 0.3 and 0.6 pH unit decrease. Under acidified conditions, larvae were still able to develop a shell during the post-embryonic phase, but higher larval mortality rates indicate that fewer larvae may metamorphose and settle in an acidified ocean. The cumulative impact of decreasing seawater pH on fertilization, embryogenesis and survival to the benthic stage is estimated to reduce the number of competent settlers by 38% for a 0.3 pH unit decrease, and by 89% for a 0.6 pH unit decrease from present conditions. Additionally, slower growth rates and a delayed metamorphosis at a smaller size were indicative for larvae developed under acidified conditions. This may further decline the recruit population size due to a longer subjection to perturbations, such as predation, during the pelagic phase. In general, early life history processes were most severely compromised at ,pH 7.5, which corresponds to seawater undersaturated with respect to aragonite. Since recent models predict a comparable decrease in pH in coastal waters in the near future, this study indicates that future populations of Macoma balthica are likely to decline as a consequence of ongoing ocean acidification
Extracellular electrical signals in a neuron-surface junction: model of heterogeneous membrane conductivity
Signals recorded from neurons with extracellular planar sensors have a wide
range of waveforms and amplitudes. This variety is a result of different
physical conditions affecting the ion currents through a cellular membrane. The
transmembrane currents are often considered by macroscopic membrane models as
essentially a homogeneous process. However, this assumption is doubtful, since
ions move through ion channels, which are scattered within the membrane.
Accounting for this fact, the present work proposes a theoretical model of
heterogeneous membrane conductivity. The model is based on the hypothesis that
both potential and charge are distributed inhomogeneously on the membrane
surface, concentrated near channel pores, as the direct consequence of the
inhomogeneous transmembrane current. A system of continuity equations having
non-stationary and quasi-stationary forms expresses this fact mathematically.
The present work performs mathematical analysis of the proposed equations,
following by the synthesis of the equivalent electric element of a
heterogeneous membrane current. This element is further used to construct a
model of the cell-surface electric junction in a form of the equivalent
electrical circuit. After that a study of how the heterogeneous membrane
conductivity affects parameters of the extracellular electrical signal is
performed. As the result it was found that variation of the passive
characteristics of the cell-surface junction, conductivity of the cleft and the
cleft height, could lead to different shapes of the extracellular signals
Some recommendations for applying gPC (generalized polynomial chaos) to modeling: An analysis through the Airy random differential equation
In this paper we study the use of the generalized polynomial chaos method to differential equations describing a model that depends on more than one random input. This random input can be in the form of parameters or of initial or boundary conditions. We investigate the effect of the choice of the probability density functions for the inputs on the output stochastic processes. The study is performed on the Airy¿s differential equation. This equation is a good test case since its solutions are highly oscillatory and errors can develop both in the amplitude and the phase. The effect of cigarette smoke exposure on the development of inflammation in lungs, gut and joints of TNFΔARE mice
The inflammatory cytokine TNF-alpha is a central mediator in many immune-mediated diseases, such as Crohn's disease (CD), spondyloarthritis (SpA) and chronic obstructive pulmonary disease (COPD). Epidemiologic studies have shown that cigarette smoking (CS) is a prominent common risk factor in these TNF-dependent diseases. We exposed TNF Delta ARE mice; in which a systemic TNF-alpha overexpression leads to the development of inflammation; to 2 or 4 weeks of air or CS. We investigated the effect of deregulated TNF expression on CS-induced pulmonary inflammation and the effect of CS exposure on the initiation and progression of gut and joint inflammation. Upon 2 weeks of CS exposure, inflammation in lungs of TNF Delta ARE mice was significantly aggravated. However, upon 4 weeks of CS-exposure, this aggravation was no longer observed. TNF Delta ARE mice have no increases in CD4+ and CD8+ T cells and a diminished neutrophil response in the lungs after 4 weeks of CS exposure. In the gut and joints of TNF Delta ARE mice, 2 or 4 weeks of CS exposure did not modulate the development of inflammation. In conclusion, CS exposure does not modulate gut and joint inflammation in TNF Delta ARE mice. The lung responses towards CS in TNF Delta ARE mice however depend on the duration of CS exposure
Computational analysis of mechanical stress–strain interaction of a bioresorbable scaffold with blood vessel

Absorb scaffold has an expansion rate lower than Xience V stent, with a less outer diameter achieved after
balloon deflation. Due to the difference in design and material properties, Absorb also shows a higher recoiling than Xience V, which suggests that additional post-dilatation is required to achieve
effective treatment for patients with calcified plaques and stiff vessels. However, Absorb scaffold induces significantly lower stresses on the artery-plaque system, which can be clinically beneficial. Eccentric plaque causes complications to stent deployment, especially non-uniform vessel expansion. Also the stress levels in the media and adventitia layers are considerably higher for the plaque with high eccentricity, for which the choice of stents, in terms of materials and designs, will be of paramount importance. Our results imply that the benefits of Absorb scaffolds are amplified
in these cases
Underwater noise levels in UK waters
Underwater noise from human activities appears to be rising, with ramifications for acoustically sensitive marine organisms and the functioning of marine ecosystems. Policymakers are beginning to address the risk of ecological impact, but are constrained by a lack of data on current and historic noise levels. Here, we present the first nationally coordinated effort to quantify underwater noise levels, in support of UK policy objectives under the EU Marine Strategy Framework Directive (MSFD). Field measurements were made during 2013-2014 at twelve sites around the UK. Median noise levels ranged from 81.5-95.5 dB re 1 μPa for one-third octave bands from 63-500 Hz. Noise exposure varied considerably, with little anthropogenic influence at the Celtic Sea site, to several North Sea sites with persistent vessel noise. Comparison of acoustic metrics found that the RMS level (conventionally used to represent the mean) was highly skewed by outliers, exceeding the 97 th percentile at some frequencies. We conclude that environmental indicators of anthropogenic noise should instead use percentiles, to ensure statistical robustness. Power analysis indicated that at least three decades of continuous monitoring would be required to detect trends of similar magnitude to historic rises in noise levels observed in the Northeast Pacific
Predictive value of laser speckle contrast analysis in systemic sclerosis. A systematic review and pilot study.
Objectives: To assess whether tools to functionally examine the microcirculation, such as laser speckle contrast analysis (LASCA), are predictive of ischaemic digital trophic lesions ([i]DTL) in patients with systemic sclerosis (SSc).
Methods: First, a systematic review (according to PRISMA) was conducted to identify studies describing a link between LASCA and SSc-related (i)DTL. In the additional pilot study, consecutive SSc patients underwent clinical and LASCA ex- aminations (to assess the peripheral blood perfusion [PBP] of both hands) at enrol- ment. For one year, a monthly telephone survey was conducted to investigate (i)DTL occurrence. Logistic regression and ROC analysis were performed.
Results: None of the three manuscripts retained through the systematic review ex- amined the predictive value of LASCA for future (i)DTL. In our pilot study, 7/106 (6.6%) SSc patients developed at least one iDTL during follow-up, with PBP not found to be predictive (OR = 0.995, p = .418; ROC–AUC = 0.597). Post hoc, when only patients not taking vasodilators were analysed (n = 57), all three who developed iDTL had an average PBP ≤ 70 PU, while only 9/54 (16.7%) patients without iDTL occurrence had such values.
Conclusion: A predictive role of LASCA for (i)DTL has not yet been described in the literature and could also not be attested by our additional pilot study, due to a lower-than-expected iDTL incidence in our day-to-day SSc population in which patients were allowed to continue their vasodilator medication. However, the prom- ising observations in the subgroup of vasodilator-naïve patients encourage further investigation of this potential added value of LASCA
