    Inter-rater agreement, sensitivity, and specificity of the prone hip extension test and active straight leg raise test

    BACKGROUND: Two clinical tests used to assess for neuromuscular control deficits in low back pain (LBP) patients are the prone hip extension (PHE) test and active straight leg raise (ASLR) test. For these tests, it has been suggested examiners classify patients as “positive” or “negative” based on the presence or absence (respectively) of specific “abnormal” lumbopelvic motion patterns. The inter-rater agreement of such a classification scheme has been reported for the PHE test, but not for the ASLR test. In addition, the sensitivity and specificity of such classification schemes have not been reported for either test. The primary objectives of the current study were to investigate: 1) the inter-rater agreement of the examiner-reported classification schemes for these two tests, and 2) the sensitivity and specificity of the classification schemes. METHODS: Thirty participants with LBP and 40 asymptomatic controls took part in this cross-sectional observational study. Participants performed 3–4 repetitions of each test whilst two examiners classified them as “positive” or “negative” based on the presence or absence (respectively) of specific “abnormal” lumbopelvic motion patterns. The inter-rater agreement (Kappa statistic), sensitivity (LBP patients), and specificity (controls) were calculated for each test. RESULTS: Both tests demonstrated substantial inter-rater agreement (PHE test: Kappa = 0.76, 95% CI = 0.57-0.95, p < 0.001; ASLR test: Kappa = 0.76, 95% CI = 0.57-0.96, p < 0.001). For the PHE test, the sensitivity was 0.18-0.27 and the specificity was 0.63-0.78; the odds ratio (OR) of “positive” classifications in the LBP group was 1.25 (95% CI = 0.58-2.72; Examiner 1) and 1.27 (95% CI = 0.52-3.12; Examiner 2). For the ASLR test, the sensitivity was 0.20-0.25 and the specificity was 0.84-0.86; the OR of “positive” classifications in the LBP group was 1.72 (95% CI = 0.75-3.95; Examiner 1) and 1.57 (95% CI = 0.64-3.85; Examiner 2). CONCLUSION: Classification schemes for the PHE test and ASLR test based on the presence or absence of specific “abnormal” lumbopelvic motion patterns demonstrated substantial inter-rater agreement. However, additional investigation is required to further comment on the clinical usefulness of the motion patterns demonstrated by LBP patients during these tests as a diagnostic tool or treatment outcome

    Molecular Carbon Chains and Rings in TMC-1

    We present mapping results in several rotational transitions of HC3N, C6H, both cyclic and linear C3H2 and C3H, towards the cyanopolyyne peak of the filamentary dense cloud TMC-1 using the IRAM 30m and MPIfR 100m telescopes. The spatial distribution of the cumulene carbon chain propadienylidene H2C3 (hereafter l-C3H2) is found to deviate significantly from the distributions of the cyclic isomer c-C3H2, HC3N, and C6H which in turn look very similar. The cyclic over linear abundance ratio of C3H2 increases by a factor of 3 across the filament, with a value of 28 at the cyanopolyyne peak. This abundance ratio is an order of magnitude larger than the range (3 to 5) we observed in the diffuse interstellar medium. The cyclic over linear abundance ratio of C3H also varies by ~2.5 in TMC-1, reaching a maximum value (13) close to the cyanopolyyne peak. These behaviors might be related to competitive processes between ion-neutral and neutral-neutral reactions for cyclic and linear species.Comment: Accepted for publication in The Astrophysical Journal, part I. 24 pages, including 4 tables, 7 figures, and figure caption

    The Use of Open Communal Grazing Designs to Screen Options for Grazing Management

    An open communal grazing design is described that enables a large number of grazing tactics to be concurrently evaluated in small plots under common grazing conditions. Pasture data indicated that the same level of utilisation occurred inside the experimental plots as in the surrounding field. However, differential grazing may occur where plots have divergent composition. The open communal design was economical using \u3c 5% of the land, livestock and fencing resources of alternative designs. The limitations of the open communal design as a research tool are also discussed

    The Analysis of Results from Paired Paddock Comparisons

    Paired-paddock comparisons are a common way of evaluating new grassland practices at a scale more relevant to farmers. They are also being used to replace or augment designed experiments and can be the only source of information available on a practice. However, it is often uncertain if the differences between paddocks are significant. Importantly, a current trend among funding organisations is to support paddock comparisons. The need for valid procedures to compare unreplicated treatments is increasingly urgent. It is suggested that a range of tools be used to infer statistical significance from using typical error values from related studies or subsampling, through to multivariate techniques to follow trends. Local ‘rules of thumb’ could be developed and data evaluated with calibrated models. A final judgement on treatment effects would need to be based upon the use of several criteria to achieve a ‘balance of probabilities’. Consideration of these problems suggests that paired-paddocks should only be used to evaluate contrasting treatments where large effects are expected and not small variations within a practice

    Adaptive Changes in Life History and Survival following a New Guppy Introduction

    Numerous studies of wild populations have shown that phenotypic traits can change adaptively on short timescales, but very few studies have considered coincident changes in major fitness components. We here examine adaptive changes in life-history traits and survival rates for wild guppies introduced into new environments. Female life-history traits in the derived (Damier River) populations diverged from the ancestral (Yarra River) population, as a result of adaptation to predation regime (high vs. low) and other aspects of the local river. Moreover, some components of the derived Damier populations, particularly juveniles, now show higher survival in the Damier than do contemporary representatives from the ancestral Yarra population. These results suggest that adaptive change can improve survival rates after fewer than 10 years (fewer than 30 guppy generations) in a new environment

    Mycobacterium bovis infection in the lion (Panthera leo) : current knowledge, conundrums and research challenges

    Mycobacterium bovis has global public-health and socio-economic significance and can infect a wide range of species including the lion (Panthera leo) resulting in tuberculosis. Lions are classified as vulnerable under the IUCN Red List of Threatened Species and have experienced a 30% population decline in the past two decades. However, no attempt has been made to collate and critically evaluate the available knowledge of M.bovis infections in lions and potential effects on population. In this review we set out to redress this. Arguments suggesting that ingestion of infected prey animals are the main route of infection for lions have not been scientifically proven and research is needed into other possible sources and routes of infection. The paucity of knowledge on host susceptibility, transmission directions and therefore host status, manifestation of pathology, and epidemiology of the disease in lions also needs to be addressed. Advances have been made in diagnosing the presence of M. bovis in lions. However, these diagnostic tests are unable to differentiate between exposure, presence of infection, or stage of disease. Furthermore, there are contradictory reports on the effects of M. bovis on lion populations with more data needed on disease dynamics versus the lion population’s reproductive dynamics. Knowledge on disease effects on the lion reproduction and how additional stressors such as drought or co-morbidities may interact with tuberculosis is also lacking.     The impact of 24 weeks of supervised endurance versus resistance exercise training on left ventricular mechanics in healthy untrained humans.

    BACKGROUND: In addition to the well-known cardiac structural adaptation to exercise training, little work examined changes in LV mechanics. With new regional and global indices available we sought to determine the effect of 24 weeks endurance versus resistance training on LV mechanics. METHODS AND RESULTS: 23 male subjects were randomly allocated to either a 24-week endurance- or resistance-training program. Pre- and post-training 2D echocardiographic images were acquired. Global LV mechanics (strain [ε]) were recorded in longitudinal, circumferential and radial planes. Rotation was assessed at apical and basal levels. In addition, longitudinal ε-volume loops, across the cardiac cycle, were constructed from simultaneous LV ε (longitudinal and transverse strain) and volume measurements across the cardiac cycle as a novel measure of LV mechanics. Marginal differences in ε and rotation data were found between groups. Post-training, we found no change in global peak ε data. Peak basal rotation significantly increased after training with changes in the endurance group (-2.2±1.9o to -4.5±3.3o) and the resistance group (-2.9±3.0o to -3.4±2.9o) . LV ε-volume loops revealed a modest rightward shift in both groups. CONCLUSIONS: Whilst most global and regional indices of LV mechanics were not significantly altered, 24 weeks of intense supervised exercise training increased basal rotation. Further studies that assess LV mechanics in larger cohorts of subjects and those with cardiovascular disease and risk factors may reveal important training impacts

    Discovery of a meteoritic ejecta layer containing unmelted impactor fragments at the base of Paleocene lavas, Isle of Skye, Scotland

    Evidence for meteorite impacts in the geological record may include the presence of shocked minerals, spherule layers, and geochemical anomalies. However, it is highly unusual to nd unmelted crystals from the actual impactor within an ejecta layer. Here we detail the rst recorded occurrence of vanadium-rich osbornite (TiVN) on Earth, from two sites on Skye, northwest Scotland, which are interpreted as part of a meteoritic ejecta layer. TiVN has only previously been reported as dust from comet Wild 2, but on Skye it has been identi ed as an unmelted phase. Both ejecta layer sites also contain niobium-rich osbornite (TiNbN), which has not previously been reported. An extraterrestrial origin for these deposits is strongly supported by the presence of reidite (a high-pressure zircon polymorph), which is only found naturally at sites of meteorite impact. Barringerite [(Fe,Ni)2P], baddeleyite (ZrO2), alabandite (MnS), and carbon-bearing native iron spherules, together with planar deformation features and diaplectic glass in quartz, further support this thesis. We demonstrate through eld relationships and Ar-Ar dating that the meteorite strike occurred during the mid-Paleocene. This is the rst recorded mid-Paleocene impact event in the region and is coincident with the onset of magmatism in the British Palaeogene Igneous Province (BPIP). The Skye ejecta layer deposits provoke important questions regarding their lateral extent at the base of the BPIP and the possibility of their presence elsewhere beneath the much larger North Atlantic Igneous Province
