    Radial artery pseudoaneurysm after a single arterial puncture for blood-gas analysis: a case report

    We report a case of a radial artery pseudoaneurysm complicating a single arterial puncture for blood-gas analysis that was treated with excision of pseudoaneurysm and suture of the defect of wall of radial artery. The puncture for continuous blood pressure monitoring and serial blood gas analysis have been reported in critically ill patients, but, to the best of our knowledge, there are no cases reported of pseudoaneurysm after a single arterial puncture for blood-gas analysis. In the reported case we think that the main cause of the pseudoaneurysm onset was an incorrect compression and/or a too much short time of compression of the radial artery after the puncture. Minor sequelae and rare complications may be minimized by careful attention to detail in the performance of such procedures and care of the patient also after a single arterial puncture

    La tesi di dottorato dal titolo “La diffusione di nuovi farmaci: un framework multidimensionale per gestire l’accesso al mercato nel sistema healthcare” analizza approfonditamente le principali problematiche di accesso al mercato, specialmente nel contesto healthcare. In particolare, lo scopo del presente studio è quello di contribuire alla letteratura esistente sulla diffusione di innovazione, analizzando come il Market Access può creare valore e supportare le innovazioni nell’attuale sistema healthcare. Il lavoro di tesi rivede i principali studi di management e di medicina, soprattutto per quel che riguarda l’innovazione del Market Access nelle discipline farmacoeconomiche e negli studi di management.Dunque, la domanda di ricerca dello studio è: “In che modo il Market Access supporta la diffusione di innovazioni?”. Per rispondere alla domanda di ricerca, l'autore ha optato per uno studio qualitativo utilizzando casi studio su dati primari e secondari rivolti a differenti stakeholder per esplorare come le MAs influenzano il processo di diffusione nel mercato healthcare. Inoltre, è stata condotta una survey rivolta ad 109 healthcare recipients appartenenti a diverse aree del sistema sanitario e farmaceutico italiano (aziende, associazioni di pazienti, payers, decision makers ecc.). I risultati dimostrano il supporto multidimensionale che il market access offre nell'attuale complessità del sistema healthcare; questa nuova pratica rappresenta un’innovazione organizzativa adottata dalle aziende farmaceutiche dove il riconoscimento del valore di una tecnologia sanitaria non si rivolge soltanto agli outcome clinici ma anche da come migliora la qualità di vita del paziente e la sostenibilità del sistema healthcare

    Quantum soliton evaporation

    Quantum evaporation may occur in a variety of systems such as superfluids, Bose-Einstein condensates, and gravitational black holes (Hawking radiation). However, to date all predictions are based on semiclassical models, e.g., the Einstein equations and classical space-time metric for a black hole and only the fluctuations are quantized. Here we use a fully quantized dynamical equation, the quantum nonlinear Schrödinger equation, to study the evolution of quantum solitons. As a result of quantum fluctuations in the center-of-mass position, the expectation value of the quantum soliton width increases and concomitantly evaporates through the emission of frequency-entangled photon pairs. The frequency of this emission decreases as the soliton evaporates due to the soliton spreading. In the final phase, the soliton mean field collapses irreversibly into a state with zero mean amplitude. These results may provide insight to quantum evaporation in other systems where a full quantum description is still to be developed and highlights that even classically stable systems may also be subject to quantum evaporation

    Pseudoaneurysm with arteriovenous fistula of the prostate after pelvic trauma: Ultrasound imaging

    Pseudoaneurysm (PA) associated with an arteriovenous fistula (AVF) of the internal pudendal artery branches are very uncommon. We report a case of post-traumatic PA with AVF connected to Santorini plexus. Diagnosis was made with trans-rectal ultrasound (TRUS) after recurrent hematuria. TRUS reported a 1.7 × 1.4 × 1.5 cm anechoic area, on anterior prostate apex close to Santorini plexus. The use of color Doppler in this area revealed high flow velocity that was indicative for AVF. The feeding artery was a distal branch of the left pudenda artery. After selective embolization was observed complete occlusion of the feeding branches and disappearance of PA with AVF. Prostate PA with concomitant symptomatic AVF detected with TRUS has not yet described in literature after pelvic trauma and represents complex diagnostic challenges. This case report suggests that the use of TURS and color Doppler can provide an important diagnostic and follow-up to address the clinical suspicion of occult vascular injuries using a noninvasive approach

    Immunotherapy for patients with advanced urothelial cancer: Current evidence and future perspectives

    In recent years, immunotherapy has produced encouraging results in a rapidly increasing number of solid tumors. The responsiveness of bladder cancer to immunotherapy was first established in nonmuscle invasive disease in 1976 with intravesical instillations of bacillus Calmette-Guérin (BCG). Very recently immune checkpoint inhibitors demonstrated good activity and significant efficacy in metastatic disease. In particular the best results were obtained with programmed death-ligand-1 (PD-L1) and programmed death-1 (PD-1) inhibitors, but many other immune checkpoint inhibitors, including anti-cytotoxic T-lymphocyte-associated protein-4 (CTLA-4) antibodies, are currently under investigation in several trials. Simultaneously other therapeutic strategies which recruit an adaptive immune response against tumoral antigens or employ externally manipulated tumor infiltrating lymphocytes might change the natural history of bladder cancer in the near future. This review describes the rationale for the use of immunotherapy in bladder cancer and discusses recent and ongoing clinical trials with checkpoint inhibitors and other novel immunotherapy agents
