    Analysis of Care Coordination for Children with Special Health Care Needs: A Parent\u27s Perspective

    Introduction. Care coordination involves organizing patient care activities and sharing information among all of the participants concerned with a patient\u27s care to achieve improved outcomes, a recent national focus. Compared to the national average, a higher percentage of Vermont children are cared for in an office that meets medical home criteria. However, there is limited research on medical home and care coordination for children with special health care needs (CSHCN) in the state of Vermont. Objectives. The goal of this study was to assess family perceptions, knowledge, and attitudes about how well care coordination is working for Vermont families with CSHCN. Methods. A paper and an electronic anonymous survey was developed for Vermont families with CSHCN. The surveys were then distributed by Vermont Family Network and the UVMMC Department of Pediatrics. Focus group interviews were also conducted at Vermont Family Network to provide family insight to explain the quantitative data. Results. 30 participants responded to the survey; only 20 completed it. The overall composite satisfaction score is 54%. This score takes into account 4 questions regarding care coordination satisfaction. Each question was formatted into a numerical value ranging from zero to five, with an overall score of 20 equating to 100% satisfaction. Discussion. Findings indicate that families with CSHCN are not satisfied with the level of care coordination currently provided. Respondents reported many barriers regarding care coordination, including lack of communication among health care providers, insurance coverage, and lack of support during transitional periods in care.     Edge selective gas detection using Langmuir films of graphene platelets

    Recent advances in large-scale production of graphene have led to the availability of solution processable platelets at the commercial scale. Langmuir-Schaefer (L-S) deposition is a scalable process for forming a percolating film of graphene platelets which can be used for electronic gas sensing. Here, we demonstrate the use of this deposition method to produce functional gas sensors, using a chemiresistor structure from commercially-available graphene dispersions. The sensitivity of the devices and repeatability of the electrical response upon gas exposure has been characterized. Raman spectroscopy and Kelvin probe force microscopy (KPFM) show doping of the basal plane using ammonia (n-dopant) and acetone (p-dopant). The resistive signal is increased upon exposure to both gases showing that sensing originates from the change in contact resistance between nanosheets. We demonstrate that Arrhenius fitting of the desorption response potentially allows measurements of the desorption process activation energies for gas molecules adsorbed onto the graphene nanosheets

    Silver nanowire array-polymer composite as thermal interface material

    Silver nanowire arrays embedded inside polycarbonate templates are investigated as a viable thermal interface material for electronic cooling applications. The composite shows an average thermal diffusivity value of 1.89x10(-5) m(2) s(-1), which resulted in an intrinsic thermal conductivity of 30.3 W m(-1) K(-1). The nanowires' protrusion from the film surface enables it to conform to the surface roughness to make a better thermal contact. This resulted in a 61% reduction in thermal impedance when compared with blank polymer. An similar to 30 nm Au film on the top of the composite was found to act as a heat spreader, reducing the thermal impedance further by 35%. A contact impedance model was employed to compare the contact impedance of aligned silver nanowire-polymer composites with that of aligned carbon nanotubes, which showed that the Young's modulus of the composite is the defining factor in the overall thermal impedance of these composites

    Functional liquid structures by emulsification of graphene and other two-dimensional nanomaterials

    Pickering emulsions stabilised with nanomaterials provide routes to a range of functional macroscopic assemblies. We demonstrate the formation and properties of water-in-oil emulsions prepared through liquid-phase exfoliation of graphene. Due to the functional nature of the stabiliser, the emulsions exhibit conductivity due to inter-particle tunnelling. We demonstrate a strain sensing application with a large gauge factor of ~40; the highest reported in a liquid. Our methodology can be applied to other two-dimensional layered materials opening up applications such as energy storage materials, and flexible and printable electronics

    Effects of down-regulating ornithine decarboxylase upon putrescine-associated metabolism and growth in Nicotiana tabacum L.

    Transgenic plants of Nicotiana tabacum L. homozygous for an RNAi construct designed to silence ornithine decarboxylase (ODC) had significantly lower concentrations of nicotine and nornicotine, but significantly higher concentrations of anatabine, compared with vector-only controls. Silencing of ODC also led to significantly reduced concentrations of polyamines (putrescine, spermidine and spermine), tyramine and phenolamides (caffeoylputrescine and dicaffeoylspermidine) with concomitant increases in concentrations of amino acids ornithine, arginine, aspartate, glutamate and glutamine. Root transcript levels of S-adenosyl methionine decarboxylase, S-adenosyl methionine synthase and spermidine synthase (polyamine synthesis enzymes) were reduced compared with vector controls, whilst transcript levels of arginine decarboxylase (putrescine synthesis), putrescine methyltransferase (nicotine production) and multi-drug and toxic compound extrusion (alkaloid transport) proteins were elevated. In contrast, expression of two other key proteins required for alkaloid synthesis, quinolinic acid phosphoribosyltransferase (nicotinic acid production) and a PIP-family oxidoreductase (nicotinic acid condensation reactions), were diminished in roots of odc-RNAi plants relative to vector-only controls. Transcriptional and biochemical differences associated with polyamine and alkaloid metabolism were exacerbated in odc-RNAi plants in response to different forms of shoot damage. In general, apex removal had a greater effect than leaf wounding alone, with a combination of these injury treatments producing synergistic responses in some cases. Reduced expression of ODC appeared to have negative effects upon plant growth and vigour with some leaves of odc-RNAi lines being brittle and bleached compared with vector-only controls. Together, results of this study demonstrate that ornithine decarboxylase has important roles in facilitating both primary and secondary metabolism in Nicotiana

    Development of Agricultural Market and Trade Policies in the CEE Candidate Countries.

    This synthesis report focuses on the evolution of agricultural market and trade policies in the Central and Eastern European (CEE) candidate countries in the period 1997 to 2001. The developments were crucially influenced by (OECD, 2000a): ⢠the situation in world agricultural markets; ⢠the overall macroeconomic development in the countries considered; ⢠the prospective EU accession; ⢠bringing domestic agricultural policy in line with the Uruguay Agreement on Agriculture (URAA). High 1997 agricultural prices on world commodity markets were followed by a marked depression in 1998. With the exemption of milk products this trend continued in 1999. Likewise the economic and financial crisis in Russia had a considerable impact on agricultural policies. It hit the regions´ exports resulting in a decline in industrial as well as agricultural output1. Thus, compared to the previous years most of the CEE candidate countries experienced a slow down or even negative rates of growth in their Gross Domestic Product (GDP) in 1998 and 1999. In addition those countries felt increased budgetary pressures. Agricultural market and trade policies largely reacted to these developments. Border protection was increased in many countries in 1998. This was combined in some cases with export subsidies, and ad hoc producer aids to mitigate the adverse effects. The prospect of EU accession also had an influence on the agricultural policy design in the region with many countries implementing EU-type policy instruments. Thus, the importance of per hectare and per head payments increased in the region, quota like measures were implemented in some countries and as part of this development Estonia introduced tariffs for agro-food imports. Finally, many countries also continued to adjust their policies to comply with their commitments agreed to in the World Trade Organisation (WTO). Despite these general tendencies there are also differences in the development of agricultural policies between the various CEE candidates. Chapter 2 therefore provides an overview of the changes of agricultural market and trade policies in each of the 10 accession countries. It addresses the policy issues market access (e.g. tariffs, special safeguard measures), export subsidies (value and quantities) and domestic support (intervention policies, direct payments, input subsidies, production quotas). Chapter 3 provides a brief assessment of recent policy developments in the region in the light of EU accession and WTO commitments. The development of prices and values, e.g. export subsidies, agricultural support expenditure, were presented in the background papers provided by the country experts in current prices in national currencies. In this synthesis report they are in addition converted in Euro. This firstly allows for a better comparison among the CEE candidate countries as well as between those countries and the EU. Some of the accession countries still suffer from high inflation and thus a strong depreciation of their currency.     Polymer of intrinsic microporosity (PIM-7) coating affects triphasic palladium electrocatalysis

    A film of the polymer of intrinsic microporosity PIM-7 is coated onto a glassy carbon electrode and the resulting effects on electron transfer reactions are studied for three different types of processes: (i) aqueous solution based, (ii) solid state surface immobilised, and (iii) electrocatalytic processes on electrodeposited palladium. The effects on reactivity for hydroquinone oxidation in aqueous phosphate buffer are shown to be linked to microporosity causing a slightly lower rate of mass transport without detrimental effects on electron transfer and reaction kinetics. Next, water-insoluble microcrystalline anthraquinone is immobilised directly into the PIM-7 film and shown to give a chemically reversible reduction process, which is enhanced in the presence of PIM-7, when compared to the case of anthraquinone immobilised directly onto bare glassy carbon. Electrodeposition of a film of nano-palladium is demonstrated to give catalytically active electrodes for the reduction/oxidation of protons/hydrogen, the reduction of oxygen, and for the oxidation of formic acid and methanol. With the PIM-7 film applied onto palladium, a mechanical stabilisation effect occurs. In addition, both the hydrogen insertion and the hydrogen evolution reactions as well as formic acid oxidation are enhanced. Effects are discussed in terms of PIM-7 beneficially affecting the interfacial reaction under triphasic conditions. The microporous polymer acts as an interfacial “gas management” layer
