    Time to Shine Volunteer Listeners Report

    This Volunteer Listeners report provides stories about older people’s experiences of social isolation as part of the evaluation Of Time to Shine, Leeds. The report details the Volunteer Listeners Approach as an idea, its development, pilot testing and full implementation during 2019. The methodology is simple, volunteers hold conversations with older people to capture their stories, and notes about these are then produced as stories. The common themes found within the stories are reported in detail here, and show that the older people telling their stories had experienced complex life circumstances, including loss, bereavement and loneliness. Their participation in Time to Shine Funded activities led to a number of individual benefits. Some also discussed being able to give back by supporting others and volunteering themselves. However, several barriers to participation were also identified

    The extremely truncated circumstellar disc of V410 X-ray 1: A precursor to TRAPPIST-1?

    Protoplanetary discs around brown dwarfs and very low mass stars offer some of the best prospects for forming Earth-sized planets in their habitable zones. To this end, we study the nature of the disc around the very low mass star V410 X-ray 1, whose SED is indicative of an optically thick and very truncated dust disc, with our modelling suggesting an outer radius of only 0.6 au. We investigate two scenarios that could lead to such a truncation, and find that the observed SED is compatible with both. The first scenario involves the truncation of both the dust and gas in the disc, perhaps due to a previous dynamical interaction or the presence of an undetected companion. The second scenario involves the fact that a radial location of 0.6 au is close to the expected location of the H2_2O snowline in the disc. As such, a combination of efficient dust growth, radial migration, and subsequent fragmentation within the snowline leads to an optically thick inner dust disc and larger, optically thin outer dust disc. We find that a firm measurement of the CO J=2J=2--1 line flux would enable us to distinguish between these two scenarios, by enabling a measurement of the radial extent of gas in the disc. Many models we consider contain at least several Earth-masses of dust interior to 0.6 au, suggesting that V410 X-ray 1 could be a precursor to a system with tightly-packed inner planets, such as TRAPPIST-1

    Topographic and hydrodynamic controls on barrier retreat and preservation: An example from Dogger Bank, North Sea

    Barrier retreat can occur due to in-place drowning, overstepping or rollover, depending on the interplay of controls such as sea-level rise, sediment supply, coastal hydrodynamic regime and topography. Offshore sedimentary archives of barriers active during rapid Holocene sea-level rise provide important records of marine transgression, which are vital analogues to support appropriate mitigation strategies for future coastal realignment under projected relative sea-level rise scenarios. This study analyses the sedimentary archive at Dogger Bank, which is a formerly-glaciated area in the North Sea. Dogger Bank experienced marine transgression due to Early Holocene rapid relative sea-level rise. An integrated dataset of vibrocores and high-resolution seismic reflection data permits a stratigraphic framework to be established, which reveals the buried coastal geomorphology of the southern Dogger Bank for the first time. A transgressive stratigraphy was identified, comprising a topographically complicated basal glacial and terrestrial succession, overlain by two phases of barrier and tidal mudflat deposition, prior to shallow marine sedimentation. Barrier phase A was a recurved barrier drowned in place, and discontinuously overstepped to barrier phase B, which experienced continuous overstepping. By linking barrier elevations to relative sea-level curves, the timing of each barrier phase was established. Both barrier phases retreated during periods of rapid sea-level rise with abundant sediment supply. Coastal hydrodynamics (increasing wave energy) and antecedent topography with spatially variable accommodation are suggested to be the main reason for differing retreat mechanisms, rather than the rate of sea-level rise. Antecedent coastal geomorphology plays a critical role in erosional and depositional patterns during transgression, and therefore on the timing, rate and location of marine inundation, which needs to be included in models that aim to forecast hazards in coastal areas

    Worldwide variation in the relative importance of hepatitis B and hepatitis C viruses in hepatocellular carcinoma: a systematic review

    We combined information published worldwide on the seroprevalence of hepatitis B surface antigen (HbsAg) and antibodies against hepatitis C virus (anti-HCV) in 27 881 hepatocellular carcinomas (HCCs) from 90 studies. A predominance of HBsAg was found in HCCs from most Asian, African and Latin American countries, but anti-HCV predominated in Japan, Pakistan, Mongolia and Egypt. Anti-HCV was found more often than HBsAg in Europe and the United States

    Misdiagnosis of Hereditary Amyloidosis as AL (Primary) Amyloidosis

    Background: Hereditary, autosomal dominant amyloidosis, caused by mutations in the genes encoding transthyretin, fibrinogen A -chain, lysozyme, or apolipoprotein A-I, is thought to be extremely rare and is not routinely included in the differential diagnosis of systemic amyloidosis unless there is a family history. Methods: We studied 350 patients with systemic amyloidosis, in whom a diagnosis of the light-chain (AL) type of the disorder had been suggested by clinical and laboratory findings and by the absence of a family history, to assess whether they had amyloidogenic mutations. Results: Amyloidogenic mutations were present in 34 of the 350 patients (9.7 percent), most often in the genes encoding fibrinogen A -chain (18 patients) and transthyretin (13 patients). In all 34 of these patients, the diagnosis of hereditary amyloidosis was confirmed by additional investigations. A low-grade monoclonal gammopathy was detected in 8 of the 34 patients (24 percent). Conclusions: A genetic cause should be sought in all patients with amyloidosis that is not the reactive systemic amyloid A type and in whom confirmation of the AL type cannot be obtained

    Health services research in the public healthcare system in Hong Kong: An analysis of over 1 million antihypertensive prescriptions between 2004-2007 as an example of the potential and pitfalls of using routinely collected electronic patient data

    <b>Objectives</b> Increasing use is being made of routinely collected electronic patient data in health services research. The aim of the present study was to evaluate the potential usefulness of a comprehensive database used routinely in the public healthcare system in Hong Kong, using antihypertensive drug prescriptions in primary care as an example.<p></p> <b>Methods</b> Data on antihypertensive drug prescriptions were retrieved from the electronic Clinical Management System (e-CMS) of all primary care clinics run by the Health Authority (HA) in the New Territory East (NTE) cluster of Hong Kong between January 2004 and June 2007. Information was also retrieved on patients’ demographic and socioeconomic characteristics, visit type (new or follow-up), and relevant diseases (International Classification of Primary Care, ICPC codes). <p></p> <b>Results</b> 1,096,282 visit episodes were accessed, representing 93,450 patients. Patients’ demographic and socio-economic details were recorded in all cases. Prescription details for anti-hypertensive drugs were missing in only 18 patients (0.02%). However, ICPC-code was missing for 36,409 patients (39%). Significant independent predictors of whether disease codes were applied included patient age > 70 years (OR 2.18), female gender (OR 1.20), district of residence (range of ORs in more rural districts; 0.32-0.41), type of clinic (OR in Family Medicine Specialist Clinics; 1.45) and type of visit (OR follow-up visit; 2.39). <p></p> In the 57,041 patients with an ICPC-code, uncomplicated hypertension (ICPC K86) was recorded in 45,859 patients (82.1%). The characteristics of these patients were very similar to those of the non-coded group, suggesting that most non-coded patients on antihypertensive drugs are likely to have uncomplicated hypertension. <p></p> <b>Conclusion</b> The e-CMS database of the HA in Hong Kong varies in quality in terms of recorded information. Potential future health services research using demographic and prescription information is highly feasible but for disease-specific research dependant on ICPC codes some caution is warranted. In the case of uncomplicated hypertension, future research on pharmaco-epidemiology (such as prescription patterns) and clinical issues (such as side-effects of medications on metabolic parameters) seems feasible given the large size of the data set and the comparability of coded and non-coded patients

    Basonuclin-2 Requirements for Zebrafish Adult Pigment Pattern Development and Female Fertility

    Relatively little is known about the generation of adult form. One complex adult trait that is particularly amenable to genetic and experimental analysis is the zebrafish pigment pattern, which undergoes extensive remodeling during post-embryonic development to form adult stripes. These stripes result from the arrangement of three classes of neural crest-derived pigment cells, or chromatophores: melanophores, xanthophores, and iridophores. Here, we analyze the zebrafish bonaparte mutant, which has a normal early pigment pattern but exhibits a severe disruption to the adult stripe pattern. We show that the bonaparte mutant phenotype arises from mutations in basonuclin-2 (bnc2), encoding a highly conserved, nuclear-localized zinc finger protein of unknown function. We show that bnc2 acts non-autonomously to the melanophore lineage and is expressed by hypodermal cells adjacent to chromatophores during adult pigment pattern formation. In bonaparte (bnc2) mutants, all three types of chromatophores differentiate but then are lost by extrusion through the skin. We further show that while bnc2 promotes the development of two genetically distinct populations of melanophores in the body stripes, chromatophores of the fins and scales remain unaffected in bonaparte mutants, though a requirement of fin chromatophores for bnc2 is revealed in the absence of kit and colony stimulating factor-1 receptor activity. Finally, we find that bonaparte (bnc2) mutants exhibit dysmorphic ovaries correlating with infertility and bnc2 is expressed in somatic ovarian cells, whereas the related gene, bnc1, is expressed within oocytes; and we find that both bnc2 and bnc1 are expressed abundantly within the central nervous system. These findings identify bnc2 as an important mediator of adult pigment pattern formation and identify bonaparte mutants as an animal model for dissecting bnc2 functions

    Subtle changes in the flavour and texture of a drink enhance expectations of satiety

    Background: The consumption of liquid calories has been implicated in the development of obesity and weight gain. Energy-containing drinks are often reported to have a weak satiety value: one explanation for this is that because of their fluid texture they are not expected to have much nutritional value. It is important to consider what features of these drinks can be manipulated to enhance their expected satiety value. Two studies investigated the perception of subtle changes in a drink’s viscosity, and the extent to which thick texture and creamy flavour contribute to the generation of satiety expectations. Participants in the first study rated the sensory characteristics of 16 fruit yogurt drinks of increasing viscosity. In study two, a new set of participants evaluated eight versions of the fruit yogurt drink, which varied in thick texture, creamy flavour and energy content, for sensory and hedonic characteristics and satiety expectations. Results: In study one, participants were able to perceive small changes in drink viscosity that were strongly related to the actual viscosity of the drinks. In study two, the thick versions of the drink were expected to be more filling and have a greater expected satiety value, independent of the drink’s actual energy content. A creamy flavour enhanced the extent to which the drink was expected to be filling, but did not affect its expected satiety. Conclusions: These results indicate that subtle manipulations of texture and creamy flavour can increase expectations that a fruit yogurt drink will be filling and suppress hunger, irrespective of the drink’s energy content. A thicker texture enhanced expectations of satiety to a greater extent than a creamier flavour, and may be one way to improve the anticipated satiating value of energy-containing beverages
