This paper aims to make some observations about the teaching and learning of reading, their relationships with children’s literature, and the formation of literary taste.     Nursing students’ knowledge about alcohol and drugs

    Los objetivos de la presente investigación fueron verificar el conocimiento de los estudiantes de enfermería sobre el uso de alcohol y otras drogas, en especial lo relacionado a drogadicción, tolerancia, abstinencia e intoxicación; conocer las razones que los estudiantes atribuyen a la drogadicción y al inicio del uso de alcohol y otras drogas; y, el interés personal hacia el problema del consumo de drogas. Se utilizó un diseño exploratorio y descriptivo; la muestra estuvo compuesta por 44 estudiantes; se usó un cuestionario semiestructurado, construido por los investigadores con base en los objetivos propuestos, conteniendo preguntas abiertas y preguntas cerradas, totalizando 24 preguntas. Fueron seguidos los procedimientos éticos y los datos fueron sometidos a un análisis estadístico descriptivo. Se evidenció que el conocimiento de los estudiantes es todavía limitado, la comprensión, sobre las razones de los pacientes el uso y la dependencia, se mostraron incompletas, también se constató que el interés por la temática está presente.O objetivo da presente pesquisa foi verificar o conhecimento de estudantes de enfermagem sobre o uso de álcool e outras drogas, em especial a drogodependência, tolerância, abstinência e intoxicação; as razões que os estudantes atribuem à drogodependência e ao início do uso de álcool e outras drogas e o interesse pessoal na problemática do consumo de drogas. Utilizou-se um desenho descritivo-exploratório; amostra composta por 44 estudantes; um questionário semiestruturado, construído pelos pesquisadores com base nos objetivos propostos, contendo perguntas abertas e fechadas, totalizando 24 questões. Foram seguidos os procedimentos éticos e os dados foram submetidos à análise descritiva exploratória. A descriptive-exploratory design was used, with a sample of 44 students, by applying a semi-structured questionnaire, constructed by the researchers based on the objectives, with open and closed questions, totaling 24 points.     Controlling competing interactions at oxide interfaces: Enhanced anisotropy in La0.7Sr0.3MnO3 films via interface engineering

    We investigated thin La0.7Sr0.3MnO3-SrTiO3 heterostructures, where the band alignment is engineered by a variation of La/Sr stoichiometry only at the interface. In thin films, the engineered interface leads to an enhancement of the reversed spin configuration that mimics bulk behavior. Microscopically, this enhancement is closely connected with an increased magnetic anisotropy as well as intercoupling between an e(g) orbital reconstruction and a corresponding anisotropic lattice fluctuation. Furthermore, a reentrant-type behavior, triggered by this intercoupling, is observed in the remanent spin state. This microscopic perspective leads to insights on developing new strategies for maintaining bulk-like properties even in very thin La0.7Sr0.3MnO3 heterostructures.open11910Ysciescopu

    Estudo da validação convergente da Escala de Contemplação Ladder aplicada por telefone em tabagistas

    OBJECTIVE: This work was designed to validate the Portuguese version of the Contemplation Ladder, whose purpose is to assess the motivational phase to quit smoking among tobacco users using a telephone service. METHOD: A cross-sectional study was conducted in a nationwide drug use information hotline. In order to assess the convergent validation, the correlation between the Contemplation Ladder and the URICA Scale was calculated, which was previously validated. RESULTS: The study included 271 tobacco users. Statistically significant correlations were found between the Contemplation Ladder scores and the scores of the URICA precontemplation (r=-0.16; p<0.01), action (r=0.15; p<0.01) and maintenance (r=0.18; p<0.01) subscales. The correlation between the URICA Scale compound score and the Contemplation Ladder was also significant (r=0.31; p<0.01). CONCLUSION: The results of our study suggest that the Contemplation Ladder can be an efficient substitute for the URICA scale (whose application lasts at least 20 minutes), without submitting the interviewee to a heavy load of questions. The study presented evidences of convergent validity for the Contemplation Ladder when applied via telephone in tobacco users.OBJETIVO: Este estudo teve como objetivo validar a versão em português da Escala de Contemplação Ladder, que visa verificar o estágio motivacional para parar de fumar em tabagistas, em um serviço de atendimento telefônico. MÉTODO: Um estudo transversal foi conduzido em um serviço de âmbito nacional de informação sobre uso de drogas por telefone. Para avaliação da validação convergente foi calculada a correlação entre a Escala de Contemplação Ladder e a Escala URICA, a qual já foi validada anteriormente. RESULTADOS: O estudo incluiu 271 tabagistas. Foram encontradas correlações estatisticamente significativas entre os escores da Escala de Contemplação Ladder e os escores das subescalas de pré-contemplação (r=-0,16; p<0,01), ação (r=0,15; p<0,01) e manutenção (r=0,18; p<0,01) da URICA. A correlação entre o escore composto da Escala URICA e a Escala de Contemplação Ladder também foi significativa (r=0,31; p<0,01). CONCLUSÃO: Os resultados do nosso estudo sugerem que a Escala de Contemplação Ladder pode ser uma alternativa para a Escala URICA (cuja aplicação dura no máximo 20 minutos), sem se impor uma carga maior de perguntas ao entrevistado.     Direct Observation of Electrostatically Driven Band Gap Renormalization in a Degenerate Perovskite Transparent Conducting Oxide

    We have directly measured the band gap renormalization associated with the Moss-Burstein shift in the perovskite transparent conducting oxide (TCO), La-doped BaSnO_{3}, using hard x-ray photoelectron spectroscopy. We determine that the band gap renormalization is almost entirely associated with the evolution of the conduction band. Our experimental results are supported by hybrid density functional theory supercell calculations. We determine that unlike conventional TCOs where interactions with the dopant orbitals are important, the band gap renormalization in La-BaSnO_{3} is driven purely by electrostatic interactions

    Comportamento cardiovascular durante a reabilitação após cirurgia de revascularização do miocárdio

    OBJECTIVE: The aim of the present study was to assess the behavior of cardiovascular variables during an in-hospital cardiovascular rehabilitation program in patients following myocardial revascularization surgery. METHODS: A total of 14 patients (mean age: 55.4 ± 6.4 years, 78.6% male) participated in the study, all of whom had a previous diagnosis of coronary insufficiency and indication for elective surgery. The protocol consisted of a group of low-impact (2-3 METs) upper/lower extremity and walking exercises performed both pre and post-operatively (3rd and 4th days). The following variables were evaluated at rest and following the exercise program: heart rate (HR, bpm); systolic arterial pressure (SAP, mmHg); diastolic arterial pressure (DAP, mmHg); mean arterial pressure (MAP, mmHg); double product (DPr, bpm/mmHg); and the Rating of Perceived Exertion (RPE) scale. RESULTS: There was a significant increase in HR and DPr in the individual analysis (P<0.001) as well as in between days (P<0.001 for HR and P<0.05 for DPr), but only attaining maximal values that were < 30% of predicted. Moreover, a negative correlation was found between the RPE scale and both SAP and MAP. CONCLUSION: The exercises proposed proved to be safe with the change in key physiologic variables throughout the experiment below recommended values for the hospitalization phase. Furthermore, the RPE scale appears to have a correlation with some hemodynamic variables and thus may be a useful tool for this group of patients. _______________________________________________________________________________ RESUMOObjetivo: O objetivo do presente estudo foi avaliar o comportamento das variáveis cardiovasculares durante um programa de reabilitação cardiovascular hospitalar em pacientes submetidos à cirurgia de revascularização do miocárdio.Métodos: Um total de 14 pacientes (idade média: 55,4 ±6,4 anos, 78,6% do sexo masculino) participaram do estudo,todos apresentavam diagnóstico prévio de insuficiência coronariana e indicação de cirurgia eletiva. O protocolo consistiu de um grupo de exercícios de baixo impacto (2-3METs) para extremidade superior e inferior e exercícios de caminhada realizados no pré e pós-operatório (3 e 4 dias).As seguintes variáveis foram avaliadas em repouso e após o programa de exercício: frequência cardíaca (FC, bpm),pressão arterial sistólica (PAS, mmHg), pressão arterial diastólica (PAD, mmHg), pressão arterial média (MAP,mmHg), duplo produto (DPr, bpm/mmHg) e escala de Índice de Percepção de Esforço (IPE).Resultados: Houve aumento significativo da FC e DPr na análise individual (P <0,001), bem como na análise entre os dias de avaliação (P <0,001 para FC e P <0,05 para DPr), mas apenas atingindo valores máximos que foram < 30% do previsto. Além disso, ocorreu correlação negativa entre a escala IPE e SAP e MAP.Conclusão: Os exercícios propostos mostraram-se seguros com a mudança de variáveis fisiológicas fundamentais em todo o experimento abaixo do valor recomendado para a fase de hospitalização. Além disso, a escala percepção de esforço parece ter correlação com algumas variáveis hemodinâmicas e, portanto, pode ser uma ferramenta útil para este grupo de pacientes

    Body composition and body fat distribution are related to cardiac autonomic control in non-alcoholic fatty liver disease patients

    BACKGROUND/OBJECTIVES: Heart rate recovery (HRR), a cardiac autonomic control marker, was shown to be related to body composition (BC), yet this was not tested in non-alcoholic fatty liver disease (NAFLD) patients. The aim of this study was to determine if, and to what extent, markers of BC and body fat (BF) distribution are related to cardiac autonomic control in NAFLD patients. SUBJECTS/METHODS: BC was assessed with dual-energy X-ray absorptiometry in 28 NAFLD patients (19 men, 51±13 years, and 9 women, 47±13 years). BF depots ratios were calculated to assess BF distribution. Subjects’ HRR was recorded 1 (HRR1) and 2 min (HRR2) immediately after a maximum graded exercise test. RESULTS: BC and BF distribution were related to HRR; particularly weight, trunk BF and trunk BF-to-appendicular BF ratio showed a negative relation with HRR1 (r 1⁄4 0.613, r 1⁄4 0.597 and r 1⁄4 0.547, respectively, Po0.01) and HRR2 (r 1⁄4 0.484, r 1⁄4 0.446, Po0.05, and r 1⁄4 0.590, Po0.01, respectively). Age seems to be related to both HRR1 and HRR2 except when controlled for BF distribution. The preferred model in multiple regression should include trunk BF-to-appendicular BF ratio and BF to predict HRR1 (r2 1⁄4 0.549; Po0.05), and trunk BF-to-appendicular BF ratio alone to predict HRR2 (r2 1⁄4 0.430; Po0.001). CONCLUSIONS: BC and BF distribution were related to HRR in NAFLD patients. Trunk BF-to-appendicular BF ratio was the best independent predictor of HRR and therefore may be best related to cardiovascular increased risk, and possibly act as a mediator in age-related cardiac autonomic control variation.info:eu-repo/semantics/publishedVersio

    Uso de drogas ilícitas em sete países da América Latina: perspectivas críticas de familiares e pessoas próximas

    This cross-sectional multi-centre study explored how family members and friends of illicit drug users perceived protective and risk factors, treatment facilities and policies and laws regarding illicit drug use. Family members and friends of illicit drug users were recruited in 10 urban health care outpatient units in 7 Latin American countries (Brazil, Colombia, Costa Rica, Ecuador, Guatemala, Honduras and Mexico) to complete a questionnaire. The majority of the respondents chose psycho-social factors over genetic or biological explanations as causes of drug problems. Respondents felt that families and governments were responsible for preventing drug problems. Church/religious institutions were most often mentioned in the context of accessible treatment. When asked about access to treatment facilities, the majority said that there were not enough. Shame about drug use, cost, and limited treatment options were most often cited as barriers to treatment.Este estudio transversal multicéntrico exploró como los familiares y personas cercanas de usuarios de drogas ilícitas perciben los factores de protección y los de riesgo, las facilidades de tratamiento, las iniciativas de prevención y la legislación relativa a las drogas ilícitas. Los familiares y personas cercanas de los usuarios de drogas ilícitas fueron reclutados en 10 unidades urbanas de atención de salud en ambulatorios, en 7 países de América Latina (Brasil, Colombia, Costa Rica, Ecuador, Guatemala, Honduras y México) para responder a una encuesta. Con respecto a las causas de los problemas de las drogas, la mayoría de los encuestados destacó los factores psicosociales como siendo más importantes que los factores genéticos o biológicos. Los encuestados consideraron que las familias y los gobiernos son quienes tienen más responsabilidad en la prevención de los problemas de drogas. La iglesia y las instituciones religiosas fueron mencionadas con mayor frecuencia en el contexto del acceso al tratamiento. Cuando se les preguntó sobre el acceso a las facilidades de tratamiento, la mayoría manifestó que éstas no eran suficientes. Como barreras para el tratamiento, citaron entre las más frecuentes, la vergüenza por el uso de las drogas, la falta de opciones para su tratamiento y el costo del mismo.Este estudo multicêntrico corte temporal explorou a perspectiva de familiares e pessoas próximas a usuários de drogas ilícitas sobre fatores de risco e proteção, serviços de tratamento, políticas e leis relacionadas ao uso de drogas ilícitas. Os familiares e pessoas próximas a usuários de drogas ilícitas foram recrutados em dez unidades de saúde, localizadas em grandes centros urbanos de sete países da América Latina (Brasil, Colômbia, Costa Rica, Equador, Guatemala, Honduras e México), para responderem um questionário. A maioria dos participantes escolheu fatores psicossociais e não fatores genéticos ou biológicos para explicar a causa dos problemas do uso de drogas. Responderam que familiares e governantes são os principais responsáveis pela prevenção dos problemas das drogas. As igrejas e outras instituições religiosas foram mencionadas com frequência dentro do contexto de acesso ao tratamento. A maioria dos entrevistados apontou que o acesso aos serviços que oferecem tratamentos aos usuários de drogas não é suficiente.     Cost-effectiveness of non-invasive methods for assessment and monitoring of liver fibrosis and cirrhosis in patients with chronic liver disease: systematic review and economic evaluation

    BACKGROUND: Liver biopsy is the reference standard for diagnosing the extent of fibrosis in chronic liver disease; however, it is invasive, with the potential for serious complications. Alternatives to biopsy include non-invasive liver tests (NILTs); however, the cost-effectiveness of these needs to be established. OBJECTIVE: To assess the diagnostic accuracy and cost-effectiveness of NILTs in patients with chronic liver disease. DATA SOURCES: We searched various databases from 1998 to April 2012, recent conference proceedings and reference lists. METHODS: We included studies that assessed the diagnostic accuracy of NILTs using liver biopsy as the reference standard. Diagnostic studies were assessed using the Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2) tool. Meta-analysis was conducted using the bivariate random-effects model with correlation between sensitivity and specificity (whenever possible). Decision models were used to evaluate the cost-effectiveness of the NILTs. Expected costs were estimated using a NHS perspective and health outcomes were measured as quality-adjusted life-years (QALYs). Markov models were developed to estimate long-term costs and QALYs following testing, and antiviral treatment where indicated, for chronic hepatitis B (HBV) and chronic hepatitis C (HCV). NILTs were compared with each other, sequential testing strategies, biopsy and strategies including no testing. For alcoholic liver disease (ALD), we assessed the cost-effectiveness of NILTs in the context of potentially increasing abstinence from alcohol. Owing to a lack of data and treatments specifically for fibrosis in patients with non-alcoholic fatty liver disease (NAFLD), the analysis was limited to an incremental cost per correct diagnosis. An analysis of NILTs to identify patients with cirrhosis for increased monitoring was also conducted. RESULTS: Given a cost-effectiveness threshold of £20,000 per QALY, treating everyone with HCV without prior testing was cost-effective with an incremental cost-effectiveness ratio (ICER) of £9204. This was robust in most sensitivity analyses but sensitive to the extent of treatment benefit for patients with mild fibrosis. For HBV [hepatitis B e antigen (HBeAg)-negative)] this strategy had an ICER of £28,137, which was cost-effective only if the upper bound of the standard UK cost-effectiveness threshold range (£30,000) is acceptable. For HBeAg-positive disease, two NILTs applied sequentially (hyaluronic acid and magnetic resonance elastography) were cost-effective at a £20,000 threshold (ICER: £19,612); however, the results were highly uncertain, with several test strategies having similar expected outcomes and costs. For patients with ALD, liver biopsy was the cost-effective strategy, with an ICER of £822. LIMITATIONS: A substantial number of tests had only one study from which diagnostic accuracy was derived; therefore, there is a high risk of bias. Most NILTs did not have validated cut-offs for diagnosis of specific fibrosis stages. The findings of the ALD model were dependent on assuptions about abstinence rates assumptions and the modelling approach for NAFLD was hindered by the lack of evidence on clinically effective treatments. CONCLUSIONS: Treating everyone without NILTs is cost-effective for patients with HCV, but only for HBeAg-negative if the higher cost-effectiveness threshold is appropriate. For HBeAg-positive, two NILTs applied sequentially were cost-effective but highly uncertain. Further evidence for treatment effectiveness is required for ALD and NAFLD.
