    Spermidine Associated to Non-Surgical Treatment of Periodontal Disease: Split Mouth Randomized Controlled Clinical Trial

    Objectives. Spermidine is an endogenous polyamine whose increase induces the autocrine remodeling into targeted cells. The aim of the present study was to evaluate the adjunctive beneficial effects of the local delivery of spermidine to periodontal non-surgical therapy on reduction of periodontal pocket. Methods. In this split mouth, blind, randomized controlled clinical trial, 20 patients with severe chronic generalized periodontal disease scheduled for cause related non-surgical periodontal treatment were enrolled. In three quadrants for each patients, three experimental teeth with probing pocket depth (PPD) 65 6mm were selected and randomly assigned to one of the following treatments: scaling and root planning (SRP)+ spermidine gel (spermidine + alginate) (group a), SRP+ placebo gel (only alginate) (positive control, group b) or SRP (negative control, group c). PPD at baseline, 3 and 6 months after treatment was assessed. Descriptive and inferential statistics was done. Results. After periodontal treatment, a reduction of PPD was observed in all sites. No differences emerged between groups at each time point. In all groups, a significant PPD reduction was observed at T1 and T2 compared to baseline (p<0.05 and p<0.001). PPD reduction from T1 to T2 was significant only in group a (p<0.001). Conclusions. After non-surgical treatment, local delivery of spermidine may induce prolonged improvement of clinical outcome

    Dental treatment of a rare case of pyoderma gangrenosum with aggressive periodontal disease

    Background and Overview: Pyoderma gangrenosum (PG) is a rare neutrophil-mediated autoinflammatory dermatosis that can involve the oral mucosa. Dental surgery is a potential triggering factor for the onset of PG lesions. The authors describe and discuss the dental management of a rare case of aggressive periodontitis in a patient with PG, from multiple tooth extractions to prosthetic rehabilitation, including administration of systemic steroid prophylaxis before surgery to prevent the potential onset of PG-related lesions. Case Description: A 22-year-old man who had a diagnosis of PG and who had aggressive periodontal disease underwent dental extractions, gingivoplastic surgery, and prosthetic rehabilitation. The patient received 8 milligrams of betamethasone intramuscularly 20 minutes before the oral surgery. The tissues healed perfectly, and no adverse effects were reported. Conclusions and Practical Implications: For minor oral surgery, prophylactic corticosteroids might help reduce the risk of developing PG-related lesions. The clinician should plan the prosthetic devices to be as atraumatic as possible

    Medical-grade silicone coated with rhamnolipid R89 is effective against Staphylococcus spp. Biofilms

    Staphylococcus aureus and Staphylococcus epidermidis are considered two of the most important pathogens, and their biofilms frequently cause device-associated infections. Microbial biosurfactants recently emerged as a new generation of anti-adhesive and anti-biofilm agents for coating implantable devices to preserve biocompatibility. In this study, R89 biosurfactant (R89BS) was evaluated as an anti-biofilm coating on medical-grade silicone. R89BS is composed of homologues of the mono- (75%) and di-rhamnolipid (25%) families, as evidenced by mass spectrometry analysis. The antimicrobial activity against Staphylococcus spp. planktonic and sessile cells was evaluated by microdilution and metabolic activity assays. R89BS inhibited S. aureus and S. epidermidis growth with minimal inhibitory concentrations (MIC99) of 0.06 and 0.12 mg/mL, respectively and dispersed their pre-formed biofilms up to 93%. Silicone elastomeric discs (SEDs) coated by R89BS simple adsorption significantly counteracted Staphylococcus spp. biofilm formation, in terms of both built-up biomass (up to 60% inhibition at 72 h) and cell metabolic activity (up to 68% inhibition at 72 h). SEM analysis revealed significant inhibition of the amount of biofilm-covered surface. No cytotoxic effect on eukaryotic cells was detected at concentrations up to 0.2 mg/mL. R89BS-coated SEDs satisfy biocompatibility requirements for leaching products. Results indicate that rhamnolipid coatings are effective anti-biofilm treatments and represent a promising strategy for the prevention of infection associated with implantable devices

    Histomorphometrical evaluation of the effects of Aminogam® gel in oral healing process of post-surgical soft tissue

    Wound healing is a dynamic process that involves a complex interaction of inflammatory cells, cytokines and mediators of extracellular matrix [1]. One of the processes that occur during tissue regeneration is angiogenesis and it is considered to have a pivotal role in wound repair. Previous studies have shown that a topical application of proteins and sodium hyaluronate to wounds can expedite the repair of damaged tissue [2]. The aim of this preliminary study is to evaluate the efficacy of Aminogam\uae gel (A\uae) (ErreKappa Euroterapici SpA, Milano), a topical medication which contains 4 amino acids (glycine, leucine, proline, lysine) and sodium hyaluronate, used to improve and accelerate gingival flap healing following molar extraction by analyzing collagen fibers amount, orientation and microvascular distribution (MVD). Ten patients (mean age 49ys) were included in the study. Two teeth (38 and 48) were extracted at an interval of 30 days. The \u201ctest\u201d site (AM) was treated with A\uae while the \u201ccontrol\u201d site (no AM) was not. Dental extraction was performed and the flaps were sutured with a consequent excess of tissue for histological processing (T0). A\uae had been applied only at the AM site for 10 days post-extraction. At suture removal, a gingivoplasty was performed and the exceeding tissue was histologically analysed (T1). Paraffin blocks were cut and slides were stained with haematoxylin-eosin and Sirius Red. No signs of inflammatory infiltrate or necrosis were observed. Sirius Red staining highlighted a lower degree of organized collagen fibers at T1 vs T0. At T0 the fibers were organized in closely packed and well-oriented bundles. At T1-no AM fibers were thin and formed a disorganized grid. At T1-AM fibers appeared thicker and the tissue appeared more mature compared to T1-no AM. Immunohistochemistry against CD31 was performed to mark endothelial cells and to calculate MVD by stereological method [3]. MVD resulted highest at T1-AM. The T1 data normalized on T0 presented a statistically significant difference (p=0.012) between AM and no AM group.     Marker-Assisted Pyramiding of Blast-Resistance Genes in a japonica Elite Rice Cultivar through Forward and Background Selection

    Rice blast, caused by Pyricularia oryzae, is one of the main rice diseases worldwide. The pyramiding of blast-resistance (Pi) genes, coupled to Marker-Assisted BackCrossing (MABC), provides broad-spectrum and potentially durable resistance while limiting the donor genome in the background of an elite cultivar. In this work, MABC coupled to foreground and background selections based on KASP marker assays has been applied to introgress four Pi genes (Piz, Pib, Pita, and Pik) in a renowned japonica Italian rice variety, highly susceptible to blast. Molecular analyses on the backcross (BC) lines highlighted the presence of an additional blast-resistance gene, the Pita-linked Pita2/Ptr gene, therefore increasing the number of blast-resistance introgressed genes to five. The recurrent genome was recovered up to 95.65%. Several lines carrying four (including Pita2) Pi genes with high recovery percentage levels were also obtained. Phenotypic evaluations confirmed the effectiveness of the pyramided lines against multivirulent strains, which also had broad patterns of resistance in comparison to those expected based on the pyramided Pi genes. The developed blast-resistant japonica lines represent useful donors of multiple blast-resistance genes for future rice-breeding programs related to the japonica group

    Property-based interest propagation in ontology-based user model

    We present an approach for propagation of user interests in ontology-based user models taking into account the properties declared for the concepts in the ontology. Starting from initial user feedback on an object, we calculate user interest in this particular object and its properties and further propagate user interest to other objects in the ontology, similar or related to the initial object. The similarity and relatedness of objects depends on the number of properties they have in common and their corresponding values. The approach we propose can support finer recommendation modalities, considering the user interest in the objects, as well as in singular properties of objects in the recommendation process. We tested our approach for interest propagation with a real adaptive application and obtained an improvement with respect to IS-A-propagation of interest values

    Propagating user interests in ontology-based user model

    In this paper we address the problem of propagating user interests in ontology-based user models. Our ontology-based user model (OBUM) is devised as an overlay over the domain ontology. Using ontologies as the basis of the user profile allows the initial user behavior to be matched with existing concepts in the domain ontology. Such ontological approach to user profiling has been proven successful in addressing the cold-start problem in recommender systems, since it allows for propagation from a small number of initial concepts to other related domain concepts by exploiting the ontological structure of the domain. The main contribution of the paper is the novel algorithm for propagation of user interests which takes into account i) the ontological structure of the domain and, in particular, the level at which each domain item is found in the ontology; ii) the type of feedback provided by the user, and iii) the amount of past feedback provided for a certain domain object

    Totally laparoscopic, multi-stage, restorative proctocolectomy for inflammatory bowel diseases. A prospective study on safety, efficacy and long-term results

    Background: Laparoscopic ileo-pouch-anal anastomosis (IPAA) has been reported as having low morbidity and several advantages. Aims: To evaluate safety, efficacy and long-term results of laparoscopic IPAA, performed in elective or emergency settings, in consecutive unselected IBD patients. Methods: All the patients received totally laparoscopic 2-stage (proctocolectomy and IPAA \u2013 stoma closure) or 3-stage (colectomy \u2013 proctectomy and IPAA \u2013 stoma closure) procedure according to their presentation. Results: From July 2007 to July 2016, 160 patients entered the study. 50.6% underwent a 3-stage procedure and 49.4% a 2-stage procedure. Mortality and morbidity were 0.6% and 24.6%. Conversion rate was 3.75%. 8.7% septic complications were associated with steroids and Infliximab treatment (p = 0.0001). 3-stage patients were younger (p = 0.0001), with shorter disease duration (p = 0.0001), minor ASA scores of 2 and 3 (p = 0.0007), lower inflammatory index and better nutritional status (p = 0.003 and 0.0001), fewer Clavien-Dindo's grade II complications (p = .0001), reduced rates of readmission and reoperation at 90 days (p = 0.03), and shorter hospitalization (p = .0001), but with similar pouch and IPAA leakage, compared to 2-stage patients. 8 years pouch failure and definitive ileostomy were 5.1% and 3.7%. Conclusion: A totally laparoscopic approach is safe and feasible, with very low mortality and morbidity rates and very low conversion rate, even in multi-stage procedures and high-risk patients

    Study of intragastric structuring ability of sodium alginate based o/w emulsions under in vitro physiological pre-absorptive digestion conditions

    In the present work, the intragastric structuring ability of o/w emulsions either stabilised (1–4%, w/w of sodium alginate (SA)) or structured with sheared ionic gel (1–3%, w/w of SA crosslinked with Ca2+) in the absence (saliva and gastric phases constituted of deionised water) or presence of in vitro pre-absorptive conditions (physiological simulated saliva and gastric fluids) was investigated. Visualisation of the morphological aspects of the gastric chymes, in the absence of multivalent counterions, demonstrated that SA stabilised systems underwent a remarkable swelling in the pH range of 2–3, whilst at the same pH range, ionic SA gel structured systems maintained their major structure configuration. When the aforementioned systems were exposed to physiological intragastric fluids, a reduction of the length and the hydrodynamic volume of the alginate fibres was detected regardless the structuring approach. On their exposure to physiological intragastric conditions (pH = 2), SA stabilised emulsions underwent sol–gel transition achieving a ca. 3- to 4-order increase of storage modulus (at 1 Hz). In the case of ionic sheared gel structured emulsions, exposure to physiological intragastric fluids resulted in a 10-fold reduction ability of their acid structuring ability, most likely due to the dialysis of egg-box dimer conformations by monovalent cations and protons and the sterical hindering of hydrogen bonding of MM and GG sequences under acidic conditions. Using of non-physiological simulated intragastric fluids was associated with overestimated structuring performance of SA regardless its physical state
