    Cardiac hemangioma of the right atrium in a neonate : fetal management and expedited surgical resection

    Cardiac hemangioma is a rare tumor with a reported incidence of 1-2%. We describe the case of a neonate with a right atrial mass that was diagnosed prenatally. The fetus developed a supraventricular tachycardia and was delivered by cesarean section in the 35th week of gestation. The infant underwent surgery after 24 hours to remove the mass which was diagnosed as a cardiac capillary-cavernous hemangioma.peer-reviewe

    Point-of-care ultrasonography—An essential skill for medical graduates?

    The recent proliferation of point-of-care ultrasonography (POCUS) in the clinical practice of many medical specialties has exposed persistent barriers to education, training and standardisation. Specialist training curriculums are already overwhelming, having grossly insufficient time available for the specialist trainees and for the small number of available trainers alike to incorporate POCUS into postgraduate education. The logical solution to overcome these barriers could be to incorporate basic POCUS education and training into the undergraduate university curriculums, introducing longitudinal integration with other relevant medical sciences. The Australasian Society of Ultrasound in Medicine already has well-established educational programmes in POCUS with standardised assessment of competency, which could potentially offer the basis for symbiosis with the Australian and New Zealand medical schools

    The case for the development and use of "ecologically valid" measures of executive function in experimental and clinical neuropsychology

    This article considers the scientific process whereby new and better clinical tests of executive function might be developed, and what form they might take. We argue that many of the traditional tests of executive function most commonly in use (e.g., the Wisconsin Card Sorting Test; Stroop) are adaptations of procedures that emerged almost coincidentally from conceptual and experimental frameworks far removed from those currently in favour, and that the prolongation of their use has been encouraged by a sustained period of concentration on “construct-driven” experimentation in neuropsychology. This resulted from the special theoretical demands made by the field of executive function, but was not a necessary consequence, and may not even have been a useful one. Whilst useful, these tests may not therefore be optimal for their purpose. We consider as an alternative approach a function-led development programme which in principle could yield tasks better suited to the concerns of the clinician because of the transparency afforded by increased “representativeness” and “generalisability.” We further argue that the requirement of such a programme to represent the interaction between the individual and situational context might also provide useful constraints for purely experimental investigations. We provide an example of such a programme with reference to the Multiple Errands and Six Element tests

    The search for factors influencing the palatability of beef continues. The relative importance of variables in the USDA quality grading system that currently predicts acceptability and palatability has been questioned. The effect of marbling on palatability is one of the more controversial aspects. Marbling is one of the prime factors in determining the quality grade of a beef carcass and researchers have shown that other variables may play an equal or more important role influencing the eating quality of beef. The purpose of this study was to evaluate the accuracy of commonly-used predictors of palatability and to consider the possible influence of other carcass measures as predictors of palatability. Palatability, as defined in this study, was measured by the subjective taste panel scores for flavor, juiciness and tenderness.     Absolute photoionization cross section measurements of the Kr I-isoelectronic sequence

    Photoionization spectra have been recorded in the 4s, 4p and 3d resonance regions for the Kr Iisoelectronic sequence using both the dual laser produced plasma technique (at DCU) to produce photoabsorption spectra, and the merged ion beam and synchrotron radiation technique (at ASTRID) to measure absolute photoionization cross sections. Profile parameters are compared for the 4s − np resonances of Rb+ and Sr2+. Many new 4p " ns, md transitions are identified with the aid of Hartree-Fock calculations, and consistent quantum defects are observed for the various ns and md Rydberg series. Absolute single and double photoionization cross sections recorded in the 3d region for Rb+ and Sr2+ ions show preferential decay via double photoionization. This is only the second report where both the DLP technique and the merged beam technique have been used simultaneously to record photoionization spectra, and the advantages of both techniques (i.e. better resolution in the case of DLP and values for absolute photoionization cross sections in the case of the merged beam technique) are highlighted

    Women’s groups’ perceptions of neonatal and infant health problems in rural Malawi

    AimsTo present the perceptions of women in rural Malawi regarding the health problems affecting neonates and infants and to explore the relevance of these perceptions for child health policy and strategy in Malawi.MethodsWomen’s groups in Mchinji district identified newborn and infant health problems (204 groups, 3484 women), prioritised problems they considered most important (204 groups, 3338 women) and recorded these problems on monitoring forms. Qualitative data was obtained through 6 focus-groupdiscussions with the women’s groups and 22 interviews with individuals living in women’s group communities but not attending groups.ResultsWomen in Malawi do not define the neonatal period accordingto any epidemiological definition. In order of importance they identified and prioritised the following problems for newborns and infants: diarrhoea, infection, preterm birth, tetanus, malaria, asphyxia, respiratory tract infection, hypothermia, jaundice, convulsions and malnutrition.ConclusionThis study suggests that women in rural Malawi collectively have a developed understanding of neonatal and infant health problems. This makes a strong argument for the involvement of lay people in policy and strategy development and also suggests that this capacity, harnessed and strengthened through community mobilisation approaches, has thepotential to improve neonatal and infant health and reduce mortality

    Characterization of Hereford and Two-Breed Rotational Crosses of Hereford With Angus and Simmental Cattle: Carcass Traits of Steers

    Calf carcass traits were evaluated for Simmental (S) x Hereford (H) and Angus (A) x H cross cows in two-breed rotations and for straightbred H. Data were grouped into seven dam breed categories: straightbred Hereford (H), F1 S x H cows (SH), S x H cows of low percentage H (SHS), S x H cows of high percentage H (HSH), F1 A x H cows (AH), A x H cows of low percentage H (AHA) and A x H cows of high percentage H (HAH). Straightbred H and crossbred SH, AH, SHS and AHA cows were mated to H bulls, HSH cows were mated to S bulls and HAH cows were mated to A bulls. Calves from S x H dams produced heavier carcasses with less fat, lower quality grade, larger longissimus area and increased estimated cutability compared to calves from H or A x H dams. Some significant intergenerational differences were observed within rotations, particularly within S x H. Calves from HSH cows mated to S bulls produced carcasses with less fat cover, lower quality grade, larger longissimus muscle area and higher estimated cutability compared to calves from SHS dams mated to H bulls. Within both rotations, evaluation of carcass weight per day of age indicated lower postweaning rate of gain for generations in which H was the sire breed. A separate analysis evaluated carcass traits of calves from SHS, HSH, AHA and HAH dam breed groups from the last 3 years of the study when calves were fed under two different postweaning management systems. With Management System One, the concentrate to roughage ratio was increased less rapidly and calves averaged 122 days older at slaughter and carcass weights averaged 128 Ib heavier compared to calves fed under Management System Two. The breed group x postweaning management system interaction effect approached significance only for marbling score, estimated cutability and kidney, pelvic and heart fat
