Expressão de genes associados à obesidade em testículos e esperma de ratos sujeitos a restrição calórica e administração de GLP-1
Caloric restriction (CR), due to its negative energy balance and GLP-1, for
being an incretin, constitute two weight loss protocols that remain poorly
investigated, particularly its impact on male reproductive function, whose
proper functioning depends on a balanced energy homeostasis. That energy
homeostasis has been related to genes linked to energy metabolism control,
namely the obesity-related genes (ORGs), such as fat mass and obesity associated (FTO), melanocortin-4 receptor (MC4R), glucosamine-6-
phosphate deaminase 2 (GNPDA2) and transmembrane protein 18
(TMEM18). Thus, this project aimed to use an animal model to study the
potential of CR and GLP-1 administration in the regulation of those ORGs
expression in testes and sperm of Wistar rats and verify if there is an impact
on sperm quality. Firstly, we observed that CR promoted a lower weight gain
and a decrease in insulin resistance. On the other hand, GLP-1 administration,
beyond the expected increase in active GLP-1 levels, did not promote any
change in glucose metabolism, hormonal profile and consequently in body
weight. Furthermore, CR promoted an increase in sperm head defects, while
the GLP-1 administration improved sperm morphology. Regarding the ORGs
we observed the presence of FTO, MC4R and TMEM18 transcripts in rat
testes and identified those transcripts, for the first time, in rat sperm.
Additionally, we identified, for the first time, the presence of GNPDA2
transcripts in rat testes and sperm. The corresponding proteins were also
identified in rat testes, being that they all presented distinct cellular locations.
CR and GLP-1 administration promoted an increase in the expression of the
ORGs in testes, however in spermatozoa the expression was not altered. We
also identified the presence of NFE2L2 (encodes for an important antioxidant
transcription factor) transcripts in rat testes and sperm and verified that their
abundance is increased, in testes, by CR and GLP-1 administration. Then we
explored, individually, the role the ORGs at the testicular and sperm levels,
namely in the response to CR and GLP-1 administration and we obtained
some clues related to the potential involvement of each of the ORGs in the
male reproductive function. Overall, we were able to perceive that CR and
GLP-1 administration promoted a general association of all ORGs with an
improvement in oxidative status both in testes and sperm.A restrição calórica (CR), pelo seu balanço energético negativo e o GLP-1,
por ser uma incretina, constituem dois protocolos de perda de peso, que
permanecem pouco investigados, principalmente no que toca ao seu impacto
na função reprodutiva masculina, cujo correto funcionamento depende de
uma homeostase energética equilibrada. Essa homeostase energética tem
sido relacionada a genes ligados ao controlo do metabolismo energético,
nomeadamente os genes relacionados à obesidade (ORGs), como gene de
obesidade e de massa de gordura associada (FTO), recetor de
melanocortina-4 (MC4R), glucosamina-6-fosfato desaminase 2 (GNPDA2) e
proteína transmembranar 18 (TMEM18). Assim, este projeto teve como
objetivo utilizar um modelo animal para estudar o potencial da CR e da
administração de GLP-1 na regulação da expressão desses ORGs nos
testículos e esperma de ratos Wistar e verificar se há impacto na qualidade
espermática. Primeiramente, observamos que a CR promoveu menor ganho
de peso e diminuição da resistência à insulina. Por outro lado, a administração
de GLP-1, além do esperado aumento nos níveis de GLP-1 ativo, não
promoveu qualquer alteração no metabolismo da glucose, no perfil hormonal
e consequentemente no peso corporal. Para além disso, a CR promoveu um
aumento nos defeitos da cabeça dos espermatozoides, enquanto a
administração de GLP-1 melhorou a morfologia espermática. Em relação aos
ORGs, observamos a presença de transcritos de FTO, MC4R e TMEM18 em
testículos de ratos e identificamos esses transcritos, pela primeira vez, em
esperma de ratos. Além disso, identificamos, pela primeira vez, a presença
de transcritos GNPDA2 em testículos e esperma de ratos. As proteínas
correspondentes também foram identificadas em testículos de ratos, sendo
que todas apresentavam localizações celulares distintas. A CR e a
administração de GLP-1 promoveram um aumento na expressão dos ORGs
nos testículos, porém no esperma a expressão não foi alterada. Também
identificamos a presença de transcritos de NFE2L2 (codifica para um
importante fator de transcrição antioxidante) em testículos e esperma de ratos
e verificamos que sua abundância é aumentada nos testículos, pela CR e
administração de GLP-1. Em seguida, exploramos, individualmente, o papel
dos ORGs nos níveis testicular e espermático, nomeadamente na resposta à
CR e à administração de GLP-1 e obtivemos algumas pistas relacionadas com
o envolvimento potencial de cada um dos ORGs na função reprodutiva
masculina. No geral, pudemos perceber que as duas intervenções
promoveram uma associação geral de todos os ORGs com uma melhoria no
estado oxidativo dos testículos e esperma.Mestrado em Bioquímic
Postpartum subcapsular liver hematoma: a rare complication successfully managed with conservative treatment
Subcapsular liver hematoma is a spontaneous bleeding between the Glisson's capsule and the liver parenchyma. It is a rare but serious complication of severe preeclampsia and/or HELLP (hemolysis, elevated liver enzyme and low platelets) syndrome that can have devastating consequences, the worst being hepatic rupture with maternal death. A young nulliparous woman, at 34 weeks ‘gestation, was admitted for evaluation of new onset hypertension. The pregnancy was uneventful, yet at 32 weeks’ gestation a severe fetal growth restriction was diagnosed. She presented a high urine protein to creatinine ratio, then a pre-eclampsia without severity criteria was diagnosed. At day 3 of hospitalization, she presented with intense epigastric pain and vaginal blood loss, so an emergency c-section was performed with the suspicion of placental abruption. Eighteen hours postpartum she complained of intense diffuse abdominal pain radiating to the right shoulder blade. Arterial hypotension and obtundation was observed. A computerized tomography was performed and revealed the presence of a bulky perihepatic hematoma measuring 12×9×21 cm. The patient went into hemorrhagic shock and so massive hemorrhage protocol was activated. After a multidisciplinary discussion a conservative management was decided. Clinical and analytical improvement was observed, and she was discharged home on postpartum day-27. The diagnosis of subcapsular liver hematoma is challenging, but it is essential to be done in a timely manner. Thus, a high index of suspicion, a prompt diagnosis and a multidisciplinary approach are the key factors for a successful outcome
Results of the Portuguese EQA program for molecular detection of SARS-CoV-2, 2020-2022
The initial strategy for containing the spread of SARS CoV 2 was the prompt identification of COVID 19 cases. National Reference laboratory implemented the biomolecular assays to detect SARS-CoV-2.
In June 2020 the Portuguese National External Quality Assessment Program (PNAEQ) in collaboration with National Reference Laboratory for Influenza and Other Respiratory Viruses (NRL) organized the first National External Quality Assessment Program (EQA) for detection of SARS CoV 2 virus by molecular methods. Fanconi Anemia in Portugal: retrospective study of 34 years of investigation at National Health Institute (1980-2014)
A Anemia de Fanconi (AF) é uma doença recessiva rara, com uma frequência
estimada de 4 a 7 por 1 000 000 de nascimentos. Caraterizase
por malformações congénitas, falência medular e hipersensibilidade
a agentes clastogénicos de DNA. Devido à grande complexidade desta
patologia a primeira abordagem de diagnóstico, consiste na análise da
instabilidade cromossómica, após cultura celular com estimulação com
agentes clastogénicos diepoxibutano (DEB) ou mitomicina C (MMC). Realizou-
se um estudo retrospetivo de 34 anos (1980-2014) em 243 amostras
com suspeita de AF e de 25 amostras de familiares de doentes de
AF, num total de 268 amostras. Nas 243 amostras suspeitas de Anemia
de Fanconi, foram identificadas 37 com AF. A idade média ao diagnóstico
foi de 7 anos, existindo um ligeiro predomínio da incidência no sexo
feminino (59%). Uma amostra foi classificada como AF(-/+). Nos familiares
de doentes com AF foram identificados 2 casos positivos, o que
perfaz 39 amostras de AF positivas. Em quatro das amostras AF negativas,
observaram-se cariotipos anormais. Estes resultados não permitem
estimar uma frequência de doentes de AF em Portugal, uma vez que
não englobam indivíduos de todas as regiões portuguesas, mas permitem
uma estimativa da frequência espectável.Fanconi anemia (FA) is a rare recessive disease with an estimated frequency
from 4 to 7 per 1 000 000 live births. It is characterized by congenital
malformations, bone marrow failure and hypersensitivity to DNA
clastogenic agents. Due to the great complexity of the pathology the first
diagnostic approach is the analysis of chromosomal instability in cultured
cells after stimulation with the clastogenic agents diepoxybutane (DEB)
or mitomycin C (MMC). We performed a retrospective study of 34 years
(1980-2014) in 243 samples suspected of FA and 25 samples from relatives
of FA patients, totalizing 268 samples. In 243 suspected samples
37 were confirmed to be FA. The average age at diagnosis was 7 years,
with a slight predominant incidence in females (59%). One sample was
classified as FA (-/+). Among the patients’ relatives, 2 positive FA cases
were identified totaling 39 positive samples. In four negative samples of
suspected FA, abnormal karyotypes were observed. Since these samples
are not representative of individuals from all Portuguese regions, the data
only allow estimating the frequency of FA in Portugal
Caracterização de uma duplicação do braço longo do cromossoma 1,por microarray de alta resolução, em mielofibrose primária: um fator de malignidade?
Os neoplasmas mieloproliferativos (NMP) são caracterizados por uma excessiva proliferação de uma ou mais linhagens da série mieloide. A mielofibrose primária (MFP) é uma das patologias deste grupo, que apresenta uma proliferação preferencial de megacariócitos e granulócitos na medula óssea (MO). Tem uma incidência anual de 0,5-1,5/100 000 indivíduos. Os doentes ao diagnóstico apresentam idades entre 60-70 anos independentemente do sexo. Os fatores de mau prognóstico são: idade > 70 anos, hemoglobina <10 g/dl; plaquetas <100 x106/L e cariotipo anormal. A maior causa de morbilidade e mortalidade da MFP é a falência da MO, ocorrência de eventos tromboembólicos, hipertensão portal, falência cardíaca e progressão para Leucemia Mieloide Aguda (LMA).
Apresentamos um caso clínico, de um individuo do sexo feminino, com 68 anos à data do diagnóstico inicial, que apresentava anemia moderada e trombocitose, sendo diagnosticada como metaplasia mieloide com mielofibrose. Foi realizado o cariotipo na MO, observando-se a presença de uma duplicação do braço longo do cromossoma 1 na região (1) (q21q32). A doente manteve-se estável e sem terapêutica até início 2009, tendo nesta altura realizado uma reavaliação do mielograma, e biopsia óssea que evidenciou uma mielofibrose em fase avançada. Fez em paralelo estudos por citogenética e a pesquisa da mutação V617F em Jak2, evidenciando a inexistência da referida mutação e confirmando-se a presença da duplicação. Iniciou terapêutica com substituto de eritropoietina. Atualmente, com 8 anos de evolução da doença apresenta-se sem queixas clínicas, independente de transfusões e mantendo a terapêutica.
A duplicação da região (1) (q21q32) associada a NMP é uma anomalia rara, estando descritos 5 casos, dos quais apenas 2 são MFP. Dado que a doente em estudo não apresenta evolução para LMA (ao contrário do descrito por outros autores em neoplasmas mieloides (NM)) foram realizados estudos por citogenética molecular (FISH, e microarray de alta resolução), que confirmaram os pontos de quebra observados e não evidenciaram outras alterações para além da descrita. Com base na história clinica da doente e nos resultados, pode-se sugerir que a duplicação da região (1) (q21q32) por si só não induz uma evolução para LMA e que a existência de duplicação de genes correlacionados com esta patologia (ex: ARNT; entre outros) não é fator suficiente para uma progressão mais agressiva, contudo deverão ser realizados mais estudos com o intuito de esclarecer o papel desta alteração nos NM
Gravidez e tabagismo : uma oportunidade para mudar comportamentos
Introduction: Maternal smoking during pregnancy is related not only to perinatal
adverse events but also to important postnatal problems. Smoking is very prevalent in women with several socio-demographic factors playing an important role. Aims: To assess the frequency of smoking as well as mothers’ change in behaviours during pregnancy. To identify socio-demographic factors associated to smoking and to assess women’s knowledge and sources of information about adverse effects of smoking during pregnancy.Methods: We started a cohort study between March and October 2003 at the Maternity, Hospital Santa Maria. After an informed consent the mothers in the post natal ward were asked to fill in a questionnaire. Results: Four hundred and seventy five mothers responded to the questionnaire and among them 30% were smokers before pregnancy. Thirty five percent stopped smoking
during pregnancy and those who continued significantly reduced the number of
cigarrettes per day. Smoking was less prevalent in married women, with higher
educational level and with stable employment (statistically significant correlations).
Forty five percent of the mothers were misinformed or had no information about the
adverse effects of smoking during pregnancy. Their main source of information were the media. Discussion: Smoking prevalence was high (30%) among the respondents but the
pregnancy lowered the numbers of cigarrettes per day. The percentage of misinformed
mothers concerned about the adverse effects of smoking during pregnancy was high (45%), which also showed that doctors’ role as a source of information was surpassed by the media. The study also brought to light the need for the health professionals to improve their competences in their role on prevention of smoking during pregnancy and the postnatal period
Co-activation of upper limb muscles during reaching in post-stroke subjects: An analysis of the contralesional and ipsilesional limbs
The purpose of this study was to analyze the change in antagonist co-activation ratio of upper-limb muscle pairs, during the reaching movement, of both ipsilesional and contralesional limbs of post-stroke subjects. Nine healthy and nine post-stroke subjects were instructed to reach and grasp a target, placed in the sagittal and scapular planes of movement. Surface EMG was recorded from postural control and movement related muscles. Reaching movement was divided in two sub-phases, according to proximal postural control versus movement control demands, during which antagonist co-activation ratios were calculated for the muscle pairs LD/PM, PD/AD, TRIlat/BB and TRIlat/BR. Post-stroke's ipsilesional limb presented lower co-activation in muscles with an important role in postural control (LD/PM), comparing to the healthy subjects during the first sub-phase, when the movement was performed in the sagittal plane (p<0.05). Conversely, the post-stroke's contralesional limb showed in general an increased co-activation ratio in muscles related to movement control, comparing to the healthy subjects. Our findings demonstrate that, in post-stroke subjects, the reaching movement performed with the ipsilesional upper limb seems to show co-activation impairments in muscle pairs associated to postural control, whereas the contralesional upper limb seems to have signs of impairment of muscle pairs related to movement.info:eu-repo/semantics/publishedVersio
Avaliação da qualidade e segurança microbiológica de refeições de sushi prontas a consumir em Portugal
Being a food product that contains perishable ingredients
and involves a significant degree of manual handling during
preparation, sushi is regarded as a potentially hazardous
food, which may lead to foodborne disease outbreaks. In
Portugal, consumption of takeaway sushi meals has strongly
increased throughout the past few years; however, there is
limited information regarding its compliance with food
quality standards. Under this context, the present study
aimed to evaluate the microbiological quality and safety of
take-away ready-to-eat sushi meals in Lisbon, Portugal. Sixty-
two samples were collected from different origins (restaurant
and hypermarket), and each sample was tested for aerobic
mesophilic microorganisms, Enterobacteriaceae, Escherichia coli, positive coagulase Staphylococci, presumptive
Bacillus cereus count, as for detection of pathogenic microorganisms,
such as Salmonella spp., Listeria monocytogenes
and Vibrio parahaemolyticus, V. cholerae and V. vulnificus. Results
revealed that 48.4% (30/62) were deemed unsatisfactory,
35.5% (22/62) were classified as borderline and only
16.1% (10/62) were considered satisfactory. Even though we
did not detect the incidence of potentially pathogenic microorganisms
in sushi, the presence of B. cereus and coagulase-
positive Staphylococci was detected at unsatisfactory
levels. Furthermore, significant differences between the
place of origin (restaurant vs. hypermarket) and type of fish
were also observed. Overall, the high number of samples
classified with a level of microbiological quality “unsatisfactory”
and “borderline” highlights the need to review good
hygiene practices, as well as the quality of the raw materials
used, to obtain a final product with a satisfactory quality and
safety levelRESUMO - Por ser um género alimentício que contém ingredientes
perecíveis e envolve um grau significativo de manipulação
manual durante a sua preparação, o sushi é considerado
um alimento potencialmente perigoso, que pode
causar surtos de doença de origem alimentar. Em Portugal,
o consumo de refeições de sushi prontas a consumir
tem aumentado ao longo dos últimos anos. No entanto, a
informação sobre o cumprimento das normas de qualidade
alimentar é limitada. Neste contexto, o presente estudo
teve como objetivo avaliar a qualidade e a segurança
microbiológica de refeições de sushi prontas para consumo
em take-way, na região de Lisboa, Portugal. Foram
colhidas 62 amostras de diferentes origens (restaurante e
hipermercado), e em cada amostra foi efetuada a contagem
de microrganismos aeróbios mesófilos, Enterobacteriaceae,
Escherichia coli, estafilococos coagulase positiva,
Bacillus cereus presuntivos, e deteção de microrganismos
patogénicos, tais como: Salmonella spp., Listeria monocytogenes,
Vibrio parahaemolyticus, Vibrio cholerae e Vibrio
vulnificus. Os resultados revelaram que 48,4% (30/62) das
amostras foram consideradas insatisfatórias, 35,5%
(22/62) foram classificadas como “borderline” e apenas
16,1% (10/62) foram consideradas como satisfatórias. Embora
não tenham sido detetados microrganismos potencialmente
patogénicos nas amostras de sushi, a presença
de B. cereus e estafilococos coagulase positivos foram detetados
em níveis insatisfatórios. Além disso, também
foram observadas diferenças significativas entre o local
de origem (restaurante vs. hipermercado) e tipo de peixe.
No geral, o elevado número de amostras classificadas
com um nível de qualidade microbiológica insatisfatória
e “borderline” evidência a necessidade de revisão das
boas práticas de higiene, bem como da qualidade das matérias-
primas utilizadas, para obter um produto final com
