Predicting health-related quality of life (EQ-5D-5 L) and capability wellbeing (ICECAP-A) in the context of opiate dependence using routine clinical outcome measures:CORE-OM, LDQ and TOP
BACKGROUND: Economic evaluation normally requires information to be collected on outcome improvement using utility values. This is often not collected during the treatment of substance use disorders making cost-effectiveness evaluations of therapy difficult. One potential solution is the use of mapping to generate utility values from clinical measures. This study develops and evaluates mapping algorithms that could be used to predict the EuroQol-5D (EQ-5D-5 L) and the ICEpop CAPability measure for Adults (ICECAP-A) from the three commonly used clinical measures; the CORE-OM, the LDQ and the TOP measures. METHODS: Models were estimated using pilot trial data of heroin users in opiate substitution treatment. In the trial the EQ-5D-5 L, ICECAP-A, CORE-OM, LDQ and TOP were administered at baseline, three and twelve month time intervals. Mapping was conducted using estimation and validation datasets. The normal estimation dataset, which comprised of baseline sample data, used ordinary least squares (OLS) and tobit regression methods. Data from the baseline and three month time periods were combined to create a pooled estimation dataset. Cluster and mixed regression methods were used to map from this dataset. Predictive accuracy of the models was assessed using the root mean square error (RMSE) and the mean absolute error (MAE). Algorithms were validated using sample data from the follow-up time periods. RESULTS: Mapping algorithms can be used to predict the ICECAP-A and the EQ-5D-5 L in the context of opiate dependence. Although both measures can be predicted, the ICECAP-A was better predicted by the clinical measures. There were no advantages of pooling the data. There were 6 chosen mapping algorithms, which had MAE scores ranging from 0.100 to 0.138 and RMSE scores ranging from 0.134 to 0.178. CONCLUSION: It is possible to predict the scores of the ICECAP-A and the EQ-5D-5 L with the use of mapping. In the context of opiate dependence, these algorithms provide the possibility of generating utility values from clinical measures and thus enabling economic evaluation of alternative therapy options. Achieving integrated treatment: a realist synthesis of service models and systems for co-existing serious mental health and substance use conditions.
Approximately 30-50% of people with serious mental illness have co-existing drug or alcohol problems (COSMHAD), associated with adverse health and social care outcomes. UK guidelines advocate both co-occurring needs being met within mental health services, but uncertainty remains about how to operationalise this to improve outcomes. Various unevaluated service configurations exist in the UK. A realist synthesis was done to identify, test, and refine programme theories of how context shapes the mechanisms through which UK service models for COSMHAD work, for whom, and in what circumstances. Structured and iterative realist searches of seven databases identified 5099 records. A two-stage screening process identified 132 papers. Three broad contextual factors shaped COSMHAD services across 11 programme theories: committed leadership, clear expectations regarding COSMHAD from mental health and substance use workforces, and clear care-coordination processes. These contextual factors led to increased staff empathy, confidence, legitimisation, and multidisciplinary ethos, which improved care coordination and increased the motivation of people with COSMHAD to work towards their goals. Our synthesis highlights that integrating COSMHAD care is complex, and both individual and cultural behavioural shifts in leadership, workforce, and service delivery are essential to ensure people with COSMHAD receive compassionate, trauma-informed care that meets their needs

Colorimetria do fruto como indicador do teor de bixina em sementes de urucum
O estudo objetivou relacionar a colorimetria do epicarpo dos frutos com o teor de bixina em sementes de urucum (Bixa orellana L.). A bixina é utilizada em diversos segmentos industriais, como farmacêutico, têxtil, de cosméticos e alimentício. Os frutos foram colhidos de árvores adultas e classificados visualmente em quatro estádios de maturação com base na coloração externa do epicarpo. Após a segregação visual, mensurou-se a coloração do fruto com um analisador digital de cores. A caracterização inicial se deu através da morfometria das sementes e da determinação do teor de água. O experimento seguiu delineamento inteiramente ao acaso, contendo quatro repetições. Frutos verdes e marrom-claro fechados apresentaram sementes com maior tamanho e teor de bixina. Para obtenção de bixina as sementes podem ser colhidas quando o fruto estiver fechado, com coloração visual verde e/ou marrom-claro ou com valores de reflectância no intervalo de 93-162; 61-144 ou 35-62 nm nas escalas vermelho, verde ou azul, respectivamente, do espectro de cores
