Critical transformation in higher education: Ethical reflections on #
Protest movements such as the #MustFall currently dominate the South African higher education landscape. This article focuses on such protest movements, paying particular attention to protests against gender-based violations at some universities, commonly referred to as #RapeMustFall, as an exemplar of the gender injustices and inequities that persist. We argue that debates and policies about gender-based violations at universities cannot and should not be overshadowed by deficient grand narratives informed by patriarchy, colonialism and capitalism. To frame this argument, we critically review the current status quo from a gender mainstreaming policy-making perspective. We then argue the merits of an ethical perspective to transformation in higher education. Physiotherapists’ perception of a community-based primary healthcare clinical education approach to undergraduate learning
Background. South African health systems are challenged by numerous stressors, such as a lack of resources, staff shortages and overburdened public sector demands. This necessitates appropriately equipped and trained healthcare professionals to meet the demands of this system. Community-based primary healthcare (PHC) clinical education is an approach towards preparing health science students to meet these demands. Clinical education is the cornerstone of undergraduate training. Physiotherapists are among the healthcare professionals who require undergraduate training that drives competence for independent practice.Objective. To explore the perceptions and experiences of physiotherapists as clinical supervisors within a physiotherapy undergraduate programme that adopted a community-based PHC approach to clinical training.Methods. An explorative qualitative approach was used, with semi-structured interviews with 10 purposively selected physiotherapists supervising students on the newly introduced platform. Data were transcribed and analysed using content analysis.Results. Seven themes emerged from the data, which relate to curriculum redress, organisational factors, stakeholder dynamics, barriers and enablers to decentralised clinical training, perceived preparedness for practice and recommendations.Conclusions. As the need for an increasing number of health professionals is realised, more innovative methods for clinical education of undergraduate health science students are required. Community-based PHC training for physiotherapy students is one such approach and was generally perceived as a valuable framework to incorporate competencies required for practice as future independent practitioners. Furthermore, improved communication between students, clinicians and academic staff was seen as a recommendation to influence clinical education
Serum resistin is associated with C-reactive protein and LDL- cholesterol in type 2 diabetes and coronary artery disease in a Saudi population
Aims
Resistin is an adipocyte-derived factor implicated in obesity-associated type 2 diabetes (T2DM). This study examines the association between human serum resistin, T2DM and coronary heart disease.
Methods
One hundred and fourteen Saudi Arabian patients (male: female ratio 46:68; age 51.4 (mean ± SD)11.7 years; median and range: 45.59 (11.7) years and BMI: 27.1 (mean ± SD) 8.1 Kgm2 median and range: 30.3 (6.3) were studied. Serum resistin and C-reactive protein (CRP), a marker of inflammation CRP levels, were measured in all subjects. (35 patients had type 2 diabetes mellitus (T2DM); 22 patients had coronary heart disease (CHD).
Results
Serum resistin levels were 1.2-fold higher in type 2 diabetes and 1.3-fold higher in CHD than in controls (p = 0.01). In addition, CRP was significantly increased in both T2DM and CHD patients (p = 0.007 and p = 0.002 respectively). The use of regression analysis also determined that serum resistin correlated with CRP levels (p = 0.04, R2 0.045).
Conclusion
The findings from this study further implicate resistin as a circulating protein associated with T2DM and CHD. In addition this study also demonstrates an association between resistin and CRP, a marker of inflammation in type 2 diabetic patients
Rubinstein-Taybi syndrome:dental manifestations and management
Rubinstein-Taybi syndrome (RSTS) is an uncommon genetic disorder characterised by a typical facies, small stature, broad angulated thumbs and intellectual impairment. Dental changes are a minor, yet significant component of the condition. Craniofacial growth retardation in RSTS is frequently complicated by unerupted teeth, while dental caries is related to the inherent intellectual deficit. Dental problems necessitate interdisciplinary management in terms of oral surgery, conservative dentistry, periodontics and orthodontics. When affected individuals are unco-operative, certain dental procedures may warrant general anaesthesia. In these instances, dental and medical staff will combine their expertise to enhance the well-being of the patient. In addition, specific dental changes may alert the medical practitioner to the possible diagnosis of RSTS. In this article we document the oro-dental manifestations and review the oro-dental approach in the management of three patients with RSTS. Our experience in South Africa may be relevant to other countries at a similar stage of development

High Transmitter CD4+ T-Cell Count Shortly after the Time of Transmission in a Study of African Serodiscordant Couples.
© 2015 Karita et al.Background: 2013 WHO guidelines recommend starting ART at CD4+ T-cell counts ≤500 cells/μL. We present the T-cell counts from adult Africans with HIV shortly following transmission to their sexual partners. Methods: HIV-discordant couples in Zambia, Uganda and Rwanda were followed prospectively and received couples counseling and condoms. HIV uninfected partners were tested for HIV at least quarterly and HIV-infected partners received HIV care and referral for ART per national guidelines. Upon diagnosis of incident HIV infection in the previously HIV-uninfected partner, a blood sample was collected from both partners to measure CD4+ T-cells and perform viral linkage. The estimated date of infection (EDI) of the incident case was calculated based on testing history. EDI was unknown for suspected transmitting partners. Results: From 2006-2011, 4,705 HIV-discordant couples were enrolled in this cohort, and 443 cases of incident HIV infection were documented. Virus linkage analysis was performed in 374 transmission pairs, and 273 (73%) transmissions were linked genetically. CD4 counts in the transmitting partner were measured a median of 56 days after EDI (mean:90.5, min:10, max:396). The median CD4 count was 339 cells/μl (mean:386.4, min:15, max:1,434), and the proportion of partners with a CD4+ T-cell count above 500/μl was 25% (95% CI:21, 31). Conclusions: In our cohort of discordant couples, 73% of HIV transmissions occurred within the relationship, and the transmitter CD4+ T cell count shortly after the transmission event was frequently higher than the WHO 2013 ART-initiation guidelines. Competition between Magnetic and Structural Transition in CrN
CrN is observed to undergo a paramagnetic to antiferromagnetic transition
accompanied by a shear distortion from cubic NaCl-type to orthorhombic
structure. Our first-principle plane wave and ultrasoft pseudopotential
calculations confirm that the distorted antiferromagnetic phase with spin
configuration arranged in double ferromagnetic sheets along [110] is the most
stable. Antiferromagnetic ordering leads to a large depletion of states around
Fermi level, but it does not open a gap. Simultaneous occurence of structural
distortion and antiferromagnetic order is analyzed.Comment: 10 pages, 10 figure
First-principles calculations of the self-trapped exciton in crystalline NaCl
The atomic and electronic structure of the lowest triplet state of the
off-center (C2v symmetry) self-trapped exciton (STE) in crystalline NaCl is
calculated using the local-spin-density (LSDA) approximation. In addition, the
Franck-Condon broadening of the luminescence peak and the a1g -> b3u absorption
peak are calculated and compared to experiment. LSDA accurately predicts
transition energies if the initial and final states are both localized or
delocalized, but 1 eV discrepancies with experiment occur if one state is
