    A pragmatic randomized waitlist-controlled effectiveness and cost-effectiveness trial of digital interventions for depression and anxiety

    Utilization of internet-delivered cognitive behavioural therapy (iCBT) for treating depression and anxiety disorders in stepped-care models, such as the UK’s Improving Access to Psychological Therapies (IAPT), is a potential solution for addressing the treatment gap in mental health. We investigated the effectiveness and cost-effectiveness of iCBT when fully integrated within IAPT stepped-care settings. We conducted an 8-week pragmatic randomized controlled trial with a 2:1 (iCBT intervention: waiting-list) allocation, for participants referred to an IAPT Step 2 service with depression and anxiety symptoms (Trial registration: ISRCTN91967124). The primary outcomes measures were PHQ-9 (depressive symptoms) and GAD-7 (anxiety symptoms) and WSAS (functional impairment) as a secondary outcome. The cost-effectiveness analysis was based on EQ-5D-5L (preference-based health status) to elicit the quality-adjust life year (QALY) and a modified-Client Service Receipt Inventory (care resource-use). Diagnostic interviews were administered at baseline and 3 months. Three-hundred and sixty-one participants were randomized (iCBT, 241; waiting-list, 120). Intention-to-treat analyses showed significant interaction effects for the PHQ-9 (b = −2.75, 95% CI −4.00, −1.50) and GAD-7 (b = −2.79, 95% CI −4.00, −1.58) in favour of iCBT at 8-week and further improvements observed up to 12-months. Over 8-weeks the probability of cost-effectiveness was 46.6% if decision makers are willing to pay £30,000 per QALY, increasing to 91.2% when the control-arm’s outcomes and costs were extrapolated over 12-months. Results indicate that iCBT for depression and anxiety is effective and potentially cost-effective in the long-term within IAPT. Upscaling the use of iCBT as part of stepped care could help to enhance IAPT outcomes. The pragmatic trial design supports the ecological validity of the findings

    A multiscale analysis of gene flow for the New England cottontail, an imperiled habitat specialist in a fragmented landscape

    Landscape features of anthropogenic or natural origin can influence organisms\u27 dispersal patterns and the connectivity of populations. Understanding these relationships is of broad interest in ecology and evolutionary biology and provides key insights for habitat conservation planning at the landscape scale. This knowledge is germane to restoration efforts for the New England cottontail (Sylvilagus transitionalis), an early successional habitat specialist of conservation concern. We evaluated local population structure and measures of genetic diversity of a geographically isolated population of cottontails in the northeastern United States. We also conducted a multiscale landscape genetic analysis, in which we assessed genetic discontinuities relative to the landscape and developed several resistance models to test hypotheses about landscape features that promote or inhibit cottontail dispersal within and across the local populations. Bayesian clustering identified four genetically distinct populations, with very little migration among them, and additional substructure within one of those populations. These populations had private alleles, low genetic diversity, critically low effective population sizes (3.2-36.7), and evidence of recent genetic bottlenecks. Major highways and a river were found to limit cottontail dispersal and to separate populations. The habitat along roadsides, railroad beds, and utility corridors, on the other hand, was found to facilitate cottontail movement among patches. The relative importance of dispersal barriers and facilitators on gene flow varied among populations in relation to landscape composition, demonstrating the complexity and context dependency of factors influencing gene flow and highlighting the importance of replication and scale in landscape genetic studies. Our findings provide information for the design of restoration landscapes for the New England cottontail and also highlight the dual influence of roads, as both barriers and facilitators of dispersal for an early successional habitat specialist in a fragmented landscape

    Proverb interpretation in fluent aphasia and Alzheimer's disease: Implications beyond abstract thinking

    This study compared proverb processing across three groups, i.e. patients with fluent aphasia (APH), patients with Alzheimer's Disease (AD), and normal control subjects (NC). Proverb stimuli were used to examine the effects of group membership and proverb familiarity in two presentation formats (i.e. spontaneous versus multiple-choice) on performance. The sensitivity of cognitive measures as predictors of ability to interpret proverbs was also investigated. In relation to NC subjects, patients with fluent APH exhibited significant difficulty formulating responses for familiar and unfamiliar spontaneous proverbs, whereas patients with AD demonstrated lower performance only on the unfamiliar proverbs. On the multiple-choice paradigm, however, patients with APH exhibited minimal difficulty. Conversely, the patients with AD manifested significant problems selecting the correct abstract response for familiar proverbs. With regard to predictors, language was relevant to familiar proverb interpretations and to proverbs presented in the spontaneous format. Cognition was a sensitive predictor for unfamiliar proverb interpretations and to the multiple-choice format. Deficits on the proverb tasks are discussed with reference to the potential breakdown of underlying linguistic and cognitive processes. The present data support the diagnostic value of proverbs in elucidating brain- behaviour relationships

    Movement velocity as a measure of exercise intensity in three lower limb exercises

    The purpose of this study was to investigate the relationship between movement velocity and relative load in three lower limbs exercises commonly used to develop strength: Leg Press, Full Squat, and Half Squat. The percentage of one Repetition Maximum (%1RM) has typically been used as the main parameter to control resistance training, however more recent research has proposed movement velocity as an alternative. Fifteen participants performed a load progression with a range of loads until they reached their one repetition maximum (1RM). Maximum instantaneous velocity V max) and mean propulsive velocity (MPV) of the knee extension phase of each exercise were assessed. For all exercises, a strong relationship between V max and the %1RM was found: Leg Press (r2adj=0.96; 95% CI for slope is [-0.0244,-0.0258], p<0.0001), Full Squat (r2adj=0.94; 95% CI for slope is [-0.0144, -0.0139], p<0.0001), and Half Squat (r2adj=0.97; 95% CI for slope is [-0.0135, -0.00143], p<0.0001), and for MPV, Leg Press (r2adj=0.96; 95% CI for slope is [- 0.0169, -0.0175], p<0.0001, Full Squat (r2adj=0.95; 95% CI for slope is [-0.0136, -0.0128], p<0.0001), and Half Squat (r2adj=0.96; 95% CI for slope is [-0.0116, 0.0124], p<0.0001). The 1RM was attained with a MPV and Vmax of 0.21±0.06 ms-1 and 0.63±0.15 ms-1, 0.29±0.05 ms-1 and 0.89±0.17 ms-1, 0.33±0.05 ms-1, and 0.95±0.13 ms-1 for Leg Press, Full Squat and Half Squat respectively. Results indicate that it is possible to determine an exercise-specific %1RM by measuring movement velocity for that exercise

    Search for Narrow Diphoton Resonances and for gamma-gamma+W/Z Signatures in p\bar p Collisions at sqrt(s)=1.8 TeV

    We present results of searches for diphoton resonances produced both inclusively and also in association with a vector boson (W or Z) using 100 pb^{-1} of p\bar p collisions using the CDF detector. We set upper limits on the product of cross section times branching ratio for both p\bar p\to\gamma\gamma + X and p\bar p\to\gamma\gamma + W/Z. Comparing the inclusive production to the expectations from heavy sgoldstinos we derive limits on the supersymmetry-breaking scale sqrt{F} in the TeV range, depending on the sgoldstino mass and the choice of other parameters. Also, using a NLO prediction for the associated production of a Higgs boson with a W or Z boson, we set an upper limit on the branching ratio for H\to\gamma\gamma. Finally, we set a lower limit on the mass of a `bosophilic' Higgs boson (e.g. one which couples only to \gamma, W, and Z$ bosons with standard model couplings) of 82 GeV/c^2 at 95% confidence level.Comment: 30 pages, 11 figure

    Measurement of the cross-section and charge asymmetry of WW bosons produced in proton-proton collisions at s=8\sqrt{s}=8 TeV with the ATLAS detector

    This paper presents measurements of the W+μ+νW^+ \rightarrow \mu^+\nu and WμνW^- \rightarrow \mu^-\nu cross-sections and the associated charge asymmetry as a function of the absolute pseudorapidity of the decay muon. The data were collected in proton--proton collisions at a centre-of-mass energy of 8 TeV with the ATLAS experiment at the LHC and correspond to a total integrated luminosity of 20.2~\mbox{fb^{-1}}. The precision of the cross-section measurements varies between 0.8% to 1.5% as a function of the pseudorapidity, excluding the 1.9% uncertainty on the integrated luminosity. The charge asymmetry is measured with an uncertainty between 0.002 and 0.003. The results are compared with predictions based on next-to-next-to-leading-order calculations with various parton distribution functions and have the sensitivity to discriminate between them.Comment: 38 pages in total, author list starting page 22, 5 figures, 4 tables, submitted to EPJC.     Measurement of the lepton charge asymmetry in W-boson decays produced in p-pbar collisions

    We describe a measurement of the charge asymmetry of leptons from W boson decays in the rapidity range 0 enu, munu events from 110+/-7 pb^{-1}of data collected by the CDF detector during 1992-95. The asymmetry data constrain the ratio of d and u quark momentum distributions in the proton over the x range of 0.006 to 0.34 at Q2 \approx M_W^2. The asymmetry predictions that use parton distribution functions obtained from previously published CDF data in the central rapidity region (0.0<|y_l|<1.1) do not agree with the new data in the large rapidity region (|y_l|>1.1).Comment: 13 pages, 3 tables, 1 figur

    Inclusive Search for Anomalous Production of High-pT Like-Sign Lepton Pairs in Proton-Antiproton Collisions at sqrt{s}=1.8 TeV

    We report on a search for anomalous production of events with at least two charged, isolated, like-sign leptons with pT > 11 GeV/c using a 107 pb^-1 sample of 1.8 TeV ppbar collisions collected by the CDF detector. We define a signal region containing low background from Standard Model processes. To avoid bias, we fix the final cuts before examining the event yield in the signal region using control regions to test the Monte Carlo predictions. We observe no events in the signal region, consistent with an expectation of 0.63^(+0.84)_(-0.07) events. We present 95% confidence level limits on new physics processes in both a signature-based context as well as within a representative minimal supergravity (tanbeta = 3) model.Comment: 15 pages, 4 figures.     Measurement of the Strong Coupling Constant from Inclusive Jet Production at the Tevatron pˉp\bar pp Collider

    We report a measurement of the strong coupling constant, αs(MZ)\alpha_s(M_Z), extracted from inclusive jet production in ppˉp\bar{p} collisions at s=\sqrt{s}=1800 GeV. The QCD prediction for the evolution of αs\alpha_s with jet transverse energy ETE_T is tested over the range 40<ETE_T<450 GeV using ETE_T for the renormalization scale. The data show good agreement with QCD in the region below 250 GeV. In the text we discuss the data-theory comparison in the region from 250 to 450 GeV. The value of αs\alpha_s at the mass of the Z0Z^0 boson averaged over the range 40<ETE_T<250 GeV is found to be αs(MZ)=0.1178±0.0001(stat)0.0095+0.0081(exp.syst)\alpha_s(M_{Z})= 0.1178 \pm 0.0001{(\rm stat)}^{+0.0081}_{-0.0095}{\rm (exp. syst)}. The associated theoretical uncertainties are mainly due to the choice of renormalization scale (^{+6%}_{-4%}) and input parton distribution functions (5%).Comment: 7 pages, 3 figures, using RevTeX.
