    Preoperative and postoperative risk factors for periprosthetic joint infection in primary total hip arthroplasty: A 1-year experience

    BACKGROUND Periprosthetic joint infection (PJI) in primary total hip replacement (THR) is one of the most important threats in orthopedic surgery, so one important surgeon's target is to avoid or early diagnose a PJI. Although the incidence of PJI is very low (0.69%) in our department, with an average follow-up of 595 d, this infection poses a serious threat due to the difficulties of treatment and the lower functional outcomes after healing. AIM To study the incidence of PJI in all operations occurring in the year 2016 in our department to look for predictive signs of potential infection. METHODS We counted 583 THR for 578 patients and observed only 4 cases of infection (0.69%) with a mean follow-up of 596 d (min 30, max 1451). We reviewed all medical records to collect the data: duration and time of the surgery, presence, type and duration of the antibiotic therapy, preoperative diagnosis, blood values before and after surgery, transfusions, presence of preoperative drugs (in particularly anticoagulants and antiaggregant, corticosteroids and immunosuppressants), presence of some comorbidities (high body mass index, blood hypertension, chronic obstructive pulmonary disease, cardiac ischemia, diabetes, rheumatological conditions, previous local infections). RESULTS No preoperative, intraoperative, or postoperative analysis showed a higher incidence of PJI. We did not find any class with evident major odds of PJI. In our study, we did not find any border value to predict PJI and all patients had similar values in both groups (non-PJI and PJI). Only some categories, such as female patients, showed more frequency of PJI, but this difference related to sex was not statistically significant. CONCLUSION We did not find any category with a higher risk of PJI in THR, probably due to the lack of few cases of infection

    Influence of Autochthonous Putative Probiotic Cultures on Microbiota, Lipid Components and Metabolome of Caciotta Cheese

    The present study was undertaken to produce probiotic Caciotta cheeses from pasteurized ewes' milk by using different combinations of autochthonous microbial cultures, containing putative probiotic strains, and evaluate their influence on gross composition, lipid components, sensory properties and microbiological and metabolite profiles of the cheeses throughout ripening process. A control cheese was produced using commercial starter cultures. The hydrophilic molecular pools (mainly composed by amino acids, organic acids, and carbohydrates) were characterized by means of H-1 NMR spectroscopy, while the cholesterol, alpha-tocopherol and fatty acid composition by HPLC-DAD/ELSD techniques. Conventional culturing and a PCR-DGGE approach using total cheese DNA extracts were used to analyze cheese microbiota and monitor the presence and viability of starters and probiotic strains. Our findings showed no marked differences for gross composition, total lipids, total cholesterol, and fatty acid levels among all cheeses during ripening. Differently, the multivariate statistical analysis of NMR data highlighted significant variations in the cheese' profiles both in terms of maturation time and strains combination. The use of autochthonous cultures and adjunct probiotic strains did not adversely affect acceptability of the cheeses. Higher levels of lactobacilli (viability of 10(8)-10(9) cfu/g of cheese) were detected in cheeses made with the addition of probiotic autochthonous strains with respect to control cheese during the whole ripening period, suggesting the adequacy of Caciotta cheese as a carrier for probiotic bacteria delivery

    Electrospun Silk Fibroin Scaffolds for Tissue Regeneration: Chemical, Structural, and Toxicological Implications of the Formic Acid-Silk Fibroin Interaction

    The dissolution of Bombyx mori silk !broin (SF) !lms in formic acid (FA) for the preparation of electrospinning dopes is widely exploited to produce electrospun SF scaffolds. The SilkBridge® nerve conduit is an example of medical device having in its wall structure an electrospun component produced from an FA spinning dope. Though highly volatile, residual FA remains trapped into the bulk of the SF nano!bers. The purpose of this work is to investigate the type and strength of the interaction between FA and SF in electrospun mats, to quantify its amount and to evaluate its possible toxicological impact on human health. The presence of residual FA in SF mats was detected by FTIR and Raman spectroscopy (new carbonyl peak at about 1,725 cm!1) and by solid state NMR, which revealed a new carbonyl signal at about 164.3 ppm, attributed to FA by isotopic 13C substitution. Changes occurred also in the spectral ranges of hydroxylated amino acids (Ser and Thr), demonstrating that FA interacted with SF by forming formyl esters. The total amount of FA was determined by HS-GC/MS analysis and accounted for 247 ± 20 !mol/g. The greatest part was present as formyl ester, a small part (about 3%) as free FA. Approximately 17% of the 1,500 !mol/g of hydroxy amino acids (Ser and Thr) theoretically available were involved in the formation of formyl esters. Treatment with alkali (Na2CO3) succeeded to remove the greatest part of FA, but not all. Alkali-treated electrospun SF mats underwent morphological, physical, and mechanical changes. The average diameter of the !bers increased from about 440 nm to about 480 nm, the mat shrunk, became stiffer (the modulus increased from about 5.5 MPa to about 7 MPa), and lost elasticity (the strain decreased from about 1 mm/mm to about 0.8 mm/mm). Biocompatibility studies with human adult dermal !broblasts did not show signi!cant difference in cell proliferation (313 ± 18 and 309 ± 23 cells/ mm2 for untreated and alkali-treated SF mat, respectively) and metabolic activity. An in-depth evaluation of the possible toxicological impact of residual FA was made using the SilkBridge® nerve conduit as case study, following the provisions of the ISO 10993-1 standard. The Potential Patient Daily Intake, calculated from the total amount of FA determined by HS-GC/MS, was 2.4 mg/day and the Tolerable Exposure level was set to 35.4 mg/day

    Saipem My Health Record: a model of an electronic health record for the management of workers' health

    Introduction. Saipem is a contractor company in Oil and Gas industry. Saipem has developed an in-house medical department, whose main objective, apart the management of medical emergencies, is prevention, health protection and promotion. In order to make these principals available and applicable at all site worldwide Saipem decided to initiate and develop the implementation of e-medicine in its daily operations. Methods.The challenging goal of the Saipem Medical Service is to make available to all employees their health data, quickly, easily accessible and secure. The basis for achieving these objectives is the adoption of standards that allow the exchange of data between patient, public and private structures thus ensuring the interoperability of information. My Health Records (MHR) is the program that allows the visualization, consultation and sharing of health information and data that constitute the clinical and health history of each Saipem worker. My HR makes it possible to have secure and exclusive access to your healthcare profile, its portability and the consequent possibility to consult it and show it in case of need, even from remote extemporaneous stations from any device connected to the internet (PC, smartphone or tablet) . The digitization of health documents through MHR is aimed at ensuring a) faster and more direct communication between employer, medical staff and employee, b) immediate usability and portability of data c) the availability of a vast amount of data in a single space d) a reduction in costs through the dematerialization of the data Results.In order to assess the impact of MHR on the working population, Saipem has prepared to send a satisfaction questionnaire to approximately 6.000 employees in order to understand the impact on the working population and lead to even more innovative solutions. The purpose of the questionnaire is also to sensitize the user / employee in the autonomous and completely free of this application. The result are show and discussed. Conclusion. MHR was created to offer all Saipem employees the opportunity to access their health records. This project represents a model to be proposed to all Italian companies that, following the dictates of Legislative Decree 81/08, apply Health Surveillance to their workers. The advantages are represented by the dematerialization of the data, the relative cost saving (shipment of health documentation, reduction of hours / work dedicated to the expiry of visits, possibility for public health to use health data without repetition of clinical and instrumental tests, vaccinations, etc) and greater attention to all Italian workers, both EU and non-EU, with a definitely positive impact on Public Health. The strategy of the MHR launch is proving successful, and from 1 June 2017 to 1 June 2018 16,764 accesses were registered

    Hierarchical TiN-Supported TsFDH Nanobiocatalyst for CO2 Reduction to Formate

    AbstractThe electrochemical reduction of CO2 to value‐added products like formate represents a promising technology for the valorization of carbon dioxide. We propose a proof‐of‐concept bioelectrochemical system (BES) for the reduction of CO2 to formate. For the first time, our device employs a nanostructured titanium nitride (TiN) support for the immobilization of a formate dehydrogenase (FDH) enzyme. The hierarchical TiN nanostructured support exhibits high surface area and wide pore size distribution, achieving high catalytic loading, and is characterized by higher conductivity than other oxide‐based supports employed for FDHs immobilization. We select the oxygen‐tolerant FDH from Thiobacillus sp. KNK65MA (TsFDH) as enzymatic catalyst, which selectively reduces CO2 to formate. We identify an optimal TiN morphology for the enzyme immobilisation through enzymatic assay, reaching a catalyst loading of 59 μg cm−2 of specifically‐adsorbed TsFDH and achieving a complete saturation of the anchoring sites available on the surface. We evaluate the electrochemical CO2 reduction performance of the TiN/TsFDH system, achieving a remarkable HCOO− Faradaic efficiency up to 76 %, a maximum formate yield of 44.1 μmol mg−1FDH h−1 and high stability. Our results show the technological feasibility of BES devices employing novel, nanostructured TiN‐based supports, representing an important step in the optimization of these devices

    Enisamium Inhibits SARS-CoV-2 RNA Synthesis.

    Pandemic SARS-CoV-2 causes a mild to severe respiratory disease called coronavirus disease 2019 (COVID-19). While control of the SARS-CoV-2 spread partly depends on vaccine-induced or naturally acquired protective herd immunity, antiviral strategies are still needed to manage COVID-19. Enisamium is an inhibitor of influenza A and B viruses in cell culture and clinically approved in countries of the Commonwealth of Independent States. In vitro, enisamium acts through metabolite VR17-04 and inhibits the activity of the influenza A virus RNA polymerase. Here we show that enisamium can inhibit coronavirus infections in NHBE and Caco-2 cells, and the activity of the SARS-CoV-2 RNA polymerase in vitro. Docking and molecular dynamics simulations provide insight into the mechanism of action and indicate that enisamium metabolite VR17-04 prevents GTP and UTP incorporation. Overall, these results suggest that enisamium is an inhibitor of SARS-CoV-2 RNA synthesis in vitro

    A robust method to quantify low molecular weight contaminants in heparin: detection of tris(2-n-butoxyethyl) phosphate

    Recently, oversulfated chondroitin sulfate (OSCS) was identified in contaminated heparin preparations, which were linked to several adverse clinical events and deaths. Orthogonal analytical techniques, namely nuclear magnetic resonance (NMR) and capillary electrophoresis (CE), have since been applied by several authors for the evaluation of heparin purity and safety. NMR identification and quantification of residual solvents and non-volatile low molecular contaminants with USP acceptance levels of toxicity was achieved 40-fold faster than the traditional GC-headspace technique, which takes similar to 120 min against similar to 3 min to obtain a (1)H NMR spectrum with a signal/noise ratio of at least 1000/1. the procedure allowed detection of Class 1 residual solvents at 2 ppm and quantification was possible above 10 ppm. 2D NMR techniques (edited-HSQC (1)H/(13)C) permitted visualization of otherwise masked EDTA signals at 3.68/59.7 ppm and 3.34/53.5 ppm, which may be overlapping mononuclear heparin signals, or those of ethanol and methanol.     Performance Assessment in Fingerprinting and Multi Component Quantitative NMR Analyses

    An interlaboratory comparison (ILC) was organized with the aim to set up quality control indicators suitable for multicomponent quantitative analysis by nuclear magnetic resonance (NMR) spectroscopy. A total of 36 NMR data sets (corresponding to 1260 NMR spectra) were produced by 30 participants using 34 NMR spectrometers. The calibration line method was chosen for the quantification of a five-component model mixture. Results show that quantitative NMR is a robust quantification tool and that 26 out of 36 data sets resulted in statistically equivalent calibration lines for all considered NMR signals. The performance of each laboratory was assessed by means of a new performance index (named Qp-score) which is related to the difference between the experimental and the consensus values of the slope of the calibration lines. Laboratories endowed with a Qp-score falling within the suitable acceptability range are qualified to produce NMR spectra that can be considered statistically equivalent in terms of relative intensities of the signals. In addition, the specific response of nuclei to the experimental excitation/relaxation conditions was addressed by means of the parameter named NR. NR is related to the difference between the theoretical and the consensus slopes of the calibration lines and is specific for each signal produced by a well-defined set of acquisition parameters
