    "Why is the Doctor a Man?" Reactions of Older Adults to a Virtual Training Doctor

    Shared decision making (SDM) is increasingly considered as the best way to reach a treatment decision in a clinical environment. However, the use of SDM in practice can be obstructed by a number of factors, such as time constraints or lack of applicability due to patient characteristics. Our project, PrepDoc, explores how a Virtual Training Doctor (VTD) can help patients overcome some of these obstacles to experiencing effective SDM during doctor's visits. In this paper, we report on user studies conducted with 19 participants in Scotland aged 65+. The goal of these studies was to identify the reactions of this audience to the PrepDoc system, evaluate its suitability within Scotland, and elicit suggestions to improve it. Our findings revealed that the idea of empowering people to participate in SDM using a virtual agent was positively received by all participants. However, the reactions to how this idea was implemented in the PrepDoc system varied greatly across participants. Based on this, our paper outlines recommendations for enhancing the user experience with VTDs, accommodating individual differences of older adults, and accounting for the national context

    KiDS+GAMA:Intrinsic alignment model constraints for current and future weak lensing cosmology

    We directly constrain the non-linear alignment (NLA) model of intrinsic galaxy alignments, analysing the most representative and complete flux-limited sample of spectroscopic galaxies available for cosmic shear surveys. We measure the projected galaxy position-intrinsic shear correlations and the projected galaxy clustering signal using high-resolution imaging from the Kilo Degree Survey (KiDS) overlapping with the GAMA spectroscopic survey, and data from the Sloan Digital Sky Survey. Separating samples by colour, we make no significant detection of blue galaxy alignments, constraining the blue galaxy NLA amplitude AIAB=0.210.36+0.37A_{\textrm{IA}}^{\textrm{B}}=0.21^{+0.37}_{-0.36} to be consistent with zero. We make robust detections (9σ\sim9\sigma) for red galaxies, with AIAR=3.180.46+0.47A_{\textrm{IA}}^{\textrm{R}}=3.18^{+0.47}_{-0.46}, corresponding to a net radial alignment with the galaxy density field, and we find no evidence for any scaling of alignments with galaxy luminosity. We provide informative priors for current and future weak lensing surveys, an improvement over de facto wide priors that allow for unrealistic levels of intrinsic alignment contamination. For a colour-split cosmic shear analysis of the final KiDS survey area, we forecast that our priors will improve the constraining power on S8S_{8} and the dark energy equation of state w0w_{0}, by up to 62%62\% and 51%51\%, respectively. Our results indicate, however, that the modelling of red/blue-split galaxy alignments may be insufficient to describe samples with variable central/satellite galaxy fractions.Comment: 27 pages (incl. 7 appendix pages), 10 figures, accepted by A&

    Community-led responses to COVID-19 within Gypsy and Traveller communities in England: A participatory qualitative research study.

    Individuals were asked to play an active role in infection control in the COVID-19 pandemic. Yet while government messages emphasised taking responsibility for the public good (e.g. to protect the National Health Service), they appeared to overlook social, economic and political factors affecting the ways that people were able to respond. We co-produced participatory qualitative research with members of Gypsy and Traveller communities in England between October 2021 and February 2022 to explore how they had responded to COVID-19, its containment (test, trace, isolate) and the contextual factors affecting COVID-19 risks and responses within the communities. Gypsies and Travellers reported experiencing poor treatment from health services, police harassment, surveillance, and constrained living conditions. For these communities, claiming the right to health in an emergency required them to rely on community networks and resources. They organised collective actions to contain COVID-19 in the face of this ongoing marginalisation, such as using free government COVID-19 tests to support self-designed protective measures including community-facilitated testing and community-led contact tracing. This helped keep families and others safe while minimising engagement with formal institutions. In future emergencies, communities must be given better material, political and technical support to help them to design and implement effective community-led solutions, particularly where government institutions are untrusted or untrustworthy

    KiDS+VIKING+GAMA:Testing semi-analytic models of galaxy evolution with galaxy-galaxy-galaxy lensing

    Several semi-analytic models (SAMs) try to explain how galaxies form, evolve and interact inside the dark matter large-scale structure. These SAMs can be tested by comparing their predictions for galaxy-galaxy-galaxy-lensing (G3L), which is weak gravitational lensing around galaxy pairs, with observations. We evaluate the SAMs by Henriques et al. (2015; H15) and by Lagos et al. (2012; L12), implemented in the Millennium Run, by comparing their predictions for G3L to observations at smaller scales than previous studies and also for pairs of lens galaxies from different populations. We compare the G3L signal predicted by the SAMs to measurements in the overlap of the Galaxy And Mass Assembly survey (GAMA), the Kilo-Degree Survey (KiDS), and the VISTA Kilo-degree Infrared Galaxy survey (VIKING), splitting lens galaxies into two colour and five stellar-mass samples. Using an improved G3L estimator, we measure the three-point correlation of the matter distribution for mixed lens pairs with galaxies from different samples, and unmixed lens pairs with galaxies from the same sample. Predictions by the H15 SAM agree with the observations for all colour-selected and all but one stellar-mass-selected sample with 95% confidence. Deviations occur for lenses with stellar masses below 9.5h2M9.5h^{-2}\mathrm{M}_\odot at scales below 0.2h1Mpc0.2h^{-1}\mathrm{Mpc}. Predictions by the L12 SAM for stellar-mass selected samples and red galaxies are significantly higher than observed, while the predicted signal for blue galaxy pairs is too low. The L12 SAM predicts more pairs of small stellar-mass and red galaxies than the H15 SAM and the observations, as well as fewer pairs of blue galaxies. Likely explanations are different treatments of environmental effects by the SAMs and different models of the initial mass function.     Population Distribution, Settlement Patterns and Accessibility across Africa in 2010

    The spatial distribution of populations and settlements across a country and their interconnectivity and accessibility from urban areas are important for delivering healthcare, distributing resources and economic development. However, existing spatially explicit population data across Africa are generally based on outdated, low resolution input demographic data, and provide insufficient detail to quantify rural settlement patterns and, thus, accurately measure population concentration and accessibility. Here we outline approaches to developing a new high resolution population distribution dataset for Africa and analyse rural accessibility to population centers. Contemporary population count data were combined with detailed satellite-derived settlement extents to map population distributions across Africa at a finer spatial resolution than ever before. Substantial heterogeneity in settlement patterns, population concentration and spatial accessibility to major population centres is exhibited across the continent. In Africa, 90% of the population is concentrated in less than 21% of the land surface and the average per-person travel time to settlements of more than 50,000 inhabitants is around 3.5 hours, with Central and East Africa displaying the longest average travel times. The analyses highlight large inequities in access, the isolation of many rural populations and the challenges that exist between countries and regions in providing access to services. The datasets presented are freely available as part of the AfriPop project, providing an evidence base for guiding strategic decisions

    Openness in Education as a Praxis: From Individual Testimonials to Collective Voices

    Why is Openness in Education important, and why is it critically needed at this moment? As manifested in our guiding question, the significance of Openness in Education and its immediate necessity form the heart of this collaborative editorial piece. This rather straightforward, yet nuanced query has sparked this collective endeavour by using individual testimonies, which may also be taken as living narratives, to reveal the value of Openness in Education as a praxis. Such testimonies serve as rich, personal narratives, critical introspections, and experience-based accounts that function as sources of data. The data gleaned from these narratives points to the understanding of Openness in Education as a complex, multilayered concept intricately woven into an array of values. These range from aspects such as sharing, access, flexibility, affordability, enlightenment, barrier-removal, empowerment, care, individual agency, trust, innovation, sustainability, collaboration, co-creation, social justice, equity, transparency, inclusivity, decolonization, democratisation, participation, liberty, and respect for diversity. This editorial, as a product of collective endeavour, invites its readers to independently engage with individual narratives, fostering the creation of unique interpretations. This call stems from the distinctive character of each narrative as they voice individual researchers’ perspectives from around the globe, articulating their insights within their unique situational contexts

    Viral Control of Mitochondrial Apoptosis

    Throughout the process of pathogen–host co-evolution, viruses have developed a battery of distinct strategies to overcome biochemical and immunological defenses of the host. Thus, viruses have acquired the capacity to subvert host cell apoptosis, control inflammatory responses, and evade immune reactions. Since the elimination of infected cells via programmed cell death is one of the most ancestral defense mechanisms against infection, disabling host cell apoptosis might represent an almost obligate step in the viral life cycle. Conversely, viruses may take advantage of stimulating apoptosis, either to kill uninfected cells from the immune system, or to induce the breakdown of infected cells, thereby favoring viral dissemination. Several viral polypeptides are homologs of host-derived apoptosis-regulatory proteins, such as members of the Bcl-2 family. Moreover, viral factors with no homology to host proteins specifically target key components of the apoptotic machinery. Here, we summarize the current knowledge on the viral modulation of mitochondrial apoptosis, by focusing in particular on the mechanisms by which viral proteins control the host cell death apparatus

    Background: Cardiovascular disease is a major comorbidity of rheumatoid arthritis (RA) and a leading cause of death. Chronic systemic inflammation involving tumour necrosis factor alpha (TNF) could contribute to endothelial activation and atherogenesis. A number of anti-TNF therapies are in current use for the treatment of RA, including certolizumab pegol (CZP), (Cimzia ®; UCB, Belgium). Anti-TNF therapy has been associated with reduced clinical cardiovascular disease risk and ameliorated vascular function in RA patients. However, the specific effects of TNF inhibitors on endothelial cell function are largely unknown. Our aim was to investigate the mechanisms underpinning CZP effects on TNF-activated human endothelial cells. Methods: Human aortic endothelial cells (HAoECs) were cultured in vitro and exposed to a) TNF alone, b) TNF plus CZP, or c) neither agent. Microarray analysis was used to examine the transcriptional profile of cells treated for 6 hrs and quantitative polymerase chain reaction (qPCR) analysed gene expression at 1, 3, 6 and 24 hrs. NF-κB localization and IκB degradation were investigated using immunocytochemistry, high content analysis and western blotting. Flow cytometry was conducted to detect microparticle release from HAoECs. Results: Transcriptional profiling revealed that while TNF alone had strong effects on endothelial gene expression, TNF and CZP in combination produced a global gene expression pattern similar to untreated control. The two most highly up-regulated genes in response to TNF treatment were adhesion molecules E-selectin and VCAM-1 (q 0.2 compared to control; p > 0.05 compared to TNF alone). The NF-κB pathway was confirmed as a downstream target of TNF-induced HAoEC activation, via nuclear translocation of NF-κB and degradation of IκB, effects which were abolished by treatment with CZP. In addition, flow cytometry detected an increased production of endothelial microparticles in TNF-activated HAoECs, which was prevented by treatment with CZP. Conclusions: We have found at a cellular level that a clinically available TNF inhibitor, CZP reduces the expression of adhesion molecule expression, and prevents TNF-induced activation of the NF-κB pathway. Furthermore, CZP prevents the production of microparticles by activated endothelial cells. This could be central to the prevention of inflammatory environments underlying these conditions and measurement of microparticles has potential as a novel prognostic marker for future cardiovascular events in this patient group.
