    What influences PhD graduate trajectories during the degree : a research-based policy agenda

    During the past two decades, PhD graduate numbers have increased dramatically with graduates viewed by governments as a means to advance the knowledge economy and international competitiveness. Concurrently, universities have also invested in policies to monitor satisfaction, retention, and timely completion-and researchers have expanded the study of PhD experience. We, as such researchers, have increasingly received invitations from university decision-makers to present research evidence which might guide their doctoral programs. Their interest provoked us to do a qualitative systematized review of research on doctoral experience-seeking evidence of practices that influenced retention, satisfaction, and completion. The result contributes a synthesis of the critical research evidence that could be used to inform doctoral education policy. We also demonstrate the possibilities of such evidence by suggesting some potential recommendations, while recognizing that there is no direct relationship between research results and their transformation into particular institutional contexts in ways that enhance doctoral experience. We hope our initiative will be taken up and extended by other researchers, particularly the research gaps we note, so we can collectively support the use of research evidence to influence both doctoral policies and practices-with the goal to better prepare PhD researchers for their futures and better support their supervisors.Peer reviewe

    How does social support contribute to engaging Post-PhD experience?

    Social support from the supervisor and the researcher community has been identified as one of the determinants for successful completion of doctoral studies. Still surprisingly little is known about the function of social support for early career Post-PhD researchers. Even less is known about the individual variation in experienced social support among Post-PhD researchers. This study explores the function of social support in terms of experienced research engagement, burnout and abandonment intentions among Post-PhD researchers. Altogether, 282 Post-PhD researchers from UK and Spanish universities completed the survey. The cluster analysis was applied. Results show that the majority of participants experienced high levels of supervisory and researcher community support. Researchers representing an Adequate Support profile were less likely to experience burnout or to consider abandonment; they also experienced a higher degree of research engagement than their less fortunate counterparts. Further, Post-PhD researchers working in research groups were more likely to display the Adequate Support profile than those working primarily on their own. Strikingly, scientists were more likely than social scientists to be represented in the Reduced Support profile. Interestingly, there were no statistically significant differences between the UK and Spanish Post-PhD or female and male researchers in support profiles.Peer reviewe

    Dermatological Manifestations in the Intensive Care Unit: A Practical Approach

    Dermatological problems are not usually related to intensive medicine because they are considered to have a low impact on the evolution of critical patients. Despite this, dermatological manifestations (DMs) are relatively frequent in critically ill patients. In rare cases, DMs will be the main diagnosis and will require intensive treatment due to acute skin failure. In contrast, DMs can be a reflection of underlying systemic diseases, and their identification may be key to their diagnosis. On other occasions, DMs are lesions that appear in the evolution of critical patients and are due to factors derived from the stay or intensive treatment. Lastly, DMs can accompany patients and must be taken into account in the comprehensive pathology management. Several factors must be considered when addressing DMs: on the one hand, the moment of appearance, morphology, location, and associated treatment and, on the other hand, aetiopathogenesis and classification of the cutaneous lesion. DMs can be classified into 4 groups: life-threatening DMs (uncommon but compromise the patient’s life); DMs associated with systemic diseases where skin lesions accompany the pathology that requires admission to the intensive care unit (ICU); DMs secondary to the management of the critical patient that considers the cutaneous manifestations that appear in the evolution mainly of infectious or allergic origin; and DMs previously present in the patient and unrelated to the critical process.     Adherence to the Western, Prudent and Mediterranean dietary patterns and breast cancer risk: MCC-Spain study

    Objective To externally validate the previously identified effect on breast cancer risk of the Western, Prudent and Mediterranean dietary patterns. Study design MCC-Spain is a multicase-control study that collected epidemiological information on 1181 incident cases of female breast cancer and 1682 control cases from 10 Spanish provinces. Three dietary patterns derived in another Spanish case-control study were analysed in the MCC-Spain study. These patterns were termed Western (high intakes of fatty and sugary products and red and processed meat), Prudent (high intakes of low-fat dairy products, vegetables, fruits, whole grains and juices) and Mediterranean (high intake of fish, vegetables, legumes, boiled potatoes, fruits, olives, and vegetable oil, and a low intake of juices). Their association with breast cancer was assessed using logistic regression models with random province-specific intercepts considering an interaction with menopausal status. Risk according to tumour subtypes ? based on oestrogen (ER), progesterone (PR) and human epidermal growth factor 2 (HER2) receptors (ER+/PR+&HER2-; HER2+; ER-/PR-&HER2-) ? was evaluated with multinomial regression models. Main outcome measures Breast cancer and histological subtype. Results Our results confirm most of the associations found in the previous case-control study. A high adherence to the Western dietary pattern seems to increase breast cancer risk in both premenopausal women (OR4thvs.1stquartile(95%CI):1.68(1.02;2.79); OR1SD-increase(95%CI): 1.19(1.01;1.40)) and postmenopausal women (OR4thvs.1stquartile(95%CI):1.48(1.07;2.05); OR1SD-increase(95%CI): 1.14(1.01;1.28)). While high adherence to the Prudent pattern did not show any effect on breast cancer, the Mediterranean dietary pattern seemed to be protective, but only among postmenopausal women (OR4thvs.1stquartile (95%CI):0.72(95% CI 0.53;0.98); p-int = 0.075). There were no significant differences by tumour subtype.
