    Birth defects in newborns and stillborns: an example of the Brazilian reality

    <p>Abstract</p> <p>Background</p> <p>This study constitutes a clinical and genetic study of all newborn and stillborn infants with birth defects seen in a period of one year in a medical school hospital located in Brazil. The aims of this study were to estimate the incidence, causes and consequences of the defects.</p> <p>Methods</p> <p>For all infants we carried out physical assessment, photographic records, analysis of medical records and collection of additional information with the family, besides the karyotypic analysis or molecular tests in indicated cases.</p> <p>Result</p> <p>The incidence of birth defects was 2.8%. Among them, the etiology was identified in 73.6% (ci95%: 64.4-81.6%). Etiology involving the participation of genetic factors single or associated with environmental factors) was more frequent 94.5%, ci95%: 88.5-98.0%) than those caused exclusively by environmental factors (alcohol in and gestational diabetes mellitus). The conclusive or presumed diagnosis was possible in 85% of the cases. Among them, the isolated congenital heart disease (9.5%) and Down syndrome (9.5%) were the most common, followed by gastroschisis (8.4%), neural tube defects (7.4%) and clubfoot (5.3%). Maternal age, parental consanguinity, exposure to teratogenic agents and family susceptibility were some of the identified risk factors. The most common observed consequences were prolonged hospital stays and death.</p> <p>Conclusions</p> <p>The current incidence of birth defects among newborns and stillbirths of in our population is similar to those obtained by other studies performed in Brazil and in other underdeveloped countries. Birth defects are one of the major causes leading to lost years of potential life. The study of birth defects in underdeveloped countries should continue. The identification of incidence, risk factors and consequences are essential for planning preventive measures and effective treatments.</p

    Brazilian multicenter study on prevalence of preterm birth and associated factors

    <p>Abstract</p> <p>Background</p> <p>The occurrence of preterm birth remains a complex public health condition. It is considered the main cause of neonatal morbidity and mortality, resulting in a high likelihood of sequelae in surviving children. With variable incidence in several countries, it has grown markedly in the last decades. In Brazil, however, there are still difficulties to estimate its real occurrence. Therefore, it is essential to establish the prevalence and causes of this condition in order to propose prevention actions. This study intend to collect information from hospitals nationwide on the prevalence of preterm births, their associated socioeconomic and environmental factors, diagnostic and treatment methods resulting from causes such as spontaneous preterm labor, prelabor rupture of membranes, and therapeutic preterm birth, as well as neonatal results.</p> <p>Methods/Design</p> <p>This proposal is a multicenter cross-sectional study plus a nested case-control study, to be implemented in 27 reference obstetric centers in several regions of Brazil (North: 1; Northeast: 10; Central-west: 1; Southeast: 13; South: 2). For the cross sectional component, the participating centers should perform, during a period of six months, a prospective surveillance of all patients hospitalized to give birth, in order to identify preterm birth cases and their main causes. In the first three months of the study, an analysis of the factors associated with preterm birth will also be carried out, comparing women who have preterm birth with those who deliver at term. For the prevalence study, 37,000 births will be evaluated (at term and preterm), corresponding to approximately half the deliveries of all participating centers in 12 months. For the case-control study component, the estimated sample size is 1,055 women in each group (cases and controls). The total number of preterm births estimated to be followed in both components of the study is around 3,600. Data will be collected through a questionnaire all patients will answer after delivery. The data will then be encoded in an electronic form and sent online by internet to a central database. The data analysis will be carried out by subgroups according to gestational age at preterm birth, its probable causes, therapeutic management, and neonatal outcomes. Then, the respective rates, ratios and relative risks will be estimated for the possible predictors.</p> <p>Discussion</p> <p>These findings will provide information on preterm births in Brazil and their main social and biological risk factors, supporting health policies and the implementation of clinical trials on preterm birth prevention and treatment strategies, a condition with many physical and emotional consequences to children and their families.</p

    Potentially probiotic acai yogurt

    The aim of this work was to evaluate the suitability of yogurt containing acai pulp as a food carrier of probiotic cultures. Probiotic yogurts containing increasing amounts of acai pulp (3, 5 and 7%) were processed and submitted to a physicochemical analysis and viable microbial count during refrigerated storage. In general, all the physicochemical parameters showed variations proportional to the amount of acai pulp in the product formulation. Probiotic activity was verified throughout refrigerated storage for all the products, although there was a fall of one logarithmic cycle for both micro-organisms during this period (10(7)-10(8) CFU/mL).61217818

    Corrimento vaginal referido entre gestantes em localidade urbana no Sul do Brasil: prevalência e fatores associados

    The purpose of this study was to determine the prevalence and risk factors associated with selfreported vaginal discharge among pregnant women in the city of Rio Grande, South Brazil. Using a cross-sectional design, a standard interview was applied to pregnant women at home by previously trained interviewers, covering the following: demographic, reproductive, and socioeconomic data, household conditions, health care, and illnesses during pregnancy, including vaginal discharge. The chi-square test was used to compare proportions, and Poisson regression was used in the multivariate analysis. Among the 339 pregnant women interviewed, 52% reported vaginal discharge. The following variables were significantly associated with the outcome: age (prevalence rate, PR = 1.49), marital status (PR =1.31), urinary tract infection (PR = 1.56),hyperglycemia (PR = 1.48), use of an intrauterine device (PR = 2.35), and history of preterm delivery (PR = 1.37), with oral contraception showing a protective effect (PR = 0.79). Prevalence of selfreported discharge was high among this group of pregnant women. Several risk factors were also identified for the disease under study. These findings can contribute to the implementation of preventive interventions.Por intermédio de delineamento transversal, buscouse determinar a prevalência e identificar fatores associados à ocorrência de corrimento vaginal referido entre gestantes da cidade de Rio Grande, Rio Grande do Sul, Brasil. Utilizando-se de questionário padrão, foram investigadas características sócio-econômicas, demográficas, reprodutivas, condições de moradia, assistência recebida e ocorrência de corrimento vaginal referido entre estas gestantes. Para as comparações entre proporções utilizou-se teste do qui-quadrado e para análise multivariada regressão de Poisson. Dentre as 339 gestantes estudadas, 51,6% referiram corrimento vaginal na gestação. As seguintes variáveis mostraramse significativamente associadas à ocorrência de corrimento vaginal referido: idade (razão de prevalências: RP= 1,49), estado civil (RP = 1,31), ocorrência de infecção urinária (RP = 1,56), hiperglicemia na gestação atual (RP = 1,48), uso de dispositivo intra-uterino (RP =2,35), ocorrência prévia de parto prematuro (RP =1,37) e utilização de anticoncepcional oral como fator de proteção (RP = 0,79). Este estudo mostrou prevalência elevada de corrimento vaginal referido entre as gestantes estudadas e permitiu identificar aquelas com maior risco de adoecer por esta causa, o que pode contribuir para a adoção de medidas preventivas

    OBJETIVO: Avaliar o risco de parto pré-termo (PPT) espontâneo na população geral a partir do estudo comparativo dos marcadores ultra-sonográficos morfológicos do colo uterino, como o sinal do afunilamento e a ausência da área glandular endocervical. MATERIAIS E MÉTODOS: Foram arroladas 361 gestantes na população geral, submetidas a exame ultra-sonográfico transvaginal entre a 21ª e 24ª semana, e verificados os resultados perinatais. RESULTADOS: A incidência de PPT espontâneo foi de 5,0%. O sinal do afunilamento foi observado em 4,2% da população estudada e em 22,2% das pacientes que evoluíram para PPT espontâneo. Tal parâmetro mostrou associação significante com PPT (p < 0,001; risco relativo de 6,68). A ausência do eco glandular endocervical (EGE) foi detectada em 2,8% das pacientes estudadas e em 44,4% das pacientes que evoluíram para PPT espontâneo. Este parâmetro demonstrou forte associação com PPT espontâneo (p < 0,001; risco relativo de 28,57). A análise de regressão logística multivariada apontou a ausência do EGE como a única variável morfológica associada ao PPT espontâneo. CONCLUSÃO: A predição do PPT espontâneo a partir de sinais ultra-sonográficos deve ser realizada contemplando marcadores biométricos e morfológicos, entre estes, a ausência do EGE. Este estudo indica uma tendência clara da marcante importância da ausência do EGE como indicador do risco para PPT espontâneo, a ser confirmada futuramente em pesquisas multicêntricas.<br>OBJECTIVE: To verify if different morphological ultrasonographic markers such as the funneling sign and the cervical gland area can be predictors of spontaneous premature delivery in pregnant women between 21 and 24 weeks of gestation. MATERIALS AND METHODS: This was a prospective cross-sectional study in which 361 women with 21 to 24 weeks of gestation were examined by transvaginal ultrasonography. The gestational age at delivery was later checked by telephone or mail. RESULTS: Spontaneous preterm delivery (SPD) occurred in 5% of the patients. Cervical funneling occurred in 4.2% of the women and in 22.2% of those who had SPD. This finding was correlated to preterm delivery (p < 0.001; relative risk of 6.68). Absence of a hypoechoic area peripheral to the cervical canal consistent with endocervical epithelium glands, namely endocervical glandular echo (EGE) feature, was detected in 2.8% of all patients and in 44.4% of those who developed spontaneous preterm labor. There was a statistically significant association of this feature to SPD (p < 0.001; relative risk of 28.57). Multivariance logistic regression analysis showed that this was the feature with strongest correlation with SPD, when compared to cervical funneling. CONCLUSION: Prediction of SPD through ultrasound features should observe biometrical and morphological signs such as absence of EGE. This is a new and important ultrasound finding that can be considered a predictor of risk for SPD, and should be confirmed in future multicentric trials

    Estudo dos ventrículos cerebrais por ultrassonografia, na criança normal, nascida a termo, de 1 a 6 meses

    OBJETIVO: Estudo dos ventrículos cerebrais por ultrassonografia, com o objetivo de estabelecer de forma simplificada parâmetros para diagnóstico das dilatações ventriculares leves. MATERIAIS E MÉTODOS: Foram estudadas, prospectivamente, 105 crianças, normais, nascidas a termo, com um total de 181 exames realizados, mensalmente até os 6 meses, através da observação de dados morfológicos e medidas. As medidas efetuadas foram: índice ventrículo/hemisfério, diâmetro anteroposterior do corno anterior e do quarto ventrículo. RESULTADOS: Obtiveram-se média, desviopadrão e percentis de normalidade das medidas estabelecidas, em cada faixa etária. A pesquisa de halo anecoico nos dois terços posteriores do plexo coroide em plano coronal VI, para avaliação dos cornos temporal/posterior, foi ausente, e o terceiro ventrículo mostrou-se como uma fenda anecoica, menor que 1 mm, em plano coronal V em todas as crianças do estudo. CONCLUSÃO: Os achados morfológicos relacionados aos cornos temporal/posterior e ao terceiro ventrículo, associados ao percentil 95 das medidas como limite superior da normalidade, podem ser utilizados para diagnóstico simplificado de dilatações ventriculares leves
