    Interpreting the Hours-Technology time-varying relationship

    We investigate the time varying relation between hours and technology shocks using a structural business cycle model. We propose an RBC model with a Constant Elasticity of Substitution (CES) production function that allows for capital- and labor-augmenting technology shocks. We estimate the model with Bayesian techniques. In the full sample, we find (i) evidence in favor of a less than unitary elasticity of substitution (rejecting Cobb-Douglas) and (ii) a sizable role for capital augmenting shock for business cycles fluctuations. In rolling sub-samples, we document that the transmission of technology shocks to hours worked has been varying over time. We argue that this change is due to the increase of the elasticity of factor substitution. That is, labor and capital became less complementary throughout the sample inducing a change in the sign and size of the response of hours.     Yield response of fennel (Foeniculum vulgare Mill.) to irrigation with saline water

    Salinity causes yield reductions if threshold levels are exceeded, while it can cause the deterioration of some qualitative parameters and/or the improvement of others. Two years (2000-2001, 2006-2007) research was carried out to evaluate the effect of water quality on yield and quality of fennel, a vegetable very common in Apulia region. We compared two types of soil and two sodium absorption ratio of irrigation water obtained by dissolving in distilled water NaCl + CaCl2 (1:1) or only NaCl, and four electrical conductivity of water of 0.7 (control), 4, 8 and 12 dS m(-1). The soil type does not show any noticeable effect on the tested parameters. The salt type influenced the plant development and the total soluble solids contained in the bulbs. The plant height, the leaf and the bulb weight were, respectively, 6, 11 and 21% higher in NaCl + CaCl2 treatment as compared to NaCl one. The plant height, the leaf and the bulb weight dropped, respectively, by 33, 49 and 71%, shifting from 0.7 to 12 dS m(-1) salinity treatments. The dry matter content and total soluble solids increased with rising salinity. In fact, shifting from the control to the more saline treatment, the leaves and the bulbs dry matter increased, respectively, by 15 and 13%, and total soluble solids increased by 17%. Moreover, the increase in salinity favoured the production of flattened bulbs. The application of the Maas and Hoffman model to salinity response confirms the ranking of this species among moderately salt sensitive species, even though the critical threshold was the same for NaCl and NaCl + CaCl2 treatments (1.26 dS m(-1)), while the slope was higher in NaCl treatment (14.24% m dS(-1)) than NaCl + CaCl2 (10.39% m dS(-1))

    Efficacy of a Non-addictive Nasal Irrigation Based on Sea Salt Enriched with Natural Enzymes among Patients with Sinusitis: An In Vivo, Randomized, Controlled Trial

    BACKGROUND: Chronic rhinosinusitis (CRS) is a common condition that is defined as inflammation of the nose and paranasal sinuses. Nasal irrigation plays an important role in the treatment of CRS. Evidence from basic research favors hypertonic saline over isotonic saline for mucociliary clearance, but evidence from clinical studies is controversial. AIM: This study aims to investigate the hypothesis that the use of daily nasal irrigation based on sea salt, enriched with natural enzymes and lysozyme, may be useful in patients with CRS. PATIENT AND METHODS: Patients (30 men and 30 women) 18–55 years old (mean age 41 ± 3 y.o.), with two episodes of acute sinusitis or one episode of chronic sinusitis per year for 2 consecutive years, were enrolled stratified by sex and age and randomly divided into two groups supplementation: Group A (test) and Group B (control/ placebo). Moreover, an exit questionnaire was asked to Group A subjects to report whether their sinus-related quality of life has gotten worse, stayed the same, or improved (scale from 0 to ±100%). RESULTS: The result showed that in the test group (A) from T0 to T1, a reduction of 17.65% for the symptoms related headache and/or facial pressure and a reduction of the 18.18%, for the symptoms relates to congestion and/ or nasal discharge. On the other hand, the control group (B) shown less difference between T0 and T1. CONCLUSIONS: This study strengthens the argument that the tested formulation is a safe, well-tolerated, long-term therapy that patients with chronic sinonasal complaints can and will use at home with minimal training and follow-up

    Single stage reconstruction of complex head and neck defects involving the skin with a single ALT flap: A ten year review

    Background: Multicomponent defects of the head and neck involving the cervical skin pose a reconstructive challenge for microsurgeons and usually requires two flaps. However, many patients who undergo such surgical treatment had prior treatment with radiotherapy and the availability of recipient vessels for free flap reconstruction may be limited. The purpose of this study was to review our experience in the reconstruction of these extensive head and neck defects using a single ALT free flap. Methods: A total of 21 patients with complex defects of the head and neck involving multiple anatomical subunits, including the overlying cervical skin, underwent reconstruction with a single ALT flap. The clinical, functional, and aesthetic outcomes of these patients were reviewed. Results: The mean hospital stay was 24 days. There was one total flap loss due to pedicle thrombosis. The patient underwent a further ALT reconstruction with no postoperative complications. Cervical fistulas occurred in three patients, and all fistulas were healed by simple wound packing. Three patients with tracheal defect had a functional tracheostoma with adequate stomal patency. A modified barium swallowing study was performed on each patient, and all of them achieved total oral intake. Among them, two patients tolerated only a pureed diet. Conclusions: Complex neck reconstruction can be accomplished with a single ALT flap with good clinical and functional results, minimal morbidity and quick recovery

    Effect of Water Regime, Nitrogen Level, and Biostimulant Application on the Water and Nitrogen Use Efficiency of Wild Rocket [Diplotaxis tenuifolia (L.) DC]

    The use of biostimulants in agriculture is an emerging technique that can contribute to improved production and resource use efficiency. This research was carried out in southern Italy to evaluate the biostimulating effects of seaweed extract (SW) and azoxystrobin (AZ) on wild rocket subjected to two water regimes (WRs) and three nitrogen levels (NLs), and grown in pots under unheated greenhouse conditions. The following treatments were compared: (i) two WRs: restoration of 100% (WR100) and 50% (WR50) of crop evapotranspiration; (ii) three NLs: 0 (N0), 75 (N75), or 150 (N150) kg ha−1 of N; and (iii) three biostimulants (BSs): an untreated control (C), and the application of AZ or SW. This paper reports the effects on N uptake (Nup), N use efficiency (NUE), and water use efficiency (WUE). The following indicators of NUE were assessed: apparent recovery efficiency (RE), internal utilization efficiency (IE), partial productivity factor (PFPn) of N supplied, agronomic efficiency (AE), and physiological efficiency (PE). The following indicators of WUE were assessed: photosynthetic WUE (p_WUE), yield WUE (Y_WUE), biomass WUE (B_WUE), and irrigation yield WUE (IY_WUE). The indicators of NUE were affected differently by treatments. RE was 20% higher with SW. IE was higher with AZ. PFPn increased by 10.4 and 8.1% with AZ and SW, respectively. AE increased by 10.9 and 19.9% after applying AZ and SW, respectively. PE rose by 6.7 and 9.3% after applying AZ and SW. AZ and SW improved p_WUE, mainly under water deficit (interaction of WR × BS). With AZ application, Y_WUE, B_WUE, and IY_WUE were higher by 17.8, 13.8, and 19.3%, respectively, while the application of SW resulted in a smaller increase (9.5–7.7 and 9.9%). SW and AZ were shown to be effective through the moderate improvement of wild rocket’s nitrogen and water use efficiency. The two biostimulants were more effective at improving p_WUE in water deficit conditions, proving to be particularly useful for farmers operating with water scarcity. Therefore, they can provide valuable support to farmers by improving the sustainability of resource use

    Orthopedic temporomandibular joint (TMJ) instability is very common among children and adults. It is often associated with pain in the cervicofacial region, and muscle contraction. To investigate whether muscle contraction can cause permanent posterior rotation of the head and whether treatment with splint and kinetotherapy is efficient, a literature review was carried out of patients with pain in the cervicofacial area. Additionally, the case of a 15-year old patient presenting with permanent posterior rotation of cranium, with no movement between the first two vertebra and pain in the cervicofacial area was reported. Kinetotherapy followed by rapid maxillary expansion improved the function of cervical vertebrae and reduced the cervicofacial pain within the first two weeks. Kinetotherapy, rapid maxillary expansion, and orthodontic treatment with a stable joint position could be a good therapy to control occipital-atlas function

    Stop-event-related potentials from intracranial electrodes reveal a key role of premotor and motor cortices in stopping ongoing movements

    In humans, the ability to withhold manual motor responses seems to rely on a right-lateralized frontal–basal ganglia–thalamic network, including the pre-supplementary motor area and the inferior frontal gyrus (IFG). These areas should drive subthalamic nuclei to implement movement inhibition via the hyperdirect pathway. The output of this network is expected to influence those cortical areas underlying limb movement preparation and initiation, i.e., premotor (PMA) and primary motor (M1) cortices. Electroencephalographic (EEG) studies have shown an enhancement of the N200/P300 complex in the event-related potentials (ERPs) when a planned reaching movement is successfully stopped after the presentation of an infrequent stop-signal. PMA and M1 have been suggested as possible neural sources of this ERP complex but, due to the limited spatial resolution of scalp EEG, it is not yet clear which cortical areas contribute to its generation. To elucidate the role of motor cortices, we recorded epicortical ERPs from the lateral surface of the fronto-temporal lobes of five pharmacoresistant epileptic patients performing a reaching version of the countermanding task while undergoing presurgical monitoring. We consistently found a stereotyped ERP complex on a single-trial level when a movement was successfully cancelled. These ERPs were selectively expressed in M1, PMA, and Brodmann's area (BA) 9 and their onsets preceded the end of the stop process, suggesting a causal involvement in this executive function. Such ERPs also occurred in unsuccessful-stop (US) trials, that is, when subjects moved despite the occurrence of a stop-signal, mostly when they had long reaction times (RTs). These findings support the hypothesis that motor cortices are the final target of the inhibitory command elaborated by the frontal–basal ganglia–thalamic network

    Evaluation of a novel technique for closure of small palatal fistula

    A palatal fistula is a pathological condition that connects the nasal cavities with the oral cavity. An oral–nasal fistula is reported as a possible post-surgical complication after the removal of oral carcinomas. The presence of a palatal fistula affects the patient’s quality of life, making it necessary to apply a prosthetic device, such as a palatal plate, to keep the nasal cavities separated from the oral one. There are several surgical techniques to close a palatal fistula, but it is not possible to define the optimal technique as the approach is extremely dependent on the characteristics of the fistula. The aim of this article is to propose a minimally invasive technique to reduce the size of palatal fistulae and to reduce the surgical difficulty (NSPF). A total of 20 patients fulfilled the inclusion criteria and were checked every two weeks. The fistula was injured with a needle every 2 weeks. Fifteen patients who healed with complete closure of the fistula reported no need for a palatal protection plate to eat, drink and speak normally. It is possible to conclude that the NSPF protocol is a valid approach for the non-surgical reduction of palatal fistulae, and it is possible, when the appropriate conditions are present, to achieve complete closure.     Activity and safety of RAD001 (everolimus) in patients affected by biliary tract cancer progressing after prior chemotherapy: a phase II ITMO study.

    BACKGROUND: Biliary tract cancer (BTC) is a highly lethal disease for which the best available therapy remains undetermined. The mammalian target of rapamycin (mTOR) pathway is up-regulated in several cancers, including BTC, and preclinical evidence indicates that mTOR inhibition may be effective in the treatment of BTC. We sought to evaluate the activity and tolerability of the mTOR inhibitor RAD001-everolimus-in patients with BTC progressing after prior chemotherapy. PATIENTS AND METHODS: This was an open-label, single-arm, phase II study (EUDRACT 2008-007152-94) conducted in eight sites in Italy. Patients with locally advanced, metastatic or recurrent BTC progressing despite previous chemotherapy received a daily oral dose of everolimus 10 mg administered continuously in 28-day cycles. The two primary end points were disease control rate (DCR) and objective response rate (ORR). Secondary end points were progression-free survival (PFS), overall survival (OS) and time-to-progression (TTP). RESULTS: Thirty-nine patients were enrolled. The DCR was 44.7%, and the ORR was 5.1%. One patient showed a partial response at 2 months and one patient showed a complete response sustained up to 8 months. The median (95% confidence interval) PFS was 3.2 (1.8-4.0) months, and the median OS was 7.7 (5.5-13.2) months. The median TTP was 2.0 (1.7-3.7) months. Most common toxicities were asthenia (43.6%), thrombocytopenia (35.9%), pyrexia (30.8%) and erythema, mainly of mild-to-moderate severity. Two patients required dose reduction due to adverse events. CONCLUSION: Everolimus demonstrated a favourable toxicity profile and encouraging anti-tumour activity. Further trials are needed to establish the role of everolimus in the treatment of BTC.
