ResumenEn este artículo se presenta una investigación desarrollada durante un curso escolar en la Comunidad Autónoma de Andalucía (2010-2011). El tema central del estudio es analizar la influencia del implante coclear en la mejora de la calidad de vida de niños que lo han recibido en sus primeros años de vida. No es el propósito de este trabajo describir íntegramente la investigación, sino que de forma selectiva se hace un recorrido por el diseño metodológico y los resultados de la investigación, que nos lleva a plantear una serie de reflexiones finales acerca de la mejora de la forma de vida y del desarrollo personal de niños que padecen hipoacusia y son portadores de un implante coclear.AbstractThis article describes a study conducted in an Andalusia Community school (year 2010-2011). Infecção do trato urinário: aspectos epidemiológicos, fisiopatológicos e manejo terapêutico

Em síntese, as ITUs são infecções causadas por bactérias, sendo a bexiga o sítio de infecção mais comum. Essa patologia é uma das principais causas de infecções nas mulheres, sendo que essas são acometidas até quatro vezes mais em relação aos homens. A faixa etária mais acometida gira em torno de 16 a 35 anos. Essa maior prevalência das mulheres em relação aos homens está relacionada à anatomia do trato urinário feminino e sua proximidade com os órgãos reprodutivos. Os principais germes envolvidos na etiopatogenia da ITU são: Escherichia coli, Enterococcus sp., Klebsiella sp., Pseudomonas aeruginosa, Proteus sp. e Staphylococcus sp. Os sintomas clássicos dessa patologia são disúria, dor suprapúbica, polaciúria e urgência miccional. Em mulheres mais velhas, pode haver também queixas menos específicas como: urina com odor fétido, sensação de esvaziamento incompleto da bexiga, constipação e hematúria. A coleta cuidadosa da amostra de urina é muito útil para fins diagnósticos. No que se refere ao tratamento da ITU, esse possui como foco principal a antibioticoterapia. Em casos de ITU aguda não complicada, a terapia dura de 03 a 05 dias, sendo os fármacos de primeira linha: nitrofurantoína, trimetoprim-sulfametoxazol e fosfomicina. Ademais, em ITUs recorrentes, pode-se lançar mão do tratamento profilático, que pode ser feito por intermédio de baixas dosagens de antimicrobianos por um período de 6 a 12 meses. As complicações mais frequentes são a persistência dos sintomas, pielonefrite, incontinência urinária, abscesso renal, prostatite crônica e insuficiência renal

Hyperdominance in Amazonian Forest Carbon Cycling

While Amazonian forests are extraordinarily diverse, the abundance of trees is skewed strongly towards relatively few ‘hyperdominant’ species. In addition to their diversity, Amazonian trees are a key component of the global carbon cycle, assimilating and storing more carbon than any other ecosystem on Earth. Here we ask, using a unique data set of 530 forest plots, if the functions of storing and producing woody carbon are concentrated in a small number of tree species, whether the most abundant species also dominate carbon cycling, and whether dominant species are characterized by specific functional traits. We find that dominance of forest function is even more concentrated in a few species than is dominance of tree abundance, with only ≈1% of Amazon tree species responsible for 50% of carbon storage and productivity. Although those species that contribute most to biomass and productivity are often abundant, species maximum size is also influential, while the identity and ranking of dominant species varies by function and by region

Long-term thermal sensitivity of Earth’s tropical forests

The sensitivity of tropical forest carbon to climate is a key uncertainty in predicting global climate change. Although short-term drying and warming are known to affect forests, it is unknown if such effects translate into long-term responses. Here, we analyze 590 permanent plots measured across the tropics to derive the equilibrium climate controls on forest carbon. Maximum temperature is the most important predictor of aboveground biomass (−9.1 megagrams of carbon per hectare per degree Celsius), primarily by reducing woody productivity, and has a greater impact per °C in the hottest forests (>32.2°C). Our results nevertheless reveal greater thermal resilience than observations of short-term variation imply. Mortality and pulmonary complications in patients undergoing surgery with perioperative SARS-CoV-2 infection: an international cohort study

Background: The impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on postoperative recovery needs to be understood to inform clinical decision making during and after the COVID-19 pandemic. This study reports 30-day mortality and pulmonary complication rates in patients with perioperative SARS-CoV-2 infection. Methods: This international, multicentre, cohort study at 235 hospitals in 24 countries included all patients undergoing surgery who had SARS-CoV-2 infection confirmed within 7 days before or 30 days after surgery. The primary outcome measure was 30-day postoperative mortality and was assessed in all enrolled patients. The main secondary outcome measure was pulmonary complications, defined as pneumonia, acute respiratory distress syndrome, or unexpected postoperative ventilation. Findings: This analysis includes 1128 patients who had surgery between Jan 1 and March 31, 2020, of whom 835 (74·0%) had emergency surgery and 280 (24·8%) had elective surgery. SARS-CoV-2 infection was confirmed preoperatively in 294 (26·1%) patients. 30-day mortality was 23·8% (268 of 1128). Pulmonary complications occurred in 577 (51·2%) of 1128 patients; 30-day mortality in these patients was 38·0% (219 of 577), accounting for 81·7% (219 of 268) of all deaths. In adjusted analyses, 30-day mortality was associated with male sex (odds ratio 1·75 [95% CI 1·28–2·40], p\textless0·0001), age 70 years or older versus younger than 70 years (2·30 [1·65–3·22], p\textless0·0001), American Society of Anesthesiologists grades 3–5 versus grades 1–2 (2·35 [1·57–3·53], p\textless0·0001), malignant versus benign or obstetric diagnosis (1·55 [1·01–2·39], p=0·046), emergency versus elective surgery (1·67 [1·06–2·63], p=0·026), and major versus minor surgery (1·52 [1·01–2·31], p=0·047). Interpretation: Postoperative pulmonary complications occur in half of patients with perioperative SARS-CoV-2 infection and are associated with high mortality. Thresholds for surgery during the COVID-19 pandemic should be higher than during normal practice, particularly in men aged 70 years and older. Consideration should be given for postponing non-urgent procedures and promoting non-operative treatment to delay or avoid the need for surgery. Taking the pulse of Earth's tropical forests using networks of highly distributed plots

Tropical forests are the most diverse and productive ecosystems on Earth. While better understanding of these forests is critical for our collective future, until quite recently efforts to measure and monitor them have been largely disconnected. Networking is essential to discover the answers to questions that transcend borders and the horizons of funding agencies. Here we show how a global community is responding to the challenges of tropical ecosystem research with diverse teams measuring forests tree-by-tree in thousands of long-term plots. We review the major scientific discoveries of this work and show how this process is changing tropical forest science. Our core approach involves linking long-term grassroots initiatives with standardized protocols and data management to generate robust scaled-up results. By connecting tropical researchers and elevating their status, our Social Research Network model recognises the key role of the data originator in scientific discovery. Conceived in 1999 with RAINFOR (South America), our permanent plot networks have been adapted to Africa (AfriTRON) and Southeast Asia (T-FORCES) and widely emulated worldwide. Now these multiple initiatives are integrated via ForestPlots.net cyber-infrastructure, linking colleagues from 54 countries across 24 plot networks. Collectively these are transforming understanding of tropical forests and their biospheric role. Together we have discovered how, where and why forest carbon and biodiversity are responding to climate change, and how they feedback on it. This long-term pan-tropical collaboration has revealed a large long-term carbon sink and its trends, as well as making clear which drivers are most important, which forest processes are affected, where they are changing, what the lags are, and the likely future responses of tropical forests as the climate continues to change. By leveraging a remarkably old technology, plot networks are sparking a very modern revolution in tropical forest science.
