    Palaeomagnetic and mineral magnetic analyses of the Deckenschotter of northern Switzerland and southern Germany

    The Deckenschotter is a fluvial to glaciofluvial gravel unit in northern Switzerland and southern Germany. The deposits are considered the oldest preserved glacial to interglacial Quaternary deposits in the northern Alpine foreland and are thus important geomorphological markers for landscape evolution. Nevertheless, the age of the deposits is only approximately known and subject to controversial debates. This study presents the results of an extensive palaeomagnetic investigation carried out on intercalated fine-grained sediments at 11 sites of the Höhere Deckenschotter (HDS) and at 5 sites of the Tiefere Deckenschotter (TDS). The HDS show reversed and normal magnetisations, indicating deposition > 0.773 Ma, while the TDS exhibit only normal directions. Age constraints for the different sites are discussed in the light of evidence from other studies. The study therefore clearly supports the efforts to determine the age of the Deckenschotter. As data from previous palaeomagnetic studies on the HDS and TDS have not been published or preserved, this is in fact the only data-based palaeomagnetic study available

    Phytotherapeutic and naturopathic adjuvant therapies in otorhinolaryngology

    Phytotherapeutic pharmaceuticals and herbal medicinal products with its roots in classical phytotherapeutic medicine have a well-established role in otolaryngological therapy, especially for diseases of the upper airways and acute and chronic infections. A thorough selection and application could mean huge benefit for the patient, in particular in cases with contraindications, chemo- and antibiotic resistance or patient request. Besides, it might spare other medications. Phytotherapeutic pharmaceuticals must fulfil the same criteria of quality, effectiveness and harmlessness of evidence-based medicine like chemical pharmaceuticals, although they are often prescribed due to its well established or traditional based use.     We compared the frequency of severe ocular complications secondary to Herpes Zoster Ophthalmicus (HZO) in 232 patients. They were divided into three groups: 1) patients without treatment (n = 164); 2) patients treated adequately (n = 48) with acyclovir (ACV; 5 x 800 mg/d orally and ophthalmic ointment 5 x /d for a minimum of 7 days, given within three days after skin eruption); and, 3) patients treated inadequately (n = 20) with ACV (only topical treatment, insufficient doses, interrupted treatment, delayed treatment). Patients with no treatment or with inadequate treatments showed the same frequency of severe ocular complications (21% (34/164) and 25% (5/20), respectively). In contrast, when adequate treatment of ACV was given complications occurred in only 4% (2/48) of cases. This study emphasizes the need for prompt (within three days after skin eruption) and adequate (5 x 800 mg/d for at least 7 days) treatment of ACV to prevent the severe complications of HZO

    Resilience in patients and spouses faced with malignant melanoma. A qualitative longitudinal study

    Life-threatening diseases like malignant tumours are associated with considerable existential distress. Little is known about the factors that promote resilience within these individuals. This longitudinal qualitative partner study aimed to analyse resilience as per Antonovsky's sense of coherence. Eight patients with malignant melanoma and their partners were interviewed. They were asked about their coping strategies, attitudes towards the meaning of life and their cancer, and comprehension of what is happening to them. The questions were asked shortly after their diagnosis was made and 6 months later. All interviews were audio-taped and later transcribed and analysed according to the method of qualitative content analysis described by P. Mayring. At baseline, the majority of statements made (261; patients = 141/spouses = 120) related to coping/manageability of disease, with only 26 statements (patients = 15/spouses = 11) related to meaning and 127 (patients = 64/spouses = 63) to comprehension. There were no significant differences between the responses of patients and their partners and no significant changes in the number of statements during the 6-month interview. The most significant theme that emerged was manageability of disease, with distraction the most commonly utilised coping skill. The comprehension and meaning themes were far less prevalent. Hence, support should focus on disease and situational manageability

    Objective: The number of mobile applications addressing health topics is increasing. Whether these apps underwent scientific evaluation is unclear. We comprehensively assessed papers investigating the diagnostic value of available diagnostic health applications using in-built smartphone-sensors. Methods: Systematic Review - Medline, Scopus, Web of Science inclusive Medical Informatics and Business Source Premier (by citation of reference) were searched from inception until December 15th, 2016. Checking of reference lists of review articles and of included articles complemented electronic searches. We included all studies investigating a health application that used in-built sensors of a smartphone for diagnosis of disease. The methodological quality of 11 studies used in an exploratory meta-analysis was assessed with the QUADAS-2 tool and the reporting quality with the STARD statement. Sensitivity and specificity of studies reporting two-by-two tables were calculated and summarized. Results We screened 3’296 references for eligibility. Eleven studies, most of them assessing melanoma screening apps, reported 17 two-by-two tables. Quality assessment revealed high risk of bias in all studies. Included papers studied 1’048 subjects (758 with the target conditions and 290 healthy volunteers). Overall, the summary estimate for sensitivity was 0.82 (95 % confidence interval (CI); 0.56 to 0.94) and 0.89 (95 %CI; 0.70 to 0.97) for specificity. Conclusions The diagnostic evidence of available health apps on Apple’s and Google’s app stores is scarce. Consumers and healthcare professionals should be aware of this when using or recommending them
