Preparatory courses for medical residency: students point of view
OBJETIVO: Conhecer a opinião dos alunos do internato do Curso de Medicina da Universidade de Brasília sobre os cursos preparatórios para a residência médica. MÉTODOS: Aplicação de questionário padronizado aos alunos do primeiro ano do internato RESULTADOS: Participaram do estudo 67 dos 74 alunos (90,5%) dos dois primeiros semestres do internato. Dentre eles, 57 (85,1%) estão matriculados ou pretendem matricular-se em cursos preparatórios. Apenas 28,4% (19 alunos) acreditam que a participação nestes cursos atrapalhe as atividades do internato. A participação nos cursos preparatórios é considerada crucial para a aprovação nas provas de residência médica por 36 entrevistados (53,7%). Quando questionados quanto ao fator mais importante para a aprovação na prova de residência, 91,0% afirmaram ser o período de estudo teórico. Já quando o enfoque do estudo é a formação profissional, 92,5% reconheceram serem mais importantes as atividades desenvolvidas durante o internato. CONCLUSÃO: A maior parte dos alunos entrevistados considera que a participação em cursos preparatórios é importante para a aprovação nas provas para ingresso nos programas de residência médica e que esta participação não atrapalha as atividades do internato.OBJECTIVE: To know the opinion of Medical interns, University of Brasilia on the preparatory courses for medical residency. RESULTS: Sixty-seven of the 74 students (90.5%) of the first two semesters of internship participated. Among them, 57 (85.1%) are enrolled or intend to enroll in preparatory courses. Only 28.4% (19 students) believe that participation in these courses disrupt the activities of the internship. Participation in preparatory courses is considered crucial to the approval in the residency exams for 36 respondents (53.7%). When asked about the most important factor for the approval in the exams, 91.0% claimed to be the period of theoretical study. But when the focus of the study is professional training, 92.5% acknowledged the activities during the internship as the most important. CONCLUSION: Most students interviewed consider that participation in preparatory courses is important to be approved in the residency programs' admission exams and that this participation does not hinder the activities of the internship
Genética da doença de Parkinson no Brasil : revisão sistemática de formas monogênicas
Background: Increasing numbers of mutations causing monogenic forms of Parkinson's disease (PD) have been described, mostly among patients in Europe and North America. Since genetic architecture varies between different populations, studying the specific genetic profile of Brazilian patients is essential for improving genetic counseling and for selecting patients for clinical trials. Objective: We conducted a systematic review to identify genetic studies on Brazilian patients and to set a background for future studies on monogenic forms of PD in Brazil. Methods: We searched MEDLINE, EMBASE and Web of Science from inception to December 2019 using terms for "Parkinson's disease", "genetics" and "Brazil". Two independent reviewers extracted the data. For the genes LRRK2 and PRKN, the estimated prevalence was calculated for each study, and a meta-analysis was performed. Results: A total of 32 studies were included, comprising 94 Brazilian patients with PD with a causative mutation, identified from among 2,872 screened patients (3.2%). PRKN mutations were causative of PD in 48 patients out of 576 (8.3%). LRRK2 mutations were identified in 40 out of 1,556 patients (2.5%), and p.G2019S was the most common mutation (2.2%). Conclusions: PRKN is the most common autosomal recessive cause of PD, and LRRK2 is the most common autosomal dominant form. We observed that there was a lack of robust epidemiological studies on PD genetics in Brazil and, especially, that the diversity of Brazil’s population had not been considered.Introdução: Um número crescente de mutações causando formas monogênicas de doença de Parkinson (DP) tem sido descrito, principalmente entre pacientes da Europa e da América do Norte. Como a arquitetura genética varia entre diferentes populações, entender os perfis genéticos específicos de pacientes brasileiros é essencial para um melhor aconselhamento genético e para a seleção de participantes para ensaios clínicos. Objetivo: Revisão sistemática para identificar estudos genéticos brasileiros na área e definir o cenário para estudos futuros das formas monogênicas de DP no Brasil. Métodos: Nós pesquisamos as bases de dados MEDLINE, EMBASE e Web of Science desde a criação até dezembro de 2019, usando termos para “Parkinson’s disease”, “genetics” e “Brazil”. A extração de dados foi feita por dois revisores independentes. Para os genes LRRK2 e PRKN, calculamos a prevalência estimada para cada estudo e realizamos uma meta-análise. Resultados: Um total de 32 estudos foram incluídos e 94 pacientes brasileiros com DP com mutações causativas foram identificados em 2872 pacientes avaliados (3.2%). As mutações no PRKN causaram DP em 48 de 576 pacientes (8.3%). As mutações no LRRK2 foram identificadas em 40 de 1566 pacientes (2.5%), sendo a mutação mais comum a p.G2019S (2.2%). Conclusões: As mutações na PRKN são a causa mais comum de DP autossômica recessiva, e as mutações no LRRK2 a causa mais comum de DP autossômica dominante. Nós observamos uma falta de estudos epidemiológicos robustos em genética de DP, especialmente por não levar em conta a diversidade de nossa população
O ultrassom transcraniano é útil para o diagnóstico da doença de Parkinson na prática clínica?
A ultrassonografia transcraniana (UTC) é um exame complementar para diagnóstico de doença de Parkinson (DP). Objetivo Avaliar as características da UTC em pacientes com DP e seus diagnósticos diferenciais e estabelecer a precisão desse exame para o diagnóstico clínico de DP após seguimento. Métodos Avaliou-se retrospectivamente 85 pacientes com suspeita clínica inicial de DP, parkinsonismo atípico (PA) ou tremor essencial (TE), todos submetidos a UTC. Um consenso de dois especialistas determinou o diagnóstico clínico final após revisar os registros médicos das consultas de seguimento. A precisão do UTC foi calculada usando métodos estatísticos Bayesianos. Resultados O achado de hiperecogenicidade da substância negra (> 20 mm2) mostrou alta sensibilidade (93,4%) e especificidade (86,6%). A razão de verossimilhança positiva mostra 6.93 vezes mais chances de diagnosticar DP do que uma condição alternativa, se o achado estiver presente. Conclusões Este estudo demonstra a utilidade prática do UTC na diferenciação de DP de condições clínicas similares quando o diagnóstico clínico é inicialmente pouco claro.Transcranial sonography (TCS) is an emerging ancillary examination for diagnosing Parkinson’s disease (PD). Objective To evaluate TCS features in patients with PD and its mimics, and establish their accuracy in predicting the final clinical diagnosis after follow-up. Methods We retrospectively studied 85 patients with an initial clinical suspicion of PD, atypical parkinsonism or essential tremor, all of whom underwent TCS. Two specialists reviewed the follow-up clinical visit records and determined the final clinical diagnosis. The accuracy analysis of the TCS was determined using Bayesian statistical methods. Results The finding of substantia nigra hyperechogenicity (> 20 mm2) showed high sensitivity (93.4%) and specificity (86.6%). The positive likelihood ratio showed 6.93-fold greater odds for diagnosing PD than an alternative condition when this finding was present. Conclusions This study revealed the practical usefulness of TCS in differentiating PD from its prevalent mimics when the clinical diagnosis was initially unclear
Floor and ceiling effects on the Montreal Cognitive Assessment in patients with Parkinson’s disease in Brazil
ABSTRACT Parkinson’s disease (PD) is a common neurodegenerative disease associated with cognitive impairment. The Montreal Cognitive Assessment (MoCA) has been used as a recommended global cognition scale for patients with PD, but there are some concerns about its application, partially due to the floor and ceiling effects. Objective: To explore the floor and ceiling effects on the MoCA in patients with PD in Brazil. Methods: Cross-sectional study with data from patients with PD from five Brazilian Movement Disorders Clinics, excluding individuals with a possible diagnosis of dementia. We analyzed the total score of the MoCA, as well as its seven cognitive domains. The floor and ceiling effects were evaluated for the total MoCA score and domains. Multivariate analyses were performed to detect factors associated with floor and ceiling effects. Results: We evaluated data from 366 patients with PD and approximately 19% of individuals had less than five years of education. For the total MoCA score, there was no floor or ceiling effect. There was a floor effect in the abstraction and delayed memory recall domains in 20% of our sample. The ceiling effect was demonstrated in all domains (80.8% more common in naming and 89% orientation), except delayed recall. Education was the main factor associated with the floor and ceiling effects, independent of region, sex, age at evaluation, and disease duration. Conclusion: The floor and ceiling effects are present in specific domains of the MoCA in Brazil, with a strong impact on education. Further adaptations of the MoCA structure for underrepresented populations may reduce these negative effects
Comprometimento cognitivo na doença de Parkinson : correlatos clínicos, neuropsicológicos e de neuroimagem

Apesar de avanços na descrição de sua história natural, há dificuldades em definir
biomarcadores de estado que se correlacionem robustamente com as manifestações clínicas e
cognitivas. O estudo buscou descrever correlatos clínicos, neuropsicológicos e de neuroimagem
de comprometimento cognitivo leve (CCL) em pacientes com DP em estágios Hoehn & Yahr (HY)
1 e 2. Em um delineamento transversal observacional, foram avaliados pacientes com DP (n=69)
e controles (n=15) com uma bateria neuropsicológica abrangente. A bateria incluiu: Escala de
Avaliação Cognitiva da doença de Parkinson (PD-CRS), Montreal Cognitive Assessment
(MoCA), Extensão de dígitos direta e inversa, Teste de Semelhanças, Modalidades Símbolo-
Dígito (SDMT), Teste das trilhas parte A e B (TMT-A, TMT-B), Bateria Automatizada de Testes
Neuropsicológicos de Cambridge (CANTAB), Escala de Avaliação Funcional Cognitiva da
doença de Parkinson (PD- CFRS). Os grupos também foram avaliados quanto aos sintomas
motores, com a Escala Unificada de Avaliação da Doença de Parkinson (MDS-UPDRS, parte 3),
e de sintomas não motores, com a MDS-UPDRS (parte 1), Escala de Sintomas Não Motores
(NMSS), Inventário de Depressão de Beck (BDI), Escala Hospitalar de Ansiedade e Depressão
(HADS), Escala de Sono da doença de Parkinson (PDSS- BR), e o Questionário de Triagem para
Sintomas de Transtorno comportamental do sono REM (RBDSQ-BR). Uma subamostra (n=64)
realizou Ressonância Magnética (RM) cerebral, com análise de segmentação hipocampal,
volumes da superfície cortical e medidas de tensor de difusão (DTI) por estatística espacial
baseada em tratos (TBSS) e tratometria, em modelos ajustados para variáveis de confundimento,
como idade, sexo, escolaridade, gravidade dos sintomas motores e volume intracraniano. DP-
CCL foi definido conforme uma aproximação das recomendações da MDS. Os indivíduos com
DP (n=69) foram organizados em dois grupos: 36,2% (n=25) no grupo DP-CCL, e 73,8% (n=44)
no grupo com DP e cognição intacta (DP- CI). O grupo DP-CCL demostrou mais sintomas
motores axiais, apatia, irritabilidade, alucinações, sonolência diurna, desregulação
dopaminérgica e distúrbios motores noturnos (dor e distonia). A frequência dos subtipos tremor-
dominante (TD) e instabilidade postural-distúrbio de marcha (PIGD), e de transtorno do sono
REM, não diferiu entre os grupos com DP. Pacientes com DP mostraram sintomas depressivos
e ansiosos mais intensos que o grupo controle. O grupo com DP mostrou desempenho inferior
em testes executivos, como torre de Londres e TMT (B-A), de abstração (Semelhanças), e testes
de memória de reconhecimento visual e espacial (Aprendizado Pareado Associado e
Reconhecimento de Padrões). O volume da porção central do corpo caloso diferiu entre DP-CCL
e controles. O grupo DP-CCL apresentou menores volumes de subcampos do hipocampo:
camada molecular, região CA1, subículo e pré-subículo. Houve contrastes nos volumes corticais
independentemente da idade, sexo, escolaridade e MDS-UPDRS-3, no giros supramarginal e
temporal superior (DP-CCL vs. DP-CI) à esquerda, e no giro fusiforme e em áreas temporais
inferiores à direita (DP-CCL vs. controle). Mudanças de integridade microestrutural foram
encontradas nas radiações talâmicas superiores, radiações ópticas, feixe fronto-occipital inferior
e fascículo longitudinal superior I, no grupo DP-CCL. O estudo complementa os modelos de
disseminação neuroanatômica da atrofia na DP. Demonstrou-se redução volumétrica de
subcampos da formação hipocampal (CA1, complexo subicular e camada molecular), que ocorre
nos estágios HY1-2, ainda com cognição intacta. O declínio cognitivo inicial inclui envolvimento
cortical posterior (córtex parietal, giros fusiforme e temporal inferior). Os resultados indicaram
haver vulnerabilidade seletiva dos feixes de substância branca que se direcionam ao lobo
occipital, ou conectam regiões pré- frontais às parietais, ligando o DP-CCL às regiões cerebrais

Despite advances in the description of its natural history, there are still difficulties in defining state
biomarkers that are robustly correlated with clinical and cognitive manifestations. The study
sought to describe the clinical, neuropsychological and neuroimaging correlates of mild cognitive
impairment (MCI) in patients with PD in Hoehn & Yahr (HY) stages I and II. From a cross-sectional
observational study, we evaluated Parkinson's patients (n=69) and controls (n=15) using a
comprehensive neuropsychological battery. The battery included: Parkinson's Disease-Cognitive
Rating Scale (PD-CRS), Montreal Cognitive Assessment (MoCA), Forward and Backwards Digit
Span, Similarities (WAIS), Symbol-Digit Modalities (SDMT), Trail Making Test Parts A and B
(TMT-A and TMT-B), Cambridge Automated Neuropsychological Test Battery (CANTAB),
Parkinson's Disease-Cognitive Functional Assessment Scale (PD-CFRS). The groups were also
assessed for motor symptoms with the Unified Parkinson's Disease Rating Scale (MDS-UPDRS,
part 3), and for non-motor symptoms with the MDS-UPDRS (part 1), Non-Motor Symptom Scale
(NMSS), Beck Depression Inventory (BDI), Hospital Anxiety and Depression Scale (HADS),
Parkinson's Disease Sleep Scale (PDSS-BR), and Rapid Eye Movement Sleep Behavior Disorder
Screening Questionnaire (RBDSQ-BR). A subsample performed quantitative brain MRI, with
analysis of hippocampal segmentation, cortical surface volumes and analysis of DTI metrics using
tractometry and FSL-TBSS, using generalized linear models adjusted for confounding variables
such as age, gender, education, severity of motor symptoms and intracranial volume. PD-MCI
was defined according to an approximation of the MDS recommendations as suggested by
Pereira et al., 2014 and Goldman et al., 2015. Subjects with PD (n=69) were organized into two
groups: 36.2% (n=25) in PD-MCI group, and 73.8% (n=44) in the group with PD and intact
cognition (PD-IC). The PD-MCI group showed more axial motor symptoms, apathy, irritability,
hallucinations, daytime sleepiness, dopaminergic dysregulation, and nocturnal motor
disturbances (pain and dystonia). The frequency of tremor dominant (TD) and postural instability-
gait disorder (PIGD) subtypes and REM sleep behavior disorder did not differ between PD groups.
The PD patient group showed more intense depressive and anxiety symptoms than controls. The
PD group underperformed on executive tests, including Stockings of Cambridge, TMT (B-A),
abstraction (Similarities), visual and spatial recognition memory tests (Paired Associates Learning
and Pattern Recognition Memory). The volume of the central subsection of the corpus callosum
differed between the PD-MCI and control groups. The PD-MCI group had smaller volumes of
hippocampal subfields: molecular layer, CA1, subiculum and pre-subiculum. There were
differences in cortical volumes, regardless of age, education, and severity of motor symptoms, on
the left hemisphere, in the supramarginal and superior temporal gyri (PD-MCI vs. PD-IC); and in
the right fusiform gyrus and inferior temporal areas (PD- MCI vs. controls). Changes in DTI scalars
were found in the right hemisphere, in the superior thalamic radiations, optic radiations, inferior
fronto-occipital bundle, and superior longitudinal fasciculus I in the PD-MCI group. Our study
complements models for the neuroanatomical spread of atrophy in PD. It demonstrated
volumetric reduction of subfields of the hippocampal formation (CA1, subicular complex and
molecular layer), which occurs in stages HY 1-2, when patients still have PD-IC. Early cognitive
decline includes posterior cortical involvement (parietal cortex, fusiform and inferior temporal
gyrus). The results indicate a selective vulnerability of the white matter bundles that address the
occipital lobe, or connect prefrontal to parietal regions, linking PD-MCI to the posterior brain
regions

Altered structural connectivity in olfactory disfunction after mild COVID-19 using probabilistic tractography
Abstract We aimed to investigate changes in olfactory bulb volume and brain network in the white matter (WM) in patients with persistent olfactory disfunction (OD) following COVID-19. A cross-sectional study evaluated 38 participants with OD after mild COVID-19 and 24 controls, including Sniffin' Sticks identification test (SS-16), MoCA, and brain magnetic resonance imaging. Network-Based Statistics (NBS) and graph theoretical analysis were used to explore the WM. The COVID-19 group had reduced olfactory bulb volume compared to controls. In NBS, COVID-19 patients showed increased structural connectivity in a subnetwork comprising parietal brain regions. Regarding global network topological properties, patients exhibited lower global and local efficiency and higher assortativity than controls. Concerning local network topological properties, patients had reduced local efficiency (left lateral orbital gyrus and pallidum), increased clustering (left lateral orbital gyrus), increased nodal strength (right anterior orbital gyrus), and reduced nodal strength (left amygdala). SS-16 test score was negatively correlated with clustering of whole-brain WM in the COVID-19 group. Thus, patients with OD after COVID-19 had relevant WM network dysfunction with increased connectivity in the parietal sensory cortex. Reduced integration and increased segregation are observed within olfactory-related brain areas might be due to compensatory plasticity mechanisms devoted to recovering olfactory function
