Growth of infants born to HIV-positive mothers fed a whey-adapted acidified starter formula with prebiotics and nucleotides
Objectives: The objectives of this study were to evaluate whether infants born to known HIV-positive mothers, but who were not themselves infected with HIV and who were fed a chemically acidified starter formula with prebiotics with or without nucleotides during their first six months, displayed growth rates equal to uninfected infants fed a chemically acidified starter formula without prebiotics or nucleotides.Design: The design was a multi-centre, double-blinded randomised controlled trial.Setting: The study was carried out in four academic hospitals, three in Johannesburg and one in Cape Town, South Africa.Subjects and intervention: The subjects were newborn infants born to consenting HIV-positive women who had previously decided not to breast feed. The infants were randomised to receive one of three milk formulas. The intervention comprised chemically acidified formula without prebiotics or nucleotides, with prebiotics only, or with prebiotics and nucleotides.Outcome measures: The outcome measures were the growth parameters through the first six months of life.Results: Of the 150 randomised infants, 50 did not complete the study and 16 (12.8% of those tested) were infected with HIV, leaving 84 infants available for analysis. All three formulas were tolerated well, with no differences in growth parameters seen with the addition of prebiotics and nucleotides. The growth rates of the study infants up to the age of six months were very good, showing an increase in Z-scores from negative values at the time of enrolment in the first week after birth to around zero for length and > 0.5 for weight.Conclusions: The three chemically acidified formulas were tolerated well and resulted in good growth over the first six months of life. No benefits were seen with the addition of prebiotics or nucleotides. The growth rates were similar to those found in previous studies of ours on biologically acidified formulas. The chemical acidification of infant formulas appears to be a realistic alternative to biological acidification should an acidified formula be required.Keywords: milk formula; acidification; probiotics; nucleotides; infant growt
Time reclaimed: temporality and the experience of meaningful work
The importance of meaningful work has been identified in scholarly writings across a range of disciplines. However, empirical studies remain sparse and the potential relevance of the concept of temporality, hitherto somewhat neglected even in wider sociological studies of organizations, has not been considered in terms of the light that it can shed on the experience of work as meaningful. These two disparate bodies of thought are brought together to generate new accounts of work meaningfulness through the lens of temporality. Findings from a qualitative study of workers in three occupations with ostensibly distinct temporal landscapes are reported. All jobs had the potential to be both meaningful and meaningless; meaningfulness arose episodically through work experiences that were shared, autonomous and temporally complex. Schutz’s notion of the ‘vivid present’ emerged as relevant to understanding how work is rendered meaningful within an individual’s personal and social system of relevances

The aim of the AIDS Accountability Workplace Scorecard is to improve HIV and AIDS workplace programmes in
the countries and sectors most affected by the disease, and improve the health of employees, their families
and communities. Through this initiative we will: / 1. Provide tools for HIV and AIDS workplace programme monitoring and evaluation
AAI has developed scorecard tools for small, medium and large workplaces, which can be used to assess a
global, regional or national HIV and AIDS programme or interventions at a specific workplace site. The
scorecards can serve as both internal monitoring and evaluation tools and as assessments to present to
stakeholders within and outside the organization. / 2. Publish annual Rankings of HIV and AIDS Workplace Programmes
Scorecard users who wish to receive a ranking analysis and recommendations for how to improve their
programmes can submit their scorecards to AAI. AAI ‘s ranking analysis will allow users to compare their
performance with others and over time also measure their own progress. Respondents will be encouraged to
publish their ranking in AAI’s yearly Ranking Reports. / 3. Share good practice
The knowledge and good practices generated through the published rankings will be used to stimulate
improved HIV and AIDS Workplace Programmes worldwide. Large networks of companies, trade union
confederations, and national and international organizations can use the scorecard as a common framework
for monitoring and evaluation of workplace programmes
Recruitment into diabetes prevention programs : what is the impact of errors in self-reported measures of obesity?
BackgroundError in self-reported measures of obesity has been frequently described, but the effect of self-reported error on recruitment into diabetes prevention programs is not well established. The aim of this study was to examine the effect of using self-reported obesity data from the Finnish diabetes risk score (FINDRISC) on recruitment into the Greater Green Triangle Diabetes Prevention Project (GGT DPP).MethodsThe GGT DPP was a structured group-based lifestyle modification program delivered in primary health care settings in South-Eastern Australia. Between 2004–05, 850 FINDRISC forms were collected during recruitment for the GGT DPP. Eligible individuals, at moderate to high risk of developing diabetes, were invited to undertake baseline tests, including anthropometric measurements performed by specially trained nurses. In addition to errors in calculating total risk scores, accuracy of self-reported data (height, weight, waist circumference (WC) and Body Mass Index (BMI)) from FINDRISCs was compared with baseline data, with impact on participation eligibility presented.ResultsOverall, calculation errors impacted on eligibility in 18 cases (2.1%). Of n = 279 GGT DPP participants with measured data, errors (total score calculation, BMI or WC) in self-report were found in n = 90 (32.3%). These errors were equally likely to result in under- or over-reported risk. Under-reporting was more common in those reporting lower risk scores (Spearman-rho = −0.226, p-value < 0.001). However, underestimation resulted in only 6% of individuals at high risk of diabetes being incorrectly categorised as moderate or low risk of diabetes.ConclusionsOverall FINDRISC was found to be an effective tool to screen and recruit participants at moderate to high risk of diabetes, accurately categorising levels of overweight and obesity using self-report data. The results could be generalisable to other diabetes prevention programs using screening tools which include self-reported levels of obesity.<br /
What do we know about emotional labour in nursing? A narrative review
Nurses have to manage their emotions and the expression of emotion to perform best care, and their behaviours pass through emotional labour (EL). However, EL seems to be an under-appreciated aspect of caring work and there is no synthetic portrait of literature about EL in the nursing profession. This review was conducted to synthesise and to critically analyse the literature in the nursing field related to EL. Twenty-seven papers were included and analysed with a narrative approach, where two main themes were found: EL strategies and EL antecedents and consequences. Hence, EL is a multidimensional, complex concept and it represents a nursing competence to provide the best care. Moreover, nurses have a high awareness of EL as a professional competence, which is a fundamental element to balance engagement with an appropriate degree of detachment to accomplish tasks for best practice, and to provide high-quality patient care

Environmental heterogeneity decreases reproductive success via effects on foraging behaviour
Environmental heterogeneity shapes the uneven distribution of resources available to foragers, and is ubiquitous in nature. Optimal foraging theory predicts that an animal's ability to exploit resource patches is key to foraging success. However, the potential fitness costs and benefits of foraging in a heterogeneous environment are difficult to measure empirically. Heterogeneity may provide higher-quality foraging opportunities, or alternatively could increase the cost of resource acquisition because of reduced patch density or increased competition. Here, we study the influence of physical environmental heterogeneity on behaviour and reproductive success of black-legged kittiwakes, Rissa tridactyla. From GPS tracking data at 15 colonies throughout their British and Irish range, we found that environments that were physically more heterogeneous were associated with longer trip duration, more time spent foraging while away from the colony, increased overlap of foraging areas between individuals and lower breeding success. These results suggest that there is greater competition between individuals for finite resources in more heterogeneous environments, which comes at a cost to reproduction. Resource hotspots are often considered beneficial, as individuals can learn to exploit them if sufficiently predictable. However, we demonstrate here that such fitness gains can be countered by greater competition in more heterogeneous environments
Defining language impairments in a subgroup of children with autism spectrum disorder
Autism spectrum disorder (ASD) is diagnosed on the basis of core impairments in pragmatic language skills, which are found across all ages and subtypes. In contrast, there is significant heterogeneity in language phenotypes, ranging from nonverbal to superior linguistic abilities, as defined on standardized tests of vocabulary and grammatical knowledge. The majority of children are verbal but impaired in language, relative to age-matched peers. One hypothesis is that this subgroup has ASD and co-morbid specific language impairment (SLI). An experiment was conducted comparing children with ASD to children with SLI and typically developing controls on aspects of language processing that have been shown to be impaired in children with SLI: repetition of nonsense words. Patterns of performance among the children with ASD and language impairment were similar to those with SLI, and contrasted with the children with ASD and no language impairment and typical controls, providing further evidence for the hypothesis that a subgroup of children with ASD has co-morbid SLI.
