Forehead Skin Blood Flow in Normal Neonates during Active and Quiet Sleep, Measured with a Diode Laser Doppler Instrument
Changes in forehead skin blood flow during active and quiet sleep were determined in 16 healthy neonates using a recently developed semi-conductor laser Doppler flow meter without light conducting fibres. Measurements were carried out at a postnatal age varying from 5 hours to 7 days. The two sleep states could be distinguished in 17 recordings. The mean skin blood flow values during active sleep were significantly higher (p<0.01) than those during quiet sleep, the mean increase being 28.1%. The variability of the flow signal, expressed as the coefficient of variation, changed significantly from 23.1% during active sleep to 18.2% during quiet sleep
Building toolkits for COPD exacerbations: lessons from the past and present
In the nineteenth century, it was recognised that
acute attacks of chronic bronchitis were harmful. 140
years later, it is clearer than ever that exacerbations
of chronic obstructive pulmonary disease (ECOPD) are
important events. They are associated with significant
mortality, morbidity, a reduced quality of life and an
increasing reliance on social care. ECOPD are common
and are increasing in prevalence. Exacerbations beget
exacerbations, with up to a quarter of in-patient episodes
ending with readmission to hospital within 30 days. The
healthcare costs are immense. Yet despite this, the tools
available to diagnose and treat ECOPD are essentially
unchanged, with the last new intervention (non-invasive
ventilation) introduced over 25 years ago.
An ECOPD is ’an acute worsening of respiratory symptoms
that results in additional therapy’. This symptom and
healthcare utility-based definition does not describe
pathology and is unable to differentiate from other causes
of an acute deterioration in breathlessness with or without
a cough and sputum. There is limited understanding of
the host immune response during an acute event and no
reliable and readily available means to identify aetiology or
direct treatment at the point of care (POC). Corticosteroids,
short acting bronchodilators with or without antibiotics
have been the mainstay of treatment for over 30 years. This
is in stark contrast to many other acute presentations of
chronic illness, where specific biomarkers and mechanistic
understanding has revolutionised care pathways. So why
has progress been so slow in ECOPD? This review examines
the history of diagnosing and treating ECOPD. It suggests
that to move forward, there needs to be an acceptance
that not all exacerbations are alike (just as not all COPD is
alike) and that clinical presentation alone cannot identify
aetiology or stratify treatment

Reflective Journaling: A Theoretical Model and Digital Prototype for Developing Resilience and Creativity
Reflection is commonly discussed as a tool for personal and professional development that is becoming increasingly important in today’s global and digital world. In this paper, we propose a model that suggests ways in which reflection, in the form of Reflective Journaling, can support the development of creativity and resilience, which are needed to enable individuals to function effectively in a fast-changing environment. In addition, the model proposes ways in which external support and progress monitoring can be used in conjunction with skills in adaptive resilience and structured creativity, to support the maintenance of reflective journaling as a habit, in the longer term, thus creating virtuous cycles of skills and behaviours that can reinforce each other. Based on our model, and additional user research, we describe the design of a first digital prototype that aims to support the use of Reflective Journaling and to develop creativity and resilience through suggested mechanisms. Initial evaluations of our prototype are positive. It has been well-received by early test users, and has the potential to address all the connections defined. We therefore suggest that the theoretical model can be used to develop digital tools, such as the one included, to help those who wish to develop the habit of reflective journaling, and through that a range of other skills associated with resilience and creative thinking. We see this as a starting point for investigating this potential in more depth
Extragalactic Radio Continuum Surveys and the Transformation of Radio Astronomy
Next-generation radio surveys are about to transform radio astronomy by
discovering and studying tens of millions of previously unknown radio sources.
These surveys will provide new insights to understand the evolution of
galaxies, measuring the evolution of the cosmic star formation rate, and
rivalling traditional techniques in the measurement of fundamental cosmological
parameters. By observing a new volume of observational parameter space, they
are also likely to discover unexpected new phenomena. This review traces the
evolution of extragalactic radio continuum surveys from the earliest days of
radio astronomy to the present, and identifies the challenges that must be

Social norms of cooperation in small-scale societies
Indirect reciprocity, besides providing a convenient framework to address the evolution of moral systems, offers a simple and plausible explanation for the prevalence of cooperation among unrelated individuals. By helping someone, an individual may increase her/his reputation, which may change the pre-disposition of others to help her/him in the future. This, however, depends on what is reckoned as a good or a bad action, i.e., on the adopted social norm responsible for raising or damaging a reputation. In particular, it remains an open question which social norms are able to foster cooperation in small-scale societies, while enduring the wide plethora of stochastic affects inherent to finite populations. Here we address this problem by studying the stochastic dynamics of cooperation under distinct social norms, showing that the leading norms capable of promoting cooperation depend on the community size. However, only a single norm systematically leads to the highest cooperative standards in small communities. Validity of a self-reported measure of familial history of obesity
<p>Abstract</p> <p>Background</p> <p>Familial history information could be useful in clinical practice. However, little is known about the accuracy of self-reported familial history, particularly self-reported familial history of obesity (FHO).</p> <p>Methods</p> <p>Two cross-sectional studies were conducted. The aims of study 1 was to compare self-reported and objectively measured weight and height whereas the aims of study 2 were to examine the relationship between the weight and height estimations reported by the study participants and the values provided by their family members as well as the validity of a self-reported measure of FHO. Study 1 was conducted between 2004 and 2006 among 617 subjects and study 2 was conducted in 2006 among 78 participants.</p> <p>Results</p> <p>In both studies, weight and height reported by the participants were significantly correlated with their measured values (study 1: r = 0.98 and 0.98; study 2: r = 0.99 and 0.97 respectively; p < 0.0001). Estimates of weight and height for family members provided by the study participants were strongly correlated with values reported by each family member (r = 0.96 and 0.95, respectively; p < 0.0001). Substantial agreement between the FHO reported by the participants and the one obtained by calculating the BMI of each family members was observed (kappa = 0.72; p < 0.0001). Sensitivity (90.5%), specificity (82.6%), positive (82.6%) and negative (90.5%) predictive values of FHO were very good.</p> <p>Conclusion</p> <p>A self-reported measure of FHO is valid, suggesting that individuals are able to detect the presence or the absence of obesity in their first-degree family members.</p
Marine Biodiversity in South Africa: An Evaluation of Current States of Knowledge
Continental South Africa has a coastline of some 3,650 km and an Exclusive Economic Zone (EEZ) of just over 1 million km2. Waters in the EEZ extend to a depth of 5,700 m, with more than 65% deeper than 2,000 m. Despite its status as a developing nation, South Africa has a relatively strong history of marine taxonomic research and maintains comprehensive and well-curated museum collections totaling over 291,000 records. Over 3 million locality records from more than 23,000 species have been lodged in the regional AfrOBIS (African Ocean Biogeographic Information System) data center (which stores data from a wider African region).
