    Educación para la convivencia en contextos escolares: una propuesta de intervención contra los malos tratos entre iguales

    Presentamos un programa de intervención contra los malos tratos entre iguales que está integrado en dos proyectos de investigación europeos – el Proyecto TMR para el estudio de la naturaleza y prevención de los malos tratos entre iguales y el Proyecto Sócrates – y se desarrolla a través de la formación del profesorado y del alumnado de un centro de enseñanza secundaria. El programa tiene como objetivo la mejora de las habilidades sociales de los miembros de la comunidad educativa, como medio para desarrollar relaciones interpersonales positivas, que se consideran básicas para la mejora del clima de convivencia. Mostramos los efectos que la mejora del clima de convivencia tiene sobre la incidencia de los malos tratos en el centro donde se realiza el estudio. Los resultados señalan una mejora del clima de convivencia en el centro tras la implementación del programa y una reducción de la incidencia del maltrato entre iguales.An intervention program against bullying is presented in this article. The intervention program is integrated in two European research projects – the TMR Project about nature and prevention of bullying and the Sócrates Project – and it is developed through out pupils and teachers’ training. The intervention program persecutes the improvement of social skills in order to develop positive interpersonal relationships those are considered the base for improving the conviviality climate. Finally, we show the effects of this climate improvement on bullying incidence in the centre where the study was made. The results point out an improvement of climate after the implementation of the program as well as a reduction of the bullying incidence

    Incidencia de los malos tratos entre iguales en alumnos de Hebrón (Palestina)

    Presentamos un estudio sobre malos tratos entre iguales realizado en la ciudad de Hebrón (Palestina). Utilizando el Bullies and Victims Questionnaire de Olweus, se presenta una descripción del fenómeno de los malos tratos entre adolescentes palestinos. La muestra está compuesta por 1492 alumnos de educación primaria y secundaria, de edades comprendidas entre los 10 y los 16 años, y pertenecientes a centros situados en ámbitos urbanos y rurales de Hebrón. Los datos muestran unas tasas de incidencia del maltrato similares a las encontradas en los países occidentales: 11,1% de víctimas, 9,5% de agresores y 78,2% de espectadores. Sin embargo, los datos también ponen de manifiesto que un mayor número de alumnos palestinos, en comparación con estudios occidentales, están involucrados en episodios de maltrato hacia los compañeros, ya sea como víctimas o como agresores. De la misma forma, los datos confirman tendencias similares en el fenómeno de los malos tratos entre iguales entre el alumnado palestino y las encontradas en el alumnado occidental.We present a study about bullying carried out in Hebron (Palestine). In the study is used the Olweus’ Bullies and Victims Questionnaire and show a description of bullying phenomena among Palestine’ teenagers. The sample is composed by 1492 students for primary and secondary education, 10-16 aged from urban and rural centres. The data shows similar incidence taxes than in occidental researches: 11,1% of victims, 9,5% of aggressors and 78,2% of bystanders. However, the data point out a higher amount of Palestine students who are exposed to violence as victims or/and aggresors, in comparison with occidental studies. In the same way, some tendencies are confirmed – those allows to characterize the bullying phenomenon – among Palestine students and are similar than founded among occidental students

    Assessing the impact of COVID-19 on liver cancer management (CERO-19).

    BACKGROUND & AIMS: The coronavirus disease 2019 (COVID-19) pandemic has posed unprecedented challenges to healthcare systems and it may have heavily impacted patients with liver cancer (LC). Herein, we evaluated whether the schedule of LC screening or procedures has been interrupted or delayed because of the COVID-19 pandemic. METHODS: An international survey evaluated the impact of the COVID-19 pandemic on clinical practice and clinical trials from March 2020 to June 2020, as the first phase of a multicentre, international, and observational project. The focus was on patients with hepatocellular carcinoma or intrahepatic cholangiocarcinoma, cared for around the world during the first COVID-19 pandemic wave. RESULTS: Ninety-one centres expressed interest to participate and 76 were included in the analysis, from Europe, South America, North America, Asia, and Africa (73.7%, 17.1%, 5.3%, 2.6%, and 1.3% per continent, respectively). Eighty-seven percent of the centres modified their clinical practice: 40.8% the diagnostic procedures, 80.9% the screening programme, 50% cancelled curative and/or palliative treatments for LC, and 41.7% modified the liver transplantation programme. Forty-five out of 69 (65.2%) centres in which clinical trials were running modified their treatments in that setting, but 58.1% were able to recruit new patients. The phone call service was modified in 51.4% of centres which had this service before the COVID-19 pandemic (n = 19/37). CONCLUSIONS: The first wave of the COVID-19 pandemic had a tremendous impact on the routine care of patients with liver cancer. Modifications in screening, diagnostic, and treatment algorithms may have significantly impaired the outcome of patients. Ongoing data collection and future analyses will report the benefits and disadvantages of the strategies implemented, aiding future decision-making.     Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Differential clinical characteristics and prognosis of intraventricular conduction defects in patients with chronic heart failure

    Intraventricular conduction defects (IVCDs) can impair prognosis of heart failure (HF), but their specific impact is not well established. This study aimed to analyse the clinical profile and outcomes of HF patients with LBBB, right bundle branch block (RBBB), left anterior fascicular block (LAFB), and no IVCDs. Clinical variables and outcomes after a median follow-up of 21 months were analysed in 1762 patients with chronic HF and LBBB (n = 532), RBBB (n = 134), LAFB (n = 154), and no IVCDs (n = 942). LBBB was associated with more marked LV dilation, depressed LVEF, and mitral valve regurgitation. Patients with RBBB presented overt signs of congestive HF and depressed right ventricular motion. The LAFB group presented intermediate clinical characteristics, and patients with no IVCDs were more often women with less enlarged left ventricles and less depressed LVEF. Death occurred in 332 patients (interannual mortality = 10.8%): cardiovascular in 257, extravascular in 61, and of unknown origin in 14 patients. Cardiac death occurred in 230 (pump failure in 171 and sudden death in 59). An adjusted Cox model showed higher risk of cardiac death and pump failure death in the LBBB and RBBB than in the LAFB and the no IVCD groups. LBBB and RBBB are associated with different clinical profiles and both are independent predictors of increased risk of cardiac death in patients with HF. A more favourable prognosis was observed in patients with LAFB and in those free of IVCDs. Further research in HF patients with RBBB is warranted

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries
