    Pregnancy Associated Breast Cancer: An Analysis of Fetal Treatment Risk

    There are several viable treatment options for patients with PABC considered un-harmful to fetal development. Trastusumab, or Herceptin, targets HER2 protein and successfully combats aggressive breast cancer. In standard doses, it appears to be safe to the fetus even when administered during the first trimester. A likely side effect of Herceptin is anhydramnios, which can be monitored for throughout the stages of a pregnancy (Shrim et al. 2008). Anthracyclines, commonly used in chemotherapy, appear to be non-toxic, and have been used to successfully cure PABC patients in their second and third trimester. However, first trimester spontaneous abortions are documented to increase dramatically in anthracycline receiving PABC patients (Ring A. E. et al. 2005). Taxanes, such as paclitaxel, docetaxel, and vinorelbine, are microtubule agents that are highly active against breast cancer. As cytotoxic drugs they can possibly disrupt organogenesis and their administration should therefore be delayed until after the first trimester. Taxanes as well as anthracycline treatments should also be halted three weeks prior to delivery to prevent the occurrence of neutropenia, a decrease in white blood cells (neutrophil) that raises the risk of infection, and allow for regeneration of platelet count in the bloodstream (Mir O. et al 2008). Breast surgery can be safely performed at any point of a pregnancy although it is recommended to be postponed until after the first trimester. External beam radiation and hormone therapy are both documented to have negative effects on a developing fetus and therefore should not be considered as possible treatment options for PABC patients. In diagnosing and assessing PABC, ionizing radiation should be replaced by chest x-rays accompanied with proper shielding to protect the fetus. Sonograms appear to be more effective in diagnosing breast cancer than mammograms, because of the changes in pregnancy related breast density. Although PABC treatment is more complicated than standard breast cancer treatment, treatment during pregnancy has been proven to be a feasible and recommended option (Loibl S. et al. 2005)

    Economically Disadvantaged Rural Multigenerational Families Raising Infants

    This study explores psychological distress, parenting behaviors and infant temperamental/behavioral risk among rural economically disadvantaged families in which mothers and grandmothers are raising an infant together. Traditional components of family stress frameworks that model the influence of economic disadvantage on child outcomes via parenting were expanded to include factors that may be particularly relevant to these multigenerational families of varying maternal age. This study also simultaneously considers the relationship between observed mother and grandmother parenting and early child risk. The findings suggest that despite the unique risks to psychological well-being faced by grandmothers, the grandmothers display higher levels of sensitive parenting. This sensitive parenting is related to infant temperamental/behavioral risk in the presence of high levels of maternal negative parenting, but only for white families. Mothers and grandmothers display similar levels of negative intrusive parenting, but different factors are linked to the observed parenting of each generation. These findings contribute to understanding the adaptive benefits and risks of three-generation households

    Increasing communication effectiveness per personality types in an effort to enhance student retention

    Indiana University-Purdue University Indianapolis (IUPUI)Student retention is a problem that continues to plaque higher education institutions whose ultimate goal is to graduate students. The reported national student retention average in 2006 was between 58 and 71.6 percent, depending on to which statistics you refer. The importance for the academic community is that “the loss of students returning to campus for another year usually results in greater financial loss and a lower graduation rate for the institution, and might also affect the way that stakeholders, legislators, parents, and students view the institution” (Lau, 2003). In order to combat low student retention rates, many have initiated a variety of programs and strategic measures to increase students’ likelihood to complete their education. These initiatives can be found in the form of committees designated to conduct research and subsequently implement programs, colleges hiring outside consultants to assist with retention strategies, and the implementation of “student success” courses into the existing curriculum. Additional measures at the campus level may include: retention merit initiatives, student satisfaction and instructor surveys, and re-entry campaigns to target withdrawn students. According to Tinto (2002), “Most institutions, in my view, have not taken student retention seriously. They have done little to change the way they organize their activities, done little to alter the student experience, and therefore done little to address the deeper roots of student attrition”. The author faults the institutions that attempt to combat the issue by simply adding a course that is “marginal to the academic life of the institution”. While he does not directly address using personality or learning styles as a tool to combat student attrition, he states that, “Institutions that are successful in building settings that educate their students, all students, not just some, are institutions that are successful in retaining their students”. This research will provide an in depth look at existing personality type and retention data, an examination of communication incidents as reported by both “graduates” and “withdrawn” students, and recommendations for implementing personality-based communication techniques in the classroom in an effort to enhance overall student satisfaction. Considering the explosive growth of web-based distance education courses and program offerings, additional considerations will be made to address the online learning environment and its unique communicative needs. It is my assertion that both student retention and overall satisfaction can be enhanced with knowledge of existing personality and learning types of both students and teachers and a modification of the communication processes to fit students’ varying styles and communicative needs. By conducting a very basic level of research on personality types, one can find an abundance of information, each assessment claiming to be more effective than the others. Several textbooks, websites, and employer profiling systems guide users to various paper or web based tests which solicit descriptors of one’s own behavior, characteristics, and tendencies. First published in 1962, one widely recognized psychometric questionnaire used frequently in career counseling is the Myers-Briggs Type Indicator. Respondents are asked to answer 93 forced-choice questions based on their preference of two words or short statements. The results are given in the form of a four letter abbreviation, each letter representing one of their four type preferences based on four dichotomies. The four dichotomies are Extraversion vs. Introversion, Sensing vs. Intuition, Thinking vs. Feeling, and Judging vs. Perceiving. “The MBTI suggests general areas of life, or careers, in which persons are most apt to be interested, motivated, and successful” (Van, 1992, p. 20). As described by John (1990), “The five-factor model is a descriptive framework within which all the important individual differences in personality are subsumed under five global traits” (as quoted in Wolfe & Johnson, 1995, p. 178). The Five Factor Model identifies the “Big Five” personality traits of its respondents and presents them as percentile scores. Measures are comprised of either self-descriptive sentences or adjectives. The Big Five factors are as follows: Openness, Conscientiousness, Extraversion, Agreeableness, and Neuroticism. “A personality taxonomy such as the “16-factor model developed by Cattell (1965), posits that there are 16 primary personality factors” (Lidy & Kahn, 2006, p. 124). Through extensive research on the subject, and self assessing with a variety of these tests, the model I have chosen to highlight here is the DiSC personality assessment. The tool measures personality types based on a word association that offers a number of descriptors and asks participants to select the one that is “most like” and “least like” them. The in-depth profile then provides a bar graph measure of each of the four dimensions and a “classical pattern” to the participants. The four dimensions of the assessment are as follows: D (Dominant), i (Influencer), S (Steadiness), C (Conscientiousness)

    Wellbeing Among Rural Grandfamilies in Two Multigenerational Household Structures

    Multigenerational households are an understudied type of grandfamily. In rural communities, these households are likely to be economically disadvantaged and underserved. Drawing from a subset (N = 63) of low-income multigenerational households in a multistate research study, Rural Families Speak About Health, the present study compares demographic characteristics, parent and child wellbeing, and family processes in two types of multigenerational household structures: one-parent/grandparent families and two-parent/grandparent families. Research on these multigenerational household configurations is rare despite the potential for different needs, strengths, and services. Results indicate no differences in economic hardship or disadvantage by household type. Children in one-parent/grandparent households were older, and mothers reported providing more elder care than in two-parent/grandparent homes. There were no statistically significant differences in mother or child wellbeing across these family structures. Differences in family processes emerged. Specifically, mothers’ reports of parenting alliances and family routines varied by household type such that mothers in one-parent/grandparent households reported stronger parenting alliances and more stable family routines than those in two-parent/grandparent families. Implications of the findings for service professionals seeking to design and implement family support and prevention programs for grandfamilies, particularly in rural areas, are discussed

    Accuracy and consistency of letter formation in children with Developmental Coordination Disorder: an exploratory study

    Background: Handwriting difficulties are frequently mentioned in descriptions of Developmental Coordination Disorder (DCD). Recent studies have shown that children with DCD pause more and produce less text than typically developing (TD) peers. This temporal dysfluency indicates a lack of automaticity in handwriting production. One possible contributing factor is the accuracy and consistency of letter formation. The aim of this study was to gain a better understanding of handwriting dysfluency by examining the accuracy and consistency of letter production both within and across different writing tasks. Method: Twenty-eight 8-15 year-old children with DCD participated in the study, with 28 typically developing (TD) age and gender matched controls. They completed the alphabet writing and copy fast tasks from the Detailed Assessment of Speed of Handwriting on a digitising writing tablet. The accuracy and consistency of letter production were examined. Results & Discussion: The DCD group had a higher percentage of errors within their letterforms than TD peers. Letter production was also less consistent between tasks. Children with DCD appear to have difficulties with the ‘allograph’ (motor program) aspect of handwriting and may require explicit teaching of letter formation

    Strategic Players for Identifying Optimal Social Network Intervention Subjects

    We present a method whereby social network ties are used to identify behavioral leaders who are situated in the network such that these individuals are: 1) able to influence other individuals who are in need of and most receptive to intervention, thereby optimizing the impact of the intervention; and 2) not embedded with ties to individuals that are likely to be behaviorally antagonistic to the intervention or that would compromise the optimal impact of intervention. In this study we developed a method that we call Strategic Players, which is a solution for identifying a set of players who are close to a target subset of the network (i.e., the target group), and far away from the subset we wish to avoid (i.e. the avoid group), where the proximity to either the target or avoid group may be facilitated by network members who are in neither group (i.e. the neutral group). This solution seeks to maximize the diffusion of the behavior to the target group while minimizing contact and influence to the avoid group. We apply this method to two different social networks, and one simulated social network

    Resistance to Peer influence Moderates the Relationship Between Perceived (But Not Actual) Peer Norms and Binge Drinking in a College Student Social Network

    Introduction: Adolescent and young adult binge drinking is strongly associated with perceived social norms and the drinking behavior that occurs within peer networks. The extent to which an individual is influenced by the behavior of others may depend upon that individual’s resistance to peer influence (RPI). Methods: Students in their first semester of college (N = 1323; 54.7% female, 57% White, 15.1% Hispanic) reported on their own binge drinking, and the perceived binge drinking of up to 10 important peers in the first-year class. Using network autocorrelation models, we investigated cross-sectional relationships between participant’s binge drinking frequency and the perceived and actual binge drinking frequency of important peers. We then tested the moderating role of RPI, expecting that greater RPI would weaken the relationship between perceived and actual peer binge drinking on participant binge drinking. Results: Perceived and actual peer binge drinking were statistically significant predictors of participant binge drinking frequency in the past month, after controlling for covariates. RPI significantly moderated the association between perceptions of peer binge drinking and participant’s own binge drinking; this association was weaker among participants with higher RPI compared to those with lower RPI. RPI did not interact with the actual binge drinking behavior of network peers

    Do Misperceptions of Peer Drinking Influence Personal Drinking Behavior? Results From a Complete Social Network of First-Year College Students

    This study considered the influence of misperceptions of typical versus self-identified important peers\u27 heavy drinking on personal heavy drinking intentions and frequency utilizing data from a complete social network of college students. The study sample included data from 1,313 students (44% male, 57% White, 15% Hispanic/Latinx) collected during the fall and spring semesters of their freshman year. Students provided perceived heavy drinking frequency for a typical student peer and up to 10 identified important peers. Personal past-month heavy drinking frequency was assessed for all participants at both time points. By comparing actual with perceived heavy drinking frequencies, measures of misperceptions of heavy drinking (accurately estimate, overestimate, underestimate) were constructed for both general and important peers. These misperceptions were then used as predictors of concurrent and prospective personal heavy drinking frequency and intentions using network autocorrelation analyses. The majority of students (84.8%) overestimated, 11.3% accurately estimated, and 3.9% underestimated heavy drinking among their general peers, whereas 42.0% accurately estimated, 36.9% overestimated, and 21.1% underestimated important peers\u27 heavy drinking. For both referents, overestimation of peer heavy drinking was associated with more frequent heavy drinking and higher drinking intentions at both time points. Importantly, the effects of underestimating and overestimating close peers\u27 drinking on personal alcohol use were significant after controlling for the influence of misperceptions of general peers\u27 heavy drinking. Close peers are a critical referent group in assessments related to social norms for young adult alcohol use. Implications for prevention and intervention are discussed
