Porcelain laminate veneers are one of the most preferred treatment methods today, due to the increasing interest in minimally invasive approach and their superior aesthetic properties. With the appreciation of the minimally invasive approach and the developing adhesive procedures, the partial laminate veneers have become more popular in dental field. In these case series, the teeth were restored with partial laminate veneers by only intervening in the fractured area. In this case series, the teeth that were restored with partial laminate veneers were prepared with different techniques. Preparation was performed without bevel in enamel margin in the case 1, with incisal overlap and bevel to create more natural transition on tooth surface in the case 2. In the case 3, preparation with incisal overlap was performed with preparation of the palatal surface of the tooth. The optical impression was made with the digital camera of the Chairside Economical Restoration of Esthetic Ceramic (CEREC) acquisition unit (Omnicam CEREC Sirona, Bensheim, Germany), in the first case. In the other cases, impression was taken with elastomeric impression materials. In addition, restorations were fabricated from feldspathic porcelain and lithium disilicate porcelain. In all the cases, restoration was cemented with resin cements. The following cases present the minimally invasive treatment of maxillary central incisors with a partial laminate veneers fabricated from different ceramic materials. New materials, procedures, and techniques provide patients an improved quality of life and a greater self-esteem with creating a confident aesthetic smile

Amaç: Bu çalışmanın amacı, termal ve mekanik yükleme döngülerinin, bir self – etch rezin yapıştırma simanının dentine bağlanma dayanımını in vitro şartlar altında değerlendirmektir.Gereç ve Yöntemler: Çürüksüz, 18 adet mandibular üçüncü molar diş üzerinde inlay kaviteleri hazırlandı. Restorasyonlar, feldspatik porselen kullanılarak hazırlandı (Vita VM9). Dentine bağlantı bir dual- cure, self- etch rezin yapıştırma simanı (Clearfil Esthetic Cement) kullanılarak sağlandı. Sonrasında dişler rastgele üç gruba ayrıldı: Grup I: Kontrol Grubu (termal veya mekanik siklus yok). Grup II: Termal siklus (5.000 termal siklus).

Regio- and stereo-chemical ring-opening reactions of the 2,3-epoxy alcohol derivative with nucleophiles: Explanation of the structures and C-2 selectivity supported by theoretical computations

Get PDF
The ring-opening reactions of (1aS,2S,6bR)-5-ethyl-2-hydroxyhexahydro-4H-oxireno[2,3-e]isoindole-4,6(5H)-dione were investigated under very mild and nonchelated conditions. C-2 selective ring-opening products were obtained with nucleophilic additions such as Cl-, Br- and N-3(-). The exact configuration of (3aS,4R,5R,6S,7aS)-5-chloro-2-ethyl-4,6-dihydroxyhexahydro-1H-isoindole-1,3(2H)-dione was determined by X-Ray diffraction analysis which was obtained from the reaction of epoxy alcohol with HCl . On the other hand, theoretical computations were carried out to explain the regioselectivity in the ring opening reaction of epoxy alcohols. The results showed that the ring-opening reaction of both epoxy alcohols proceeds in a kinetically controlled manner and regioselectivity occurs depending on the transition state. Get PDF
OBJECTIVE: Differential diagnosis of idiopathic pulmonary fibrosis (IPF) is important among fibrotic interstitial lung diseases (ILD). This study aimed to evaluate the rate of IPF in patients with fibrotic ILD and to determine the clinical-laboratory features of patients with and without IPF that would provide the differential diagnosis of IPF. MATERIAL AND METHODS: The study included the patients with the usual interstitial pneumonia (UIP) pattern or possible UIP pattern on thorax high-resolution computed tomography, and/or UIP pattern, probable UIP or possible UIP pattern at lung biopsy according to the 2011 ATS/ERSARS/ALAT guidelines. Demographics and clinical and radiological data of the patients were recorded. All data recorded by researchers was evaluated by radiology and the clinical decision board. RESULTS: A total of 336 patients (253 men, 83 women, age 65.8 +/- 9.0 years) were evaluated. Of the patients with sufficient data for diag-nosis (n=300), the diagnosis was IPF in 121 (40.3%), unclassified idiopathic interstitial pneumonia in 50 (16.7%), combined pulmonary fibrosis and emphysema (CPFE) in 40 (13.3%), and lung involvement of connective tissue disease (CTD) in 16 (5.3%). When 29 patients with definite IPF features were added to the patients with CPFE, the total number of IPF patients reached 150 (50%). Rate of male sex (p<0.001), smoking history (p<0.001), and the presence of clubbing (p=0.001) were significantly high in patients with IPE None of the women <50 years and none of the men <50 years of age without a smoking history were diagnosed with IPE Presence of at least 1 of the symptoms suggestive of CTD, erythrocyte sedimentation rate (ESR), and antinuclear antibody (FANA) positivity rates were significantly higher in the non-IPF group (p<0.001, p=0.029, p=0.009, respectively). CONCLUSION: The rate of IPF among patients with fibrotic ILD was 50%. In the differential diagnosis of IPF, sex, smoking habits, and the presence of clubbing are important. The presence of symptoms related to CTD, ESR elevation, and EANA positivity reduce the likelihood of IPF

Diagnosis of comorbid migraine without aura in patients with idiopathic/genetic epilepsy based on the gray zone approach to the International Classification of Headache Disorders 3 criteria

Get PDF
BackgroundMigraine without aura (MwoA) is a very frequent and remarkable comorbidity in patients with idiopathic/genetic epilepsy (I/GE). Frequently in clinical practice, diagnosis of MwoA may be challenging despite the guidance of current diagnostic criteria of the International Classification of Headache Disorders 3 (ICHD-3). In this study, we aimed to disclose the diagnostic gaps in the diagnosis of comorbid MwoA, using a zone concept, in patients with I/GEs with headaches who were diagnosed by an experienced headache expert.MethodsIn this multicenter study including 809 consecutive patients with a diagnosis of I/GE with or without headache, 163 patients who were diagnosed by an experienced headache expert as having a comorbid MwoA were reevaluated. Eligible patients were divided into three subgroups, namely, full diagnosis, zone I, and zone II according to their status of fulfilling the ICHD-3 criteria. A Classification and Regression Tree (CART) analysis was performed to bring out the meaningful predictors when evaluating patients with I/GEs for MwoA comorbidity, using the variables that were significant in the univariate analysis.ResultsLonger headache duration (<4 h) followed by throbbing pain, higher visual analog scale (VAS) scores, increase of pain by physical activity, nausea/vomiting, and photophobia and/or phonophobia are the main distinguishing clinical characteristics of comorbid MwoA in patients with I/GE, for being classified in the full diagnosis group. Despite being not a part of the main ICHD-3 criteria, the presence of associated symptoms mainly osmophobia and also vertigo/dizziness had the distinguishing capability of being classified into zone subgroups. The most common epilepsy syndromes fulfilling full diagnosis criteria (n = 62) in the CART analysis were 48.39% Juvenile myoclonic epilepsy followed by 25.81% epilepsy with generalized tonic-clonic seizures alone.ConclusionLonger headache duration, throbbing pain, increase of pain by physical activity, photophobia and/or phonophobia, presence of vertigo/dizziness, osmophobia, and higher VAS scores are the main supportive associated factors when applying the ICHD-3 criteria for the comorbid MwoA diagnosis in patients with I/GEs. Evaluating these characteristics could be helpful to close the diagnostic gaps in everyday clinical practice and fasten the diagnostic process of comorbid MwoA in patients with I/GEs

WOS: 000355962800016PubMed ID: 25740306The aim of this study was to investigate the influence of denture cleansers on surface roughness (Ha, mu m) of two hard permanent reline resins and a conventional acrylic denture base resin. Fifty specimens of each material, measuring 10 mmx2 mm were randomly divided into 1 control and 4 experimental subgroups (n=10). Surface roughness values were measured using profilometer before and after immersion in distilled water and in 4 different denture cleansers for 8 h for 140 days. The Kruskal Wallis and Wilcoxon Signed Ranks tests were used for comparison among groups with a significance level of alpha=0.05. Immersion in denture cleansers significantly increased the surface roughness of reline resins and the difference among the cleansers was statistically significant (p=0.059). Denture cleansers can cause significant surface roughness alterations on hard permanent reline resins and denture base resin where their effect may differ according to the selected resin material
