Znaczenie pomiaru cząstkowej rezerwy przepływu wieńcowego (FFR) a iFR — czy potrzebujemy nowej techniki do oceny istotności zwężenia naczyń wieńcowych?
Fractional flow reserve (FFR) is a technique, which allows the accurate assessment of stenosis severity and can be easily and quickly measured during coronary angiography. For past years, FFR has been a basic tool in interventional cardiology, allowing to assess the potential benefit of revascularisation of coronary artery disease. Instantaneous wave-free ratio (iFR) is a recently developed, helpful physiological index, used to evaluate the severity of stenosis, calculated by measuring the resting pressure gradient across a coronary lesion during the portion of diastole. During the American College of Cardiology 2017 Scientific Session in Washington, the outcomes of two large, randomised trials were presented: iFR-SWEDEHEART and DEFINE-FLAIR, comparing iFR vs FFR with a combined total of 4,500 patients. The outcomes show, that iFR is non-inferior to FFR, with similar rates of major adverse cardiac events. We have to wait for the results from further randomised studies, comparing FFR with iFR and long-term follow-up of patients, who were assigned to undergo evaluation of stenosis severity. Pomiar cząstkowej rezerwy przepływu wieńcowego (FFR) jest precyzyjną oceną istotności zwężenia w naczyniach wieńcowych, którą można łatwo i szybko wykonać w trakcie koronarografii. Pomiar ten od lat stanowi podstawowe narzędzie w kardiologii interwencyjnej przy podejmowaniu decyzji dotyczącej stentowania zmienionego naczynia wieńcowego. Z kolei rozkurczowa ocena gradientu ciśnienia (iFR) jest stosunkowo nowym, pomocniczym narzędziem w ocenie istotności zwężenia naczynia wieńcowego, mierzącym gradient przez zwężenie w trakcie rozkurczu. Podczas konferencji American College of Cardiology w 2017 roku podczas Scientific Sessions w Waszyngtonie przedstawiono wyniki 2 dużych, randomizowanych badań, iFR-SWEDEHEART i DEFINE-FLAIR, służących porównaniu bezpośrednio iFR i FFR u blisko 4500 pacjentów. Opublikowane wyniki wskazują, że iFR jest nie gorszym narzędziem diagnostycznym niż FFR, z porównywalnym głównym punktem końcowym w ocenie rocznej. Pozostaje czekać na kolejne wyniki badań, w których zostaną porównane FFR i iFR oraz dalszą, odległą obserwację chorych poddanych analizie istotności zwężeń w naczyniach wieńcowych

Co nowego w niewydolności serca?
Niewydolność serca należy do chorób o wybitnie złym rokowaniu. Mimo intensywnie wdrażanych
działań prewencyjnych częstość występowania niewydolności serca nadal rośnie, podobnie
jak zainteresowanie jej tematyką wśród wielu grup badawczych na całym świecie. Optymalne
postępowanie w niewydolności serca, zarówno w jej stabilnym stadium, jak i w okresie
dekompensacji, jest jednym z najtrudniejszych problemów w codziennej praktyce klinicznej.
Przestrzeganie wytycznych ESC, dotyczących rozpoznawania i leczenia ostrej i przewlekłej
niewydolności serca, przekłada się na wymierne korzyści kliniczne, jednak istniejące braki
w dowodach naukowych sprawiają, że leczenie pacjentów z niewydolnością serca nierzadko
pozostaje empiryczne. Na podstawie wyników kilkunastu badań klinicznych minionego roku
rozstrzygnięto niektóre problemy w postępowaniu w niewydolności serca, które przedstawiono

Niewydolność serca i współistniejąca przewlekła obturacyjna choroba płuc — problemy diagnostyczne i terapeutyczne
Niewydolność serca i przewlekła obturacyjna choroba płuc (POChP) to jednostki chorobowe
przyczyniające się w istotny sposób do wzrostu chorobowości i śmiertelności we współczesnym
społeczeństwie. Szacuje się, że u 20–30% chorych z niewydolnością serca występuje POChP.
Dokładny wywiad kliniczny, wykonanie nieinwazyjnych badań serca, oznaczenie peptydów
natriuretycznych (BNP i NT-proBNP) w surowicy krwi pomagają z kolei rozpoznać współistniejącą
niewydolność serca u chorego z POChP. W obu grupach w trakcie postępu choroby
rozwijają się podobne zmiany patologiczne w mięśniach szkieletowych. Celem leczenia nie
powinna być tylko poprawa wskaźników sercowych i płucnych, ale również odwrócenie niekorzystnych
zmian w mięśniach szkieletowych. Ich patomechanizmu do końca dokładnie nie
poznano. Prawdopodobnie znaczenie ma brak odpowiedniego wysiłku fizycznego oraz ogólnoustrojowy
proces zapalny w przebiegu niewydolności serca i POChP zaburzający równowagę
procesów produkcji i degradacji białek mięśni szkieletowych. Istotny wpływ na terapię obu
współistniejących jednostek chorobowych ma również częsta obawa przed włączaniem beta-
-adrenolityków u chorego z POChP, co skutkuje pozbawieniem pacjenta z rozpoznaną niewydolnością
serca praktycznie najważniejszego leku w terapii tej choroby. (Folia Cardiologica

Giant post−infarction pseudoaneurysm of the left ventricle manifesting as severe heart failure
A 57 year-old female was admitted for chronic heart failure (HF) with NYHA class IV symptoms. Transthoracic echocardiography
revealed ruptured left ventricular (LV) lateral and posterior wall between their basal and middle segments resulting in
giant, round pseudoaneurysm formation with a diameter of 12 cm. Bidirectional flow through a 2.9 cm orifice between the
LV and the pseudoaneurysm cavity was shown. A 12-cm diameter pseudoaneurysm was resected and the orifice was closed
with a Dacron patch. Twelve months after the diagnosis, the patient is in a stable condition with NYHA class II HF symptoms.

Twelve months clinical outcome after bioresorbable vascular scaffold implantation in patients with stable angina and acute coronary syndrome : data from the Polish National Registry
Introduction: There are limited data describing bioresorbable vascular scaffold (BVS) implantation in complex lesions. Only short-term clinical outcomes are available for patients with acute coronary syndrome (ACS). Aim: To evaluate 12-month clinical outcome, safety and effectiveness of BVS implantation in complex lesions and in stable angina (SA) or ACS. Material and methods: Five hundred ninety-one patients with SA/ACS were enrolled between October 2012 and November 2013 in 30 invasive cardiology centres in Poland. At least one BVS implantation during percutaneous coronary intervention (PCI) was the only inclusion criteria. The clinical endpoint was the occurrence of a major adverse cardiovascular event (MACE) (all-cause death, myocardial infarction (MI), clinically driven target lesion revascularisation (TLR) with urgent PCI or target vessel revascularisation (TVR) with urgent coronary artery bypass grafting (CABG)) and device-oriented composite endpoint (DOCE) (cardiac death, urgent target vessel revascularisation with PCI/CABG, target vessel MI) during 12-month follow-up. Results: After 12 months TLR with urgent PCI was significantly more often reported in patients with diagnosed UA (4.59%; p < 0.02) in comparison with other PCI indications. No significant differences were found in terms of composite MACE endpoint, cumulative MACE (p = 0.09), stent thrombosis (p = 0.2) or restenosis (p = 0.2). There were no significant differences in cumulative MACE and composite MACE endpoint between patients with no/mild versus moderate/severe tortuosity and no/mild versus moderate/severe calcification of the target vessel. No significant difference was found between groups of patients with or without bifurcation of the target vessel. Device-oriented composite endpoint was significantly more often reported in the ACS group (3.2% vs. 0.47%; p < 0.03), most frequently in patients with diagnosed UA (5.5%). Conclusions: Bioresorbable vascular scaffold can be successfully and safely used for ACS treatment and in lesions of higher complexity
Immunoglobulin E as a marker of the atherothrombotic process in patients with acute myocardial infarction
Background: Clot formation is a crucial moment in the patophysiology of acute coronary
syndromes. The aim of this research was to assess the relationship between immunoglobulin E
(IgE), lipid parameters and chosen hemostatic markers. The role of IgE as a possible participant
in the atherothrombotic process was also investigated.
Methods: A total of 80 patients with acute myocardial infarction (MI) was enrolled in the
study. Concentrations of IgE, plasma lipid parameters, lipoprotein(a), markers of thrombin
generation (TAT, AT III), markers of fibrinolysis (tPA:Ag, PAI-1:Ag, PAP, D-dimers) and
markers of endothelial damage (von Willebrand factor) were measured in blood samples
collected immediately after admission, before any treatment administration.
Results: In patients with acute MI and with IgE concentration above 100 kU/l, IgE values
were strongly, positively correlated with LDL concentration (p < 0.05), lipoprotein(a) concentration
(p < 0.02) and negatively correlated with HDL plasma levels (p < 0.02). Exclusion of
patients with IgE concentration lower than 150 kU/l strengthened the correlation between IgE
concentration and LDL (p < 0.002) and lipoprotein(a) (p < 0.01) levels. It also revealed
a significant correlation between IgE and TAT (p < 0.001), IgE and AT III (p < 0.002), and
IgE and D-dimers (p < 0.05). IgE and TAT values measured 7, 14 and 40 days after
infarction also showed significant positive correlation between increments of these parameters.
Conclusions: In patients with acute MI, a significant increase of thrombinogenesis and
fibrinolysis markers is observed. Positive correlation between IgE concentration above 100 kU/l
and markers of thrombinogenesis activation, lipid parameters and lipoprotein(a) levels, with
significance increasing with IgE concentration and constant positive correlation between increments of IgE and TAT, can serve as evidence of IgE participation in the atherothrombotic

Ocena związku stężeń immunoglobuliny E z markerami aktywacji krzepnięcia i parametrami lipidowymi u osób z ostrym zawałem serca
Wstęp: Tworzenie skrzepliny jest momentem krytycznym w patofizjologii ostrych zespołów
wieńcowych. Celem niniejszej pracy była ocena związku immunoglobuliny E (IgE) z wybranymi
markerami aktywacji trombinogenezy, fibrynolizy, uszkodzenia śródbłonka i parametrami
lipidowymi oraz ocena roli IgE jako ewentualnego współuczestnika procesu miażdżycowo-zakrzepowego.
Metody: Badanie przeprowadzono u 80 chorych z ostrym zawałem serca. Stężenia parametrów
lipidowych, lipoproteiny(a), markerów generacji trombiny (TAT, AT III), markerów
fibrynolizy (tPA:Ag, PAI-I:Ag, PAP, D-dimery) i uszkodzenia śródbłonka (czynnik von Willebranda)
oznaczano we krwi pobranej bezpośrednio po przyjęciu do szpitala oraz przed rozpoczęciem
leczenia.
Wyniki: U chorych z ostrym zawałem serca, ze stężeniem IgE powyżej 100 kU/l stwierdzono
statystycznie istotny dodatni związek log (IgE) ze stężeniem LDL (p < 0,05) i lipoproteiny(a)
(p < 0,02) oraz ujemny z HDL (p < 0,02). Pominięcie pacjentów ze stężeniem IgE poniżej 150 kU/l
wzmocniło siłę korelacji log (IgE) z LDL (p < 0,002) i lipoproteiną(a) (p < 0,01) i wykazało
istotny związek IgE z TAT (p < 0,001), AT III (p < 0,002) i D-dimerami (p < 0,05).
Zarówno 7., 14., jak i 40. dnia po zawale utrzymywała się dodatnia znamienna korelacja
przyrostów stężeń IgE z przyrostami stężeń TAT (p < 0,02).
Wnioski: U osób z ostrym zawałem serca stwierdza się istotny wzrost stężeń osoczowych
markerów trombinogenezy i aktywacji fibrynolizy. Obserwowana dodatnia korelacja stężeń
IgE powyżej 100 kU/l z markerami wewnątrznaczyniowej aktywacji trombinogenezy, parametrami
lipidowymi i lipoproteiną(a) o wzrastającej istotności wraz ze wzrostem stężenia IgE
oraz utrzymująca się dodatnia znamienna korelacja przyrostów stężeń IgE z przyrostami
stężeń TAT po zawale mogą świadczyć o współuczestniczeniu IgE w procesie miażdżycowo-zakrzepowym. (Folia Cardiologica Excerpta 2007; 2: 309-316
Multicenter registry of Impella-assisted high-risk percutaneous coronary interventions and cardiogenic shock in Poland (IMPELLA-PL)
Background: Impella is a percutaneous mechanical circulatory support device for treatment of cardiogenic shock (CS) and high-risk percutaneous coronary interventions (HR-PCIs). IMPELLA-PL is a national retrospective registry of Impella-treated CS and HR-PCI patients in 20 Polish interventional cardiological centers, conducted from January 2014 until December 2021.Aims: We aimed to determine the efficacy and safety of Impella using real-world data from IMPELLA-PL and compare these with other registries.Methods: IMPELLA-PL data were analyzed to determine primary endpoints: in-hospital mortality and rates of mortality and major adverse cardiovascular and cerebrovascular events (MACCE) at 12 months post-discharge.Results: Of 308 patients, 18% had CS and 82% underwent HR-PCI. In-hospital mortality rates were 76.4% and 8.3% in the CS and HR-PCI groups, respectively. The 12-month mortality rates were 80.0% and 18.2%, and post-discharge MACCE rates were 9.1% and 22.5%, respectively. Any access site bleeding occurred in 30.9% of CS patients and 14.6% of HR-PCI patients, limb ischemia in 12.7% and 2.4%, and hemolysis in 10.9% and 1.6%, respectively.Conclusions: Impella is safe and effective during HR-PCIs, in accordance with previous registry analyses. The risk profile and mortality in CS patients were higher than in other registries, and the potential benefits of Impella in CS require investigation
