    Detection of terminal complement components in experimental immune glomerular injury

    Detection of terminal complement components in experimental immune glomerular injury. Complement mediates glomerulonephritis by inflammatory cell-dependent and non-inflammatory cell-independent effects on glomerular permeability. The latter may involve terminal components of the complement system. We examined several models of immunologic renal injury in the rat by immunofluorescence (IF) for terminal complement components C5, C6, C7, and C8 in glomeruli using antisera to human C5-8, which cross-react with the analogous rat complement components. Rats with the heterologous and autologous phases of passive Heymann nephritis (PHN) had proteinuria and 1 to 2+ capillary wall deposits of heterologous or rat IgG, rat C3, and C5-8. Complement depletion with cobra venom factor (CVF) significantly decreased proteinuria in both models and prevented deposition of all complement components. Rats with active Heymann nephritis had similar deposits of rat IgG and C5-8. Rats with anti-GBM nephritis and aminonucleoside nephrosis had severe proteinuria which was not affected by CVF treatment and deposits of C5-8 were absent. The presence of terminal complement components in immune deposits in experimental glomerular disease correlates with a functional role for complement in mediating glomerular injury. These data support the hypothesis that the terminal complement pathway may be a major mediator of some types of immune glomerular injury.Détection des constituants terminaux du complément au cours de lésions immunes glomérulaires expérimentales. Le complément est le médiateur d'une glomérulonéphrite par des effets inflammatoires cellule-dépendants, et non inflammatoires cellule-indépendants sur la perméabilité glomérulaire. Ces derniers pourraient mettre en jeu les constituants terminaux du système complémentaire. Nous avons examiné plusieurs modèles de lésions rénales immunologiques chez le rat par la immunofluorescence (IF) en ce qui concerne les constituants complémentaires terminaux C5, C6, C7, et C8 dans les glomérules en utilisant des antisérums contre C5-8 humain qui croisent avec les constituants complémentaires analogues du rat. Des rats dans les phases hétérologue et autologue d'une néphrite passive de Heymann (PHN) avaient une proléinurie et des dépôts sur les parois capillaires 1 à 2 + d'IgG hétérologue ou de rat, de C3 et de C5-8 de rat. Une déplétion complémentaire avec du facteur de venin de cobra (CVF) a diminué significativement la protéinurie dans les deux modèles et a prévenu le dépôt de tous les constituants complémentaires. Des rats atteints d'une nephrite active de Heymann avaient des dépôts identiques d'IgG et de C5-8 de rat. Des rats atteints de néphrite anti-GBM et de néphrose aux aminoglucosides avaient une protéinurie sévère non affectée par le traitement au CVF et les dépôts de C5-8 étaient absents. La présence des constituants terminaux de complément dans les dépôts immuns lors des glomérulopathies expérimentales est corrélée avec un rôle fonctionnel du complément dans la médiation des lésions glomérulaires.     Prediction of recurrent Clostridium difficile infection using comprehensive electronic medical records in an integrated healthcare delivery system

    BACKGROUNDPredicting recurrentClostridium difficileinfection (rCDI) remains difficult. METHODS. We employed a retrospective cohort design. Granular electronic medical record (EMR) data had been collected from patients hospitalized at 21 Kaiser Permanente Northern California hospitals. The derivation dataset (2007–2013) included data from 9,386 patients who experienced incident CDI (iCDI) and 1,311 who experienced their first CDI recurrences (rCDI). The validation dataset (2014) included data from 1,865 patients who experienced incident CDI and 144 who experienced rCDI. Using multiple techniques, including machine learning, we evaluated more than 150 potential predictors. Our final analyses evaluated 3 models with varying degrees of complexity and 1 previously published model.RESULTSDespite having a large multicenter cohort and access to granular EMR data (eg, vital signs, and laboratory test results), none of the models discriminated well (c statistics, 0.591–0.605), had good calibration, or had good explanatory power.CONCLUSIONSOur ability to predict rCDI remains limited. Given currently available EMR technology, improvements in prediction will require incorporating new variables because currently available data elements lack adequate explanatory power.Infect Control Hosp Epidemiol2017;38:1196–1203</jats:sec

    Assessment of the wintering area of Red Knots in MaranhĂŁo, northern Brazil, in February 2005

    To assess population size and the conservation status of the Red Knot Calidris canutus rufa population in Maranhão, N Brazil, an aerial census and field studies were conducted in February 2005. The aerial count showed a population of 7,575 Knots, which is down about 600 from a previous census in the 1980s. However, the count for all shorebird species combined was only 24,000 compared to 198,600 in the 1980s, paralleling a world-wide trend of population decline in shorebirds. Resightings of colour-banded knots confirmed that this is a separate population from the larger wintering population in Tierra del Fuego. All species of shorebirds captured in Maranhão were found to be infested with feather lice and mites. Body masses of knots in Maranhão were significantly lower than in Tierra del Fuego, and about half the birds were below the hypothesized fat-free mass of the species. Blood and feather samples were taken from 38 Knots for subsequent assessment of virus loads, and for detecting sites where primary feather moult had occurred. This will enable us to establish whether significant mortality is associated with pathogen loads and the energetic demands of delayed moulting. The small size of the Maranhão population and the loss of another 13,000 knots this winter from the Tierra del Fuego population means that both are now endangered. Brochures on the need for Red Knot conservation were designed and printed, and have been circulated among fishing communities and school classes in Maranhão.Fil: Baker,Allan J..     Notes on the Music: A social data infrastructure for music annotation

    Beside transmitting musical meaning from composer to reader, symbolic music notation affords the dynamic addition of layers of information by annotation. This allows music scores to serve as rudimentary communication frameworks. Music encodings bring these affordances into the digital realm; though annotations may be represented as digital pen-strokes upon a score image, they must be captured using machine-interpretable semantics to fully benefit from this transformation. This is challenging, as annota- tors’ requirements are heterogeneous, varying both across different types of user (e.g., musician, scholar) and within these groups, de- pending on the specific use-case. A hypothetical all-encompassing tool catering to every conceivable annotation type, even if it were possible to build, would vastly complicate user interaction. This additional complexity would significantly increase cognitive load and impair usability, particularly in dynamic real-time usage con- texts, e.g., live annotation during music rehearsal or performance. To address this challenge, we present a social data infrastructure that facilitates the creation of use-case specific annotation toolkits. Its components include a selectable-score module that supports customisable click-and-drag selection of score elements (e.g., notes, measures, directives); the Web Annotations data model, extended to support the creation of custom, Web-addressable annotation types supporting the specification and (re-)use of annotation palettes; and the Music Encoding and Linked Data (MELD) Javascript client library, used to build interfaces that map annotation types to render- ing and interaction handlers. We have extended MELD to support the Solid platform for social Linked Data, allowing annotations to be privately stored in user-controlled Personal Online Datastores (Pods), or selectively shared or published. To demonstrate the feasi- bility of our proposed approach, we present annotation interfaces employing the outlined infrastructure in three distinct use-cases: scholarly communication; music rehearsal; and rating during music listening
