A krónikus lymphocytás leukaemia korszerű molekuláris diagnosztikája és kezelése az új célzott terápiák korszakában
Chronic lymphoid leukaemia (CLL) has a heterogeneous clinical course depending on many clinical and molecular prognostic markers, which play an important role in the selection of the best treatment option. So far, TP53 disruption is the key prognostic and predictive factor assisting treatment decisions, especially in the era of novel therapies. Asymptomatic patients in early stages of the disease will still benefit from watchful waiting. In the frontline setting, chemoimmunotherapy is still the standard care in the majority of standard risk CLL patients. New classes of drugs like kinase inhibitors and BCL-2 inhibitors (ibrutinib, idelalisib and venetoclax) are the treatment of choice in CLL patients with relapsed/refractory disease, with the exception of high risk disease, where the optimal treatment is frontline ibrutinib monotherapy. In the near future, integrating next generation sequencing into the routine diagnostics would help the development of individual CLL patient management and to choose an optimal treatment strategy. Absztrakt:
A krónikus lymphocytás leukaemia (CLL) heterogén klinikai lefolyású
lymphoproliferativ betegség, amelyben számos klinikai és molekuláris
prognosztikai marker nyújt segítséget a leghatékonyabb terápia megválasztásában.
Eddig a TP53-defektus bizonyult kulcsfontosságú prognosztikai
és prediktív faktornak, amely befolyásolja a terápiás döntést, különösen az új
célzott kezelések érájában. A tünetmentes, korai stádiumú betegek nem igényelnek
kezelést, követésük javasolt (úgynevezett „watch and wait”). Első vonalban a
standard rizikójú CLL-es betegek többségében a standard kezelés továbbra is a
kemo-immuno terápia. Az új orálisan alkalmazható kis molekulájú gyógyszereknek,
mint a kinázinhibitorok (KI) és a Bcl-2-gátlók (ibrutinib, idelalisib és
venetoclax), elsősorban relabáló/refrakter CLL kezelésében van helyük, ez alól
kivétel az ibrutinib-monoterápia, amely a nagy rizikójú
(TP53-defektust hordozó) betegek első vonalbeli kezelésére is
javasolt. A nem túl távoli jövőben az új generációs szekvenálás diagnosztikába
történő integrálása támogathatja majd a CLL-es betegek személyre szabott

Multiplex ligatiofüggő szondaamplifikáció az onkohematológiai kutatásban és diagnosztikában
Absztrakt:
A malignus hematológiai betegségek kialakulását, progresszióját, illetve
terápiával szemben mutatott rezisztenciáját kísérő genetikai eltéréseket ma már
egyre alaposabban ismerjük. A klinikailag releváns abnormalitásoknak a
mindennapi diagnosztika keretein belül való célzott kimutatása gyors, megbízható
és költséghatékony módszereket igényel. A multiplex ligatiofüggő
szondaamplifikáció a genomikus kópiaszám-eltérések vizsgálatának hatékony
eszköze, mellyel 55–60 lókusz egyidejűleg analizálható. Az eljárás lehetőséget
nyújt prognosztikai és prediktív markerek átfogó felderítésére, így alkalmazása
hatékonyan kombinálható a kariotipizálással és fluoreszcencia in
situ hibridizációval, melyek jelenleg a legelterjedtebb
diagnosztikus technikák citogenetikai aberrációk kimutatására. Ezenkívül a
módszer képes a metilációs státusz célzott meghatározására és specifikus
mutációk detektálására is, 24 órán belül eredményt szolgáltatva. Az alábbiakban
bemutatjuk a multiplex ligatiofüggő szondaamplifikáció technikai hátterét,
összefoglaljuk előnyeit és korlátait, valamint megbeszéljük az onkohematológiai
kutatásban és diagnosztikában betöltött szerepét. Végezetül, az új generációs
szekvenáláshoz kapcsolódó, közelmúltbeli technológiai újítások fényében

Abstract:
Genetic abnormalities associated with the development, progression and treatment
resistance of hematological malignancies are extensively characterized. Rapid,
reliable and cost-efficient techniques are needed to screen the clinically
relevant aberrations in routine diagnostics. Multiplex ligation-dependent probe
amplification is an efficient tool to analyze genomic copy number aberrations at
55–60 different genomic loci. The method allows the profiling of prognostic and
predictive markers; thus, it can efficiently be combined with karyotyping and
fluorescence in situ hybridization, the most commonly used
diagnostic techniques to detect cytogenetic lesions. Furthermore, the method can
interrogate methylation status and unravel point mutations at specific sites,
providing results in 24 hours. Here, we describe the technical background of
multiplex ligation-dependent probe amplification, summarize its advantages and
limitations as well as discuss its role in oncohematological diagnostics and
research. Finally, future outlook is provided, with emphasis on recent
technological advances related to next-generation sequencing. Orv Hetil. 2018;

Enhancer of zeste homologue 2 (EZH2) is a reliable immunohistochemical marker to differentiate malignant and benign hepatic tumors
BACKGROUND: The immunohistochemical demonstration of Enhancer of zeste homologue 2 (EZH2) proved to be a useful marker in several tumor types. It has been described to distinguish reliably hepatocellular carcinomas from liver adenomas and other benign hepatocellular lesions. However, no other types of malignant liver tumors were studied so far. METHODS: To evaluate the diagnostic value of this protein in hepatic tumors we have investigated the presence of EZH2 by immunohistochemistry in hepatocellular carcinomas and other common hepatic tumors.EZH2 expression was examined in 44 hepatocellular carcinomas, 23 cholangiocarcinomas, 31 hepatoblastomas, 16 other childhood tumor types (rhabdomyosarcoma, neuroblastoma, Wilms' tumor and rhabdoid tumor), 17 metastatic liver tumors 24 hepatocellular adenomas, 15 high grade dysplastic nodules, 3 biliary cystadenomas, 3 biliary hamartomas and 3 Caroli's diseases. RESULTS: Most of the malignant liver tumors were positive for EZH2, but neither of the adenomas, cirrhotic/dysplastic nodules, reactive and hamartomatous biliary ductules stained positively. CONCLUSIONS: Our immunostainings confirm that EZH2 is a sensitive marker of hepatocellular carcinoma, but its specificity is very low, since almost all the investigated malignant liver tumors were positive regardless of their histogenesis. Based on these results EZH2 is a sensitive marker of malignancy in hepatic tumors. In routine surgical pathology EZH2 could be most helpful to diagnose cholangiocarcinomas, because as far as we know this is the first marker to distinguish transformed and reactive biliary structures. Although hepatoblastomas also express EZH2, the diagnostic significance of this observation seems to be quite limited whereas, the structurally similar, other blastic childhood tumors are also positive.
