    Management of boxers fracture with single antegrade bent K-wire

    Background: Fractures of the neck of fifth metacarpal are commonly encountered in orthopaedic practice. Management of these fractures is a debatable topic with treatment varying from conservative management to surgical fixation. In our study we used a pre-bent k wire in antegrade fashion to achieve fracture fixation. We found that pre-bent K wire provides an excellent reduction with good correction of neck shaft angle and good restoration of movements at the metcarpo-phalyngeal and inter-phalyngeal joints. Pre-bent K wire provides an efficient and economical modality of fixation in fracture neck of fifth metacarpal fractures.Methods: All patients presenting with fracture neck of fifth metacarpal were managed by pre-bent K wire in antegrade fashion by a single surgeon using a single prebent K wire.Results: All the fractures involved the dominant hand (16 right and 6 left). The mean duration of surgery was 24 mins. Intra operatively the mean no of X ray shoots were 12. At the end of the follow-up there was no significant difference in the TAM in the affected and the unaffected hand. The p=0.17. The mean DASH at the end of the follow up was 2.42. The mean fracture healing time was 45 days.Conclusions: Boxers fracture is very common fracture type in young active adults. Conservative management is usually associated with shorting and angular deformities. Percutenous fixation of the fracture with bent K wire placed in antegrade fashion provides good stability at the fracture site.

    Comparative study between transforaminal epidural selective nerve root block versus interlaminar epidural in the treatment of lumbar radicular leg pain

    Background: Sciatic neuralgia is a result of nerve root oedema because of the inflammatory, immunological and mechanical factors. Steroid injections play an important role in the management of sciatic radiculopathy. Steroids act by reducing the oedema around the nerve roots and decreasing pain. Locally administered steroids have the advantage of reduced dosage and targeted delivery around the nerve roots. This forms the basis of epidural steroid injections. This can be given around the nerve root in the transforaminal space or in the interlaminar space. There is a paucity of literature comparing the two techniques of epidural steroid injections. We have done a randomized comparative trial, to compare the effectiveness of the two modalities of injection in the management of a single level unilateral foraminal disc herniation.Methods: Patients were randomized in two groups Group A: Transforaminal epidural (SNRB) and Group B: interlaminar epidural. Same dose of steroid was used in each group.Results: Immediate post injection, 2 week and 1 month transforaminal epidural (SNRB) was better compare to interlaminar epidural however at the end of 3 months the difference was not significant (p=0.08).Conclusions: Both transforaminal and interlaminar epidural injection are effective form of treatment in mild to moderate grade of disc disease. Both the technique provide short lasting relief in the symptoms associated with disc pathology, however pain management is better in transforaminal group compared to interlaminar group. Thus these techniques can be considered for delaying surgery and providing intermitant relief.

    Mangroves occur in tropical and subtropical regions, including harsh arid areas. Little is known about how the environmental conditions of deserts influence the ecology of mangrove seedlings. The seedlings of the mangrove Avicennia marina were examined in situ in a natural stand of the southern Red Sea coast of Saudi Arabia to (1) estimate and compare the growth rate of A. marina between selected microhabitats with different tidal exposures, and (2) examine the influence of sandstorms on the growth and survival of the seedlings. Samplings were conducted in four zones established according to their tidal exposure: low tidal exposure (Z1), medium tidal exposure (Z2), high tidal exposure with numerous burrows (Z3), and high tidal exposure with a few or no burrows (Z4). Vertical growth and mortality of the seedlings and selected environmental variables were quantified. The results show that seedling growth rates differed significantly between the sampling zones, the highest growth being found in the high tidal regions (Z3 followed by Z4) and the lowest growth in Z1. Growth rate followed a significant decreasing pattern over time, coinciding with increasing air temperature and decreasing relative humidity. Sandstorms showed a marked increase in July, leading to massive dust deposition that caused extensive mortality of the seedlings by burial. Our study highlights that seedling growth can be affected by the extent of tidal inundation and that sandstorms act as a natural stressor.Los manglares proliferan en regiones tropicales y subtropicales, incluidos los desiertos. Sin embargo, se sabe poco acerca de c贸mo las condiciones ambientales de los desiertos influyen en la ecolog铆a de las pl谩ntulas de manglares. Las pl谩ntulas del manglar Avicennia marina se examinaron in situ en un manglar natural de la costa sur del Mar Rojo de Arabia Saudita para (1) estimar y comparar la tasa de crecimiento de A. marina entre microh谩bitats seleccionados con diferente exposici贸n a las mareas, y (2) examinar la influencia de las tormentas de arena en el crecimiento y supervivencia de las pl谩ntulas. Los muestreos se realizaron en cuatro zonas establecidas en funci贸n de su exposici贸n a las mareas: exposici贸n a las mareas bajas (Z1), exposici贸n a las mareas medias (Z2), exposici贸n a las mareas altas con numerosas madrigueras de cangrejos (Z3) y exposici贸n a las mareas altas pero con pocas o ninguna madriguera (Z4). Se cuantific贸 el crecimiento vertical y la mortalidad de las pl谩ntulas, as铆 como las variables ambientales seleccionadas. Los resultados muestran que las tasas de crecimiento de las pl谩ntulas difirieron significativamente entre las zonas de muestreo con el mayor crecimiento encontrado en las regiones de marea alta (Z3 seguida de Z4) y el crecimiento m谩s bajo en Z1. La tasa de crecimiento sigui贸 un patr贸n decreciente significativo a lo largo del tiempo, que coincidi贸 con el aumento de la temperatura del aire y la disminuci贸n de la humedad relativa. Las tormentas de arena aumentaron en julio, causando una deposici贸n masiva de polvo que enterr贸 a las pl谩ntulas, desencadenando una gran mortalidad.     Thermal Stresses in Maize: Effects and Management Strategies

    Climate change can decrease the global maize productivity and grain quality. Maize crop requires an optimal temperature for better harvest productivity. A suboptimal temperature at any critical stage for a prolonged duration can negatively affect the growth and yield formation processes. This review discusses the negative impact of temperature extremes (high and low temperatures) on the morpho-physiological, biochemical, and nutritional traits of the maize crop. High temperature stress limits pollen viability and silks receptivity, leading to a significant reduction in seed setting and grain yield. Likewise, severe alterations in growth rate, photosynthesis, dry matter accumulation, cellular membranes, and antioxidant enzyme activities under low temperature collectively limit maize productivity. We also discussed various strategies with practical examples to cope with temperature stresses, including cultural practices, exogenous protectants, breeding climate-smart crops, and molecular genomics approaches. We reviewed that identified quantitative trait loci (QTLs) and genes controlling high- and low temperature stress tolerance in maize could be introgressed into otherwise elite cultivars to develop stress-tolerant cultivars. Genome editing has become a key tool for developing climate-resilient crops. Moreover, challenges to maize crop improvement such as lack of adequate resources for breeding in poor countries, poor communication among the scientists of developing and developed countries, problems in germplasm exchange, and high cost of advanced high-throughput phenotyping systems are discussed. In the end, future perspectives for maize improvement are discussed, which briefly include new breeding technologies such as transgene-free clustered regularly interspaced short palindromic repeat (CRISPR)/CRISPR-associated (Cas)-mediated genome editing for thermo-stress tolerance in maize

    Background There is a substantial gap in provision of adequate surgical care in many low- and middle-income countries. This study aimed to identify the economic burden of unmet surgical need for the common condition of appendicitis. Methods Data on the incidence of appendicitis from 170 countries and two different approaches were used to estimate numbers of patients who do not receive surgery: as a fixed proportion of the total unmet surgical need per country (approach 1); and based on country income status (approach 2). Indirect costs with current levels of access and local quality, and those if quality were at the standards of high-income countries, were estimated. A human capital approach was applied, focusing on the economic burden resulting from premature death and absenteeism. Results Excess mortality was 4185 per 100 000 cases of appendicitis using approach 1 and 3448 per 100 000 using approach 2. The economic burden of continuing current levels of access and local quality was US 92492millionusingapproach1and92 492 million using approach 1 and 73 141 million using approach 2. The economic burden of not providing surgical care to the standards of high-income countries was 95004millionusingapproach1and95 004 million using approach 1 and 75 666 million using approach 2. The largest share of these costs resulted from premature death (97.7 per cent) and lack of access (97.0 per cent) in contrast to lack of quality. Conclusion For a comparatively non-complex emergency condition such as appendicitis, increasing access to care should be prioritized. Although improving quality of care should not be neglected, increasing provision of care at current standards could reduce societal costs substantially
