The application of lean technologies to optimize oncological screening in the periodic health examination of adult population groups
Background. Improving organizational technologies for early detection of cancer using targeted programs for large-scale cancer screening is a priority task of primary health care. The aim: to assess the potential of lean technologies to optimize cancer screening during periodic health examination of the adult population.Materials and methods. To develop an optimal organizational and functional model of cancer screening, a working group was created; information about the current state of the process was collected.Results. A comprehensive analysis of the state of the process revealed the following problems: violation of the algorithms of oncological screening at stage 1 of periodic health examination (13.8 %); cases of non-completion of stage 2 of periodic health examination by patients with suspected colon cancer (20.4 %); queues for endoscopic examinations (14 days) and, as a consequence, not a quick visit to the oncologist of the primary oncology department (17â18 days). After optimization of the periodic health examination: there is no queue for endoscopy for patients with suspected oncopathology; accelerated access of patients with suspected oncopathology to the oncologist of the primary oncology department (from 17â18 to 5 days; p < 0.01). Increased awareness of patients about the rules for preparing for endoscopic examination and a reminder of the date and time of the upcoming endoscopy, there was an increase in the number of patients who completed stage 2 of health examination to exclude/confirm oncopathology (from 79.6 to 90 %; p < 0.05). The frequency of violations by doctors of the oncological screening algorithm decreased from 13.8 to 2.3 % (p < 0.01).Conclusion. The use of lean manufacturing marketing strategies in primary health care has helped to identify, eliminate and control the causes of problems during the first and second stages of clinical examination of patients with suspected oncopathology
Fe/Co Alloys for the Catalytic Chemical Vapor Deposition Synthesis of Single- and Double-Walled Carbon Nanotubes (CNTs). 1. The CNTâFe/CoâMgO System
Mg0.90FexCoyO (x + y ) 0.1) solid solutions were synthesized by the ureic combustion route. Upon reduction at 1000 °C in H2-CH4 of these powders, Fe/Co alloy nanoparticles are formed, which are involved in the formation of carbon nanotubes, which are mostly single and double walled, with an average diameter close to 2.5 nm. Characterizations of the materials are performed using 57Fe Mošssbauer spectroscopy and electron microscopy, and a well-established macroscopic method, based on specific-surface-area measurements, was applied to quantify the carbon quality and the nanotubes quantity. A detailed investigation of the Fe/Co alloysâ formation and composition is reported. An increasing fraction of Co2+ ions hinders the dissolution of iron in the MgO lattice and favors the formation of MgFe2O4-like particles in the oxide powders. Upon reduction, these particles form R-Fe/Co particles with a size and composition (close to Fe0.50Co0.50) adequate for the increased production of carbon nanotubes. However, larger particles are also produced resulting in the formation of undesirable carbon species. The highest CNT quantity and carbon quality are eventually obtained upon reduction of the iron-free Mg0.90Co0.10O solid solution, in the absence of clusters of metal ions in the starting material. Rhinocerebral Form of Invasive Mycosis in COVID-19 Patients: Clinical Course, Diagnosis, Treatment Experience
Background. The incidence of invasive mycoses is reported to increase among patients with a history of new coronavirus infection COVID-19. Adhesion and damage of endothelial cells by zygomycetes lead to fungus angioinvasion, release of a large number of fungal proteases, lipases and mycotoxins, as well as vascular thrombosis, subsequent necrosis of tissues.Objectives. Improvement of the diagnosis and treatment for COVID-19 associated invasive mycosis.Methods. Examination and treatment of 143 patients with invasive mycosis of the maxillofacial area in the period from August 2021 to May 2022 at the Maxillofacial Unit of Regional Clinical Hospital of Emergency Medical Care, Krasnodar Krai, Russia. The diagnosis was confi rmed by cytological, culture-based methods, as well as pathomorphological study of surgical specimen. Follow-up monitoring was carried out in 1, 3, 6 months, with repeated CT and MRI investigations. The group under follow-up monitoring included 91 patients. The incidence of signs was determined via descriptive statistics methods in proportion of the total number of observations and expressed as a percentage. Statistika 12.5 and Excel 2010 (Windows 10) were used in the study, the graphical representation of the material was made in the same programs.Results. Manifestation of invasive mycoses of the maxillofacial area occurred during the early convalescent period of coronavirus infection in specialized infectious hospitals or within two weeks after discharge from the hospital. A typical clinical picture includes rhinocerebral zygomycosis with the development of fungal osteomyelitis of the upper jaw, nasal bones, zygomatic bone, ethmoidal labyrinth bones, sphenoid bone, frontal bone. 17 cases (11.9%) had a lethal outcome as a result of disseminated form of fungal infection caused by Mucorales (14 patients), Aspergillus (3 patients) with damage to the brain, lungs, kidneys and fatal massive necrotic lesions of the craniofacial area. An integrated approach embraced early antifungal therapy (Amphotericin B) as well as surgical debridement of the lesions and provided a positive effect in treatment.Conclusion. Invasive fungal infections require prompt diagnosis and early prescription of antifungal therapy (prior to histological confi rmation of diagnosis) to minimize consequences and prevent fatal outcome. Due to the nonspecifi c nature of clinical symptoms and diffi culties of early diagnosis of invasive zygomycosis and aspergillosis, it is necessary to be alert to potential invasive aggressive mycoses in immunocompromised patients who have a history of a new coronavirus infection COVID-19
Thermodynamic characteristics of the classical n-vector magnetic model in three dimensions
The method of calculating the free energy and thermodynamic characteristics
of the classical n-vector three-dimensional (3D) magnetic model at the
microscopic level without any adjustable parameters is proposed. Mathematical
description is perfomed using the collective variables (CV) method in the
framework of the model approximation. The exponentially decreasing
function of the distance between the particles situated at the N sites of a
simple cubic lattice is used as the interaction potential. Explicit and
rigorous analytical expressions for entropy, internal energy, specific heat near
the phase transition point as functions of the temperature are obtained. The
dependence of the amplitudes of the thermodynamic characteristics of the system
for and on the microscopic parameters of the interaction
potential are studied for the cases and . The obtained
results provide the basis for accurate analysis of the critical behaviour in
three dimensions including the nonuniversal characteristics of the system.Comment: 25 pages, 5 figure
Experimental Preventive Anti-Tularemia Preparation
Designed is an experimental preparation of a prototype chemical tularemia vaccine (PCTV). It is composed of protective antigenic complex (PAC) of tularemia microbe and S-layer protein (Slp) of plague microbe. Determined is optimum ratio of these components in the preparation and schedule of its administration. Displayed are the results of its testing as regards physical-chemical properties, reactogenicity, specific activity and impact on the immune system of laboratory animals. It is found out that preparation of the prototype is non-toxic for white mice and Guinea pigs and has no damaging effect on their immune systems. Single-stage subcutaneous immunization with PCTV induces elaboration of high-level adaptive immunity in laboratory animals within 14â21 days: specific antibody generation and stimulation of immune system cell component. PCTV protective index for white mice in case of experimental tularemia, caused by
Francisella tularensis subsp. holarctica, is 87,5 % on average; in case of infecting with
F. tularensis subsp. nearctica â 50 %; and high-level immunity in both cases. High potency of the experimental preparation against tularemia caused by subsp. holarctica (protective index is 75 %) and high-grade immunity persistence is verified on the model of Guinea pigs too
Disorder-Induced Critical Phenomena in Hysteresis: Numerical Scaling in Three and Higher Dimensions
We present numerical simulations of avalanches and critical phenomena
associated with hysteresis loops, modeled using the zero-temperature
random-field Ising model. We study the transition between smooth hysteresis
loops and loops with a sharp jump in the magnetization, as the disorder in our
model is decreased. In a large region near the critical point, we find scaling
and critical phenomena, which are well described by the results of an epsilon
expansion about six dimensions. We present the results of simulations in 3, 4,
and 5 dimensions, with systems with up to a billion spins (1000^3).Comment: Condensed and updated version of cond-mat/9609072,``Disorder-Induced

Search for the standard model Higgs boson in the H to ZZ to 2l 2nu channel in pp collisions at sqrt(s) = 7 TeV
A search for the standard model Higgs boson in the H to ZZ to 2l 2nu decay
channel, where l = e or mu, in pp collisions at a center-of-mass energy of 7
TeV is presented. The data were collected at the LHC, with the CMS detector,
and correspond to an integrated luminosity of 4.6 inverse femtobarns. No
significant excess is observed above the background expectation, and upper
limits are set on the Higgs boson production cross section. The presence of the
standard model Higgs boson with a mass in the 270-440 GeV range is excluded at
95% confidence level.Comment: Submitted to JHE
Search for anomalous t t-bar production in the highly-boosted all-hadronic final state
A search is presented for a massive particle, generically referred to as a
Z', decaying into a t t-bar pair. The search focuses on Z' resonances that are
sufficiently massive to produce highly Lorentz-boosted top quarks, which yield
collimated decay products that are partially or fully merged into single jets.
The analysis uses new methods to analyze jet substructure, providing
suppression of the non-top multijet backgrounds. The analysis is based on a
data sample of proton-proton collisions at a center-of-mass energy of 7 TeV,
corresponding to an integrated luminosity of 5 inverse femtobarns. Upper limits
in the range of 1 pb are set on the product of the production cross section and
branching fraction for a topcolor Z' modeled for several widths, as well as for
a Randall--Sundrum Kaluza--Klein gluon. In addition, the results constrain any
enhancement in t t-bar production beyond expectations of the standard model for
t t-bar invariant masses larger than 1 TeV.Comment: Submitted to the Journal of High Energy Physics; this version
includes a minor typo correction that will be submitted as an erratu
Measurement of the t t-bar production cross section in the dilepton channel in pp collisions at sqrt(s) = 7 TeV
The t t-bar production cross section (sigma[t t-bar]) is measured in
proton-proton collisions at sqrt(s) = 7 TeV in data collected by the CMS
experiment, corresponding to an integrated luminosity of 2.3 inverse
femtobarns. The measurement is performed in events with two leptons (electrons
or muons) in the final state, at least two jets identified as jets originating
from b quarks, and the presence of an imbalance in transverse momentum. The
measured value of sigma[t t-bar] for a top-quark mass of 172.5 GeV is 161.9 +/-
2.5 (stat.) +5.1/-5.0 (syst.) +/- 3.6(lumi.) pb, consistent with the prediction
of the standard model.Comment: Replaced with published version. Included journal reference and DO
