Implementation of an Overall Design of a Flexible Manufacturing System
AbstractIn this study, an implementation of the design of an FMS is performed to calculate necessary number, utilization and sequence of workstations and plant layout for given production quantities of different parts, processing sequence and times. Analytical bottleneck model and rank order clustering techniques are used in the calculation and analysis of the system. Manufacturing cells are constituted for similar parts to simplify the analysis of the system and to efficient use of workstations. Results are compared with the conventional manufacturing conditions to determine the effectiveness of the implementation
The Classification of Obsessive–Compulsive and Related Disorders in the ICD-11
Background To present the rationale for the new Obsessive–Compulsive and Related Disorders (OCRD) grouping in the Mental and Behavioural Disorders chapter of the Eleventh Revision of the World Health Organization’s International Classification of Diseases and Related Health Problems (ICD-11), including the conceptualization and essential features of disorders in this grouping. Methods Review of the recommendations of the ICD-11 Working Group on the Classification for OCRD. These sought to maximize clinical utility, global applicability, and scientific validity. Results The rationale for the grouping is based on common clinical features of included disorders including repetitive unwanted thoughts and associated behaviours, and is supported by emerging evidence from imaging, neurochemical, and genetic studies. The proposed grouping includes obsessive–compulsive disorder, body dysmorphic disorder, hypochondriasis, olfactory reference disorder, and hoarding disorder. Body-focused repetitive behaviour disorders, including trichotillomania and excoriation disorder are also included. Tourette disorder, a neurological disorder in ICD-11, and personality disorder with anankastic features, a personality disorder in ICD-11, are recommended for cross-referencing. Limitations Alternative nosological conceptualizations have been described in the literature and have some merit and empirical basis. Further work is needed to determine whether the proposed ICD-11 OCRD grouping and diagnostic guidelines are mostly likely to achieve the goals of maximizing clinical utility and global applicability. Conclusion It is anticipated that creation of an OCRD grouping will contribute to accurate identification and appropriate treatment of affected patients as well as research efforts aimed at improving our understanding of the prevalence, assessment, and management of its constituent disorders
Retroperitoneal Extragonadal Nonseminomatous Germ Cell Tumor with Synchronous Orbital Metastasis
A huge retroperitoneal tumor with a right orbital mass was detected and proved to be an extragonadal nonseminomatous germ cell tumor on biopsy. BEP chemotherapy caused some regression in orbital mass however no change in retroperitoneal tumor size as well as serum tumor marker levels occurred. Herein, we present a rarely seen entity of extragonadal retroperitoneal nonseminomatous germ cell tumor with synchronous orbital metastases and discuss its diagnosis and management
Antituberculosis drug resistance patterns in two regions of Turkey: a retrospective analysis
BACKROUND: The emergence of Mycobacterium tuberculosis strains resistant to antituberculosis agents has recently received increased attention owing largely to the dramatic outbreaks of multi drug resistance tuberculosis (MDR-TB). METHODS: Patients residing in Zonguldak and Kayseri provinces of Turkey with, pulmonary tuberculosis diagnosed between 1972 and 1999 were retrospectively identified. Drug susceptibility tests had been performed for isoniazid (INH), rifampin (RIF), streptomycin (SM), ethambutol (EMB) and thiacetasone (TH) after isolation by using the resistance proportion method. RESULTS: Total 3718 patients were retrospectively studied. In 1972–1981, resistance rates for to SM and INH were found to be 14.8% and 9.8% respectively (n: 2172). In 1982–1991 period, resistance rates for INH, SM, RIF, EMB and TH were 14.2%, 14.4%, 10.5%, 2.7% and 2.9% (n: 683), while in 1992–1999 period 14.4%, 21.1%, 10.6%, 2.4% and 3.7% respectively (n: 863). Resistance rates were highest for SM and INH in three periods. MDR-TB patients constituted 7.3% and 6.6% of 1982–1991 and 1992–1999 periods (p > 0.05). CONCLUSION: This study demonstrates the importance of resistance rates for TB. Continued surveillance and immediate therapeutic decisions should be undertaken in order to prevent the dissemination of such resistant strains
The Incidence and Management of Pleural Injuries Occurring during Open Nephrectomy
Objective. To evaluate the incidence, management, and risk factors of pleural injuries occurring during open nephrectomy. Methods. Between June 2004/and June 2008, 165 patients (167 renal units) underwent open simple (n = 37, 22.2%), partial (n = 39, 23.4%) or radical (n = 91, 54.5%) nephrectomy in our institution.
Results. Flank, Chevron, and abdominal midline incisions were used in 148(88.6%), 17(10.2%), and in 2(1.2%) surgical procedures, respectively. Ribs were excised in 109(65.3%) procedures (11th rib, 10th-11th ribs, and 11th-12th ribs). Intraoperative pleural injuries were detected in 20(12%) procedures, 16(80%) were treated successfully with simple evacuation technique, and 4 required chest tube insertion. Age, sex, surgery type, incision type, and surgery site were not associated with pleural injury occurrence (P > .05). Rib resection was the only parameter associated with pleural injury occurrence. Conclusion. Pleural injuries occur in 12% of open nephrectomy procedures, and 80% can be repaired successfully. Few of them (2.4%) need chest tube insertion. Laparoscopic management of a cavitated noncommunicating rudimentary uterine horn of a unicornuate uterus: a case report
<p>Abstract</p> <p>Introduction</p> <p>A unicornuate uterus with a rudimentary horn is the most uncommon uterine anomaly of the female genital tract. It has an estimated frequency of one in 100,000 among the fertile female population. This anomaly results from the abnormal maturation of one Müllerian duct with the normal development of the contralateral one.</p> <p>Case presentation</p> <p>We report here the case of a 14-year-old Caucasian girl who came to our hospital with intense dysmenorrhea. Imaging techniques revealed a unicornuate uterus with a rudimentary horn and a large hematosalpinx. We performed a laparoscopic removal of this uterine anomaly without any complication in the postoperative period.</p> <p>Conclusion</p> <p>In our case report, we demonstrate that laparoscopy is the best approach for the treatment of IIb Müllerian abnormalities. Laparoscopy resulted in anatomical and reproductive results equivalent to those offered by a laparotomic approach, but with the additional advantages of minimally invasive surgery, such as better cosmetic results and postoperative period, which are essential for very young patients.</p
A Performance Comparison According to Number of Wavelengths and Topologies on PCSA Reservation Mechanism for OBS
Abstract A performance comparison according to different number of wavelengths and topologies on OBS has been studied in this paper. Preemptive Channel Scheduling Algorithm (PCSA) has been used as reservation mechanism in OBS. In terms of performance criteria, loss rate in bytes, access delay and end-to-end delay are considered. A 2-state MMPP (Markov Modulated Poisson Process) traffic generator is used. Four different types of Mesh and Ring topologies are used. NS2 Network Simulation tool is used for our tests. In OBS algorithms, bursts are created using a hybrid model that takes into account both timeout and maximum length threshold mechanisms. In nodes, in order to satisfy QoS requirements, priority based queuing and Regulative Wavelength Grouping (RWG) are used. In priority based queuing, packets (bursts) are sent according to their priority order. In this study, the effects of generated traffic according to the topologies and the effects of increased number of wavelengths are shown by access delays. According to the simulation studies, the success of byte drop rate increases while the number of wavelengths increases. The results that obtained on mesh topologies are better than the results of ring topologies according to our simulation results
Box-Cox Regression Method in Time Scaling
Hata terimi ile bağımlı değişkenin süreklilik ve normal dağılma varsayımıbozulduğu durumlarda λj, j = 1, 2, ..., k, kuvvet dönüşümü ile tanımlanan Box-Coxregresyon yöntemi kullanılmaktadır. Y'ler üzerindeki λj, j = 1, 2, ..., k, kuvvetdönüşümünün hangi λj değerinde Hata Kareler Toplamı (HKT)' nı minimum yaptığıdurum ele alınmaktadır. Box-Cox regresyon yöntemi, regresyon fonksiyonunundoğrusal olmayan durumu, sabit olmayan hata varyansları ve hata terimlerinindağılışlarının çarpıklığını düzeltmek için Y'nin üzerinde dönüşüm yapılması açısındanoldukça uygundur. Bu çalışmada fark ve diferansiyel analizin birlikte ele alındığızaman skalası türev kavramı kullanılarak Box-Cox regresyon yöntemi kullanmanınavantaj ve dezavantajları incelenmiştir. Box-Cox regression method with λj, for j = 1, 2, ..., k, power transformation can beused when dependent variable and error term of the linear regression model do notsatisfy the continuity and normality assumptions. The situation obtaining the smallestmean square error when optimum power λj, transformation for j = 1, 2, ..., k, of Y hasbeen discussed. Box-Cox regression method is especially appropriate to adjustexistence skewness or heteroscedasticity of error terms for a nonlinear functionalrelationship between dependent and explanatory variables.
