Periodontal inflammation as a potential driver of HIV low level viremia
HIV can be successfully suppressed to undetectable levels by antiretroviral therapy (ART) in most people with HIV (PWH). However, a small proportion continues to have persistent low-level viremia (LLV) during ART. A presumed source of LLV is production or replication from viral reservoirs, which are maintained in the presence of ART. It is unknown whether the oral cavity can be considered an HIV reservoir. As periodontal inflammation is a common problem in PWH, we hypothesize that periodontal inflammation in the oral cavity activates (latently) infected cells and thus might be associated with LLV. We included 11 individuals with HIV LLV, and compared HIV-RNA levels in saliva and plasma at baseline and at week 24 after switch of ART. We compared the LLV-group at baseline with 11 age-matched controls with suppressed viremia. To investigate the severity of periodontitis we used Periodontal Inflamed Surface Areas (PISA) by measuring probing depth, gingival recession, bleeding on probing and clinical attachment level. Severity of periodontitis was classified according to the CDC-AAP case definition. Additional insights in periodontal inflammation were obtained by comparing immune activation markers and the presence of periodontal pathogens. In four individuals of the LLV group, residual levels of HIV-RNA were detected in saliva at baseline (N = 1) or at week 24 (N = 2) or both (N = 1). Of the four individuals with LLV, three had residual levels of HIV-RNA in saliva. All 22 individuals had moderate to severe periodontitis. PISA was not significantly different between cases with LLV and controls. Similarly, periodontal pathogens were frequently observed in both groups. Total activated HLA-DR+CD38+ CD4+ cells and CD8+ cells were significantly higher in the LLV group than in the control group (p = <0.01). No immune markers were associated with LLV. In conclusion, periodontal inflammation is an unlikely driver of HIV LLV compared to HIV suppressed individuals
Postoperative circadian patterns in wearable sensor measured heart rate:a prospective observational study
PURPOSE: This study aimed to describe the 24-hour cycle of wearable sensor-obtained heart rate in patients with deterioration-free recovery and to compare it with patients experiencing postoperative deterioration.METHODS: A prospective observational trial was performed in patients following bariatric or major abdominal cancer surgery. A wireless accelerometer patch (Healthdot) continuously measured postoperative heart rate, both in the hospital and after discharge, for a period of 14 days. The circadian pattern, or diurnal rhythm, in the wearable sensor-obtained heart rate was described using peak, nadir and peak-nadir excursions.RESULTS: The study population consisted of 137 bariatric and 100 major abdominal cancer surgery patients. In the latter group, 39 experienced postoperative deterioration. Both surgery types showed disrupted diurnal rhythm on the first postoperative days. Thereafter, the bariatric group had significantly lower peak heart rates (days 4, 7-12, 14), lower nadir heart rates (days 3-14) and larger peak-nadir excursions (days 2, 4-14). In cancer surgery patients, significantly higher nadir (days 2-5) and peak heart rates (days 2-3) were observed prior to deterioration.CONCLUSIONS: The postoperative diurnal rhythm of heart rate is disturbed by different types of surgery. Both groups showed recovery of diurnal rhythm but in patients following cancer surgery, both peak and nadir heart rates were higher than in the bariatric surgery group. Especially nadir heart rate was identified as a potential prognostic marker for deterioration after cancer surgery.</p
Experiments Participation Act:Know what works for whom
In recent years (1 October 2017 - 31 December 2019), six municipalities have conducted experiments in the assistance under the Temporary Decree Experiments Participation Act. The aim was to improve the implementation of the Participation Act through less control and coercion and more room for own direction, extra guidance and customization. The municipalities have asked the research institutes to scientifically investigate the results of these experiments. In order to be able to compare, the researchers used the same questionnaire in all six municipalities, the same outcome measures were used and the analysis model is also the same. However, because the experiments are designed at the municipal level, the differences between the municipalities and the experiments implemented are too big to really be able to combine the data. For a complete comparison and interpretation of the six experiments, we refer to the local final reports. The experiments, however, each individually, but above all in conjunction, provide relevant information for (future) policy that can fuel the discussion about the future of social security in the Netherlands. That is why we decided to write this joint semi-trailer to provide an initial insight into the broader outcomes on a selection of outcome measures. After a brief summary of the conclusions, we will discuss the various experiments and some important outcome measures in more detail
Dynamics of Low-Level Viremia and Immune Activation after Switching to a Darunavir-Based Regimen
There is an ongoing debate regarding whether low-level viremia (LLV), in particular persistent LLV, during HIV treatment with optimal adherence originates from low-level viral replication, viral production, or both. We performed an observational study in 30 individuals with LLV who switched to a boosted darunavir (DRV)-based therapy. In-depth virological analyses were used to characterize the viral population and the (activity) of the viral reservoir. Immune activation was examined using cell-bound and soluble markers. The primary outcome was defined as the effect on HIV-RNA and was categorized by responders (50 cp/mL). At week 24, 53% of the individuals were considered responders, 40% non-responders, and 7% could not be assigned. Sequencing showed no evolution or selection of drug resistance in the non-responders. Production of defective virus with mutations in either the protease (D25N) or RT active site contributed to persistent LLV in two individuals. We show that in about half of the study participants, the switch to a DRV-based regimen resulted in a viral response indicative of ongoing low-level viral replication as the cause of LLV before the switch. Our data confirm that in clinical management, high genetic barrier drugs like DRV are a safe choice, irrespective of the source of LLV
Differential Genotypic Evolution of HIV-1 Quasispecies in Cerebrospinal Fluid and Plasma:A Systematic Review
HIV-1 enters the central nervous system by passing the blood-brain barrier during primary infection. Despite the introduction of combination antiretroviral therapy, the prevalence of HIV-associated neurocognitive disorders remains high and is probably related to ongoing viral neuropathological processes. The central nervous system forms a distinct physiological, cellular, and pharmacological environment. We aimed to systematically review whether the central nervous system also constitutes a distinct virological compartment, allowing differential genotypic evolution of HIV-1. Only original research papers that compared paired plasma/cerebrospinal fluid samples for drug resistance associated mutations as defined by the IAS-USA Drug Resistance Mutations Panel or compared viral envelope (env) patterns or coreceptor prediction were included. If available, HIV RNA levels were included in the analysis, with a relevant difference defined as 0.5 log(10) copies/ml Data from 35 reports with heterogeneous study design and methods was pooled and statistical analysis was performed as appropriate. A total of 555 subjects with 671 samples could be included in this review.
