Evaluation of prophylactic Effect of Remifentanil on Succinylcholine-Induced Myalgia in Humans
Purpose: To assess the efficacy of remifentanil in preventing succinylcholine-induced myalgia in humans.Methods: Sixty healthy adults scheduled for elective surgery under general anesthesia were enrolled in a double-blind study and randomly allocated to two groups of thirty patients. Patients in Group I (remifentanil group) were pretreated with remifentanil 1 ìg/kg one minute prior to induction of anesthesia, while patients in Group II (saline group) received an equivalent volume of saline. Anesthesia was induced in both groups with fentanyl 1 µg/kg, propofol 2.0 mg/kg and succinylcholine 1.5 mg/kg. Postoperative myalgia was assessed 12, 24, and 48 h after induction and graded as nil, mild, moderate, or severe.Results: Fifty nine patients completed the study. The demographic data for both groups were comparable (p > 0.05). Postoperative myalgia at 12, 24, and 48 h after induction were 34.5, 34.5 and 14.1 % in group I, and 60, 53.4 and 30 % in group II, respectively (p > 0.05).Conclusion: Prophylactic use of remifentanil 1 µg/kg intravenously is ineffective in the prevention of postoperative myalgiaKeywords: Remifentanil, Succinylcholine, Myalgia, Propofol, Prophylaxi
Investigation of the effects of constant darkness and light on blood serum cholesterol, insulin and glucose levels in healthy male rats
This study was designed to investigate the effects of constant darkness and light on changes of serum cholesterol, insulin and glucose levels in healthy male rats. In this study, healthy male rats (n = 30) were divided into 3 groups of tens and kept at various light/dark conditions: Control 12:12 light/dark (LD); constant darkness (DD), and constant light (LL) groups for 2 weeks. Blood samples were obtained from retro-orbital sinus before start of experiment and on the 7th and 14th days of the experimental period. The serum cholesterol and glucose levels were measured by the enzymatic method and insulin levels were measured using insulin kit by enzyme-linked immunosorbent assay (ELISA) method. The results of the study showed that the levels of serum cholesterol and glucose on the 7th and 14th days of the experimental period in DD group significantly decreased compared to the LD and LL groups (p < 0.05). On the 14th day of experiment, we observed significant decrease of serum insulin level in the constant darkness group compared with the two other groups (p < 0.05). The study showed that on the 7th and 14th days of experiment, constant light significantly increased serum glucose level without having any significant effects on serum cholesterol and insulin levels. Also, the long period of time (14 days) was found to be more effective in the serum of these metabolic parameters changes than the short period (7 days).Key words: Constant darkness, light, cholesterol, glucose, insulin, healthy male rats
Comparing efficacy of montelukast versus doxycycline in treatment of moderate acne
Background: Treatment of acne is an important issue for reducing the cosmetic and psychological burden of disease. Regarding the inflammatory effect of LT-B4 in acne lesions and action mechanism of Montelukast, this study was performed to determine the efficacy of Montelukastin acne treatment comparison with doxycycline. Materials and Methods: In a randomized clinical trial that was performed in Dermatology Clinic in a Training Tertiary Health Care Center in Tehran, Iran since January 2012 to May 2014, 52 patients with moderate acne were evaluated. The included patients were randomly assigned to receive doxycycline 100 mg/day plus 1 Clindamycin solution (Group 1) or Montelukast 5 mg daily plus 1 clindamycin solution (Group 2). The acne severity index was measured and compared between two groups at baseline (on admission), 1-month and 3 months later. Independent-Sample-T, Chi-Square, and Repeated-Measure ANOVA tests were used and were considered statistically significant at P < 0.05. Results: The mean age was 26.8 ± 7.1 in Group 1 and25 ± 4.8 in Group 2 (P = 0.1). 73 women and 26.7 4 men in Group 1 and 86.7 women, and 13.3 men in Group 2 (P = 0.01). The mean acne severity index at baseline was 18.2 ± 6.1 and 19 ± 4.2 in Montelukast and doxycycline group, respectively (P = 0.679). The mean acne severity index after 1-month was 10.5 ± 6.2 and 12.9 ± 3.3 in Montelukast and doxycycline group, respectively (P = 0). Finally, the mean acne severity index after 3 months follow-up was 8.6 ± 4.8 and 8.2 ± 1.2 in Montelukast and doxycycline group, respectively (P = 0.01). There was no significant difference between two groups regarding the amount of decrease in acne severity index across the study (P = 0.186). However, each groups showed a significant reduction in the acne severity index, separately (P = 0.001). Conclusion: It may be concluded that Montelukast is an effective and safe medication for moderate-level acne treatment. © 2015, Isfahan University of Medical Sciences(IUMS). All rights reserved
Prevalence of unplanned pregnancy and factors related to maternal-fetal attachment in Zanjan, 2017
Background: Despite the progress of family planning programs, a significant proportion of pregnancies are still unplanned which threatens the different dimensions of community health. Unplanned pregnancy affects parent's-child association. Maternal-fetal attachment provides a model for the child's mental-social function at present and in the future.
Objectives: This study was conducted to determine the prevalence of unplanned pregnancy and related factors of maternal-fetal attachment in pregnant women referring to Zanjan health centers in 2017.
Methods: This descriptive correlational study was part of a clinical trial that was performed on 184 pregnant women who referred to health centers for routine prenatal care from October to February 2017 in Zanjan, Iran. Using multi-stage sampling method, health centers of Zanjan were divided into three categories based on social and economic situation. Then, from each category, three centers were selected, randomly. The inclusion criteria comprised being pregnant, satisfaction to participate in the study, have at least reading and writing skills, lack of the history of obstetric complications, psychological disease and medicine use, lack of known psychological disease, lack of narcotic substances abuse, and living in Zanjan City. Data collection tool included demographic checklists and maternal-fetal attachment questionnaire, which completed in self-report method. The data of this study were analyzed by appropriate statistical tests by SPSS v.16 software.
Results: Among the participants 58.2% of women had planned pregnancy, 36.4% had unplanned pregnancy, and 5.4% had unwanted pregnancy. Maternal-fetal attachment scores were significantly higher in the planned pregnancy group 84(75-93) than the unplanned pregnancy group 57(54-60) and unwanted pregnancy group 56(48-64) (P˂0.001). The highest sub-scale in the planned pregnancy group was related to the attributing characteristics and intentions. Also, the most subscale in unplanned and unwanted pregnancies related to giving of self. There was a significant relationship between age, education, socioeconomic level, number of pregnancies, number of children and contraceptive method with type of pregnancy (P˂0.001).
Conclusion: Based on the results of the study, it seems that reduction in unplanned pregnancy will enhance the maternal-fetal attachment and will improve the mother role and social-psychological health of the chil
Noncontact respiration monitoring techniques in young children:A scoping review
Pediatric sleep-related breathing disorders, or sleep-disordered breathing (SDB), cover a range of conditions, including obstructive sleep apnea, central sleep apnea, sleep-related hypoventilation disorders, and sleep-related hypoxemia disorder. Pediatric SDB is often underdiagnosed, potentially due to difficulties associated with performing the gold standard polysomnography in children. This scoping review aims to: (1) provide an overview of the studies reporting on safe, noncontact monitoring of respiration in young children, (2) describe the accuracy of these techniques, and (3) highlight their respective advantages and limitations. PubMed and EMBASE were searched for studies researching techniques in children <12 years old. Both quantitative data and the quality of the studies were analyzed. The evaluation of study quality was conducted using the QUADAS-2 tool. A total of 19 studies were included. Techniques could be grouped into bed-based methods, microwave radar, video, infrared (IR) cameras, and garment-embedded sensors. Most studies either measured respiratory rate (RR) or detected apneas; n = 2 aimed to do both. At present, bed-based approaches are at the forefront of research in noncontact RR monitoring in children, boasting the most sophisticated algorithms in this field. Yet, despite extensive studies, there remains no consensus on a definitive method that outperforms the rest. The accuracies reported by these studies tend to cluster within a similar range, indicating that no single technique has emerged as markedly superior. Notably, all identified methods demonstrate capability in detecting body movements and RR, with reported safety for use in children across the board. Further research into contactless alternatives should focus on cost-effectiveness, ease-of-use, and widespread availability.</p
Ultra-wideband radar for simultaneous and unobtrusive monitoring of respiratory and heart rates in early childhood:A Deep Transfer Learning Approach
Unobtrusive monitoring of children’s heart rate (HR) and respiratory rate (RR) can be valuable for promoting the early detection of potential health issues, improving communication with healthcare providers and reducing unnecessary hospital visits. A promising solution for wireless vital sign monitoring is radar technology. This paper presents a novel approach for the simultaneous estimation of children’s RR and HR utilizing ultra-wideband (UWB) radar using a deep transfer learning algorithm in a cohort of 55 children. The HR and RR are calculated by processing radar signals via spectrogram from time epochs of 10 s (25 sample length of hamming window with 90% overlap) and then transforming the resultant representation into 2-dimensional images. These images were fed into a pre-trained Visual Geometry Group-16 (VGG-16) model (trained on ImageNet dataset), with weights of five added layers fine-tuned using the proposed data. The prediction on the test data achieved a mean absolute error (MAE) of 7.3 beats per minute (BPM < 6.5% of average HR) and 2.63 breaths per minute (BPM < 7% of average RR). We also achieved a significant Pearson’s correlation of 77% and 81% between true and extracted for HR and RR, respectively. HR and RR samples are extracted every 10 s.</p

Long-Acting Cabotegravir and Rilpivirine after Oral Induction for HIV-1 Infection.
BACKGROUND: Long-acting injectable regimens may simplify therapy for patients with human immunodeficiency virus type 1 (HIV-1) infection. METHODS: We conducted a phase 3, randomized, open-label trial in which adults with HIV-1 infection who had not previously received antiretroviral therapy were given 20 weeks of daily oral induction therapy with dolutegravir-abacavir-lamivudine. Participants who had an HIV-1 RNA level of less than 50 copies per milliliter after 16 weeks were randomly assigned (1:1) to continue the current oral therapy or switch to oral cabotegravir plus rilpivirine for 1 month followed by monthly injections of long-acting cabotegravir plus rilpivirine. The primary end point was the percentage of participants who had an HIV-1 RNA level of 50 copies per milliliter or higher at week 48 (Food and Drug Administration snapshot algorithm). RESULTS: At week 48, an HIV-1 RNA level of 50 copies per milliliter or higher was found in 6 of 283 participants (2.1%) who received long-acting therapy and in 7 of 283 (2.5%) who received oral therapy (adjusted difference, -0.4 percentage points; 95% confidence interval [CI], -2.8 to 2.1), a result that met the criterion for noninferiority for the primary end point (margin, 6 percentage points). An HIV-1 RNA level of less than 50 copies per milliliter at week 48 was found in 93.6% who received long-acting therapy and in 93.3% who received oral therapy (adjusted difference, 0.4 percentage points; 95% CI, -3.7 to 4.5), a result that met the criterion for noninferiority for this end point (margin, -10 percentage points). Of the participants who received long-acting therapy, 86% reported injection-site reactions (median duration, 3 days; mild or moderate severity, 99% of cases); 4 participants withdrew from the trial for injection-related reasons. Grade 3 or higher adverse events and events that met liver-related stopping criteria occurred in 11% and 2%, respectively, who received long-acting therapy and in 4% and 1% who received oral therapy. Treatment satisfaction increased after participants switched to long-acting therapy; 91% preferred long-acting therapy at week 48. CONCLUSIONS: Therapy with long-acting cabotegravir plus rilpivirine was noninferior to oral therapy with dolutegravir-abacavir-lamivudine with regard to maintaining HIV-1 suppression. Encrypted federated learning for secure decentralized collaboration in cancer image analysis.
Artificial intelligence (AI) has a multitude of applications in cancer research and oncology. However, the training of AI systems is impeded by the limited availability of large datasets due to data protection requirements and other regulatory obstacles. Federated and swarm learning represent possible solutions to this problem by collaboratively training AI models while avoiding data transfer. However, in these decentralized methods, weight updates are still transferred to the aggregation server for merging the models. This leaves the possibility for a breach of data privacy, for example by model inversion or membership inference attacks by untrusted servers. Somewhat-homomorphically-encrypted federated learning (SHEFL) is a solution to this problem because only encrypted weights are transferred, and model updates are performed in the encrypted space. Here, we demonstrate the first successful implementation of SHEFL in a range of clinically relevant tasks in cancer image analysis on multicentric datasets in radiology and histopathology. We show that SHEFL enables the training of AI models which outperform locally trained models and perform on par with models which are centrally trained. In the future, SHEFL can enable multiple institutions to co-train AI models without forsaking data governance and without ever transmitting any decryptable data to untrusted servers
