    effectiveness of aquatic therapy in post-surgery rehabilitation of the rotator cuff

    Introduction: rotator cuff injuries very often require surgical repair and subsequent rehabilitation. The aim of this study was to compare the effectiveness of a traditional and an aquatic post-surgery rehabilitation program of the rotator cuff in 20 adult (60±5y) patients. Methods:patients were randomly assigned to a traditional (TR; n=10) and to an aquatic (WR; n=10) rehabilitation group. Both programs started 15 days after the surgery, lasted 3 months (3 times-a-week), and consisted in passive mobilization followed by strengthening exercises. The only WR group underwent to an adjunctive aquatic mobilization program. Before (pre-intervention) and after (post-intervention) the programs, patients underwent the following assessments: shoulder range of motion (ROM), in both flexion and extra rotation positions; pain perception (visual analogue scale); simple shoulder test. Results: pre vs. post-intervention results were compared in each group by means of a dependent-samples t-Test on each variables. An independent-samples t-Test was used to analyze pre-intervention differences between TR and WR groups; since no significant differences were found, the same test was used to analyze post-intervention differences. Pre- vs post-intervention results showed significant improvements in all variables in both groups (p 0,005) Discussion: although the water-based program did not result more effective than the traditional program at the end of the three months, the aquatic rehabilitation seemed to improve shoulder ROM faster than the traditional one and to be better tolerated by the patients.     Hand Posture Effects on Handedness Recognition as Revealed by the Simon Effect

    We investigated the influence of hand posture in handedness recognition, while varying the spatial correspondence between stimulus and response in a modified Simon task. Drawings of the left and right hands were displayed either in a back or palm view while participants discriminated stimulus handedness by pressing either a left or right key with their hands resting either in a prone or supine posture. As a control, subjects performed a regular Simon task using simple geometric shapes as stimuli. Results showed that when hands were in a prone posture, the spatially corresponding trials (i.e., stimulus and response located on the same side) were faster than the non-corresponding trials (i.e., stimulus and response on opposite sides). In contrast, for the supine posture, there was no difference between corresponding and non-corresponding trials. Control experiments with the regular Simon task showed that the posture of the responding hand had no influence on performance. When the stimulus is the drawing of a hand, however, the posture of the responding hand affects the spatial correspondence effect because response location is coded based on multiple reference points, including the body of the hand

    Physiological responses to pedaling on a water stationary bike at different immersion heights

    Only a few studies[1-4] have investigated oxygen consumption (V’O2) and heart rate (HR) responses to pedaling on a stationary bike in water (WSB), while literature is still lacking on the effects elicited by variations in immersion depth. 14 subjects (8 M, 6 F, age 30±6y, weight 67±14kg, BMI 22.8±2.3kg/m2, fat mass 21,4±3%) performed 2 testing sessions and 2 exercise sessions: i) dry-land incremental exercise test (cycle-ergometer) to age-predicted maximum HR (HR); ii) underwater (hip-height) incremental exercise test (on a WSB) to exhaustion; iii/iv) two 2-stage (18-minutes each) underwater pedaling exercises on a WSB (Aqquatix S.r.l., Italy) at 2 different immersion heights (armpit/hip). Breath-by-breath V’O2 and beat-by-beat HR were recorded continuously throughout dry-land sessions. The intensities of the exercise sessions were 45%-55% (stage#1) and 70%-80% (stage#2) of the underwater V’O2 peak (a metronome was set at the V’O2 peak pedaling rate). A repeated-measures (2 exercise intensities) 2-way (armpit/hip heights) ANOVA was performed on each variable and the Bonferroni test was used for post-hoc comparisons. Compared to the hip-height condition, pedaling immersed at the armpit level elicited significantly lower V’O2 and HR (significant solely at high-intensity for HR) responses, either exercising at high (25.5±4.6 vs. 29.1±4.8 mL∙kg-1∙min-1; 127±14 vs. 140±19 beats∙min-1) or low (17.9±3.6 vs. 20.1±3.6 mL∙kg-1∙min-1; 105±16 vs. 110±15 beats∙min-1) intensity. While peak V’O2 didn’t differ significantly between dry-land and underwater tests (36.2±5.4 vs. 38.8±5.8 mL∙kg-1∙min-1), mean HRs at both intensities and both water heights were significantly lower when expressed as percentage of the HR resulting from the underwater test to exhaustion, rather than applying the 220-age prediction equation.Universidad de Málaga.     Resistance training modulates reticulum endoplasmic stress, independent of oxidative and inflammatory responses, in elderly people

    [EN] Aging is related to changes in the redox status, low-grade inflammation, and decreased endoplasmic reticulum unfolded protein response (UPR). Exercise has been shown to regulate the inflammatory response, balance redox homeostasis, and ameliorate the UPR. This work aimed to investigate the effects of resistance training on changes in the UPR, oxidative status, and inflammatory responses in peripheral blood mononuclear cells of elderly subjects. Thirty elderly subjects volunteered to participate in an 8-week resistance training program, and 11 youth subjects were included for basal assessments. Klotho, heat shock protein 60 (HSP60), oxidative marker expression (catalase, glutathione, lipid peroxidation, nuclear factor erythroid 2-related factor 2, protein carbonyls, reactive oxygen species, and superoxide dismutase 1 and 2), the IRE1 arm of UPR, and TLR4/TRAF6/pIRAK1 pathway activation were evaluated before and following training. No changes in the HSP60 and Klotho protein content, oxidative status markers, and TLR4/TRAF6/pIRAK1 pathway activation were found with exercise. However, an attenuation of the reduced pIRE1/IRE1 ratio was observed following training. Systems biology analysis showed that a low number of proteins (RPS27A, SYVN1, HSPA5, and XBP1) are associated with IRE1, where XBP1 and RPS27A are essential nodes according to the centrality analysis. Additionally, a gene ontology analysis confirms that endoplasmic reticulum stress is a key mechanism modulated by IRE1.     A Prospective Open-Label Observational Study of a Buffered Soluble 70 mg Alendronate Effervescent Tableton Upper Gastrointestinal Safety and Medication Errors: The GastroPASS Study

    Upper gastrointestinal (GI) side effects are a main reason for discontinuing bisphosphonate treatment, an important therapeuticoption for osteoporosis patients. Consequently, the development of novel formulations with improved tolerability is warranted. Inthis multicenter prospective, observational, postauthorization safety study conducted in Italy and Spain, postmenopausal women(PMW) with osteoporosis (naïve to bisphosphonates) were treated weekly with a buffered soluble alendronate 70 mg effervescent(ALN-EFF) tablet (Binosto®) and followed for 12 3 months. Information was collected on adverse events (AEs), medication errors,persistence, and compliance using the Morisky-Green questionnaire. Patients (N = 1028) aged 67 9 years (mean SD) receivedALN-EFF weekly. The cumulative incidence of upper GI AEs (oesophageal toxicity, gastritis, gastric ulcers, and duodenitis) relatedto ALN-EFF (primary endpoint) was 9.6% (95% condence interval [CI] 7.9–11.6%), the vast majority being of mild intensity. The mostfrequently occurring upper GI AEs related to ALN-EFF were dyspepsia (2.7%), gastroesophageal reux disease (2.4%), and nausea(2.2%). None of the relevant upper GI AEs listed in the primary endpoint and no serious AEs were reported. At least one medicationerror occurred in 29.9% (95% CI 27.1–32.8%) of patients. However, the majority of medication errors were associated with adminis-tration instructions applicable to any oral bisphosphonate and only seven medication errors were associated with the ALN-EFF for-mulation. ALN-EFF was discontinued in 209 of 1028 (20.3%) patients. The most frequent reasons for discontinuation were AEsrelated to ALN-EFF (46.9%) and patients’ decision (42.6%). Compliance with ALN-EFF was high, reected by a mean Morisky-Greenscore of 92.8 18.6. PMW with osteoporosis treated with ALN-EFF in a real-world setting experienced few upper GI AEs. In addition,they had a low discontinuation and high compliance compared with other formulations, suggesting that ALN-EFF may increasepatient satisfaction and therefore long-term adherence and efcac

    The clustering of Galaxies in the SDSS-III Baryon Oscillation Spectroscopic Survey : including covariance matrix errors

AGS acknowledges support from the Trans-regional Collaborative Research Centre TR33 ‘The Dark Universe’ of the German Research Foundation (DFG).We present improved methodology for including covariance matrices in the error budget of Baryon Oscillation Spectroscopic Survey (BOSS) galaxy clustering measurements, revisiting Data Release 9 (DR9) analyses, and describing a method that is used in DR10/11 analyses presented in companion papers. The precise analysis method adopted is becoming increasingly important, due to the precision that BOSS can now reach: even using as many as 600 mock catalogues to estimate covariance of two-point clustering measurements can still lead to an increase in the errors of ∼20 per cent, depending on how the cosmological parameters of interest are measured. In this paper, we extend previous work on this contribution to the error budget, deriving formulae for errors measured by integrating over the likelihood, and to the distribution of recovered best-fitting parameters fitting the simulations also used to estimate the covariance matrix. Both are situations that previous analyses of BOSS have considered. We apply the formulae derived to baryon acoustic oscillation (BAO) and redshift-space distortion (RSD) measurements from BOSS in our companion papers. To further aid these analyses, we consider the optimum number of bins to use for two-point measurements using the monopole power spectrum or correlation function for BAO, and the monopole and quadrupole moments of the correlation function for anisotropic-BAO and RSD measurements.Publisher PDFPeer reviewe
