    Considerable Variability in the Efficacy of 8% Capsaicin Topical Patches in the Treatment of Chronic Pruritus in 3 Patients with Notalgia Paresthetica

    Notalgia paresthetica (NP) is a focal neuropathic itch condition manifesting in intense chronic or recurrent episodic itch in a hyperpigmented, macular, uni- or bilateral skin area located below and/or medially to the scapulae. Achieving satisfactory relieve in NP patients is challenging. In this case-series three female NP patients were treated with 8% capsaicin patches following a spatial quantification of their alloknetic area with a von Frey filament. The use of a von Frey filament in order to delimit the precise area of itch sensitization and thus patch application, proved clinically feasible. Although 8% topical capsaicin relieved itch in all three patients, the duration of the effectiveness varied greatly from only 3 days to >2 months. The treatment was well tolerated in the patients and there appear to be no significant hindrances to applying this treatment with NP as an indication, although it may only exhibit satisfactory effectiveness in certain patients. Placebo-controlled double-blinded trials are needed to confirm the effectiveness of the treatment and assess predictive parameters of the treatment outcome

    Mixing of ultracold atomic clouds by merging of two magnetic traps

    We demonstrate a method to make mixtures of ultracold atoms that does not make use of a two-species magneto-optical trap. We prepare two clouds of 87Rb atoms in distinct magnetic quadrupole traps and mix the two clouds by merging the traps. For correctly chosen parameters the mixing can be done essentially without loss of atoms and with only minor heating. The basic features of the process can be accounted for by a classical simulation of particle trajectories. Such calculations indicate that mixing of different mass species is also feasible, opening the way for using the method as a starting point for making quantum gas mixtures.Comment: 12 pages, 13 figures.     Stop-and-go kinetics in amyloid fibrillation

    Many human diseases are associated with protein aggregation and fibrillation. Using glucagon as a model system for protein fibrillation we show that fibrils grow in an intermittent fashion, with periods of growth followed by long pauses. Remarkably, even if the intrinsic transition rates vary considerably in each experiment, the probability of being in the growing (stopping) state is very close to 1/4 (3/4), suggesting the presence of 4 independent conformations of the fibril tip.     Progressive Resistance Training and Cancer Testis (PROTRACT) - Efficacy of resistance training on muscle function, morphology and inflammatory profile in testicular cancer patients undergoing chemotherapy: design of a randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>Standard treatment for patients with disseminated germ cell tumors is combination chemotherapy with bleomycin, etoposide and cisplatin (BEP). This treatment is highly effective, but the majority of patients experience severe adverse effects during treatment and are at risk of developing considerable long-term morbidity, including second malignant neoplasms, cardiovascular disease, and pulmonary toxicity. One neglected side effect is the significant muscular fatigue mentioned by many patients with testicular cancer both during and after treatment. Very limited information exists concerning the patho-physiological effects of antineoplastic agents on skeletal muscle. The primary aim of this study is to investigate the effects of BEP-treatment on the skeletal musculature in testicular cancer patients, and to examine whether the expected treatment-induced muscular deterioration can be attenuated or even reversed by high intensity progressive resistance training (HIPRT).</p> <p>Design/Methods</p> <p>The PROTRACT study is a randomized controlled trial in 30 testicular cancer patients undergoing three cycles of BEP chemotherapy. Participants will be randomized to either a 9-week HIPRT program (STR) initiated at the onset of treatment, or to standard care (UNT). 15 healthy matched control subjects (CON) will complete the same HIPRT program. All participants will take part in 3 assessment rounds (baseline, 9 wks, 21 wks) including muscle biopsies, maximum muscle strength tests, whole body DXA scan and blood samples. <it>Primary outcome</it>: mean fiber area and fiber type composition measured by histochemical analyses, satellite cells and levels of protein and mRNA expression of intracellular mediators of protein turnover. Secondary outcomes: maximum muscle strength and muscle power measured by maximum voluntary contraction and leg-extensor-power tests, body composition assessed by DXA scan, and systemic inflammation analyzed by circulating inflammatory markers, lipid and glucose metabolism in blood samples. Health related Quality of Life (QoL) will be assessed by validated questionnaires (EORTC QLQ-C30, SF-36).</p> <p>Discussion</p> <p>This study investigates the muscular effects of antineoplastic agents in testicular cancer patients, and furthermore evaluates whether HIPRT has a positive influence on side effects related to chemotherapy. A more extensive knowledge of the interaction between cytotoxic-induced physiological impairment and exercise-induced improvement is imperative for the future development of optimal rehabilitation programs for cancer patients.</p> <p>Trial Registration</p> <p>Current Controlled Trials <a href="http://www.controlled-trials.com/ISRCTN32132990">ISRCTN32132990</a>.</p

    What is marine biodiversity? Towards common concepts and their implications for assessing biodiversity status

    Biodiversity' is one of the most common keywords used in environmental sciences, spanning from research to management, nature conservation, and consultancy. Despite this, our understanding of the underlying concepts varies greatly, between and within disciplines as well as among the scientists themselves. Biodiversity can refer to descriptions or assessments of the status and condition of all or selected groups of organisms, from the genetic variability, to the species, populations, communities, and ecosystems. However, a concept of biodiversity also must encompass understanding the interactions and functions on all levels from individuals up to the whole ecosystem, including changes related to natural and anthropogenic environmental pressures. While biodiversity as such is an abstract and relative concept rooted in the spatial domain, it is central to most international, European, and national governance initiatives aimed at protecting the marine environment. These rely on status assessments of biodiversity which typically require numerical targets and specific reference values, to allow comparison in space and/or time, often in association with some external structuring factors such as physical and biogeochemical conditions. Given that our ability to apply and interpret such assessments requires a solid conceptual understanding of marine biodiversity, here we define this and show how the abstract concept can and needs to be interpreted and subsequently applied in biodiversity assessments

    A Prototype Decision Support Tool for Ballast Water Risk Management using a Combination of Hydrodynamic Models and Agent-Based Modelling

    We report the development of a prototype Decision Support Tool (DST) for modelling the risks of spreading of non-indegenous invasive species via ballast water. The DST constitutes of two types of models: A 3D hydrodynamical model calculates the currents in the North Sea and Danish Straits, and an agent-based model estimates the dispersal of selected model organisms with the prevailing currents calculated by the 3D hydrodynamical model. The analysis is concluded by a post processing activity, where scenarios of dispersal are combined into an interim estimate of connectivity within the study area. The latter can be used as a tool for assessment of potential risk associated with intentional or unintentional discharges of ballast water. We discuss how this prototype DST can be used for ballast water risk management and outline other functions and uses, e.g. in regard to ecosystem-based management and the implementation of the EU Marine Strategy Framework Directive.https://commons.wmu.se/nsbwo/1001/thumbnail.jp

    Itch sensitization?:A systematic review of studies using quantitative sensory testing in patients with chronic itch

    As well established for patients with chronic pain, patients suffering from chronic itch also exhibit signs of peripheral and central sensitization. This has been linked to parallel neuroplastic sensitization processes. However, for chronic itch, sensitization has not yet been systematically assessed, studied, and hence validated. This review (Prospero CRD42016043002) summarizes and meta-analytically evaluates whether sensory aberrations including sensitization for itch occur in chronic itch. Databases PubMed, Embase, and Cochrane Library were searched for studies investigating somatosensory sensitivity assessment by quantitative sensory testing stimuli, including experimental cutaneous chemical pruritic provocations, in patients with chronic itch from skin/neurological conditions and compared with healthy controls. Outcomes were extracted for lesional and nonlesional skin, and risk of biases were assessed. Meta-analyses were performed when sufficient quantitative data were available. Of 4667 identified articles, 46 were included and 25 were eligible for meta-analyses. Patients (66% atopic dermatitis [AD]) were found more sensitive than the controls to histamine-evoked itch in lesional skin (standardized mean difference [SMD]: 0.66 confidence interval [CI]: 0.16-1.15), but not nonlesionally (SMD: −0.26 [CI: −0.58 to 0.06]). Cowhage did not evoke more itch in nonlesional skin of patients as compared to the controls (SMD: 0.38 [CI: −0.04 to 0.81]). For numerous other chemical provocations as well as for mechanical, thermal, and electrical stimulation paradigms, results were ambiguous or based on few studies. Patients with chronic itch are only robustly sensitized to various chemical pruritic stimuli when applied lesionally. More studies on somatosensory aberrations in chronic itch conditions other than AD are needed to establish whether sensitization is robustly present across chronic itch conditions.Health and self-regulatio
