    Examining the link between socio-economic position and mental health in early adolescents

    Mental health (MH) is fundamental to good quality of life and contributes to success in society. Healthy and confident children are more likely to turn out into healthy and confident adults. Adverse family situations such as low socio-economic position (SEP) may have negative consequences for child and adolescent MH. However, little is known about the link between low family SEP and MH. This thesis examined four aspects of this link: 1) the domain-specific link, 2) the mediation link, 3) the interaction link, and 4) the link with MH service use. The link between low SEP and MH was found to be larger for the externalizing than internalizing problem domain and the SEP-MH relationship was partly mediated by environment-related life stressors, particularly for internalizing problems. Familial psychopathology and low SEP were independent and non-interacting risk factors for offspring MH problems. Finally, the true association between SEP and MH service use was obscured by severity of mental problems. Maternal education, independent of other indices of SEP and severity of MH problems, significantly predicted MH service use. In conclusion, the effect of low family SEP on MH may be domain-specific, partly mediated by environment-related life stressors and the synergy between low SEP and parental psychopathology appear not to confer additional risks for mental problems in offspring. Further, failure to correct for severity of mental problems obscures the true association between SEP and MH service use in early adolescents. Finally, although the effects of SEP on MH may be relatively small, the negative SEP-MH association may be cumulative as suggested by previous studies. Since early adolescence is still at the beginning of the life-course, small SEP differences in early life may give rise to increasing differences later in the life. Therefore, interventions to ameliorate the negative effect of low SEP in families may be an important investment.

    Cohort profile: mental health following extreme trauma in a northern Ugandan cohort of War-Affected Youth Study (The WAYS Study).

    War experiences are associated with the risk of long-term mental health problems. The War-affected Youths (WAYS) Study comprises a cohort of 539 youths (61% female) aged between 18 to 25 (at baseline) randomly sampled from the population of war-affected youths in northern Uganda. The study aims to chart the trajectory of long-term mental health consequences of war and the roles of individual, family, and community contextual risk and protective factors in influencing the course of mental health using Social Ecology Model, thus, addressing both the individual and its social ecology. Knowledge of postwar contexts may inform policy and guide interventions on postwar psychosocial adjustment and reintegration in conflict-prone Great Lakes region of Africa (Rwanda, Burundi, DR Congo, Uganda, Central African Republic, and South Sudan). Two waves of data collection have been conducted and more data collection is planned. At baseline, information on demographic characteristics, pre-war experiences, psychosocial outcomes, coping, stigma/discrimination, family and community acceptance and relationship, family functioning, and post-war experiences were obtained. At follow-up, information on general health, gender-based violence, PTSD, social skills, trauma memory quality, rumination, self-esteem, and psychosocial outcomes were collected. Approval to access the data can be obtained on application to the Principal Investigator upon submission of a research proposal with ethical approval from the applicant's institution.     Sexual violence and general functioning among formerly abducted girls in Northern Uganda: the mediating roles of stigma and community relations--the WAYS study.

    BACKGROUND: Although sexual violence in war is associated with long-term mental health problems, little is known about its association with general functioning and the factors that explain this association. This study aims to illuminate the path from sexual violence to poor functioning. The prevalence of sexual violence among formerly abducted girls in Northern Uganda was assessed as well as the extent to which stigma and community relations explain the association between sexual violence and general functioning. METHOD: In a cross-sectional analysis using data from the WAYS study (N = 210, baseline age 22.06, SD = 2.06, minimum-maximum 18-25), the extent of mediation of the association between sexual violence and general functioning was assessed in multiple regression models. RESULTS: Sexual violence was found to be associated with increased stigma, poor community relations, and poor general functioning. The association between sexual violence and general functioning was mediated by stigma and community relations. The bootstrap results indicated significant mediation by stigma of 47 % (95 % confidence interval [CI] 35 to 78 % and by community relations of 67 % (95 % CI: 52 to 78 %) in the association between sexual violence and general functioning. CONCLUSION: Thus, poor functioning among formerly abducted girls is largely mediated by stigma and poor community relations. However, due to the relatively small effect sizes of the associations, targeted interventions to prevent impaired functioning may have only modest benefits to the formerly abducted girls. Interventions to alleviate the toxic effects of sexual violence in formerly abducted girls would benefit from a holistic approach that targets stigma and poor relationships within communities

    Intrusive memories and depression following recent non-traumatic negative life events in adolescents

    Background: Research in adults suggests that intrusive memories are not just found in individuals with post-traumatic stress disorder (PTSD), yet there is little evidence concerning the phenomenology of intrusive memories in children and adolescents. The present study investigated the frequency of intrusive memories following a recent negative event in an adolescent school sample, and considered the application of cognitive theory to understanding the maintenance of intrusive memories of recent negative events, and their role in maintaining depression. Methods: High school students (aged 11-18 years; n = 231) completed questionnaires concerning affect experienced during a recent negative event, the frequency of subsequent intrusive memories, memory quality, thought suppression, post-traumatic stress and depressive symptoms. Results: Most participants had experienced at least one intrusive memory in the previous week, at similar rates for traumatic events and life events. In non-trauma exposed youth, peri-event affect and memory quality accounted for unique variance in a regression model of intrusive memory frequency, while peri-event affect, memory quality, and intrusive memory frequency accounted for unique variance in a regression model of depression. Limitations: The study needs replication in younger children. Interview methods may be required to ensure that intrusive memories are being assessed and not intrusive thoughts or ruminations. Conclusions: Intrusive memories are common reaction to negative events in adolescents, and may be involved in maintaining subsequent depressed mood. The nature of event memories may have a role in the maintenance of such psychopathology, and may be a target for psychological interventions in this age group

    Returning home: forced conscription, reintegration, and mental health status of former abductees of the Lord's Resistance Army in northern Uganda

    <p>Abstract</p> <p>Background</p> <p>Since the late 1980s, the Lord's Resistance Army (LRA), a spiritualist rebel group in northern Uganda, has killed and mutilated thousands of civilians and abducted an estimated 52,000 to 75,000 people to serve as soldiers, porters, and sex slaves for its commanders. This study examines the types of violence to which former abductees have been exposed and the extent to which these acts have affected their psychological well-being.</p> <p>Methods</p> <p>This is a cross-sectional study of 2,875 individuals selected through a multi-stage stratified cluster sampling design conducted in 8 districts of northern Uganda. Multivariate logistic regressions were performed with symptoms for Post-traumatic Stress Disorder (PTSD) and depression as the main outcome measures.</p> <p>Results</p> <p>One-third of the respondents (33%) self-reported having experienced abduction (49% among the Acholi, the largest tribal group in northern Uganda). Over half (56%) of all the respondents and over two-thirds of those who experienced abduction met the criteria for symptoms of post-traumatic stress disorder (PTSD). Multivariate analysis shows that several factors increased the risk of former LRA abductees developing symptoms of PTSD. These factors included gender (females were more susceptible than males), being a member of the Acholi ethnic group, participating in or witnessing a cumulative number of traumatic events, and encountering difficulties re-integrating into communities after abduction. Factors associated with increased risk of meeting criteria for symptoms of depression included older age of males at the time of abduction, lower score on social relationship scale, high incidence of general traumatic event exposure, high incidence of forced acts of violence, and problems reintegrating into communities after abduction.</p> <p>Conclusion</p> <p>Abduction and forced conscription of civilians has affected the psychological well-being of a significant number of northern Ugandans. The sources of psychological trauma are multiple, ranging from witnessing to being forced to commit violent acts, and compounded by prolonged exposure to violence, often for months or years. Community-based mental health care services and reintegration programs are needed to facilitate the reintegration of former abductees back into their communities.</p

    Socioeconomic status and antisocial behaviour among children and adolescents: A systematic review and meta-analysis

    Previous research on the association between socioeconomic status (SES) and child and adolescent antisocial behaviour has produced mixed findings showing variation in the strength of association. This systematic review and meta-analysis aimed to summarise evidence on the relationship between socioeconomic status and broadly conceptualised antisocial behaviour, investigating variation across a range of antisocial subtypes and other potential moderators, including age, sex and informant. We identified 133 studies containing data suitable for effect size calculation, and 139 independent effect sizes were analysed (total N = 339868). The global meta-analysis showed that lower family socioeconomic status was associated with higher levels of antisocial behaviour. Moderation analyses revealed this relationship was stronger where callous–unemotional traits were the outcome, and where antisocial behaviour was reported by parents or teachers rather than self-reported. The relationship between family SES and antisocial behaviour, however, was independent of higher-level constructs such as national income inequality. These results indicate that SES can be considered a robust correlate of broadly conceptualised antisocial behaviour but the strength of this relationship may depend on the antisocial subtype under investigation and the design of the stud

    PTSD, depression and anxiety among former abductees in Northern Uganda

    <p>Abstract</p> <p>Background</p> <p>The population in Northern Uganda has been exposed to extreme levels of traumatic stress and thousands abducted forcibly became rebel combatants.</p> <p>Methods</p> <p>Using structured interviews, the prevalence and severity of posttraumatic stress disorder (PTSD), depression and anxiety was assessed in 72 former abducted adults, 62 of them being former child soldiers.</p> <p>Results</p> <p>As retrospective reports of exposure to traumatic stress increased, anxiety and PTSD occurrence increased (r = .45). 49% of respondents were diagnosed with PTSD, 70% presented with symptoms of depression, and 59% with those of anxiety. In a multiple linear regression analysis four factors could best explain the development of PTSD symptoms: male respondents (sex) living in an IDP-Camp (location) with a kinship murdered in the war (family members killed in the war) and having experienced a high number of traumatic events (number of traumatic events) were more likely to develop symptoms of PTSD than others. In disagreement to a simple dose-response-effect though, we also observed a negative correlation between the time spent with the rebels and the PTSD symptom level.</p> <p>Conclusions</p> <p>Former abductees continue to suffer from severe mental ill-health. Adaptation to the living condition of rebels, however, may lower trauma-related mental suffering.</p

    Income and social rank influence UK children's behavioral problems : a longitudinal analysis

    Children living in low‐income households face elevated risks of behavioral problems, but the impact of absolute and relative income to this risk remains unexplored. Using the U.K. Millennium Cohort Study data, longitudinal associations between Strengths and Difficulties Questionnaire scores and absolute household income, distance from the regional median and mean income, and regional income rank were examined in 3‐ to 12‐year‐olds (n = 16,532). Higher absolute household incomes were associated with lower behavioral problems, while higher income rank was associated with lower behavioral problems only at the highest absolute incomes. Higher absolute household incomes were associated with lower behavioral problems among children in working households, indicating compounding effects of income and socioeconomic advantages. Both absolute and relative incomes therefore appear to influence behavioral problems
