Best Educational Practices in Pediatric Emergency Medicine During Emergency Medicine Residency Training: Guiding Principles and Expert Recommendations
The state of pediatric emergency medicine (PEM) education within emergency medicine (EM) residency programs is reviewed and discussed in the context of shifting practice environments and new demands for a greater focus on the availability and quality of PEM services. The rapid growth of PEM within pediatrics has altered the EM practice landscape with regard to PEM. The authors evaluate the composition, quantity, and quality of PEM training in EM residency programs, with close attention paid to the challenges facing programs. A set of best practices is presented as a framework for discussion of future PEM training that would increase the yield and relevance of knowledge and experiences within the constraints of 3- and 4-year residencies. Innovative educational modalities are discussed, as well as the role of simulation and pediatric-specific patient safety education. Uncharted Paths: Hospital Networks in Critical Care
Wide variation between hospitals in the quality of critical care lead to many potentially avoidable
deaths. Regionalization of critical care is a possible solution; regionalization has been implemented
for trauma and neonatal intensive care, and it is under active discussion for medical and cardiac critical
care. However, regionalization is only one possible approach to reorganizing critical care services. This
commentary introduces the technique of network analysis as a framework for the following: (1)
understanding how critically ill patients move between hospitals, (2) defining the roles hospitals play
in regional care delivery, and (3) suggesting systematic improvements that may benefit population
health.
We examined transfers of critically ill Medicare patients in Connecticut in 2005 as a model system. We
found that patients are systematically transferred to more capable hospitals. However, we find the
standard distinction of hospitals into either “secondary hospitals” or “tertiary hospitals” poorly
explains observed transfer patterns; instead, hospitals show a continuum of roles. We further examine
the implications of the network pattern in a simulation of quarantine of a hospital to incoming
transfers, as occurred during the severe acute respiratory syndrome epidemic.
Network perspectives offer new ways to study systems to care for critically ill patients and provide
additional tools for addressing pragmatic problems in triage and bed management, regionalization,
quality improvement, and disaster preparedness. Emergency department crowding in The Netherlands: managers’ experiences
__Abstract__
__Background__
In The Netherlands, the state of emergency department (ED) crowding is unknown.
Anecdotal evidence suggests that current ED patients experience a longer length of stay
(LOS) compared to some years ago, which is indicative of ED crowding. However, no
multicenter studies have been performed to quantify LOS and assess crowding at Dutch EDs.
We performed this study to describe the current state of emergency departments in The
Netherlands regarding patients’ length of stay and ED nurse managers’ experiences of
crowding.
__Methods__
A survey was sent to all 94 ED nurse managers in The Netherlands with questions regarding
the type of facility, annual ED census, and patients’ LOS. Additional questions included
whether crowding was ever a problem at the particular ED, how often it occurred, which time
periods had the worst episodes of crowding, and what measures the particular ED had
undertaken to improve patient flow.
__Results__
Surveys were collected from 63 EDs (67%). Mean annual ED visits were 24,936 (SD ±
9,840); mean LOS for discharged patients was 119 (SD ± 40) min and mean LOS for
admitted patients 146 (SD ± 49) min. Consultation delays, laboratory and radiology delays,
and hospital bed shortages for patients needing admission were the most cited reasons for
crowding. Admitted patients had a longer LOS because of delays in obtaining inpatient beds.
Thirty-nine of 57 respondents (68%) reported that crowding occurred several times a week or
even daily, mostly between 12:00 and 20:00. Measures taken by hospitals to manage
crowding included placing patients in hallways and using a fasttrack with treatment of
patients by trained nurse practitioners.
__Conclusions__
Despite a relatively short LOS, frequent crowding appears to be a nationwide problem
according to Dutch ED nurse managers, with 68% of them reporting that crowding occurred
several times a week or even daily. Consultations delays, laboratory and radiology delays,
and hospital bed shortage for patients needing admission were believed to be the most
important factors contributing to ED crowding
Emergency department triage: an ethical analysis
<p>Abstract</p> <p>Background</p> <p>Emergency departments across the globe follow a triage system in order to cope with overcrowding. The intention behind triage is to improve the emergency care and to prioritize cases in terms of clinical urgency.</p> <p>Discussion</p> <p>In emergency department triage, medical care might lead to adverse consequences like delay in providing care, compromise in privacy and confidentiality, poor physician-patient communication, failing to provide the necessary care altogether, or even having to decide whose life to save when not everyone can be saved. These consequences challenge the ethical quality of emergency care. This article provides an ethical analysis of "routine" emergency department triage. The four principles of biomedical ethics - viz. respect for autonomy, beneficence, nonmaleficence and justice provide the starting point and help us to identify the ethical challenges of emergency department triage. However, they do not offer a <it>comprehensive </it>ethical view. To address the ethical issues of emergency department triage from a more comprehensive ethical view, the care ethics perspective offers additional insights.</p> <p>Summary</p> <p>We integrate the results from the analysis using four principles of biomedical ethics into care ethics perspective on triage and propose an integrated clinically and ethically based framework of emergency department triage planning, as seen from a comprehensive ethics perspective that incorporates both the principles-based and care-oriented approach.</p
