    Analysis of Nkx3.1:Cre-driven Erk5 deletion reveals a profound spinal deformity which is linked to increased osteoclast activity

    Extracellular signal-regulated protein kinase 5 (ERK5) has been implicated during development and carcinogenesis. Nkx3.1-mediated Cre expression is a useful strategy to genetically manipulate the mouse prostate. While grossly normal at birth, we observed an unexpected phenotype of spinal protrusion in Nkx3.1:Cre;Erk5fl/fl (Erk5fl/fl) mice by ~6ā€“8 weeks of age. X-ray, histological and micro CT (ĀµCT) analyses showed that 100% of male and female Erk5fl/fl mice had a severely deformed curved thoracic spine, with an associated loss of trabecular bone volume. Although sex-specific differences were observed, histomorphometry measurements revealed that both bone resorption and bone formation parameters were increased in male Erk5fl/fl mice compared to wild type (WT) littermates. Osteopenia occurs where the rate of bone resorption exceeds that of bone formation, so we investigated the role of the osteoclast compartment. We found that treatment of RANKL-stimulated primary bone marrow-derived macrophage (BMDM) cultures with small molecule ERK5 pathway inhibitors increased osteoclast numbers. Furthermore, osteoclast numbers and expression of osteoclast marker genes were increased in parallel with reduced Erk5 expression in cultures generated from Erk5fl/fl mice compared to WT mice. Collectively, these results reveal a novel role for Erk5 during bone maturation and homeostasis in vivo

    Although theories of specific language impairment grounded in universal grammar (UG ) have advanced the description of SLI considerably, they provide limited utility as far as treatment is concerned. Because UG assumes deficits in language principles and parameter setting, remediation of the difficulty is not possible; rather, reliance on compensatory mechanisms is recommended. Compensatory mechanisms rely on the same learning principles as are adopted by theorists that adopt a more emergentist view. Thus, we agree with Ambridge, Pine, and Lieven that a UG-based approach is redundant and recommend focusing efforts on identifying and strengthening treatment strategies associated with general learning principles instead

    What effect have NHS commissionersā€™ policies for body mass index had on access to knee replacement surgery in England?:An interrupted time series analysis from the National Joint Registry

    ObjectiveTo assess the impact of local commissioners' policies for body mass index on access to knee replacement surgery in England.MethodsA Natural Experimental Study using interrupted time series and difference-in-differences analysis. We used National Joint Registry for England data linked to the 2015 Index of Multiple Deprivation for 481,555 patients who had primary knee replacement surgery in England between January 2009 and December 2019. Clinical Commissioning Group policies introduced before June 2018 to alter access to knee replacement for patients who were overweight or obese were considered the intervention. The main outcome measures were rate per 100,000 of primary knee replacement surgery and patient demographics (body mass index, Index of Multiple Deprivation, independently-funded surgery) over time.ResultsRates of surgery had a sustained fall after the introduction of a policy (trend change of -0.98 operations per 100,000 population aged 40+, 95% confidence interval -1.22 to -0.74, PConclusionsBody mass index policy introduction was associated with decreases in the rates of knee replacement surgery across localities that introduced policies. This affected all patient groups, not just obese patients at whom the policies were targeted. Changes in patient demographics seen after policy introduction suggest these policies may increase health inequalities and further qualitative research is needed to understand their implementation and impact

    Quantifying the sensitivity of distributive fluvial systems to changes in sediment supply and lake level using stratigraphic forward modelling

    Stratigraphic forward modelling has been used to quantify the sensitivity of sandbody connectivity in a distributive fluvial system to changes in sediment supply and lake level. Recent stratigraphic forward modelling using SedsimX from StrataMod Pty Limited of the Oligocene to Miocene Huesca distributive fluvial system in northern Spain was used as a baseā€case for this sensitivity analysis. Based on literature research and initial modelling, a sediment supply sensitivity range of 0.22 to 21.85 km3/kyr and lakeā€level sensitivity range of āˆ’1000 to 1000 mm/kyr were used. Results show that the stratigraphic architecture of the modelled distributive fluvial system is more sensitive to changes in sediment supply than to changes in lake level. While an increase in the rate of sediment supply results in an increase in preserved average grain size, aggradation rates and sandbody connectivity at the same distance from the apex, the average grain size, aggradation rate and sandbody connectivity all decrease with increasing distance from fan apex. The main difference in the stratigraphic architecture can be found in the proximal zones. Only oversupplied models, with much higher sediment supply than the baseā€case, deposited fully amalgamated channelized deposits with laterally continuous, tabular beds with occasional scoured surfaces. Models with baseā€case sediment supply contain channelized sandy deposits within a fineā€grained floodplain environment. Models with sediment supply much lower than the baseā€case had no deposition in the proximal zone. Lakeā€level rise leads to reduced distal erosion of sediments, concentration of silts close to the lake shore, and higher aggradation rates and thicker sandbodies in the proximal zone. The sensitivity analysis highlights that the parameters governing the formation of distributive fluvial systems have different weightings but are ultimately all interconnected and interdependent. This quantitative framework can be used as a predictive tool for subsurface exploration in distributive fluvial systems
