Geriatric medicines development: improving regulatory strategies towards establishing an evidence-based benefit-risk balance in the geriatric population

Nesta dissertação, é apresentada uma explicação detalhada sobre o envelhecimento e as doenças relacionadas com a idade de forma a permitir caracterizar o doente geriátrico e, posteriormente, fazer a ligação com os requisitos necessários a um eficaz desenvolvimento de medicamentos centrado no paciente geriátrico. A legislação que, actualmente, suporta a aprovação de medicamentos para comercialização e a execução de ensaios clínicos, assim como a interacção entre o ambiente dos ensaios clínicos e o sujeito geriátrico são objecto de uma análise profunda. É proposto um conjunto de medidas regulamentares para a melhoria do desenvolvimento de medicamentos geriátricos baseado em evidências.The worldwide growing geriatric population, with high incidence in Europe, raises a sense of responsibility on geriatric patient representation in medicines development, considering younger adults’ studies data cannot be directly transposed to the elderly. In this study, a detailed understanding of aging and age-related diseases is presented for the characterization of the geriatric patient to further make the connection with the requirements needed for an effective geriatric patient-centric medicinal products development. The current regulatory that supports medicinal products marketing approval and clinical trials execution as well as the interaction between clinical trials environment and the geriatric subjects are deeply analyzed. A set of regulatory proposals is presented for the improvement of evidence-based geriatric medicines development

experiência da alteridade, liberta assim de referências exclusivas a origem ou destino das
personagens, transforma percursos migratórios em questionamentos ontológicos, com
implicações significativas nos planos da narrativa e da leitura literária. A questão da voz e
a enunciação em primeira pessoa são traços significativos do hibridismo de qualquer das
obras, em termos conceptuais e de género literário
O limite como pretexto : análise de três casos de estudo da obra de Álvaro Siza Vieira

O presente trabalho tem como principal objectivo apoiar-se no conceito abstracto de limite, como fio conductor e orientador deste estudo, que tem como pretexto uma incursão pela obra do arquitecto Álvaro Siza Vieira, na senda da compreensão e entendimento dos valores da mesma. Esta dissertação apoia-se em três obras arquitectónicas: Piscinas das Marés de Leça da Palmeira, Bairro da Quinta da Malagueira em Évora e o projecto dos Terraços de Bragança em Lisboa. Primeiramente, procuraremos incidir sobre o universo das características que definem Álvaro Siza Vieira como arquitecto e autor, que se destaca por uma abordagem sensível e poética nas suas intervenções. Seguiremos um percurso pelo contexto histórico e enquadramento de cada um dos projectos que este estudo convoca. Em seguida, será feita uma abordagem ao conceito de limite, aplicado ao pensamento da arquitectura, que conduzirá, por fim, à análise interpretativa de cada obra individualmente, tendo em consideração a nossa linha de raciocínio do limite.In what way can the concept of limit be constituted as an operative argument for the regulation of an architectural solution towards the surrounding territory with which it has to deal? The main objective of this work is to rely on the abstract concept of limit, as a conductor wire, guiding the study, that has as a pretext an incursion for the work of the architect Álvaro Siza Vieira, in the path of understanding the values of his work. This dissertation is based on three architectural works: Leça da Palmeira Swimming Pools, Malagueira Planning neighborhood in Évora and the Bragança Terrace in Lisbon. First, we will focus on the universe of characteristics that define Álvaro Siza Vieira as an architect and author, who stands out for a sensitive and poetic approach in his interventions. We will follow a path through the historical context and framing of each of the projects that this study calls. Next, an approach will be taken to the concept of limit, applied to the thought of the architecture, that will lead, finally, to the interpretative analysis of each work individually, taking into account our line of reasoning of the limit
Cerâmicas da Idade do Ferro II do depósito votivo de Garvão
Garvão foi um importante local sagrado da Idade do Ferro II, onde em 1982 foi
acidentalmente descoberto um impressionante depósito votivo [1]. Na escavação foram
recuperados uma enorme variedade de materiais, sobretudo cerâmica, os quais foram
depositados intencionalmente e cuidadosamente organizados de modo a optimizar o espaço
disponível. A cerâmica recuperada mostra que durante a Idade do Ferro este sítio
arqueológico foi um ponto de fusão das sociedades ibéricas com fortes influências celtas
(interior da Península Ibérica) e do mundo Mediterrâneo.
A análise estilística das cerâmicas permitiu a classificação do conjunto cerâmico em
diferentes grupos tipológicos. Relacionando o estudo material com aspectos geológicos
pretende-se contribuir para uma melhor compreensão das sociedades que produziram estas
cerâmicas e das suas interacções no SW da Península durante a Idade do Ferro II. Deste
modo, estabeleceu-se uma metodologia de estudo multi-analítica. Aplicando técnicas
modernas das ciências dos materiais e os princípios da ciências físicas (por exemplo,
geologia e química) pode-se obter respostas e uma melhor compreensão da importância de
Garvão nesta área da Península Ibérica. A combinação de técnicas como microscopia
electrónica com possibilidade de análise elementar, difracção de raios-X e análise térmica
permitem identificar a composição mineralógica destas cerâmicas que extrapolada para a
geologia regional, permitem importantes contributos para o conhecimento das sociedades
que produziram estes materiais de uso comum. Especial ênfase é concedida à fonte de
proveniência das matérias-primas, aos aspectos tecnológicos e, às rotas de circulação de
mercadorias, ideias e crenças religiosas.
Os primeiros resultados [2] são sobre um grupo de cerâmicas em que foram utilizados dois
tipos de matérias-primas: um núcleo inicial com desengordurantes mais grosseiros e a
construção da peça com material mais fino.

Iron age pottery from Garvão votive deposit
The Iberian Peninsula was an attractive area in ancient times due to its abundance in geological resources. Hence, the Iberian Iron Age is a period of successive social and political transformations, sometimes resulting in conflicts [1]. In 1982, an impressive votive deposit was discovered in Garvão (SW Portugal) revealing an important Iron Age II holy site [2]. Beside some uncertainties, this region is marked by very strong Mediterranean cultural influence but the inhabitants are celtic. The materials recovered (mainly pottery) were intentionally deposited, carefully arranged in order to optimize the available space [2, 3]. The pottery recovered shows that during Iron Age this archeological site was a merging geostrategic point of the Iberian societies with strong influences of the Mediterranean world and the Iberia celtic influences.
Pottery fragments are one of the most common signs of human occupation. Its aesthetic aspects are extensively studied as one of mankind’s earliest expressions of representational art. But the ceramic characteristics depend also on the technology accessibility, the relationship of the societies with their environment, especially with the availability of raw materials, and on the commercial and cultural connections with other societies [4, 5]. Raw materials used by these communities provide essential information to understand specific historical periods. The geological resources are particularly useful for this purpose because even processed they can figure out identifiable signatures of provenance or technology used. Moreover, their intermittent occurrence and human necessity justify the existence of trade routes.
A detailed study of the pottery based on stylistic analysis was envisaged which allowed the classification into different clusters. Special emphasis will be given to the provenance of the pottery, the technological aspects and the relationship between populations and Garvão holy site. The study of geological raw materials applying and combining modern techniques of earth materials sciences and the principles of physical sciences (e.g. geology and chemistry) can provide answers and a better understanding of the Garvão importance in this area of the Iberian Peninsula. A multi-analytical methodology was setup using optical and electron microscopy, infrared spectroscopy, X-ray fluorescence and X-ray diffraction. The combination of these techniques applied to selected Garvão pottery materials allowed the recognition of the ceramic phase composition, firing temperature, kiln atmosphere, ceramic manufacturing processes and origin of raw materials. With this study, connections with Mediterranean areas and with local production areas were underlined revealing that Garvão was a multicultural area.

Isotopic investigation in the region of Pax Julia during paleochristian occupation: paleodiets and mobility
In this study, diet and mobility of the Palaeochristian populations from the Roman
villae of São Cucufate and Cegonha, and from
the necropolis of Alpendre dos Lagares, located
in the Beja region in southern Portugal, was
investigated by isotopic analysis.
Osteological tissues provide information on diet and mobility of the individuals.
Isotopic analysis of delta 13C and delta 15N of bones can be used to determine the food intake of ancient populations whose dietary habits are not well known due to the lack of archaeological evidence. Isotopic ratios of the bone organic (delta 13C and delta 15N) and inorganic fractions (delta 13C) can provide information on the types of plants
ingested , the amount of animal resources,
terrestrial versus marine resources, as well as breastfeeding and weaning practices.
Individual mobility can be assessed by the
measurement of 87Sr/86Sr in the inorganic fraction of teeth and bones. The local 87Sr/86Sr geological ignature where the individual spent its childhood is recorded in the teeth, while 87Sr/86Sr recorded in bones relates to the place where the individual spent the last 10 years before death. Comparison of the Sr isotopic signature of teeth and bones can be used to infer about the individual’s mobility pattern.
Bone isotopic analysis can be compromised by
the diagenetic processes which occur during
burial. In this study, FTIR (Fourier Transformed Infrared Spectroscopy) and XRD (X-Ray Diffraction) analyses were used to assess the diagenetic processes impacting the skeletons. Dietary isotopic analysis of studied populations indicates a mixed terrestrial diet, with some small inter- and intra-populational variations. Dietary isotopic values obtained for the Paleochristian population of Pax Julia are roughly similar to Roman populations in the Mediterranean area. In terms of mobility, analysis of the Cegonha individuals proved they were mainly local with some evidence of limited movements
Cuatro altares de La Vera, Cáceres
This paper describes four Roman altars recently found in La Vera, which is the name given to the country lying between Tiétar river and Sierra de Gredos, in the Spanish province of Cáceres. Of the nine inscriptions, three epigraphs are dedications to local deities: <i>Band(-) Vortiacius,</i> a new divine couple, <i>Ulisus and Ulisona;</i> and <i>Quangeius,</i> this one being placed <i>in fano,</i> which is the second occurrence of that word in Spanish epigraphy. The last piece is a tombstone so dilapidated that it is difficult to tell apart even the normal features of an ancient epitaph.<br><br>Se describen cuatro altares inscritos recientemente descubiertos en La Vera, comarca cacereña situada en la orilla septentrional del río Tiétar. De las nuevas inscripciones, tres son altares dedicados a lo que se han llamado “dioses indígenas”: <i>Band(-) Vortiacius,</i> lo que parece una nueva paredría (<i>Ulisus y Ulisona</i>) y <i>Quangeius;</i> nótese que este se colocó <i>in fano,</i> la segunda mención epigráfica del término aparecida hasta ahora en la Península. La última pieza es un epitafio, pero tan estropeado por la erosión, que apenas se reconocen los elementos característicos de esta clase de epígrafes
Placa funerária da Herdade da Ponte dos Frades
Estuda-se a placa destinada a um jazigo funerário romano, que apenas tem o nome da primeira personagem a ser aí sepultada. Sugere-se que seja, por isso, o elemento principal da família e o proprietário primeiro da villa onde a placa se encontrou, nos arredores de Pax Iulia. O monumento, pela forma como o cidadão C. Cosconius vem identificado (sem cognomen), deverá datar-se dos primórdios do séc. I da nossa era
Desempenho sísmico do templo romano de Évora
O presente artigo tem como propósito avaliar o desempenho sísmico do templo romano de Évora. O modelo numérico adotado para as análises temporais não-lineares do templo é composto por 156 blocos e é baseado no método dos elementos discretos. O desempenho sísmico do templo foi avaliado através do método da análise dinâmica incremental. Neste estudo foram considerados os dois tipos de sismo previstos no Eurocódigo 8. Para cada sismo tipo foram considerados três acelerogramas independentes, compatíveis com o espetro de resposta regulamentar. As análises sísmicas foram realizadas para as duas direções principais da estrutura. No total foram realizadas 84 análises sísmicas temporais não lineares. Os resultados obtidos indicam que, para o sismo regulamentar previsto para Évora, a estrutura irá registar algum dano mas não irá colapsar. No entanto, a análise dinâmica incremental mostra que cerca de 2,5% dos blocos colapsa quando a aceleração máxima de pico regulamentar é duplicada
